AIMTo determine the sensitivity of macroscopic appearance for predicting histological diagnosis at sites other than duodenum in pediatric celiac disease (CD).METHODSEndoscopic and histologic findings in pediatric patients undergoing upper endoscopy for first-time diagnosis of CD at Stollery Children’s Hospital from 2010-2012 were retrospectively *** charts from 140 patients were reviewed. Esophageal and gastric biopsies were taken in 54.3% and 77.9% of patients, respectively. Endoscopic appearance was normal in the esophagus and stomach in 75% and 86.2%. Endoscopic esophageal diagnoses were eosinophilic esophagitis (EE) (11.8%), esophagitis (7.9%), glycogenic acanthosis (1.3%) and non-specific abnormalities (3.9%). Endoscopic gastric diagnoses were gastritis (8.3%), pancreatic rest (0.9%), and non-specific abnormalities (4.6%). Histology was normal in 76.3% of esophageal and 87.2% of gastric specimens. Abnormal esophageal histology was EE (10.5%), esophagitis (10.5%), glycogenic acanthosis (1.3%) and non-specific (1.3%). Gastritis was reported in 12.8% of specimens. Sensitivity and specificity of normal endoscopy for predicting normal esophageal histology was 86.2% and 61.1%, and for normal gastric histology was 87.4% and 21.4%.CONCLUSIONIn the absence of macroscopic abnormalities, routine esophageal and gastric biopsy during endoscopy for pediatric CD does not identify major pathologies. These findings have cost and time saving implications for clinical practice.

Aim of Investigation: Hyperpolarization-activated cyclic nucleotide-gated（HCN） channels induced an inward Na+-K+ mixed currents（Ih） which is involved in the nociceptive transmission and neuropathic pain. The expression of Ih was found in spinal lamina II（also named substantia gelatinosa, SG） neurons, especially in the GABAergic neurons which labelled green fluorescent protein（GFP） in GAD67-GFP mice. To know in detail the role of HCN channels in regulating nociceptive transmission, we investigated the contribution of presynaptic HCN channels to excitatory and inhibitory inputs of GABAergic SG neurons. Methods: Juvenile（3-5 weeks） male GAD67-GFP C57/BL6 transgenic mice were used. Whole-cell patch-clamp recordings were applied to SG neurons in transverse spinal cord slices to investigate the spontaneous excitatory and inhibitory postsynaptic currents（s EPSCs and s IPSCs）. Results: Consist with previous studies, 88.9%（n = 136/153 neurons） of recorded GFPlabelled SG neurons expressed Ih and the average amplitude was 217.9 ± 15.2 p A. Perfusion of 10 mM of ZD7288（an antagonist of HCN channel） did not affect the s EPSCs of GFP-labelled neurons（103 ± 5% of control, n = 9/9 neurons）. Neither did 50 mM of forskoin（113 ± 6% of control, n = 7/7 neurons）, an activator of adenylate cyclase. However, ZD7288 significantly decreased s IPSC frequency by 37 ± 6% but not affect their amplitude in 78%（n = 7/9 neurons） of GFP-labelled neurons. In the remaining two neurons, no detectable effects of ZD7288 on s EPSCs were found in one of them, but ZD7288 increased the s EPSC frequency by 45% in the other neuron. Consistently, forskolin dramatically increased s IPSC frequency by 64 ± 29% in 50%（n = 5/10 neurons） of GFP-labelled neurons. In the remaining five neurons, no detectable effects of forskolin on s EPSCs were found in two of them, but forskolin decreased the s EPSC frequency by 34 ± 6% in the other three neurons. Objective: T-type calcium channels are widely distributed in central and peripheral nervous system including spinal lamina II(also named substantia gelatinosa, SG) neurons. Activation of T-type channels induces an inward Ca current(I) which contribute to regulation of nociceptive transmission. In order to know in detail the cell-specific expression and function of I in SG neurons, we investigated the pharmacological characterization of I and the morphology of the SG neurons with or without I. Methods: Whole-cell patch-clamp recordings were applied to SG neurons in spinal cord slices prepared from male juvenile Sprague–Dawley rats. Neurobiotin 488 was added to the intracellular solutions to label the somata, dendrites and axon in a fraction of SG neurons recorded. Results: I was recorded in nearly 46% of the SG neurons(n = 177/384) with the average amplitude of 85.80 ± 7.49 p A. The slope of activation curve and voltage for half-maximal current(V) of I is 3.75 and-53.18 m V, respectively. The slope and V of inactivation curve was-7.31 and V =-79.69 m V, respectively. Moreover, NiCl, an antagonist of Ttype calcium channels, inhibited I in a concentration-dependent manner and the IC50 was 89.2 mM when fitted with a Hill equation(Hill coefficient = 1.7). In morphological experiments, 111 SG neurons were used for analysis. Five major cell types were identified: islet, central, radial, vertical, and unclassified cells. Among them, I was recorded in 23 neurons(21 %). Moreover, the proportion of I in each subtypes of SG neurons were: islet(35 %, n = 8/23), central(21 %, n = 5/23), radial(9 %, n = 2/23), vertical(22 %, n = 5/23), and unclassified(13%, n = 3/23) neurons. Conclusions: Most SG neurons showed a functional I that was blocked by NiCl in a concentration-dependent and reversible manner.
