Efficacy and safety of Chinese herbal medicine as adjunctive therapy in sepsis patients with bloodstream infection:a propensity-matched analysis

OBJECTIVE:To investigate the efficacy and safety of Chinese herbal medicine in treating sepsis patients with bloodstream ***:A 6-year retrospective study was carried out at a university hospital in *** sepsis patients with bloodstream infection were *** primary outcome was 28-day mortality after *** score method was used to adjust for possible confounding.28-day mortality was estimated by KaplanMeier analysis and compared using the log-rank *** regression analysis was carried out to identify factors impacting in-hospital mortality ***:Following the application of the propensity score method,a total of 176 patients were *** all-cause 28-day mortality in the control group and Chinese herbal medicine group was 21.6%and 14.8%,***-Meier survival analysis showed that Chinese herbal medicine was associated with a lower hazard ratio(HR)in all-cause 28-day death compared with the control group[HR=0.44,95%CI(0.22,0.90),Ps were similar between the two groups(P>0.05).Blood-activating and stasis-eliminating herb administration was associated with reduced inhospital mortality among sepsis patients with bloodstream infection[HR=0.54,95%CI(0.34,0.94),PsIONs:Chinese herbal medicine,especially the blood-activating and stasis-eliminating herb,might have certain efficacy and safety in treating sepsis patients with bloodstream *** should prescribe blood-activating and stasis-eliminating herb in treating these two coalescent critical diseases as long as no contraindications ***,further studies are needed to validate our results.

BACKGROUND surgical resection and liver transplantation(LT)are the most effective curative options for hepatocellular carcinoma(HCC).However,few patients with huge HCC(>10 cm in diameter),especially those with portal vein tumor thrombus(PVTT),can receive these *** internal radiation therapy(sIRT)can be used as a conversion therapy for them because it has the dual benefit of shrinking tumors and increasing residual hepatic ***,in patients with huge HCC,high lung absorbed dose often prevents them from receiving *** sUMMARY A 35-year-old man was admitted because of emaciation and pain in the hepatic region for about 1 *** computed tomography scan showed a 20.2 cm×19.8 cm tumor located in the right lobe–left medial lobes with right portal vein and right hepatic vein *** the pathological type of HCC was confirmed by biopsy,two conversions were *** first one was drug-eluting bead transarterial chemoembolization plus hepatic arterial infusion chemotherapy and lenvatinib and sintilimab,converted to sIRT,and the second one was sequential sIRT with continued systemic *** tumor size significantly decreased from 20.2 cm×19.8 cm to 16.2 cm×13.8 cm,then sequentially to 7.8 cm×6.8 *** the meantime,the ratio of spared volume to total liver volume increased gradually from 34.4%to 55.7%,then to 62.9%.Furthermore,there was visualization of the portal vein,indicating regression of the tumor ***,owing to the new tumor in the left lateral lobe,the patient underwent LT instead of resection without major *** Patients with inoperable huge HCC with PVTT could be converted to sIRT first and accept surgery sequentially.

Hyperlipidemia is a major risk factor for erectile dysfunction(ED).Oxidative stress and phenotypic modulation of corpus cavernosum smooth muscle cells(CcsMCs)are the key pathological factors of ED.N-acetylcysteine(NAC)can inhibit oxidative stress;however,whether NAc can alleviate pathological variations in the corpus cavernosum and promote erectile function recovery in hyperlipidemic rats remains unclear.A hyperlipidemia model was established using 27 eight-week-old male sprague-Dawley(sD)rats fed a high-fat and high-cholesterol diet(hyperlipidemic rats,HR).In addition,9 male sD rats were fed a normal diet to serve as controls(NC).HR rats were divided into three groups:HR,HR+normal saline(Ns),and HR+NAC(n=9 for each group;Ns or NAc intraperitoneal injections were administered daily for 16 weeks).subsequently,the lipid profiles,erectile function,oxidative stress,phenotypic modulation markers of ccsMCs,and tissue histology were *** experimental results revealed that erectile function was significantly impaired in the HR and HR+Ns groups,but enhanced in the HR+NAC *** lipid levels,over-activated oxidative stress,and multi-organ lesions observed in the HR and HR+Ns groups were improved in the HR+NAC ***,the HR group showed significant phenotypic modulation of CCsMCs,which was also inhibited by NAC *** report focuses on the therapeutic effect of NAc in restoring erectile function using a hyperlipidemic rat model by preventing CcsMC phenotypic modulationand attenuating oxidativestress.

Dear Editor,Corneal endotheliitis,first reported in 1982 as an autoimmune disorder by Khodadoust and Attarzadeh[1],is now recognized to be frequently induced by viral infections,including varicella zoster virus(VZV),cytomegalovirus(CMV)and herpes simplex virus(HsV),that can cause inflammatory and immune-related injuries or direct damages to corneal endothelial cells[2-3].In cases with this type of endotheliitis of viral origin,manifestations may include conjunctival ciliary congestion or mixed congestion,corneal subepithelial bulla,localized corneal edema,keratic precipitates(KP)characteristic to the edema area,puckered posterior elastic lamina,mild anterior chamber reaction,intraocular pressure(IOP)elevation in presence of inflammation in trabecular meshwork,and endothelial decompensation[3-4].Accurate diagnosis of viral corneal endotheliitis is challenging due to its complex presentation,leading to a high misdiagnosis *** the best of our knowledge,there was no previous report of viral corneal endotheliitis presenting with corneal endothelial defects in the English literature.

Early insulin therapy is capable to achieve glycemic control and restoreβ-cell function in newly diagnosed type 2 diabetes(T2D),but its effect on cardiovascular outcomes in these patients remains *** this nationwide real-world study,we analyzed electronic health record data from 19 medical centers across China between 1 January 2000,and 26 May *** included 5424 eligible patients(mean age 56 years,2176 women/3248 men)who were diagnosed T2D within six months and did not have prior cardiovascular *** Cox regression models were used to estimate the associations of early insulin therapy(defined as the first-line therapy for at least two weeks in newly diagnosed T2D patients)with the incidence of major cardiovascular events including coronary heart disease(CHD),stroke,and hospitalization for heart failure(HF).During 17,158 persons years of observation,we documented 834 incident CHD cases,719 stroke cases,and 230 hospitalized cases for *** diagnosed T2D patients who received early insulin therapy,compared with those who did not receive such treatment,had 31%lower risk of incident stroke,and 28%lower risk of hospitalization for *** significant difference in the risk of CHD was *** found similar results when repeating the aforesaid analysis in a propensity-score matched population of 4578 patients and with inverse probability of treatment weighting *** findings suggest that early insulin therapy in newly diagnosed T2D may have cardiovascular benefits by reducing the risk of incident stroke and hospitalization for HF.

Wearable multilead ECG sensing systems using on-skin stretchable and breathable dry adhesives

Electrocardiogram(ECG)monitoring is used to diagnose cardiovascular diseases,for which wearable electronics have attracted much attention due to their lightweight,comfort,and long-term *** study developed a wearablemultilead ECG sensing system with on-skin stretchable and conductive silver(Ag)-coated fiber/silicone(AgCF-s)dry *** and normal adhesion to pigskin(0.43 and 0.20 N/cm2,respectively)was optimized by the active control of fiber density and mixing ratio,resulting in close contact in the electrode–skin *** breathableAgCF-s dry electrodewas nonallergenic after continuous fit for 24 h and can be reused/cleaned(>100 times)without loss of *** AgCF encapsulated inside silicone elastomers was overlapped to construct a dynamic network under repeated stretching(10%strain)and bending(90°)deformations,enabling small intrinsic impedance(0.3,0.1 Hz)and contact impedance variation(0.7 k)in high-frequency vibration(70 Hz).All hard/soft modules of the multilead ECG system were integrated into lightweight clothing and equipped with wireless transmission for signal *** synchronous acquisition of I–III,aVR,aVL,aVF,and V4 lead data,the multilead ECG sensing system was suitable for various scenarios,such as exercise,rest,and sleep,with extremely high signal-to-noise ratios.
