Comparison of retinal nerve fiber layer and macular thickness measurements with Stratus OCT and OPKO/OTI OCT devices in healthy subjects

AIM: To compare retinal nerve fiber layer(RNFL) and macular thickness measurements obtained with the Stratus optical coherence tomography(OCT) and OPKO/OTI OCT ***: Included in the study were 59 eyes of 30 participants. All measurements for each eye were done on the same day with both devices. Student’s paired t-tests were used to compare the central macular thickness and RNFL measurements of the Stratus OCT and OPKO/OTI OCT. Pearson correlation was used to assess the relationship between the devices. Coefficient of variation(COV) was calculated to assess intersession ***: Using both the Stratus OCT and OPKO/OTI OCT, respectively, the measured mean average RNFL thicknesses were 98.9±11.1 μm and 115.1±9.6 μm(P =0.001),and the measured mean central retinal thicknesses(CRT)were 196.2 ±18.8 μm and 204.5 ±21.1 μm(P <0.001).Measured by the two devices, the RNFL thickness values were correlated in all quadrants, as were the retinal thickness values except the inferior outer sector. COV for average RNFL and CRT thickness were 2.9% and 4.6%for Stratus OCT, and 2.1% and 4.2% for OPKO/OTI OCT,***: We found good reproducibility of RNFL and retina thickness measurements for both Stratus OCT and OPKO/OTI OCT devices. However, even though the two OCT systems provided statistically correlated results,the values for both RNFL and macular thickness were statistically different. RNFL and macular thickness measurements with the OPKO/OTI OCT were higher than that of the Stratus OCT; therefore, the two OCT systems cannot be used interchangeably for the measurements of RNFL and macular thickness.

Prevalence and risk factors of lens opacities in rural populations living at two different altitudes in China

AIM： To investigate the prevalence of and risk factors for lens opacities in populations living at two different altitudes in China.·METHODS： A total of 813 subjects aged ≥40y in Lhasa（Tibet Autonomous Region, China. Altitude： 3658 m） and Shaoxing（Zhejiang Province, China. Altitude： 15 m） were underwent eye examinations and interviewed in this cross-sectional study. Participants＇ lens opacities were graded according to the Lens Opacities Classification System II（LOCS II） and the types of opacities with LOCS II scores ≥2 were determined. Univariate and stepwise logistic regression were used to evaluate the associations of independent risk factors with lens opacities.· RESULTS： Lens opacities were significantly more prevalent in the high-altitude than in the low-altitude area（χ2=10.54, P 〈0.001）. Lens opacities appear to develop earlier in people living at high than at low altitude. The main types of lens opacity in Lhasa and Shaoxing were mixed（23.81%） and cortical（17.87%）,respectively. Independent risk factors associated with all lens opacities were age, ultraviolet（UV） radiation exposure,and educational level. Compared with participants aged40-49 y, the risk of lens opacities increased gradually from 2 to 85 times per 10 y [odds ratio（OR）=2.168-84.731,P 〈0.05）. The risk of lens opacities was about two times greater in participants with the highest UV exposure than in those with the lowest exposure（OR=2.606, P =0.001）.Educational level was inversely associated with lensopacities; literacy deceased the risk by about 25%compared with illiteracy（OR=0.758, P =0.041）.·CONCLUSION： Old age, higher UV exposure and lower educational level are important risk factors for the development of lens opacities. Lens opacities are more prevalent among high-altitude than low-altitude inhabitants.

Effect of biometric characteristics on biomechanical properties of the cornea in cataract patient

AIM： To determine the impact of biometric characteristics on the biomechanical properties of the human cornea using the ocular response analyzer （ORA） and standard comprehensive ophthalmic examinations before and after standard phacoemulsificaUon. METHODS： This study comprised 54 eyes with cataract with significant lens opacification in stages I or II that underwent phacoemulsification （2.8 mm incision）. Corneal hysteresis （CH）, corneal resistance factor （CRF）, Goldmann-correlated intraocular pressure （IOPg）, and corneal-compensated intraocular pressure （IOPcc） were measured by ORA preoperatively and at lmo postoperatively. Biometric characteristics were derived from corneal topography [TMS-5, anterior equivalent （EQTMS） and cylindric （CYLTMS） power], corneal tomography [Casia, anterior and posterior equivalent （EQaCASC, EQpCASIA） and cylindric （CYLaCASIA, CYLpCASIA） power], keratometry [IOLMaster, anterior equivalent （EQIOL） and cylindric （CYL,oL） power] and autorefractor [anterior equivalent （EQAR）]. Results from ORA were analyzed and correlated with those from all other examinations taken at the same time point. RESULTS： Preoperatively, CH correlated with EQpCASIA and CYLpCASIA only （P=0.001, P=0.002）. Postoperatively, IOPg and IOPcc correlated with all equivalent powers （EQTMS, EQIOL, EQAR, EQaCASIA, and EQpCASIA）（P=0.001, P=0.007, P=0.001, P= 0.015, P=0.03 for IOPg and P〈0.001, ,0=0.003, P〈0.001, P= 0.009, P=0.014 for IOPcc）. CH correlated postoperatively with EQaCASIA and EQpCASIA only （P=0.021, P=0.022）. CONCLUSION： Biometric characteristics may significantly affect biomechanical properties of the cornea in terms of CH, IOPcc and IOPg before, but even more after cataract surgery.

Management of hypotony and flat anterior chamber associated with glaucoma filtration surgery

AIMTo determine the effectiveness of pharmacological and interventional treatment of hypotony and flat anterior chamber (FAC) resulting from glaucoma filtration *** retrospectively examined the medical records of fifty-two trabeculectomy patients (52 eyes) who developed postoperative hypotony and FAC. The management and associated complications of hypotony, changing intraocular pressure (IOP) and best corrected visual acuity (BCVA) were *** the 52 patients with hypotony, 29 (56%) had a grade 1 FAC, 21 (40%) had a grade 2 FAC, and only 2 had a grade 3 FAC. There was no significant difference between the mean preoperative IOP and the mean IOP at three and six months after surgery. Thirteen eyes (25%) required antiglaucomatous medication three months after surgery. The mean BCVA at 6mo after surgery was significantly reduced as compared with the mean preoperative *** and FAC following trabeculectomy are associated with troublesome complications that require pharmacological and/or surgical treatment. Thus, close follow-up is essential for affected patients.

Colorectal cancer surveillance in inflammatory bowel disease:Practice guidelines and recent developments

Patients with long-standing inflammatory bowel disease(IBD)involving at least 1/3 of the colon are at increased risk for colorectal cancer(CRC).Advancements in CRC screening and surveillance and improved treatment of IBD has reduced CRC incidence in patients with ulcerative colitis and Crohn’s *** cases of CRC are thought to arise from dysplasia,and recent evidence suggests that the majority of dysplastic lesions in patients with IBD are visible,in part thanks to advancements in high definition colonoscopy and *** practice guidelines have supported the use of chromoendoscopy with targeted biopsies of visible lesions rather than traditional random *** are encouraged to endoscopically resect visible dysplasia and only recommend surgery when a complete resection is not *** technologies such as virtual chromoendoscopy are emerging as potential tools in CRC *** with IBD at increased risk for developing CRC should undergo surveillance colonoscopy using new approaches and techniques.

Down-regulation of protein kinase C alpha/ezrin signals in light-induced phagocytic crisis of retinal pigment epithelium cells

AIM： To investigate the roles of PKC-α/ezrin signals in phagocytosis crisis of retinal pigment epithelium（RPE） cells in light damage model. METHODS： Light induced mice RPE injury model was established by continuously irradiating cool white light at different exposure time（0, 4, 8h light intensity： 4.18×10^-6 J/cm^2）. In vitro, human ARPE-19 cells treated with the doses and intensity（1.57×10^-6 J/cm^2） of laser irradiation. Histology analysis was evaluated by hematoxylin and eosin（HE） staining. In vivo RPE phagocytosis was quantified by measuring the accumulation of photoreceptor outer segments in the sub-retinal space. In vitro RPE phagocytosis was assessed by calculating the relative fluorescence intensity of FITC-labeled microspheres in ARPE-19 cells. To further investigate the molecular mechanism, the activation of PKC-α/ezrin signal was evaluated by Western blot in vivo and in ***： HE staining revealed that the thickness of outer nuclear layer decreased significantly after 4 and 8h light exposure. By immunostaining with rhodopsin, a significant greater accumulation of photoreceptor outer segment was noticed after light injury. In vitro, light injuredRPE cells had less phagocytic activity in a dose dependent manner than that of the normal control（P〈0.01）. Western blot suggested the activation of PKC-α/ezrin signaling was down-regulated in a dose-dependent manner after light exposure. CONCLUSION： Our data suggest that light induced phagocytic crisis of RPE cells may result from the downregulation of PKC-α/ezrin signaling.

Effect of two different doses of intravitreal bevacizumab with temporal retina-sparing laser photocoagulation for retinopathy of prematurity

This study aims to compare the efficacy and safety between two different doses of intravitreal bevacizumab（IVB） injection with temporal retina-sparing laser（TRSL） photocoagulation for retinopathy of prematurity（ROP）. We retrospectively evaluated 22 eyes of ROP infants who underwent IVB combined with partial TRSL for stage 3＋ zone I or posterior zone II ROP. Laser photocoagulation was applied on the avascular retina, sparing two-discdiameter width temporal avascular area anterior to ridge. A half dose（0.625 mg） or minimal dose（0.25 mg） of IVB was conducted. Four eyes in minimal dose group were retreated with IVB and laser photocoagulation on the spared retina. Of those 4 retreated eyes, three developed preretinal hemorrhage around the ridge after the first treatment, resulting in fibrotic macular dragging. A half dose of IVB may be more effective than a minimal dose with partial TRSL for ROP. Preretinal hemorrhage may be a harbinger of poor prognosis.

Comment on concurrent removal of intravitreal lens fragments after phacoemulsification with pars plana vitrectomy prevents development of retinal detachment

I read with interest the article entitled ＂Concurrent removal of intravitreal lens fragments afterphacoemulsification with pars plana vitrectomy prevents development of retinal detachment＂ by Chalam et al . In this study, none of the patients developed retinal detachment （RD） during the one-year follow-up after concurrent removal lens fragments following phacoemulsification with pars plana vitrectomy （PPV）. The authors suggested that concurrent PPV for retained lens fragments after cataract surgery might prevent development of rhegmatogenous retinal detachment （RRD）, because early PPV prevents development of intraocular inflammation and inhibits vitreous contraction, a common cause of retinal tears and detachment.
