Aortic dissection(AD) is a cardiovascular emergency with a high mortality rate, and the risk of death is estimated to increase by 1%-2% per hour. AD is complicated by variable clinical manifestations, and early detection and treatment can give more time for the treatment of *** is the most common cause of AD. In China, 65%-75% of patients with AD have poor blood pressure control. Patients with AD who are less than40 years of age are more likely to suffer from genetic diseases. Connective tissue disease(CTD) plays a dominant role in genetic diseases. Recently, inflammatory immunity, oxidative stress, obstructive sleep apnea-hypopnea syndrome(OSAHS), and other risk factors are being further studied. The new pathogenic factors should be explored to provide an accurate and effective prevention and treatment basis and new diagnostic methods and targets for drug therapy for AD.

Essential hypertension is a highly prevalent pathological condition that is considered as one of the most relevant cardiovascular risk factors and is an important cause of morbidity and mortality around the world. Despite the fact that mechanisms underlying hypertension are not yet fully elucidated,a large amount of evidence shows that oxidative stress plays a central role in its pathophysiology. Oxidative stress can be defined as an imbalance between oxidant agents,such as superoxide anion,and antioxidant molecules,and leads to a decrease in nitric oxide bioavailability,which is the main factor responsible for maintaining the vascular tone. Several vasoconstrictor peptides,such as angiotensin Ⅱ,endothelin-1 and urotensin Ⅱ,act through their receptors to stimulate the production of reactive oxygen species,by activating enzymes like NADPH oxidase andxanthine oxidase. The knowledge of the mechanism described above has allowed generating new therapeutic strategies against hypertension based on the use of antioxidants agents,including vitamin C and E,N-Acetylcysteine,polyphenols and selenium,among others. These substances have different therapeutic targets,but all represent antioxidant reinforcement. Several clinical trials using antioxidants have been made. The aim of the present review is to provide new insights about the key role of oxidative stress in the pathophysiology of essential hypertension and new clinical attempts to demonstrate the usefulness of antioxidant therapy in the treatment of hypertension.

Although the observed progress in the cardiovascular disease treatment, the incidence of new and recurrent coronary artery disease remains elevated and constitutes the leading cause of death in the developed countries. Three-quarters of deaths due to cardiovascular diseases could be prevented with adequate changes in lifestyle, including increased daily physical activity. New evidence confirms that there is an inverse dose-response relationship between physical activity and cardiovascular disease and mortality risk. However, participation in moderate to vigorous physical activity may not fully attenuate the independent effect of sedentary activities on increased risk for cardiovascular diseases. Physical activity also plays an important role in secondary prevention of cardiovascular diseases by reducing the impact of the disease, slowing its progress and preventing recurrence. Nonetheless, most of eligible cardiovascular patients still do not benefit from secondary prevention/cardiac rehabilitation programs. The present review draws attention to the importance of physical activity in the primary and secondary prevention of cardiovascular diseases. It also addresses the mechanisms by which physical activity and regular exercise can improve cardiovascular health and reduce the burden of the disease.

cardiovascular diseases are affected by multiple factors like genetic as well as environmental hence they reveal factorial nature. The evidences that genetic factors are susceptible for developing cardiovascular diseases come from twin studies and familial aggregation. Different ethnic populations reveal differences in the prevalence coronary artery disease(CAD) pointing towards the genetic susceptibility. With progression in molecular techniques different developments have been made to comprehend the disease physiology. Molecular markers have also assisted to recognize genes that may provide evidences to evaluate the role of genetic factors in causation of susceptibility towards CAD. Numerous studies suggest the contribution of specific "candidate genes", which correlate with various roles/pathways that are involved in the coronary heart disease. Different studies have revealed that there are large numbers of genes which are involved towards the predisposition of CAD. However, these reports are not consistent. One of the reasons could be weak contribution of genetic susceptibility of these genes. Genome wide associations show different chromosomal locations which dock, earlier unknown, genes which may attribute to CAD. In the present review different ApoAI-CⅡI-AIV gene clusters have been discussed.

Aim:To determine the tolerance and acceptance of hepatic venous pressure gradient(HVPG)measurements in patients with liver ***:This prospective international multicenter study included 271 patients with cirrhosis who were scheduled to undergo HVPG measurement between October 2019 and June *** related to the tolerance and acceptance of HVPG measurements were collected using descriptive ***:HVPG measurements were technically successful in all 271 patients,with 141(52.0%)undergoing HVPG measurement *** complication rate was 0.4%.Postoperative pain was significantly lower than preoperative expected pain(p<0.001)and intraoperative pain(p<0.001),and intraoperative pain was also significantly lower than preoperative expected pain(p=0.036).No,mild,moderate,severe,and intolerable discomfort scores were reported by 36.9%,44.6%,11.1%,6.3%,and 0.4%of these patients,respectively,during HVPG measurement and by 54.6%32.5%,11.4%,1.5%,and 0%,respectively,after HVPG *** these patients,39.5%had little understanding and 10%had no understanding of the value of HVPG measurement,with 35.1%and 4.1%regarding HVPG measurements as being of little or no help,*** patients reported that they would definitely(15.5%),probably(46.9%),or possibly(29.9%)choose to undergo additional HVPG measurements again,and 62.7%regarded the cost of the procedure as ***:HVPG measurement was safe and well‐tolerated in patients with cirrhosis,but patient education and communication are warranted to improve the acceptance of this procedure.

Effects of whole body vibration training on cardiorespiratory fitness and quality of life in patients with subacute stroke:A randomized controlled trial

Background Cardiorespiratory fitness reduction is potentially related to function/structure,and activity of the stroke patients may be associated with increased risk of cardiovascular disease. Whole body vibration(WBV)training is an efficient alternative therapy for neurological conditions. However,no study has investigated the effects of WBV training on cardiorespiratory fitness in patients with subacute stroke. This single-blinded randomized controlled trial was conducted to assess the effects WBV training on cardiorespiratory fitness and quality of life in individuals with subacute stroke. Methods Thirty-three people with subacute stroke(66.56±5.25 years)were enrolled from 87 patients initially screened(25 patients were excluded by inclusion/exclusion criteria,13 patients for other reasons,16 patients declined participation). Participants were randomly assigned to the low-intensity WBV group(LWBV),high-intensity WBV group(HWBV)and a control group(No WBV).All participants of low-and high-intensity WBV group were exercised on a vibration platform,and the control group performed the same exercise without WBV. Outcome measurements included oxygen uptake(VO2)rate and walking distance during the 6-Minute Walk Test(6 MWT),and quality of life(Short-Form 12 Health Survey,SF-12). The measurements were performed by a researcher at baseline and immediately after the 8-week WBV intervention period. Results Statistical analysis revealed a significant time effect for VO2 rate and walking distance achieved during 6 MWT,and the SF-12 physical composite score domain in all three groups after the 8-week treatment period(P0.05). Conclusions The addition of the 8-week WBV program to the leg exercise protocol is not more effective in enhancing cardiorespiratory fitness and quality of life than leg exercises alone in subacute stroke p

Cell transplantation is an attractive potential therapy for heart diseases. For example, myocardial infarction(MI) is a leading cause of mortality in many countries. Numerous medical interventions have been developed to stabilize patients with MI and, although this has increased survival rates, there is currently no clinically approved method to reverse the loss of cardiac muscle cells(cardiomyocytes) that accompanies this disease. Cell transplantation has been proposed as a method to replace cardiomyocytes, but a safe and reliable source of cardiogenic cells is required. An ideal source would be the patients' own somatic tissue cells, which could be converted into cardiogenic cells and transplanted into the site of MI. However, these are difficult to produce in large quantities and standardized protocols to produce cardiac cells would be advantageous for the research community. To achieve these research goals, small molecules represent attractive tools to control cell behavior. In this editorial, we introduce the use of small molecules in stem cell research and summarize their application to the induction of cardiogenesis in noncardiac cells. Exciting new developments in this field are discussed, which we hope will encourage cardiac stem cell biologists to further consider employing small molecules in their culture protocols.

Early detection and diagnosis of stable coronary artery disease (SCAD) is essential for proactive secondary prevention of myocardial infarction (MI), control of disease progress, and reduction of mortality. Clinical decision-making in modern medicine is increasingly dependent on cardiovascular imaging techniques. 2012 ACCF/AHA/ACP/AATS/PCNA/SCAI/STS guideline for the diagnosis and management of patients with stable ischemic heart disease has been issued by American Heart Association (AHA). European Society of cardiology (ESC) has issued 2013 ESC guidelines on the management of stable coronary artery diseased. Non-invasive imaging examination is recommended as the cornerstone for diagnosis in these guidelines. In China, despite the availability of multiple non-invasive imaging modalities such as electrocardiogram (ECG),echocardiography, radionuclide myocardial perfusion imaging (MPI), cardiovascular magnetic resonance imaging (CMR), and coronary computed tomography angiography (CCTA) in clinical practice, the utilization, clinical indication, and technical operations are not standardized. In the course of diagnosis and treatment, imaging examinations have been focused on "emphasizing structure and disregarding function; simplifying diagnosis and stressing therapy". In addition, clinicians' training and knowledge of examination techniques are limited and thus they do not master indications and contraindications of each specific modality. Moreover, current Chinese guidelines for the diagnosis and treatment of chronic stable angina pectoris and expert consensus for examination techniques have not provided standard and systematic guidance for non-invasive imaging examination pathways.

cardiovascular disease(CVD)is the number one cause of death in *** induced by gene mutations,heart disease,or hERG K+channel inhibitors is a serious CVD that can lead to sudden death or heart *** cardiomyocyte-based biosensors can record extracellular potentials and mechanical beating ***,parameter extraction and examination by the naked eye are the traditional methods for analyzing arrhythmic beats,and it is difficult to achieve automated and efficient arrhythmic recognition with these *** this work,we developed a unique automated template matching(ATM)cardiomyocyte beating model to achieve arrhythmic recognition at the single beat level with an interdigitated electrode impedance detection *** ATM model was established based on a rhythmic template with a data length that was dynamically adjusted to match the data length of the target beat by spline *** performance of the ATM model under long-term astemizole,droperidol,and sertindole treatment at different doses was *** results indicated that the ATM model based on a random rhythmic template of a signal segment obtained after astemizole treatment presented a higher recognition accuracy(100%for astemizole treatment and 99.14%for droperidol and sertindole treatment)than the ATM model based on arrhythmic *** believe this highly specific ATM method based on a cardiomyocyte beating model has the potential to be used for arrhythmia screening in the fields of cardiology and pharmacology.
