Surgical treatment for primary hyperparathyroidism

Objective To document the recent surgical results in the treatment of primary hyperparathyroidism (HPT) at Queen Mary Hospital. Methods From 1995 to 1996, 30 patients underwent 31 operations for primary HPT. The data of 11 men and 19 women with ages ranging from 19 to 86 years (median: 57 years) were prospectively recorded with emphasis on the need of preoperative localization. Results Symptoms of hypercalcemia were present in 20 (67%) and complications in 17 (57%) patients, respectively. Seventy seven localization studies were performed in 28 patients (average: 2.6 tests/patient). Localization was accurate in 12 of 23 (52%) ultrasonographies, 11 of 26 (42%) CT scans and 16 of 27 (59%) scintigraphies. Twenty six patients had a single adenoma excised while 3 patients with multiple endocrine neoplasia type Ⅰ (MEN Ⅰ) had subtotal parathyroidectomy for multiglandular hyperplasia during cervical exploration. Immediate normocalcemia was achieved in 29 patients. One patient had persistent hypercalcemia due to a supernumerary fifth gland in the superior mediastinum that was successfully excised in a second operation. One patient had a unilateral vocal cord paralysis and 4 patients needed calcium supplement on discharge. During a median follow up of 5 months, all patients were normocalcemic with one requiring calcium supplements. Conclusion Surgical treatment for primary HPT is a safe procedure and is associated with a high success rate. In our experience routine preoperative localization study is not cost effective.

Cloud computing plays an important role in today’s Internet environment,which meets the requirements of scalability,security and reliability by using virtualization *** technology is one of the two mainstream virtualization *** lightweight,high deployment efficiency make container technology widely used in large-scale cloud *** container technology has created huge benefits for cloud service providers and tenants,it cannot meet the requirements of security monitoring and management from a tenant ***,tenants can only run their security monitors in the target container,but it is not secure because the attacker is able to detect and compromise the security *** this paper,a secure external monitoring approach is proposed to monitor target containers in another management *** management container is transparent for target containers,but it can obtain the executing information of target containers,providing a secure monitoring *** monitors running inside management containers are secure for the cloud host,since the management containers are not *** implement the transparent external management containers by performing the one-way isolation of processes and *** process one-way isolation,we leverage Linux namespace technology to let management container become the parent of target *** mounting the file system of target container to that of the management container,file system one-way isolation is *** with the existing host-based monitoring approach,our approach is more secure and suitable in the cloud environment.

Influence of reference states on Jupiter’s dynamo simulations

Jupiter’s magnetic field is thought to be generated in its deep metallic hydrogen region through dynamo action,yet the detailed dynamic process remains poorly *** simulations have produced Jupiter-like magnetic fields,albeit using different control parameters and reference state *** this study,we investigate the influence of different reference state models,based on ab initio calculations and based on the polytropic equation of *** doing so,we perform five anelastic convection dynamo simulations that can be divided into two *** each group,different reference states are used while other control parameters and conditions are set to be *** find the reference state model can be very influential for the simulations in which buoyancy force is dominant over the Lorentz *** this regime,different dynamical outcomes can be attributed to the effective buoyancy force resulting from different reference *** simulations in which the Lorentz force is dominant over the buoyancy force,however,dynamo actions tend to be insensitive to different reference state *** Jupiter’s dynamo is in a strong field regime,i.e.,the Lorentz force plays a dominant role,our numerical results suggest that Jupiter’s internal reference state,which remains poorly constrained,may play a minor role in the dynamo process of the planet.

We study the behaviors of three-dimensional double and triple Weyl fermions in the presence of weak random poten*** performing the Wilsonian renormalization group(RG)analysis,we reveal that the quasiparticle experiences strong renormalization which leads to the modification of the density of states and quasiparticle *** further utilize the RG analysis to calculate the classical conductivity and show that the diffusive transport is substantially corrected due to the novel behavior of the quasiparticle and can be directly measured by experiments.

Effect of peripheral nerve on the neurite growth from retinal explants in culture

The effect of peripheral nerve （PN）on neurite outgrowth from retinal explants of adult hamsters was examined. Cultures of retinal explants, and co-cultures of retinal explants and PN were performed using chick retinal basement membrane （BM） as substrate. The presence of PN increases the number and length of neurite outgrowth. In addition, a high proportion of neurites situated close to PN tend to grow towards it. Since there was no contact between retinal explants and PN, we suggest that PN might secrete diffusible substances to attract the neurites to grow towards it.

The critical roles of mitophagy in cerebral ischemia

Mitochondria play a kay role in various cell processes including ATP production, Ca^2＋ homeostasis, reactive oxygen species （ROS） generation, and apoptosis. The selective removal of impaired mitochondria by autophagosome is known as mitophagy. Cerebral ischemia is a common form of stroke caused by insuf- ficient blood supply to the brain. Emerging evidence suggests that mitophagy plays important roles in the pathophysiological process of cerebral ischemia. This review focuses on the relationship between ischemic brain injury and mitophagy. Based on the latest research, it describes how the signaling pathways of mitophagy appear to be involved in cerebral ischemia.

Joint timing and frequency synchronization in coherent optical OFDM systems

In this paper,we review our joint timing and frequency synchronization algorithms in coherent optical orthogonal frequency division multiplexing(CO-OFDM)*** first present a timing estimation method by designing the pattern of the training symbol,whose timing metric has a sharp and clear peak,to ensure accurate timing offset(TO)estimation *** we provide both data-aided(DA)and blind(BL)approaches to estimate the carrier frequency offset(CFO).For the DA algorithm,we utilize the same training symbol structure as the timing estimation does,while for the BL algorithm,we utilize the zero-subcarrier power(ZSP)to achieve full-range CFO *** that our joint timing and frequency synchronization approaches require only one OFDM symbol,which ensure not only the data transmission efficiency,but also the TO and CFO estimation performance.A modified BL ZSP algorithm is proposed to further improve the CFO estimation performance by taking the power average over a series of OFDM *** effectiveness of the TO estimation algorithm,and both the DA and BL CFO estimation algorithms are verified and demonstrated in both simulations and experiments.

Is there an anatomical endophenotype for neurodevelopmental disorders? A review of dual disorder anatomical likelihood estimation (ALE) meta-analyses of grey matter volumes

The term "neurodevelopmental disorder" broadly encompasses conditions thought to arise early in development and includes schizophrenia, bipolar disorder and autism among others. These conditions share a number of genetic and environmental risk factors postulated to lead to common difficulties in socio-emotional processing, communication and cognitive function. The alternative position is that, while the same traits are affected across these conditions, the nature or direction in which they are modified may be distinct. MRI studies provide a rapidly expanding and rich database which we propose can be used to contribute to this debate. Anatomical likelihood estimation (ALE) is a method of meta-analysis applied to voxel-based MRI studies. We have adapted this method to explore the extent to which schizophrenia and bipolar disorder and schizophrenia and autism share a common brain structural phenotype. We will review this work here and discuss whether there is sufficient other evidence to justify a common framework for further research into the inter-relatedness of such conditions.

Computer simulation on reliability of retention index with FDG-PET and optimization of dual-time-point imaging protocol

The inherent noise in positron emission tomography (PET) leads to the instability of quantitative indicators, which may affect the diagnostic accuracy for differentiating malignant and benign lesions in the management of lung cancer. In this paper, the reliability of retention index (RI) is systematically investigated by using computer simulation for the dual-time-point imaging protocol. The area under the receiver operating characteristic (ROC) curve is used to evaluate the optimal protocol. Results demonstrate that the reliability of RI is affected by several factors including noise level, lesion type, and imaging schedule. The RIs with small absolute values suffer from worse reliability than those larger ones. The results of ROC curves show that over delayed second scan cannot help to improve the diagnostic performance further, while an early first scan is expected. The method of optimization based on ROC analysis can be easily extended to comprise as many lesions as possible.

To condemn a conglomerate merger for the foreclosure effect of post-merger activities, one should examine the anticipated conduct against the same liability rules under the law of agreement and the law of abuse of dominance as if the conduct has been materialized Given the inherent uncertainty of pre-merger assessment, it requires a high standard of proof to satisfy the adjudicator that the merger in question would lead to anticompetitive effect in all the circumstances. Conglomerate merger, which will reinforce market concentration or increase entry barriers, should be stringently scrutinized. Saying so, the pro-efficiency effects of conglomerate merger are appreciated One shouM not take it for granted that agglomeration will be restrained through conglomerate merger control.
