Influence of preterm birth on the association between gestational diabetes mellitus and childhood developmental vulnerability:a causal mediation analysis

Background Epidemiological studies examining the direct and indirect effects of gestational diabetes mellitus(GDM)on offspring early childhood developmental vulnerability are ***,the aims of this study were to estimate the direct and indirect effects of GDM(through preterm birth)on early childhood developmental *** We conducted a retrospective population-based cohort study on the association between gestational diabetes mellitus and early childhood developmental vulnerability in children born in Western Australia(WA)using maternal,infant and birth records from the Midwives Notification,Hospitalizations,Developmental Anomalies,and the Australian Early Development Census(AEDC)*** used two aggregated outcome measures:developmentally vulnerable on at least one AEDC domain(DV1)and developmentally vulnerable on at least two AEDC domains(DV2).Causal mediation analysis was applied to estimate the natural direct(NDE),indirect(NIE),and total(TE)effects as relative risks(RR).Results In the whole cohort(n=64,356),approximately 22%were classified as DV1 and 11%as DV2 on AEDC *** of the natural direct effect suggested that children exposed to GDM were more likely to be classified as DV1(RR=1.20,95%CI:1.10-1.31)and DV2(RR=1.34,95%CI:1.19-1.50)after adjusting for potential *** 6%and 4%of the effect of GDM on early childhood developmental vulnerability was mediated by preterm birth for DV1 and DV2,*** Children exposed to gestational diabetes mellitus were more likely to be developmentally vulnerable in one or more AEDC *** biological mechanism for these associations is not well explained by mediation through preterm birth.

Objective:To correlate the utility of the Fundamentals of Laparoscopic Surgery(FLS)manual skills program with the Objective Structured Assessment of Technical Skills(OSATS)global rating scale in evaluating operative ***:The Asian Urological Surgery Training and Educational Group(AUSTEG)Laparoscopic Upper Tract Surgery Course implemented and validated the FLS program for its usage in laparoscopic surgical ***’basic laparoscopic skills were assessed using three different training models(peg transfer,precision cutting,and intra-corporeal suturing).They also performed live porcine laparoscopic surgery at the same *** surgery skills were assessed by blinded faculty using the OSATS rating ***:From March 2016 to March 2019,a total of 81 certified urologists participated in the course,with a median of 5 years of post-residency *** differences in task time did not reach statistical significance,those with more surgical experience were visibly faster at completing the peg transfer and intra-corporeal suturing FLS ***,they took longer to complete the precision cutting task than participants with less *** OSATS scores correlated weakly with all three FLS tasks(peg transfer time:r=0.331,r^(2)=0.110;precision cutting time:r=0.240,r^(2)=0.058;suturing with intracorporeal knot time:r=0.451,r^(2)=0.203).Conclusion:FLS task parameters did not correlate strongly with OSATS globing rating scale *** FLS task models demonstrated strong validity,it is important to assimilate the inconsistencies when benchmarking technical proficiency against real-life operative competence,as evaluated by FLS and OSATS,respectively.

Markers of Oocyte Quality to Enhance Human IVF Outcomes: A Bibliographic Review

The markers of oocyte quality have remained a major controversy in the field of embryology due to the subjectivity of the different methods of oocyte assessment. Various scholars use oocyte quality and oocyte competence interchangeably. Oocyte quality can be defined as the overall health of an oocyte whereas oocyte competence refers to the ability of an oocyte to be fertilized and develop into a healthy embryo. Diminished oocyte quality is believed to be a result of alterations in oocyte growth and maturation processes that stem from several pelvic and systemic factors before and after oocyte retrieval. In this review, we focus on the morphological and nonmorphological markers of oocyte quality. Strict restrictions that limit the number of oocytes fertilized in various countries have triggered researchers around the world to come up with the most appropriate and noninvasive markers that enhance oocyte selection and optimize IVF outcomes. PubMed, Google Scholar, and the Cochrane Library were used to search for peer-reviewed, original articles about oocyte quality markers. The review was written in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Morphological markers are commonly used, but they are subjective, and no single marker can be used exclusively to predict oocyte competence and subsequent embryonic development potential. Furthermore, transcriptomics of differentially expressed genes in cumulus cells and assessment of metabolomics and other contents of follicular fluid have shown greater precision. However, their specificity to the different quality determinants needs further research.

Evaluation of soil loss severity and ecological restoration approach for sustainable agriculture in the Hindu Kush, Karakoram and Himalaya region

Soil erosion has resulted in removal of the topsoils containing fine soil particles and plant nutrients, causing decrease in soil fertility in the Hindu Kush, Karakoram and Himalaya(HKH) region. The existing production of cereal crop grains has been reduced to one third of the potential crop grains production owing to land degradation and poor farming practices. It is necessary to assess risk of soil loss and identify appropriate controlling measures to address issues of low agriculture productivity and water insecurity in the region. In the present study, severity of soil loss was predicted using Revised Universal Loss Equation(RUSLE) and ecological measures were identified for sustainable mountain agriculture in the HKH region of Upper Indus Basin, Pakistan. Overall 62.6% area was found to have very low risk of soil loss, i.e., 100 t/(ha·yr) in about 6.8% and 7.4% areas respectively. The mean rate of soil loss was about 41.9 t/(ha·yr) in the Hindu Kush, 31.1 t/(ha·yr) in the Himalayas, 18.8 t/(ha·yr) in the Karakoram and overall 29.7 t/(ha·yr) in the three HKH ranges. As such no considerable measures have been adopted by the communities for restoration of the degraded areas except raising fruit/farm trees and supporting limited social forestry for their livelihoods. The slopes cleared for cultivation and susceptible to erosion may be stabilized through sowing/planting of multi-purpose plant species and formation of proper bench terraces. The conservation of forest ecosystem and pastures at higher elevations would help in reducing overland water flow, risk of flash flood hazard and minimizing sediment loads in the downstream. Site-Specific Nitrogen Management in Dry Direct-Seeded Rice Using Chlorophyll Meter and Leaf Colour Chart

The need to maintain high rice yields and improve fertilizer nitrogen(N)-use efficiency has fueled the use of tools such as leaf colour chart(LCC) and chlorophyll meter(SPAD meter) in managing fertilizer N based on colour of the leaf. Field experiments were conducted during 2011 to 2013 at Ludhiana, India to assess the need for basal N application and to establish critical threshold values of leaf greenness as measured by LCC and SPAD meter for formulating strategies for in-season management of fertilizer N in dry direct-seeded rice(DDSR). Avoiding application of N at sowing did not adversely affect rice grain yield, indicating that basal N application in DDSR was not necessary and might lead to reduced N-use efficiency. Monitoring N uptake rate during the growing season of DDSR suggested that N uptake rate peaked at the two growth stages: maximum tillering(42 to 56 days after sowing(DAS))and panicle initiation stages(70 to 84 DAS). Using the Cate-Nelson procedure, critical LCC and SPAD meter values for fertilizer N application worked out to be 4 and 37, respectively. Real-time fertilizer N management strategy based on applying 30 kg N ha-1whenever SPAD meter or LCC readings fell below the critical values maintained optimum rice yields along with higher N-use efficiency than that observed by following blanket recommendation for fertilizer N in the region. The fixed-time variable-dose strategy consisted of applying prescriptive doses of 20 kg N ha-1at 14 DAS and 30 kg N ha-1at 28 DAS and corrective doses of 30, 40 or 50 kg N ha-1at 49 and 70 DAS depending upon LCC shade to be ≥ 4, 4–3.5, or < 3.5 and SPAD meter readings to be ≥ 40, 40–35, or< 35, respectively. This strategy also resulted in optimal rice yield along with higher N-use efficiency as compared to the blanket recommendation. This study revealed that in DDSR, fertilizer N could be managed more efficiently using the tools of LCC and SPAD meter than the current blanket recommendation.

Transplanting embryonic stem cells onto damaged human corneal endothelium

AIM To investigate whether human embryonic stem cells(hESCs) could be made to attach, grow and differentiate on a human Descemet's membrane(DM).METHODS Spontaneously differentiated hESCs were transferred onto a human corneal button with the endothelial layer removed using ocular sticks. The cells were cultured on a DM for up to 15 d. The genetically engineered hESC line expressed green fluorescent protein, which facilitated identification during the culture experiments, tissue preparation, and analysis. To detect any differentiation into human corneal endothelial-like cells, we analysed the transplanted cells by immunohistochemistry using specific *** We found transplanted cells form a single layer of cells with a hexagonal shape in the periphery of the DM. The majority of the cells were negative for octamer-binding transcription factor 4 but positive for paired box 6 protein, sodium potassium adenosine triphosphatase(NaKATPase), and Zona Occludens protein 1. In four of the 18 trials, the transplanted cells were found to express CK3, which indicates that the stem cells differentiated into corneal epithelial cells in these cases. CONCLUSION It is possible to get cells originating from hESCs to become established on a human DM, where they grow and differentiate into corneal endothelial-like cells in vitro.

The impact of male overweight on semen quality and outcome of assisted reproduction

It is well-documented that male overweight and obesity causes endocrine disorders that might diminish the male reproductive capacity; however, reports have been conflicting regarding the influence of male body mass index （BMI） on semen quality and the outcome of assisted reproductive technology （ART）. The aim of this study was to investigate whether increased male BMI affects sperm quality and the outcome of assisted reproduction in couples with an overweight or obese man and a non-obese partner. Data was prospectively collected from 612 infertile couples undergoing ART at a Danish fertility center. Self-reported information on paternal height and weight were recorded and BMI was calculated. The men were divided into four BMI categories： underweight BMI 〈 20 kgm^-2, normal BMI 20-24.9 kg m^-2, overweight BMI 25-29.9 kgm^-2 and obese BMI 〉 30 kgm^-2. Conventional semen analysis was performed according to the World Health Organization guideline and sperm DNA integrity was analyzed by the Sperm Chromatin Structure Assay （SCSA）. No statistically significant effect of male BMI was seen on conventional semen parameters （sperm concentration, total sperm count, seminal volume and motility） or on SCSA-results. Bowel endometriosis: Colorectal surgeon's perspective in a multidisciplinary surgical team

Endometriosis is a gynecological condition that presents as endometrial-like tissue outside the uterus and induces a chronic inflammatory reaction. Up to 15% of women in their reproductive period are affected by this condition. Deep endometriosis is defined as endometriosis located more than 5 mm beneath the peritoneal surface. This type of endometriosis is mostly found on the uterosacral ligaments, inside the rectovaginal septum or vagina, in the rectosigmoid area, ovarian fossa, pelvic peritoneum, ureters, and bladder, causing a distortion of the pelvic anatomy. The frequency of bowel endometriosis is unknown, but in cases of bowel infiltration, about 90% are localized on the sigmoid colon or the rectum. Colorectal involvement results in alterations of bowel habits such as constipation, diarrhea, tenesmus, dyschezia, and, rarely, rectal *** diagnosis must be made in case of irritable bowel syndrome,solitary rectal ulcer syndrome,and a rectal tumor.A precise diagnosis about the presence,location,and extent of endometriosis is necessary to plan surgical *** laparoscopic treatment has become the standard of *** on the size of the lesion and site of involvement,fullthickness disc excision or bowel resection needs to be performed by an experienced colorectal *** outcomes,following bowel resection for severe endometriosis,regarding pain and recurrence rate are good with a pregnancy rate of 50%.

Prevention of Iatrogenic Cervical Cancer

Cervical carcinogenesis consists of natural occurring spontaneous cellular processes which may lead to self-organized dissipative structures of cervical cancers what was first explained in 1977 after several years of my biochemical, biophysical, hormonal and clinical studies. That was possible thanks to monograph “Biochemie der Tumoren” written in 1942 by Nobel Prize winner H. von Euler with my master Prof. B. Skarzynski. Today I express my gratitude to Nobelist Harald zur Hausen and his team for they discovered the nuclide sequences of HPV in genomes of cervical cancer cells which opened the possibility to describe the causal role of information in formula of reality. Vaccines built from the protein capsid of HPV have proved only the pathogenic information about the virus because of its lack of DNAs. All the theories of carcinogenesis have properly described this event from methodologically different point of view. The point is that one should understand the thermodynamic rules underlying each of these approaches. Neoplasms are self-organized from the cells of the patient, who did not provide the necessary conditions for cellular metabolism as defined in the moment of appearance of its zygote. In light of medical thermodynamics all oncogenic factors can divide into sufficient or necessary to events for creating a dissipathogenic cellular status. Cervical cancer is a tumor associated with the human papillomavirus as only its pathogenic dissipathogenic factors, but the genome of cervical carcinoma cells maybe the original source of many types of HPV from the peeled off cancer cells of the uterine cervix. Many things are known to increase the risk of carcinogenesis which as a natural process is an alternative of cellular or social death. Neoplasm cell is an effect of carcinogenesis, but not a causal point at which it begins its existence.
