Meta-analysis of probiotics for the treatment of irritable bowel syndrome

Irritable bowel syndrome （IBS） is a chronic condition affecting 3%-25% of the general population. As no curative treatment is available, therapy is aimed at reducing symptoms, often with little success. Because alteration of the normal intestinal microflora has been observed in IBS, probiotics （beneficial microbes taken to improve health） may be useful in reducing symptoms. This paper systematically reviews randomized, controlled, blinded bials of probiotics for the treatment of IBS and synthesizes data on efficacy across trials of adequate quality. Pubr4ed, Medline, Google Scholar, NIH registry of clinical trials, metaRegister, and the Cochrane Central Register of Controlled Trials were searched from 1982-2007. We also conducted secondary searches of reference lists, reviews, commentaries, relevant articles on associated diseases, books and meeting abstracts. Twenty trials with 23 probiotic treatment arms and a total of 1404 subjects met inclusion criteria. Probiotic use was associated with improvement in global IBS symptoms compared to placebo [pooled relative risk （RRpooled） 0.77, 95% confidence interval （95% CI） 0.62-0.94]. Probiotics were also associated with less abdominal pain compared to placebo [RRpooled = 0.78 （0.69-0.88）]. Too few studies reported data on other IBS symptoms or on specific probiotic strains to allow estimation of a pooled RR. While our analyses suggest that probiotic use may be associated with improvement in IBS symptoms compared to placebo, these results should be interpreted with caution, given the methodological limitations of contributing studies. Probiotics warrant further study as a potential therapy for IBS.

Comprehensive studies identify motor neuron spectrum disorders including amyotrophic lateral sclerosis(ALS)as globally rising fatal disorders with the highest prevalence in aging populations,influenced by ethnicity and ancestry(GBD 2016 Motor Neuron Disease Colla borators,2018).While~10% of diagnoses involve a family history(fALS),most cases are considered sporadic(sALS).However,population-based studies suggest that even cases without a common index mutation impart heritability(Ryan et al.,2019),indicating a crucial role of rare and as yet unknown genetic denominators.

Reversibility of diabetes mellitus: Narrative review of the evidence

The global disease burden of diabetes mellitus is high. It is well-established that prediabetes is reversible but it is unclear whether diabetes is reversible once it has been diagnosed. The objective of this narrative review is to review the evidence of reversibility of diabetes me-llitus and stimulate interest in prolonged remission as a treatment target. The current evidence for bariatric surgery is stronger than intensive medical management and the evidence is stronger for type 2 diabetes pa-tients compared with type 1 diabetes patients. It is also unclear whether non obese diabetes patients would benefit from such interventions and the duration of diabetes before diabetes become irreversible. Further research is needed in this area especially with regards to the subgroup of diabetes patient who will benefit from these interventions and the long term safety and efficacy remains unknown especially with intensive me-dical management.

Diabetes mellitus continues to present a large social,financial and health system burden across the *** relationship between age of onset of the different types of diabetes and all-cause mortality is *** this review paper,the relationship between age of onset of the different types of diabetes and all-cause mortality will be reviewed and an update of the current evidence will be *** is strong evidence of the relationship between age of onset of type 2 diabetes mellitus(T2 DM) and all-cause mortality,good evidence of the relationship between age of onset of T1 DM and all-cause mortality and no evidence of the relationship between age of onset of gestational diabetes or prediabetes and all-cause *** research is needed to look at whether aggressive management of earlier onset of T2 DM can help to reduce premature mortality.

Validation of Pervasive Cloud Task Migration with Colored Petri Net

Mobile devices are resource-limited, and task migration has become an important and attractive feature of mobile clouds. To validate task migration, we propose a novel approach to the simulation of task migration in a pervasive cloud environment. Our approach is based on Colored Petri Net（CPN）. In this research, we expanded the semantics of a CPN and created two task migration models with different task migration policies： one that took account of context information and one that did not. We evaluated the two models using CPN-based simulation and analyzed their task migration accessibility, integrity during the migration process, reliability, and the stability of the pervasive cloud system after task migration. The energy consumption and costs of the two models were also investigated. Our results suggest that CPN with context sensing task migration can minimize energy consumption while preserving good overall performance.

Autonomous reconfigurable hybrid tail-sitter UAV U-Lion

We present in this work the development of a novel hybrid unmanned aircraft platform, U-Lion,which has both vertical take-off and landing(VTOL) and cruising flight capabilities. Our design is in tail-sitter structure with reconfigurable wings, which combines the advantages of a fixed-wing plane and a rotor helicopter effectively. This allows it to transit from vertical take-off to hovering, before flying in cruise mode for efficient long duration flight. The propulsion comes from two coaxial contra-rotating motors fixed on a gimbal mechanism,which can change the direction of the motors for the required thrust. This thrust-vectored propulsion system primarily provides control in the VTOL mode but also enhances flight capabilities in the cruise mode. The hybrid aircraft is equipped with GPS and airspeed sensors, and has an onboard avionic system with advanced flight control algorithms to perform fully autonomous VTOL and cruising flights, in addition to transiting effectively between VTOL and cruising flight modes. The overall design has been successfully verified by actual flight experiments.

Bayesian methods in reporting and managing Australian clinical indicators

Sustained clinical improvement is unlikely without appropriate measuring and reporting techniques. Clinical indicators are tools to help assess whether a standard of care is being met. They are used to evaluate the potential to improve the care provided by healthcare organisations(HCOs). The analysis and reporting of these indicators for the Australian Council on healthcare Standards have used a methodology which estimates, for each of the 338 clinical indicators, the gains in the system that would result from shifting the mean proportion to the 20 th centile. The results are used to provide a relative measure to help prioritise quality improvement activity within clinical areas, rather than simply focus on "poorer performing" HCOs. The method draws attention to clinical areas exhibiting larger between-HCO variation and affecting larger numbers of patients. HCOs report data in six-month periods, resulting in estimated clinical indicator proportions which may be affected by small samples and sampling variation. Failing to address such issues would result in HCOs exhibiting extremely small and large estimated proportions and inflated estimates of the potential gains in the system. This paper describes the 20 th centile method of calculating potential gains for the healthcare system by using Bayesian hierarchical models and shrinkage estimators to correct for the effects of sampling variation, and provides an example case in Emergency Medicine as well as example expert commentary from colleges based upon the reports. The application of these Bayesian methods enables all collated data to be used, irrespective of an HCO's size, and facilitates more realistic estimates of potential system gains.

Controlling chaos based on a novel intelligent integral terminal sliding mode control in a rod-type plasma torch

An integral terminal sliding mode controller is proposed in order to control chaos in a rod-type plasma torch *** this method, a new sliding surface is defined based on a combination of the conventional sliding surface in terminal sliding mode control and a nonlinear function of the integral of the system states. It is assumed that the dynamics of a chaotic system are unknown and also the system is exposed to disturbance and unstructured uncertainty. To achieve a chattering-free and high-speed response for such an unknown system, an adaptive neuro-fuzzy inference system is utilized in the next step to approximate the unknown part of the nonlinear dynamics. Then, the proposed integral terminal sliding mode controller stabilizes the approximated system based on Lyapunov＇s stability theory. In addition, a Bee algorithm is used to select the coefficients of integral terminal sliding mode controller to improve the performance of the proposed method. Simulation results demonstrate the improvement in the response speed, chattering rejection, transient response,and robustness against uncertainties.
