To the Editor:Probiotics are a promising treatment modality for functional constipation(FC);however,the factors affecting individual responses to probiotics remain *** evidence has identified that there is a strong relationship among the gut microbiota and constipation and related gut-brain axis.[1]The gut microbiota may modulate the gut functions via gut metabolites or trigger the release of gut hormones,such as peptide YY,gastric inhibitory polypeptide,and 5-hydroxytryptamine.[2]In turn,gut hormones affect gut secretion,motility,and sensation through their receptors located on epithelial,enteric,and smooth muscle cells.[2]Based on these findings,exogenous probiotics have been used in patients with constipation;however,few have produced consistent results.[3]In this study,we investigated the efficacy and safety of several specific probiotics strains for constipation,and also examined the potential reasons for the individualized effects of probiotics,which may facilitate interventional decisionmaking for FC patients.

Hepatocellular carcinoma(HCC)is a common malignant tumor with poor prognosis and high *** this study,we demonstrated a novel vaccine targeting HCC and tumor neovascular endothelial cells by fusing recombinant MHCC97H cells expressing porcine a-1,3-galactose epitopes(a Gal)and endorphin extracellular domains(END)with dendritic cells(DCs)from healthy ***^(+)/Gal^(+)-MHCC97H/DC fusion cells induced cytotoxic T lymphocytes(CTLs)and secretion of interferon-gamma(IFN-γ).CTLs targeted cells expressing a Gal and END and tumor *** fused cell vaccine can effectively inhibit tumor growth and prolong the survival time of human hepatoma mice,indicating the high clinical potential of this new cell based vaccine.

AIM To evaluate trends and disparities in hepatocellular carcinoma(HCC) outcomes among Hispanic patients in the United States with a focus on tumor stage at *** We retrospectively evaluated all Hispanic adults(age > 20) with HCC diagnosed from 2004 to 2014 using United States Surveillance, Epidemiology, and End Results(SEER) cancer registry data. Tumor stage was assessed by SEER-specific staging systems and whether HCC was within Milan criteria at *** logistic regression models evaluated for predictors of HCC within Milan criteria at *** Overall, Hispanics accounted for 19.8% of all HCC(73.3% men, 60.9% had Medicare or commercial insurance, 33.5% Medicaid, and 5.6% uninsured). Thirtyeight percent of Hispanic HCC patients were within Milan criteria at *** latter time periods, significantly more patients were diagnosed with HCC within Milan criteria, and in 2013-2014, 42.6% had HCC within Milan criteria. On multivariate regression, Hispanic males(OR vs females: 0.76, 95%CI: 0.68-0.83, P 65 years(OR vs age < 50: 0.67, 95%CI: 0.58-0.79, P < 0.001),and uninsured patients(OR vs Medicare/commercial: 0.49, 95%CI: 0.40-0.59, P <0.001) were significantly less likely to have HCC within Milan criteria at *** While one in five HCC patients in the United States are of Hispanic ethnicity,only 38% were within Milan criteria at time of diagnosis, and thus over 60% wereineligible for liver transplantation, one of the primary curative options for HCC patients. AIM To clarify the association of malignancy with mesenteric panniculitis-like changes on computed tomography(CT).METHODS All abdominal CT scans performed at North Shore University HealthS ystem showing mesenteric panniculitis from January 2005 to August 2010 were identified in the Radnet(Rad Net Corporation, Los Angeles, CA) database. Patients with a new or known diagnosis of a malignancy were included for this analysis. Longitudinal clinical histories were obtained from electronic medical *** In total, 147794 abdominal CT scans were performed during the study period. Three hundred and fiftynine patients had mesenteric panniculitis(MP)-like abnormalities on their abdominal CT. Of these patients, 81 patients(22.6%) had a known history of cancer at the time of their CT scan. Nineteen(5.3%) had a new diagnosis of cancer in concurrence with their CT, but the majority of these(14/19, 74%) were undergoing CT as part of a malignancy evaluation. Lymphomas were the most common cancers associated with MPlike findings on CT(36 cases, 36%), with follicular lymphoma being the most frequent subtype(17/36). A variety of solid tumors, most commonly prostate(7) and renal cell cancers(6) also were seen. CT follow up was obtained in 56 patients. Findings in the mesentery were unchanged in 45(80%), worsened in 6(11%), and improved in 5 patients(9%). Positron emission tomography(PET) scans performed in 44 patients only showed a positive uptake in the mesenteric mass in 2 patients(5%). CONCLUSION A new diagnosis of cancer is uncommon in patients with CT findings suggestive of MP. MP-like mesenteric abnormalities on CT generally remain stable in patients with associated malignancies. PET scanning is not recommended in the evaluation of patients with mesenteric panniculitis-like findings on CT.
