Multiple factors are incriminated in the etiopathogeny of necrotizing enterocolitis (NEC) in premature infants, including oral feeding, vascular abnormalities, increase in pro- inflammatory cytokines, and inappropriate response of the intestinal barrier to bacterial microflora. CARD15/NOD2 is a gene recently recognized as important in the innate response to gut flora and is involved in Crohn’ s disease susceptibility. We thus tested its putative role in NEC. Ten children (seven boys and three girls) suffering from NEC who were admitted to Robert Debré hospital between 1999 and 2002 were retrospectively included in the study. Genetic screening of the 11 constant exons and the exon- intron junctions of CARD15/NOD2 by direct sequencing revealed no novel mutations of that gene in NEC patients. Furthermore, the three main mutations of CARD15/NOD2 (R702W, G908R, and 1007fs) associated with susceptibility to Crohn’ s disease were not found in these patients. Our results suggest that CARD15/NOD2 does not play a major role in genetic susceptibility to NEC.

Hepatocellular adenomas are benign tumors that can be difficult to *** refine their classification,we performed a comprehensive analysis of their genetic,pathological,and clinical features.A multicentric series of 96 liver tumors with a firm or possible diagnosis of hepatocellular adenoma was reviewed by liver *** all cases,the genes coding for hepatocyte nuclear factor 1α(HNF1α)and β-catenin were *** tumors were mutated in both HNF1αand β-catenin enabling tumors to be classified into 3 groups,according to *** with HNF1αmutations formed the most important group of adenomas(44 cases).They were phenotypically characterized by marked steatosis(P < 10-4),lack of cytological abnormalities(P < 10-6),and no inflammatory infiltrates(P < 10-4).In contrast,the group of tumors defined by β-catenin activation included 13 lesions with frequent cytological abnormalities and pseudo-glandular formation(P < 10-5).The third group of tumors without mutation was divided into two subgroups based on the presence of inflammatory *** subgroup of tumors consisting of 17 inflammatory lesions,resembled telangiectatic focal nodular hyperplasias,with frequent cytological abnormalities(P = 10-3),ductular reaction(P < 10-2),and dystrophic vessels(P =.02).In this classification,hepatocellular carcinoma associated with adenoma or borderline lesions between carcinoma and adenoma is found in 46%of the β-catenin-mutated tumors whereas they are never observed in inflammatory lesions and are rarely found in HNF1αmutated tumors(P =.004).In conclusion,the molecular and pathological classification of hepatocellular adenomas permits the identification of strong genotype-phenotype correlations and suggests that adenomas with β-catenin activation have a higher risk of malignant transformation.

Background:Populations of French Polynesia(FP),where France performed atmospheric tests between 1966 and 1974,experience a high incidence of differentiated thyroid cancer(DTC).However,up to now,no sufficiently large study of DTC genetic factors in this population has been performed to reach definitive *** research aimed to analyze the genetic factors of DTC risk among the native FP ***:We analyzed more than 300000 single nucleotide polymorphisms(SNPs)genotyped in 283 DTC cases and 418 matched controls born in FP,most being younger than 15 years old at the time of the first nuclear *** analyzed the genetic profile of our cohort to identify population *** then completed a genome-wide analysis study on the whole ***:We identified a specific genetic structure in the FP population reflecting admixture from Asian and European *** identified three regions associated with increased DTC risk at 6q24.3,10p12.2,and *** lead SNPs at these loci showed respective p-values of 1.66×10^(−7),2.39×10^(−7),and 7.19×10^(−7) and corresponding odds ratios of 2.02,1.89,and ***:Our study results suggest a role of the loci 6q24.3,10p12.2 and 17q21.32 in DTC ***,a whole genome sequencing approach would be better suited to characterize these factors than genotyping with microarray chip designed for the Caucasian ***,the functional impact of these three new loci needs to be further explored and validated.

Postoperative Analgesia and Cesarean Section under General Anesthesia: Multicenter Study

Background: Neuraxial anesthesia with intrathecal morphine is the reference technique in cesarean section anesthesia for the management of postoperative analgesia. If there is a contraindication to this, general anesthesia is required. The objective of the study was to evaluate the analgesic effectiveness of 4 analgesic techniques performed during cesarean section under general anesthesia in two centers with different anesthetic practices (North Franche Comté Hospital and Omar Bongo Ondimba Army Training Hospital). Method: This is a retrospective and descriptive study over 2 years, from January 1, 2019 to December 31, 2020. It involved evaluating the analgesic effectiveness and tolerance of morphine in the epidural catheter, wound infiltration, intravenous analgesia and Transversus Abdominous Plane block (TAP block) from the post-anesthesia care unit (PACU) until the 4th post-operative day. Results: Of the 354 cesarean sections performed, 84 (11.14%) received general anesthesia. The average age was 32.27 years. Acute fetal distress was the first indication for cesarean section (45.2%), followed by hemorrhagic placenta previa (10.7%) and prolapse of the cord (8.33%). Morphine in the epidural catheter was the most used (47.6%) followed by parietal infiltration (36.9%), intravenous analgesia (13.1%) and TAP block (2.38%). The analgesic effectiveness was comparable between the techniques from postoperative day 0 to day 4. No difference in side effects. Postoperative morphine consumption was significantly reduced (p = 0.011) in the infiltration (9 mg) and TAP block (9mg) groups compared to the epidural catheter (16 mg) and intravenous analgesia (17 mg). No difference in 02 rehabilitation criteria (ambulation, first bowel movement). No difference in the occurrence of chronic pain. Conclusion: In the event of a cesarean section under general anesthesia, there are effective and well-tolerated alternatives to neuraxial anesthesia, particularly regional anesthesia

Objective： Plasma phospholipid transfer protein （PLTP） is a key determinant of lipoprotein metabolism, and both animal and human studies converge to indicate that PLTP promotes atherogenesis and its thromboembolic complications. Moreover, it has recently been reported that PLTP modulates inflammation and immune responses. Although earlier studies from our group demonstrated that PLTP can modify macrophage activation, the implication of PLTP in the modulation of T-cell-mediated immune responses has never been investigated and was therefore addressed in the present study. Approach and results： In the present study, we demonstrated that PLTP deficiency in mice has a profound effect on CD4＋ ThO cell polarization, with a shift towards the anti-inflammatory Th2 phenotype under both normal and pathological conditions. In a model of contact hypersensitivity, a significantly impaired response to skin sensitization with the hapten-2,4-dinitrofluorobenzene （DNFB） was observed in PLTP-deficient mice compared to wild-type （WT） mice. Interestingly, PLTP deficiency in mice exerted no effect on the counts of total white blood cells, lymphocytes, granulocytes, or monocytes in the peripheral blood. Moreover, PLTP deficiency did not modify the amounts of CD4＋ and CD8＋ T lymphocyte subsets. However, PLTP-deficiency, associated with upregulation of the Th2 phenotype, was accompanied by a significant decrease in the production of the pro-Thl cytokine interleukin 18 by accessory cells. Conclusions： For the first time, this work reports a physiological role for PLTP in the polarization of CD4＋ T cells toward the pro-inflammatory Th I phenotype.

Immune checkpoint inhibitors are today an immense hope in the management of cancers. However, since their widespread use, many cases of insulin-requiring diabetes appearing suddenly, as fulminant diabetes have been reported. Here, we describe 4 cases that occurred at different times after the beginning of immune checkpoint inhibitor therapy with Nivolumab alone or associated with Ipilimumab. There are 3 cases of newly diagnosed diabetes and 1 case of known type 2 diabetes formerly quite well balanced with Metformin. The clinical and biological characteristics of these patients are quite similar. They were all insulin-requiring at the discovery of diabetes and remained so throughout their follow-up. This type of diabetes which looks like type 1 diabetes seems rather to be a new entity.

Bone loss can occur as a result of various pathologies,traumas and injuries and poor bone healing leads to functionally debilitating condition,loss of self-sufficiency and deterioration in life *** the increasing incidence of facial trauma and the emergence of new procedural techniques,advanced scaffolds are currently developed as substitutes for bone tissue *** this study,we investigated the capability of a chemically cross-linked e-caprolactone-based poly(esterurethane-urea)(PCLU)scaffold to support bone *** vitro assays demonstrated that PCLU scaffolds could be colonized by cells through direct cell seeding and cell migration from outside to scaffold ***,PCLU scaffolds could provide a suitable environment for stem cells proliferation in a 3D spatial arrangement,and allowed osteogenic differentiation under appropriate *** vivo results revealed the osteogenic properties of PCLU scaffolds through a drilled-hole femoral bone defect repair improvement in *** histology and microtomography analysis,we showed that PCLU scaffolds fit well the bone cavity and were eventually entrapped between the newly formed ***,no sign of inflammation or rejection was *** envision that PCLU scaffolds can provide the clinicians with a substitute having appropriate characteristics for the treatment of bone defects.
