UBOM-4, a New Scale for Psychic Function and Energy: General Population Normative Values and Influencing Parameters

Objective: We introduce Utena’s Brief Objective Measures of 4 Axes for Psychic Function and Energy (UBOM-4), a new brief test battery for evaluating psychic function concerning intellect, emotion, volition, and imagination. To date, insufficient general population data on the UBOM have been collected. This study aimed to establish the normative values for the general population in UBOM-4 and examine the effects of age, sex, and mental health status on the values. Methods: The General health Questionnaire, 12-item version (GHQ-12) and UBOM-4 were administered to 149 individuals working in or attending professional schools or universities (73 males, 76 females;age: 19 - 65 years). Participants were classified as healthy based on their GHQ-12 responses and psychiatrist interviews. Results: UBOM-4 measures (mean ± SD) were: pulse rate difference (PRD or UBOM-pulse), −0.72 ± 4.31 beats/min;ruler catching time (RCT or UBOM-ruler), 18.42 ± 3.73 cm;degree of randomness (DOR or UBOM-random), 0.94 ± 0.18;mean randomizing time (MRT or UBOM-time), 0.60 ± 0.31 sec. The Baum Test drawings were classified as usual (86%) and unusual patterns (14%). Sex effects were observed for RCT (UBOM-ruler) and DOR (UBOM-random), but not for PRD (UBOM-pulse) and MRT (UBOM-time). Therefore, the normative values for RCT (UBOM-ruler) (males: 17.37 ± 3.25 cm;females: 19.42 ± 3.90 cm) and DOR (UBOM-random) (males: 0.92 ± 0.18;females: 0.95 ± 0.18) should be differentiated by sex. The patterns of the Baum Test drawings were affected by age, with individuals over 51 years showing the unusual pattern more frequently (42%). Discussion: Compared to previous results for healthy individuals, the present results were similar for RCT (UBOM-ruler), DOR (UBOM-random), MRT (UBOM-time), and the drawing pattern, but different for PRD (UBOM-pulse). Characteristics and Factors Associated with Obesity and Thinness among Children with Developmental Disorders

Obesity in childhood or adolescence has been recognized to be a risk factor for the onset of lifestyle-related diseases, not only in healthy children, but also in children with developmental disorders. Therefore, this study was conducted to examine the characteristics of obesity and thinness as assessed by the body fat percentage among children with developmental disorders during certain growth periods. It was also designed to investigate those factors associated with obesity and thinness based on a lifestyle and behavioral questionnaire. The subjects included 260 children from 5 to 18 years old with developmental disorders. The results of the study showed that a decrease in thinness and increase in obesity with ageing exhibited more noticeable trends among those children with mental retardation. The factors associated with obesity in children with developmental disorders were characterized by the dietary content, eating behaviors, and food preferences particular to such children, as well as low physical activity and a family history of obesity. The results of this study suggested the importance of continuous guidance along with family participation in order to improve obesity among children with developmental disorders, while focusing on the characteristics of certain growth periods.

Evaluating Obesity Index among Children with Developmental Disorders

The prevalence of obesity not only among adults but also among children has been increasing globally. Furthermore, obese children reportedly go on to be obese in adulthood. Obesity is likely to cause lifestyle-related diseases not only in able-bodied individuals but also in disabled children. Specific cognitive behavior observed in disabled children often hinders the provision of lifestyle guidance, such as nutritional and physical exercise instructions. To prevent such situations, early identification of obesity is required to improve lifestyle habits through diet and exercise in disabled children. This study included 285 children with developmental disorders. To assess a childhood obesity index, three obesity-related parameters were compared: the degree of obesity in school health, which has been used to evaluate the health of school children in Japan;abdominal circumference, which is useful for predicting visceral fat obesity;and the waist-to-height ratio (WHtR), which reflects visceral fat and physique. The abdominal circumference was significantly dependent on age. The degree of obesity and WHtR did not show a significant association with age. WHtR was significantly associated with the degree of obesity in school health. The WHtR is easily calculated as compared to the degree of index in school health which needs rather complicated calculations depending on age and age-specific coefficients. The study findings suggest that WHtR might be an easy-to-use obesity index comparable to the degree of obesity in school health in children with developmental disorders.

Using Cross Entropy as a Performance Metric for Quantifying Uncertainty in DNN Image Classifiers: An Application to Classification of Lung Cancer on CT Images

Cross entropy is a measure in machine learning and deep learning that assesses the difference between predicted and actual probability distributions. In this study, we propose cross entropy as a performance evaluation metric for image classifier models and apply it to the CT image classification of lung cancer. A convolutional neural network is employed as the deep neural network (DNN) image classifier, with the residual network (ResNet) 50 chosen as the DNN archi-tecture. The image data used comprise a lung CT image set. Two classification models are built from datasets with varying amounts of data, and lung cancer is categorized into four classes using 10-fold cross-validation. Furthermore, we employ t-distributed stochastic neighbor embedding to visually explain the data distribution after classification. Experimental results demonstrate that cross en-tropy is a highly useful metric for evaluating the reliability of image classifier models. It is noted that for a more comprehensive evaluation of model perfor-mance, combining with other evaluation metrics is considered essential. .

An Ethical Practice Intervention Program for Pediatric Nurses with Varied Nursing Experience

Aim: This study aimed to elucidate experience-related differences in the impact of an intervention program promoting ethical nursing practices among pediatric nurses with 0 - 5 years’ experience in varying settings. Methods: The pediatric nursing care model, defining 24 elements of pediatric nursing practice, was developed to educate nurses with pediatric nursing experience only, as well as those with experience in other settings, on ethical nursing practices toward pediatric patients and their families. Participants’ level of knowledge and frequency of application of these practices were assessed before, directly after and two months after the intervention, with a final follow-up three months post-intervention. Results: Results showed that among those who have experience in nursing areas other than pediatrics, the frequency of practicing items related to children’s rights and dignity significantly increased (p Conclusions: Participants showed an increased awareness of issues related to ethical pediatric nursing practice for patients’ and their parents.

Psychological Preparation of Children for Surgery: Awareness Survey Targeting Medical Professionals

The objective of this study was to conduct a nation-wide survey to investigate the state of: 1) the prevalence of psychological preparation of children undergoing surgery;and 2) the awareness of psychological preparation of children by medical professionals (physicians and nurses). We also aimed to identify the issues that need to be addressed next. A total of 178 physicians and 291 nurses working in hospitals where children undergo surgeries participated in this study. Anonymous self-administered questionnaire survey. Physicians and nurses in Japan are aware of the need to provide age-appropriate psychological preparation for children undergoing medical procedures. In current practice, however, our study revealed that surgical orientations are given solely to parents in the absence of the child in approximately 30% of cases. While approximately 60% of respondents felt that the uses of “children’s picture books and booklets” are good methods for delivering explanations to children, only about 20% of respondents were implementing psychological preparation specific to each age group, and half of the respondents were providing verbal explanations alone. Meanwhile, 77% responded that it is “time-consuming” and 48% said, “on the contrary, it may heighten their anxiety”. Also 34.3% said that they “do not know how to explain to a child” and 54.8% of these medical professionals worked in mixed wards. We observed a gap between ideal requirements and current practices. We found that it is essential to: 1) organize training sessions for knowledge acquisition;2) consciously strive to be informed of the actual post-surgery conditions and outcomes of the children;and 3) acquire physical and financial support.

Studying the Reinforcement Effect of a Seminar on Ethical Practices among Pediatric Nurses

Aim: This study aimed to examine the effect of a seminar to continuously reinforce ethical nursing practices among pediatric nurses. Methods: This seminar was based on a 24-point pediatric nursing care model (PNCM) that provided specific examples of basic ethical nursing practices for children undergoing medical procedures and their families. Participants’ knowledge level and application frequencies, regarding these practices, were assessed face-to-face in the first session and via email 2 and 5 months later, using the PNCM checklist and qualitative data about ethical practices. Results: Results revealed that 8 PNCM items about frequency remained the same as the first session after 5 months;however, 16 items reported increased application. The qualitative data collected 2 months later suggested that the nurses could have adapted better to implement ethical pediatric nursing;however, the one collected 5 months later confirmed the effect of the seminar on ethical nursing practices with children and their families. The continuous effect of the seminar became more evident from the responses received 5 months later than those received 2 months later. Conclusions: The findings confirmed a reinforced effect on ethical pediatric nursing practice with child patients and their parents after 5 months.
