Prevalence of pulmonary embolism at autopsy among elderly patients in a Chinese general hospital

Objectives To investigate the prevalence and characteristics of pulmonary embolism （PE） at autopsy in a Chinese general hospital over a period of 10 years, and to evaluate the antemortem achievement of the prophylaxis, diagnosis, and treatment of PE. Methods All medical records of deaths from the West Branch of the Chinese People＇s Liberation Army （PLA） general hospital were retrospectively re- viewed, for the period of January 1, 2006 to December 31, 2015. Cases in which autopsies had been performed were identified and further analyzed. The numbers and detailed characteristics of patients who had PEs were noted. Prophylactic measures, along with diagnosis and treatment of PE, were recorded, if performed. Results During the 10-year period, 1057 patients died in the study hospital and 278 necrop- sies were performed （autopsy rate： 26.3%）. Nine patients were found to have PE （3.2%）, and in seven of these patients （2.5%）, the PE was considered to be fatal. Embolisms were found in the trunk and bilateral main branches of the pulmonary artery tree in all seven of the fatal PE cases. Right intracardiac thrombosis was detected in five of the nine PE patients （55.6%）. All patients with PE had been hospitalized in medical departments, and only one had undergone surgery during hospitalization. Antemortem prophylaxis was performed in two of the nine PE cases （22.2%）. None of the 9 patients had received a clinical diagnosis of PE before death. Conclusions The incidence of and death rate associated with PE may vary among different races and regions. Continuous monitoring of PE by means of necropsy in certain representative medical institutions is necessary.

Background: The Covid-19 pandemic, which is an infection due to the SARS Cov-2 virus, mainly causes severe damage to the respiratory system, but several studies have reported neurological damage with different manifestations including headaches, seizures, encephalopathies, and stroke. We report a series of 10 patients infected with Covid-19 who had an ischemic stroke at the Yaoundé General Hospital, Cameroon. Patients and Methods: We carried out a prospective study for 12 months between April 2020 and March 2021 by collecting data from patients admitted for Covid-19 at the Yaoundé general hospital. Were included all patients with a diagnosis of stroke confirmed by brain imaging (CT/MRI) following a focal neurological deficit of sudden onset and whose test (nasopharyngeal swab confirmed by PCR) for Covid-19 was positive. Patients diagnosed with a stroke more than 2 weeks (14 days) after the positive Covid-19 PCR test were excluded. Results: The prevalence of ischemic stroke in this hospital-based series was 1.46%. These were six men and four women. The mean NIHSS score was 13.57. The average age of our patients was 58 years. Brain imaging was performed in each patient revealing hypodense lesion in a large arterial territory in 8 of them, for two patients we found multilacunar infarcts. All patients had negative HIV serology and D-dimers greater than 500 μg/L. Conclusion: This work on strokes in Covid-19 infected patients shows that it is a fairly frequent neurological complication, which can occur even in young subjects without known risk factors, the symptoms of viral infection are not systematically present before the onset of stroke.

AIM： TO explore the prevalence of gallstone disease （GSD） in Taiwan and condition-associated factors related to it. METHODS： We studied a total of 2386 healthy adults （1235 males and 1151 females） voluntarily admitted to Cheng Hsin general hospital for a paid physical check-up between January 2002 and December 2002. Blood samples and ultrasound sonography results were collected. RESULTS： The overall prevalence of GSD among this study-population was 5.3%, including 1.7% （n=40） having a single stone, 2.3% （n = 55） having multiple stones, and 1.3% （n = 31） having cholecystectomy. The prevalence revealed a statistically significant increase with increasing age （P〈 0.0001）. Females exhibited a greater prevalence of multiple stones than did males （3.0% vs 1.7%, P= 0.04）. Using multiple logistic regression analysis, the following appeared to be significantly related to the prevalence of GSD： older age （40-49 years vs 〈40 years, OR= 1.63 [95% CI： 0.76-3.48], 50-59 years vs 〈40 years, OR=4.93 [95% CI： 2.43-9.99], 60-69 years vs 〈40 years, OR = 6.82 [95% CI： 3.19-14.60], ≥70 years vs 〈40 years, OR= 10.65 [95% CI： 4.78-23.73]）, higher BMI （≥27 kg/m^2 vs 〈24 kg/m^2, adjusted OR= 1.74 [95% CI： 1.04-2.88]）, and higher FPG （≥ 126 mg/dL vs 〈110 mg/dL, OR= 1.71, 95% CI： 1.01-2.96）. CONCLUSION： Older age （≥50 years）, obesity （BMI ≥ 27 kg/m^2）, and type 2 diabetes （FPG ≥126 mg/dL） are associated with the prevalence of GSD.

Treatment of Helicobacter pylori in surgical practice:A randomised trial of triple versus quadruple therapy in a rural district general hospital

AIM: To compare a lansoprazole-based triple versus quadruple therapy for Helicobacter pylori (H pylori) eradication with emphasis on side effect prof ile,patient compliance and eradication rate at a rural district general hospital in Wales,United Kingdom. METHODS: One hundred one patients with H pylori infection were included in the study. Patients were randomised to receive triple therapy comprising of lansoprazole 30 mg,amoxycillin 1 g,clarithromycin 500 mg,all b.d. (LAC),or quadruple therapy comprising of lansoprazole 30 mg b.d.,metronidazole 500 mg t.d.s.,bismuth subcitrate 240 mg b.d.,and tetracycline chloride 500 mg q.d.s. (LMBT). Cure was defi ned as a negative 13C urea breath test 2 mo after treatment. RESULTS: Seven patients were withdrawn after randomisation. Fifty patients were assigned to LAC group and 44 to LMBT group. The intention-to-treat cure rates were 92% and 91%,whereas the per-protocol cure rates were 92% and 97%,respectively. Side effects were common,with 56% experiencingmoderate to severe symptoms in the LAC group and 59% in the LMBT group. Symptoms of vomiting,diarrhoea and black stools were significantly more common in the LMBT group. Patient compliance was 100% for triple therapy and 86% for quadruple therapy (P < 0.01). One-third of patients in both groups were still taking acid-reducing medications at six-month follow-up. CONCLUSION: One-week triple and quadruple therapies have similar intention-to-treat eradication rates. Certain side effects are more common with quadruple therapy,which can compromise patient compliance. Patient education or modifi cations to the regimen are alternative options to improve compliance of the quadruple regimen.

The Quest for Understanding Cutaneous Leishmaniasis in Northern Province,Sri Lanka:An Analysis of Clinical Data From the District general hospital,Vavuniya

The study aimed to examine the clinical and epidemiological patterns of cutaneous leishmaniasis(CL)in patients attending the Dermatology Unit,District general hospital in Vavuniya,Sri Lanka.A total of 77 patients clinically suspected for CL were interviewed through a structured questionnaire,and skin-lesion samples were obtained between January 2016 and January *** definitive diagnosis of CL was made through microscopic identification of smears,histopathological examination of biopsies and kDNA *** modalities were chosen based on the location of the lesions on the body and complexity of the *** 77 suspected patients,68 were confirmed for CL,with a mean age of 34.6(±12.7)years,and included 54 males(79.4%)and 14 females(20.6%).Being a male was a significant risk factor(P=0.032,OR=4.82)associated with *** were observed mainly on the exposed areas of the body,of which the forearm(22.1%)was the most commonly affected *** lesions(75.0%)and ulcerated nodules with central crust(39.7%)were the prominent features among the infected *** age group of 21–40 years was significantly associated with healing lesions(P=0.028,marginal effect=0.55).However,a significant negative relationship was detected between ulcerated nodular lesions and lesion healing(P=0.0436,marginal effect=−0.375).Males are at higher risk of *** diagnosis and specific treatment,along with preventive measures such as protective clothing and sand fly repellents,can limit the spread of the *** is a need for a comprehensive approach to prevent and control the transmission of CL.

Breast Cancer Survival in Cameroon: Analysis of a Cohort of 404 Patients at the Yaoundégeneral hospital

This study aimed to estimate the survival rate of breast cancer in a group of patients followed up at the Yaoundé general hospital in Cameroon. A retrospective review of records of patients managed for breast cancer between 1995 and 2007 was carried out at the Yaoundé general hospital. Survival analysis was carried out with survival defined as the time between the date of unequivocal diagnosis of cancer and the date of last follow-up or death. Survival curves were plotted in R.3.1.1 software. Mean age of the patients was 47.5 ± 12.36 years. Most of the patients (67.9%) presented with advanced breast cancer disease (stage III and IV). Overall patient survival rate was 30% at 5 years and 13.2% at 10 years. Median overall survival time was 2 (1.9 - 3) years. There was a correlation between survival and the stage of disease. The highest survival rates were recorded in stages I and II while the lowest rates were recorded in stage IV. There was no statistically significant difference in survival among the age groups (p = 0.15). Overall survival rates of breast cancer are 30% at 5 years and 13.2% at 10 years among Cameroonian patients and are lower compared with 90% and 82% respectively at 5 years and 10 years in some developed countries.

The responses of nursing system to the outbreak of SARS in a general hospital

Severe Acute Respiratory Syndrome (SARS), referred to as 'contagious atypical pneumonia' is the fastest spreading communicable disease known in recent history. This disease is different from other kinds of respiratory contagion due to the following aspects: the syndrome happens quickly and the infectivity is high. New problems encountered by the nursing management staff are the precipitate, pestiferous, and large numbers of in-patients and the understanding of appropriate responses, within large complex hospitals, to ensure patient care is actively supported and infectivity is prevented. To deal with these problems, we implemented the following strategies; reinforcement of the management system; optimization of the nursing flow; amendments to related hospital policies; collection of relevant information; adherence to stringent disinfection protocols; isolation of areas; reallocation of hospital resources and more efficient utilization of resources. The above strategies have reinforced the nursing management system's active response, guaranteed daily operation of the hospital's nursing activities, including the quality of care provided by the nurses, and ensured the health of patients, nurses and doctors.

A study of anemia in women with asymptomatic malaria parasitaemia at their first antenatal care visit at the general hospital,Ikot Ekpene,Akwa Ibom State,Nigeria
