Efficacy and safety of clarithromycin for patients with sepsis or septic shock:a systematic review and meta-analysis

Background:Clarithromycin exerts an immunomodulatory role in several human ***,whether this effect improves the prognosis in patients with sepsis remains controversial,and higher levels of clinical evidence are urgently *** the best of our knowledge,no meta-analysis to date has reported the clinical efficacy and safety of clarithromycin in ***:A comprehensive literature search of PubMed,EMBASE,and the Cochrane Library was conducted up to December 31,*** randomized controlled trials comparing the clinical efficacy and safety of clarithromycin with controls among patients with sepsis or septic shock were *** were pooled by applying a fixed-effects model and a relative risk(RR)estimate with 95%confidence intervals(CIs)using Review manager(version 5.3;Cochrane Collaboration,Copenhagen,Denmark).Results:Three randomized controlled trials involving a total of 910 patients were *** pooled results confirmed that clarithromycin had no beneficial effect on progression to multiple organ dysfunction syndrome(RR:1.51;95%CI:1.02-2.25;P=0.04;I^(2)=0%),28-day mortality(RR:1.09;95%CI:0.87-1.36;P=0.46;I^(2)=0%),and 90-day mortality(RR:0.86;95%CI:0.71-1.03;P=0.10;I^(2)=81%)in patients with sepsis or septic ***,there was no difference in other serious adverse events between patients who received clarithromycin and those in the control group(RR:1.02;95%CI:0.87-1.19;P=0.83;I^(2)=18%).Conclusion:Our meta-analysis did not reveal an improvement to short-term outcomes in patients with sepsis treated with ***,administration of clarithromycin did not increase the risk of adverse events.

Influence of Sanitization Process on Maintaining Sweet Potato Quality for Export

The whole cold-chain for exporting sweet potato(native variety“Abees”),to foreign market included immediate curing operation directly after harvest helped in healing skin texture,however,in order to reduce postharvest soft rot(Rhizopus stolonifer)incidence following trimming,and washing,ultraviolet light(UV-C)treatment was used as a main sanitizer for eliminating the soft *** of the roots to UV-C(254 nm)was applied in a UV-C room on freshly harvested and cured sweet potato while rolling up on a movable line at 20 cm distance for 1,2,and 3 *** combining UV-C treatment with chlorine(200 ppm)on roots,marked and significant reduction of the total microbial load and Rhizopus potential was achieved on root surfaces respectively compared with chlorine *** also reduced soft rot percentage to almost 0%*** 3 months of cold-storage,quality assessment of sweet potato showed that root characteristics were markedly *** ability of UV-C light to induce phenylalanine ammonia lyase(PAL)enzyme activity in root tissue and maintain the activities of peroxidase and polyphenol oxidase,however with slight increase,was ***-C caused an increase of phenol content in sweet potato tissue that made an activation of defense reaction against the rot causal *** the exposure time to UV-C light increased,a higher content of phenols ***,UV-C application caused decrease in sugar content of root tissue that is flavored by soft rot-causal pathogen.

Arterial Duct Stenting Versus Modified Blalock-Taussig Shunt in Patient with Ductal-Dependent Pulmonary Circulation: Systematic Review & Meta-Analysis

Ferritin and mortality in hemodialysis patients with COVID-19:A systematic review and meta-analysis

Objective:To investigate the difference in serum ferritin levels between deceased and surviving regular hemodialysis patients with ***:We conducted a systematic search across four databases following the PRISMA statement *** reporting ferritin levels and mortality of regular hemodialysis patients with COVID-19 were *** the random-effects model,we performed a meta-analysis to determine the mean difference in serum ferritin levels between the studied groups,along with their corresponding 95%confidence *** meta-analysis was carried out using Review manager 5.4 and Stata ***:A total of 1013 patients from seven studies were included in this *** meta-analysis showed higher mean serum ferritin in the deceased compared to surviving regular hemodialysis patients with COVID-19,with a mean difference of 449.43 ng/mL[95%CI(244.07,654.80),P<0.0001;I2=58%,P=0.003].Conclusions:Our study found a higher mean of serum ferritin levels in the deceased compared to surviving regular hemodialysis patients with COVID-19.

Tailoring wind properties by various passive roughness elements in a bound- ary-layer wind tunnel

Boundary-layer wind tunnel provides a unique platform to reproduce urban, suburban and rural atmospheric boundary layer （ABL） by using roughness devices such as vortex generators, floor roughness, barrier wails, and slots in the extended test-section floor in the contraction cone. Each passive device impacts wind properties in a certain way. In this study, influence of various passive devices on wind properties has been investigated. Experi- ments using eighteen different configurations of the passive devices have been carried out to simulate urban, sub-urban, and rural climate conditions in a boundary-layer wind tunnel. The effect of each configuration on the wind characteristics is presented. It was found that higher barrier height and more number of roughness elements on the floor, generated higher turbulence and therefore higher model scale factors were obtained. However, in- creased slot width in the extended test-section floor in the contraction cone of the wind tunnel seemed to have a little effect on wind characteristics.

Clinical effect of operative vs nonoperative treatment on humeral shaft fractures:Systematic review and meta-analysis of clinical trials

BACKGROUND Whether operation is superior to non-operation for humeral shaft fracture remains *** hypothesized that operation could decrease the nonunion and reintervention rates and increase the functional *** To compare the clinical efficacy between operative and nonoperative approaches for humeral shaft *** We searched the PubMed,Web of Science,ScienceDirect,and Cochrane databases from 1990 to December 2023 for clinical trials and cohort studies comparing the effects of operative and conservative methods on humeral shaft *** investigators independently extracted data from the eligible studies,and the other two assessed the methodological quality of each *** quality of the included studies was assessed using the Cochrane risk bias or Newcastle-Ottawa *** nonunion,reintervention and the overall complications and functional scores were pooled and analyzed using Review manager software(version 5.3).RESULTS A total of four randomized control trials and 13 cohort studies were included,with 1285 and 1346 patients in the operative and nonoperative groups,*** in the operative group were treated with a plate or nail,whereas those in the conservative group were managed with splint or functional *** studies were assessed as having a high risk of bias,and the other 13 were of a low risk of bias according to the Newcastle-Ottawa Scale or Cochrane risk bias *** operative group had a significantly decreased rate of nonunion[odds ratio(OR)0.30;95%CI:0.23 to 0.40,reintervention(OR:0.33;95%CI:0.24 to 0.47),and overall complications(OR:0.62;95%CI:0.49 to 0.78)].The pooled effect of the Disabilities of Arm,Shoulder,and Hand score showed a significant difference at 3[mean difference(MD)-8.26;95%CI:-13.60 to-2.92],6(MD:-6.72;95%CI:-11.34 to-2.10),and 12 months(MD:-2.55;95%CI:-4.36 to-0.74).The pooled effect of Visual Analog Scale scores and the Constant-Murley score did not significantly differ between
