Adnexal torsion is defined by the existence of a partial or complete torsion of the ovary and a tubal portion around its vascular pedicle. It is a rare gynecological emergency. We report the case of adnexal torsion on a large postpartum ovarian cyst in a 40-year-old patient. The diagnosis was suspected based on clinical and ultrasound signs and confirmed by exploratory laparotomy. The appearance of the cyst was suggestive of a mucinous cystadenoma of the ovary. The treatment was radical by performing an adnexectomy. The surgical piece weighed 5.2 kg. The postoperative course was simple. Anatomopathological examination had not been carried out;the parents had buried the operating piece.

Background: Urinary tract infections (UTIs) are a frequent reason for consultation and lead to a significant and sometimes inappropriate prescription of antibiotics. The latter favors antibiotic resistance and an increase in mortality as well as the cost of treatment. The present study aims to contribute to the fight against antibiotic resistance of enterobacteria. Methods: This is a prospective study from January to December, 2021 in the Microbiology laboratory of the General Reference hospital (GRH) of niamey including 3369 urine samples. The antibiotic resistance of enterobacteria was determined using the Viteck-2 method. Results: At least 280 strains of Enterobacteriaceae were isolated from the patient’s urine. Among these strains, Escherichia coli was the most predominant (74.64%), followed by Klebsiella pneumoniae (16.07%) and Enterobacter cloacae (7.14%) and other enterobacteria 2.15%. These Enterobacteriaceae are more common in community patients than in hospitalized patients. The average age of patients is 52 years and the age group most affected by these enterobacteria is 46 - 60 years (23.33%). The female sex is the most affected sex with (51.07%) against (48.97%) for the male sex with 1.04 as sex ratio. The hospitalization departments most affected by these enterobacteria are Nephrology (29.23%) and Endocrinology (21.54%). Up to 75% of the Enterobacteriaceae isolates show high resistance to ampicillin, amoxicillin-clavulanic acid, ticarcillin, piperacillin-tazobactam, cefoxitin, cefixime, ceftazidime, ceftriaxone, cotrimoxazole, nalidixic acid and ofloxacin. Conclusion: The high rate of antibiotic resistance among enterobacteria in urine is of concern. Only a few Enterobacteriaceae show low resistance to ertapenem, imipenem, amikacin, gentamicin, fosfomycin and nitrofurantoin. Therefore, these antibiotics are recommended as first line treatment for urinary tract infections.

Introduction: Neonatal seizures are one of the most challenging situations for paediatricians. The objective of this work was to study the epidemiological and diagnostic aspects and short-term outcomes of neonatal seizures at Issaka Gazoby Maternity hospital in niamey. Patients and Methods: This was a prospective study from November 2020 to April 2021 in the neonatology department of Issaka Gazoby Maternity hospital. All newborns aged 0 to 28 days hospitalized for seizures and/or having convulsions during hospitalization were included. Neonatal characteristics, diagnostic aspects, and their outcomes were studied. Data were analyzed using SPSS version 20 software. Results: Of the 3.068 newborns admitted, 69 cases of neonatal seizures were recorded (2.24%). The sex ratio was 1.22, and 94.2% of neonates were born at term. Generalized crises were found in 50.7%. The main etiologies were perinatal asphyxia (46.4%) and early-onset neonatal infection (40.6%). The death rate was 20.3%. Neonates died between one (1) and three (3) days of age in 42.9%. The main death causes were perinatal asphyxia (50%) and early-onset neonatal infection (21.4%). Conclusion: Neonatal seizures are uncommon frequent, with a semiology dominated by generalized seizures. Mortality is high. The reinforcement of preventive measures is necessary.

Introduction: IgG4 disease is an orphan, fibro-inflammatory autoimmune disease of recent discovery whose thoracic involvement is rarer. We report a case of Pulmonary and orbital bifocal IgG4 disease with a review of the literature. Observation: This is a 71-year-old patient with a history of hypertension, dyslipidemia, smoking cessation at 15 PA, with progressive dyspnoea, weight loss of 4 kg with PS = 0 for 2 months. The thoracic CT scan revealed 3 pseudotumoral lung lesions of the LIG, LID and LM. The histology of the two CT-guided lung biopsies and the LIG wedge had objectified inflammatory lesions without signs of malignancy. The evolution was marked by the occurrence of a right orbital edema. The cerebral scanner found a voluminous right orbital inflammatory pseudotumor. Biopsy with histology found fibroinflammatory lesions with lymphoplasmacytic infiltrates and positive immunolabeling with anti-IgG4 antibodies. The PET scanner had objectified pulmonary and pleural parenchymal consolidations and moderately hypermetabolic mediastinal ADP with max SUV between 3 and 6. The patient was put on corticosteroid therapy with a favorable outcome. Conclusion: IgG4 disease is rare and difficult to diagnose despite well-defined and consensual diagnostic criteria and classification. The discovery of new biomarkers facilitates the diagnosis and monitoring of patients. Well-codified corticosteroid therapy is effective but possibility of recurrence. The current challenge remains the lack of data on the follow-up of these patients to assess the risk of neoplasia (lymphoma).

Introduction: Cerebral palsy is the most common motor disorder of childhood according to the Center for Disease Control and Prevention (CDC). It is much more common in premature infants. The majority of fixed organic lesions occur during the development of the central nervous system. Objective: To study the epidemiological, clinical, paraclinical and prognostic profile of children aged 0 to 5 years with Cerebral Palsy (CP) seen in consultation in the medical neurology department of the National hospital of niamey. Methods: This was a transverse descriptive study with prospective collection of patient records (0 - 5 years) over a 6-month period. Results: During our study, 100 children aged 0 to 5 years had to be consulted. The prevalence was 61%. 62.3% of patients were male. The mean age was 2.01 years. The majority of the patients were from the urban area (63.93%). Almost half of the mothers had no schooling. 4.92% of patients were from consanguineous marriages. Pregnancies were monitored with at least 4 prenatal visit in 86.98% of cases and 83.61% of children were born at term. In 75.41% of cases, patients were born by eutocic delivery. In 36.07% of cases, the patients were resuscitated at birth. Fetal distress was present in 29.5%. Seizures were present in 49.18% of the patients of which 50% were generalized. Tonicity was not good in 35.15% and among them 52.63% had hypertonia. 18.03% of patients had a motor deficit. For 34.43% of the patients the first nerve was affected, followed by the third nerve with 13.11%. 65.57% of the patients had fixed retardation and for 78.69% of the patients the retardation was homogeneous. Neuromalaria, meningitis and hydrocephalus were the most incriminated causes of delay in 63.51%, 19.67% and 18.03% of cases respectively. Anticonvulsants and psychoanaleptics were the most used in 49.18% and 32.79% of cases respectively. 32.79% of patients had received motor rehabilitation. All mothers had received psychotherapy. We report three cases of neurofibromatosis type 1 disease with literature review, collected in the department of neurology and internal medicine from National hospital of niamey (HNN). Two of them were men and the first signs were noted by the mother at the birth in 2 cases. Only one case of consanguinity was observed. Clinically, light brown spots on the skin, neurofibromas, Lisch nodules were constantly observed. Histopathological’s exam confirmed neurofibromas. Moreover, cutaneous and ophthalmological manifestations lead to the diagnostic. Two cases of orthopedic complications were observed: one scoliosis and one Congenital dysplasia of the long bones. There was no specific treatment. Neurofibromatosis type 1 or von Recklinghausen’s disease is the most frequent phacomatosis and its diagnosis is usually composed of a set of clinical criteria of the National Institute Health (Bethesda, 1988).

Introduction: Goiter is the most common endocrine disease in the world. The objective of our study is to assess and compare the morbidity linked to the surgical management of goiters and thyroid tumors in the general surgery department of the niamey National hospital. Result: We collected data on a prospective basis on 112 cases having undergone a thyroidectomy between March 2019 and April 2020. The female sex is predominant with 96.4%, i.e. a sex ratio of 0.037. The average age is 41.2 years with extremes ranging from 22 to 72 years old. The average duration of evolution of goiter is 7.5 years with extremes ranging from 5 months to 20 years old. The first reason for consultation is anterior cervical swelling. TSH testing confirmed euthyroidism in 107 patients (95.5%). Therapeutically, loboisthmectomy is the most performed procedure with 43.7%. The postoperative hormonal assessment revealed 32% hypothyroidism at 6 months, with a significant difference for cases of total thyroidectomy. The occurrence of postoperative hypocalcemia was significantly greater after total and subtotal thyroidectomy. There is no significant difference in terms of recurrent complications. Conclusion: Thyroidectomies must be performed after adequate preparation and information to the patient on possible postoperative complications, hence the need for close medical monitoring immediately and remotely.

The aim of this study was to present the epidemiological, clinical and therapeutic aspects of adult urological cancers in the department of urology at Zinder National hospital. Patients and Methods: This was a cross-sectional study of adult primary urologic cancers during the period of January 2019 to December 2023. Data were obtained from the record of patients admitted to urology, and the medical oncology department. The parameters studied were;frequency, age, sex, urogenital distribution, anatomopathological type, and therapeutic aspects. Data were recorded on Excel 2013 and analyzed with Epi-info version 7.2.5. Results: Epidemiology: About 289 cancers were diagnosed through 6017 consultations during the period, corresponding to 4.80% of urological pathologies: prostate cancer (n = 221;76.47%), bladder cancer (n = 46;15.92%), kidney cancer (n = 13;04.5%), testis cancer (n = 09;03.11%). Median age of patients was 50.42 years. Prostate cancer: was Adenocarcinoma in 100% (n = 221) with Gleason scores > 7 (77.83%) and prostatique specific antigen (PSA) > 20 ng/ml in 90.04%. Extension evaluation 134 cases (60.63%). Treatment was chirurgical castration in (62.44%) and resistance to castration appeared between 8 and 11 months. hospital mortality for prostate cancer was 16.29%. Bladder cancer was found in 46 cases with Sex ratio 7/1. Cystoscopy was performed for all. Histology revealed squamous cell carcinoma (n = 41;89.13%), and (n = 5;10.87%) urothelial. Endoscopic resection performed (n = 14;30.43%). Kidney cancers were 13 cases (10 men, 03 women). Nephrectomy was performed in (n = 8;61.54%) cases and Histology revealed renal cell cancer in 76.92%. Testicular cancers were diag 09 cases. History of cryptorchidism was found in 4 cases, 4-year infertility in 3 cases. Orchidectomy was done in all cases. Chemotherapy in 3 cases. Introduction: Since it is impossible to establish a diagnosis in the presence of hyperlymphocytosis not secondary to lymphocytic hyperactivation, we considered a B-lymphoid hematopathy with a non-specific phenotypic profile. We report one case of this. Observation: This is a forty-eight (48) year old patient with hyperlymphocytosis at 139,000 elements per cubic millimeter, heterogeneous splenomegaly at 25.6 cm in diameter on abdominal ultrasound without detectable deep or peripheral lymphadenopathy. Peripheral blood cytology shows lymphocyte cell proliferation suggestive of the circulating phase of chronic lymphoproliferative B syndrome. The expression profile of cell membrane markers did not allow for the definition of a specific phenotypic profile. Faced with this immunophenotyping result, we considered a B-lymphoid hemopathy with a non-specific phenotypic profile. After three courses, the MINICHOP treatment was used to achieve partial remission with wasting of more than 80% of the evaluable masses. Conclusion: Despite the contribution of immunophenotyping in the diagnosis of lymphoproliferative syndromes, it is possible to consider the diagnosis of a B-lymphoid hemopathy with a phenotypic non-specific profile of CD45+, monotypic kappa, CD19+, FMC7+, CD22+, CD5−, CD79b−, CD23−, CD43−, CD38−, CD200−.

Malignant non-Hodgkins lymphoma (MHNL) of the uterus is uncommon. We report a case diagnosed on the basis of histologic and immunohistochemical studies of a hysterectomy specimen induced by a very painful pelvic mass in a 50-year-old patient with no previous history of the disease. It was classified as Ann Arbor IV Bb after imaging, given the medullary infiltration and signs of clinical and biological evolutivity: the patient had received two courses of chemotherapy, CHOP protocol. She died 23 days after the second treatment due to a hypertensive crisis.
