SMAD7 expression in CAR-T cells improves persistence and safety for solid tumors

Despite the tremendous progress of chimeric antigen receptor T(CAR-T)cell therapy in hematological malignancies,their application in solid tumors has been limited largely due to T-cell exhaustion in the tumor microenvironment(TME)and systemic toxicity caused by excessive cytokine *** a key regulator of the immunosuppressive TME,TGF-βpromotes cytokine synthesis via the NF-κB ***,we coexpressed SMAD7,a suppressor of TGF-βsignaling,with a HER2-targeted CAR in engineered T *** novel CAR-T cells displayed high cytolytic efficacy and were resistant to TGF-β-triggered exhaustion,which enabled sustained tumoricidal capacity after continuous antigen ***,SMAD7 substantially reduced the production of inflammatory cytokines by antigen-primed CAR-T ***,SMAD7 downregulated TGF-βreceptor I and abrogated the interplay between the TGF-βand NF-κB pathways in CAR-T *** a result,these CAR-T cells persistently inhibited tumor growth and promoted the survival of tumor-challenged mice regardless of the hostile tumor microenvironment caused by a high concentration of TGF-β.SMAD7 coexpression also enhanced CAR-T-cell infiltration and persistent activation in patient-derived tumor ***,our study demonstrated the feasibility of SMAD7 coexpression as a novel approach to improve the efficacy and safety of CAR-T-cell therapy for solid tumors.

Theranostic nanoparticles with tumor-specific enzyme-triggered size reduction and drug release to perform photothermal therapy for breast cancer treatment

Although progress has been indeed made by nanomedicines, their efficacies for cancer treatment remain low, consequently leading to failures in translation to clinic. To improve the drug delivery efficiency,nanoparticles need to change size so as to fully utilize the enhanced permeability and retention(EPR) effect of solid tumor, which is the golden principle of nanoparticles used for cancer treatment. Herein, we employed cationic small-sized red emission bovine serum albumin(BSA) protected gold nanocluster(Au NC@CBSA,21.06 nm) to both load indocyanine green(ICG) and act as imaging probe to realize theranostic. Then Au NC@CBSA-ICG was fabricated with negatively charged hyaluronic acid(HA) to form Au NC@CBSAICG@HA, which was about 200 nm to easily retain at tumor site and could be degraded by tumor-specific hyaluronidase into small nanoparticles for deep tumor penetration. The HA shell also endowed Au NC@CBSA-ICG@HA with actively targeting ability and hyaluronidase-dependent drug release. Furthermore, the quenching and recovery of fluorescence revealed the interaction between ICG and carrier, which was essential for the investigation of pharmacokinetic profiles. No matter in vitro or in vivo, Au NC@CBSAICG@HA showed markedly anti-tumor effect, and could suppress 95.0% of tumor growth on mice breast cancer model. All results demonstrated Au NC@CBSA-ICG@HA was potential for breast cancer therapy.

Minimally invasive surgery for upper gastrointestinal cancer: Our experience and review of the literature

It not only results in better cosmetic outcomes, but also reduces intraoperative blood loss and postoperative pain, leading to faster recovery; however, endoscopically enhanced anatomy and improved hemostasis via positive intracorporeal pressure generated by CO2 insufflation have not contributed to reduction in early postoperative complications or improvement in long-term outcomes. Since 1995, we have been actively using MIS for operable patients with resectable upper GI cancer and have developed stable and robust methodology in conducting totally laparoscopic gastrectomy for advanced gastric cancer and prone thoracoscopic esophagectomy for esophageal cancer using novel technology including da Vinci Surgical System (DVSS). We have recently demonstrated that use of DVSS might reduce postoperative local complications including pancreatic fistula after gastrectomy and recurrent laryngeal nerve palsy after esophagectomy. In this article, we present the current status and future perspectives on MIS for gastric and esophageal cancer based on our experience and a review of the literature.

Effect of different minimally invasive gastric cancer surgical approaches on postoperative intestinal mucosal barrier function

Background:The aim of this study was to investigate the effects of different minimally invasive surgical procedures on intestinal muco-sal barrier ***:In this study,76 patients who underwent minimally invasive gastric cancer surgery were selected,and peripheral blood was collected to test the levels of serum plasma D-lactic acid,diamine oxidase,and bacterial endotoxin before and 1 and 3 days after *** markers were compared at different time points before and after surgery to understand the recovery of the intestinal muco-sal barrier function in patients after ***:On the first postoperative day,the change in serum D-lactic acid relative to the preoperative levels was significantly(Psurgery group(4.05[-0.195,6.917 mmol/L])than in the robot-assisted surgery group(7.56[5.190,12.145 mmol/L]).Both the serum D-lactic acid and bacterial endotoxin levels were significantly higher on the first postoperative day compared with preoperative levels,and although they showed a gradual decrease by the third day,they remained significantly higher than the pre-operative levels(Psurgery group,the robotic surgery group showed larger changes in the postoperative serum D-lactic acid level,suggesting that the robotic surgery resulted in greater damage to the barrier function of the intestinal *** serum D-lactic acid and bacterial endotoxin levels were significantly higher in postoperative patients and showed a trend to gradually decrease,suggesting that the intestinal mucosal barrier function of patients after minimally invasive gastric cancer surgery is damaged and t
