A lthough advanced stage aggressive non-Hodgkin'slymphomas and Hodgkin's disease are thought to be che-motherapy-responsive cancers, a considerable number of patients either relapse or never attain a remission. High-dose therapy(HDT) followed by autologous stem cell transplantation(ASCT) is often the only possibility of cure for most of these patients. However, many controversial issues still remain with respect to HDT/ASCT for lymphomas, including its role for, the optimal timing of transplantation, the best conditioning regimen and the potential use of localized radiotherapy or immunologic methods to decrease post-transplant recurrence. Recently, mainly due to the unavailability of carmustine, several novel conditioning protocols have been clinically developed, with the aim of improving the overall outcome by enhancing the anti-lymphoma effect and, at the same time, by reducing short and long-term toxicity. Furthermore, the better safety profiles of novel approaches would definitively allow patients aged more than 65-70 years to benefit from this therapeutic option. In this review, we will briefly discuss the most relevant and recent data available regarding HDT/ASCT in lymphomas.

Objective:To compare the prolactin, progesterone, luteinizing hormone, follicle stimulating hormone, estradiol, antisperm antibodies, tumor necrosis factorα, total antioxidant capacity, prostaglandin E2 and F2α between primary and secondary infertile women in ***:One hundred and two volunteers infertile women (test group), aged 20-45 years attending the infertility clinic in the University of Calabar Teaching Hospital (UCTH) were recruited. Fourteen of the women had primary infertility while 88 of them had secondary infertility. Sixty apparently healthy, age matched women served as the control group. Five millilitres of blood was collected, allowed to clot and serum was obtained from the subjects and Enzyme Linked Immunosorbent Assay method was used for prolactin, progesterone, estradiol, follicle stimulating hormone (FSH) and luteinizing hormone (LH), prostaglandin E2and F2α, human TNF-α,Chlamydia trachomatis IgG, antisperm antibodies assay, while serum total antioxidant capacity was assessed ***:The result shows on comparison that primary infertile women has a significantly higher level of progesterone than those with secondary infertility (P>0.05). There was a significant difference in the levels of prolactin and total antioxidant capacity in the primary and secondary infertility when compare to the control group atP>0.05. Their mean ages were (31.10±5.37) years and (33.10±4.91) years respectively. There was a positive correlation between TAC and FSH, TNF and anti-sperm anti-bodies in the test group ofr=0.207 andr=0.632;P>0.05 ***: These findings suggest no alterations in levels of prostaglandin F2α, TNF and anti-oxidant between primary and secondary infertile females.

AIM To collect data from joint replacement in inhibitor patients, evaluate haemostatic and patient outcomes, and analyse the *** We report our 21-year, single-centre cumulative experience of 15 joint arthroplasties in six inhibitor *** Two low responder inhibitor patients were in the early days treated with FVIII, whereas bypassing agents were used in the rest of the high responder patients. The primary haemostatic outcome was good in 8/15, fair in 4/15 and poor in 3/15 operations. The overall patient outcome, including joint health and patient satisfaction, was good in 10/15, fair 4/15 and poor in 1/15. No deep infections were observed. Cost analysis was most beneficial in low responders and in two immune-tolerized, high responder patients. In all cases, factor replacement comprised the main treatment *** Our experience supports the initial use of bypassing agents as well as preoperative immune-tolerance induction when possible. Despite the challenges of haemostasis and severe joint disease, total joint arthroplasty can reach a good outcome, even in inhibitor patients. The risk for deep infection might be smaller than previously reported. Individual planning, intense multidisciplinary teamworkand execution of operations should be centralised in a professional unit.

AIM To analyse the risk of pregnancy(a prothrombotic state) in patients with Budd-Chiari Syndrome(BCS). METHODS Retrospective study of pregnancy in women with known BCS at single center from January 2001 to December 2015. RESULTS Out of 53 females with BCS, 7 women had 16 pregnancies. Median age at diagnosis of BCS in these women was 25 years(range 21-34 years). At least one causal factor for BCS was identified in 6 women(86%). Six women had undergone radiological decompressive treatment. All patients had anticoagulation. Six fetuses were lost before 20 wk gestation in 2 women. There were 9 deliveries over 32 wk gestation and one delivery at 27 wk. All infants did well. Seven babies were born by emergency caesarean section. There were no cases of thrombosis. Two patients had notable vaginal(PV) bleeding in 3 pregnancies. None of the patients had variceal haemorrhage. Two patients were diagnosed with pulmonary hypertension, one during pregnancyand the other in the post-partum period. There was no maternal *** Maternal outcomes in patients with treated BCS are favourable and fetal outcomes beyond 20 wk gestation are good. There has been increased rate of caesarean section. Pulmonary hypertension is an important finding that needs further validation. These patients should be managed in centers experienced in treating high-risk pregnancies.

A Rare Entity of Angioimmunoblastic T-Cell Lymphoma

Introduction: Angioimmunoblastic T Cell Lymphoma (AITL) is a well-recognized subtype of peripheral T cell lymphoma. It occurs predominantly in the lymph nodes with presence of systemic symptoms and carries a dismal prognosis. AITL accounts for about 1% - 2% of all cases of non-Hodgkin lymphoma. Case presentation: A 59-year-old gentleman of Chinese ethnicity with no prior medical illness presented to Tengku Ampuan Afzan Hospital with a two-month history of fever, anorexia, unintentional weight loss and generalized lymphadenopathies. Physical examination revealed diffuse lymphadenopathies involving the cervical, axillary and inguinal regions bilaterally. He had hepatosplenomegaly. An excisional biopsy of the cervical and inguinal lymph nodes was compatible with AITL. The bone marrow biopsy demonstrated disease infiltration. He was treated with 6 cycles of Etoposide-CHOP (cyclophosphamide, doxorubicin, vincristine, prednisolone) induction chemotherapy followed by consolidation high dose therapy-autologous stem cell transplant (HDT-ASCT). He achieved complete remission on 18-Fluoro- deoxyglucose Positron Emission Tomography (18-FDG-PET) imaging. His bone marrow biopsy showed disease clearance. Conclusion: The diagnosis of AITL remains challenging and often poses a dilemma to clinicians and lymphoma pathologists. The natural history of AITL remains very variable with many of them relapsing subsequently despite achieving prior complete remission.

Objective To evaluate the application of anti-T-lymphocyte globulin (ATG) based nonmyeloablative but profoundly immunosuppressive regimens followed by donor lymphocyte infusion (DLI) for the treatment of hematologic *** The protocol was designed to minimize the intensity of the conditioning regimen to the range of nonmyeloablative therapies based on ATG with low-dose busulfan (Bu) and Cytoxan (CTX) (15-19.5 mg/kg, 8 mg/kg and 80 mg/kg, respectively). The patients received the first lymphocytic infusion from HLA-identical sibling donors on days 28-30 after transplant, and the first T cell dosage of 10 6/kg followed by the escalated dosage in the range of (0.5-1.5)×10 8/kg. The total number of procedures were performed at a median of 4.2 procedures (range of 2-8 procedures).Results Engraftment was documented in all six patients in the form of donor-recipient hematopoietic cells mixed chimera at early-stage posttransplant, which was converted gradually into complet chimera by DLI in four patients. Graft-versus-host disease (GVHD) developed in three of six cases, only one of which was severe. To date,four patients are disease free and *** Allogeneic donor stem cell engraftment into host can be achieved by nonmyeloablative conditioning regimen based on ATG. Transient mixed donor-recipient hematopoietic cell mixed with chimeras may be successfully converted into complete chimerism by DLI posttransplant. GVHD remains major clinical concern in our study.
