Hepatocellular carcinoma(HCC)is the most prevalent cancer of the hepatobiliary tract and the third leading cause of cancer-related mortality *** and bevacizumab combination is currently considered among the frontline treatment modalities for advanced unresectable *** studies examining this combination were focused on evaluating its *** numerous case reports documenting some side effects,there is a limited number of large-scale studies assessing these side *** this article,we comment on the case report by Park et al published recently,reporting a fatal intra-tumoral hemorrhage in a patient with HCC who received systemic therapy in the form of the combination of atezolizumab and bevacizumab.

High-grade pancreatic intraepithelial neoplasia is a challenging diagnosis and itdoes not exhibit mass lesions. It is suspected based on changes in the mainpancreatic duct in magnetic resonance cholangiopancreatography. Sometimesonly an unclear duct shows in magnetic resonance cholangiopancreatographywith no focal strictures and upstream dilatation of the main pancreatic duct. Serialpancreatic juice cytology is valuable in diagnosis of those patients.

Characteristics and outcomes of cholangiocarcinoma by region in Thailand: A nationwide study

AIM To identify the potential risk factors of cholangiocarcinoma, we determined the characteristics of cholangiocarcinoma patients among 5 different regions of Thailand. METHODS All patients diagnosed with cholangiocarcinoma between 2008 and 2013 were identified using the Nationwide Hospital Admission Data registry(n = 39421). Baseline characteristics, comorbidities and survival were abstracted. RESULTS The annual incidence during the study period was stable in all regions. Most patients lived in the Northeast(62.8%), followed by the North(16.9%), Central(12.3%), Bangkok(5.4%), and South(n = 2.6%) regions(P < 0.0001). Significantly more cholangiocarcinoma patients had diabetes, cirrhosis, and chronic viral hepatitis B/C infection than noncholangiocarcinoma participants(diabetes: 11.42% vs 5.28%; cirrhosis: 4.81% vs 0.92%; hepatitis B: 0.74% vs 0.12%; and hepatitis C: 0.50% vs 0.10%, P < 0.0001 for all, respectively). The overall 1-year mortality rate was 81.7%, with a stable trend over time. CONCLUSION Diabetes and chronic liver diseases may be associated with cholangiocarcinoma in the Thai population.

Objective:To obtain the prevalence with clinical symptoms of Cyclospora cayetanensis(***),a coceidian protozoan parasite,in Thailand which is the cause of an intestinal infection characterized by sporadic-to-frequent explosive ***:In a field surveyconducted by the faculty of tropical *** *** part of the existing parasite-control program,a total of 2 540 faecal samples from villagers in Nan Province,Thailand,were collected and examined to determine the prevalence and clinical characteristics of parasitic ***:Twelve cases of *** infection were found duritig faecal examination of schoolchildren aged 5-12 *** exhibited obvious clinical symptoms,especially evidence of diarrhea:5 of 12 had loose faeces,one reported frequent symptoms of abdominal discomfort,and another had pale conjunctiva with low *** children were generally ***:This finding confirms a public-health issue with potentially serious consequences whereby children can be exposed to an environment contaminated with food-und water-borne transmitted oocysts,anil can hence become infected with ***.

Proton pump inhibitors(PPIs),the most commonly used antisecretory medications in the management of reflux illness,virtually eliminate elective surgery for ulcer disease,and relegate anti-reflux surgery to patients with gastroesophageal reflux disease(GERD)who are inadequately managed by medical ***,PPI medications still leave some therapeutic demands of GERD ***,up to 40%-55%of daily PPI users have chronic symptoms,due to PPI ***-competitive acid blockers(P-CABs)transcend many of the problems and limits of PPIs,delivering quick,powerful,and extended acid suppression and allowing for treatment of numerous unmet ***,it has become clear that compromised mucosal integrity plays a role in the etiology of *** a result,esophageal mucosal protection has emerged as a novel and potential treatment *** increasing body of research demonstrates that when P-CABs are used as primary drugs or add-on drugs(to regular treatment),they provide a considerable extra benefit,particularly in alleviating symptoms that do not respond to PPI therapy.

Significant progress has been achieved in the treatment of metastatic colorectal cancer(mCRC)patients during the last 20 *** are currently numerous treatments available for the first-line treatment of *** molecular technologies have been developed to reveal novel prognostic and predictive biomarkers for *** development of next-generation sequencing and wholeexome sequencing,which are strong new tools for the discovery of predictive molecular biomarkers to facilitate the delivery of customized treatment,has resulted in tremendous breakthroughs in DNA sequencing technology in recent *** appropriate adjuvant treatments for mCRC patients are determined by the tumor stage,presence of high-risk pathologic characteristics,microsatellite instability status,patient age,and performance ***,targeted therapy,and immunotherapy are the main systemic treatments for patients with *** the fact that these novel treatment choices have increased overall survival for mCRC,survival remains optimal for individuals with non-metastatic *** molecular technologies currently being used to support our ability to practice personalized medicine;the practical aspects of applying molecular biomarkers to regular clinical practice;and the evolution of chemotherapy,targeted therapy,and immunotherapy strategies for the treatment of mCRC in the front-line setting are all reviewed here.

Use of buffy coat thick films in detecting malaria parasites in patients with negative conventional thick films

Objective:To determine the frequency of malaria parasite detection from the buffy coal blood films by using capillary tube in falciparum malaria patients with negative conventional thick ***:Thirty six uncomplicated falciparum malaria patients confirmed by conventional thick and thin films were included in the *** patients were treated with artemisinin combination therapy at Hospital for tropical Diseases,Bangkok,Thailand for 28 *** for conventional blood films were conducted every 6 hours until negative parasitemia,then daily fingerpricks for parasite checks were conducted until the patients were discharged from hospital. Blood samples were also concurrently collected in 3 heparinized capillary tubes at the same time of fingerpricks for conventional blood films when the prior parasitemia was negative on thin films and parasitemia was lower than 50 parasites/200 white blood cells by thick *** first negative conventional thick films were compared with buffy coat thick films for parasite identification. Results:Out of 36 patients with thick films showing negative for asexual forms of parasites, buffy coat films could detect remaining 10 patients(27.8%) with asexual forms of Plasmodium ***:The study shows that buffy coat thick films are useful and can detect malarial parasites in 27.8%of patients whose conventional thick films show negative parasitemia.

Dengue virus(DENV) is a mosquito-transmitted virus that is expanding across the world,The incidence of dengue infection,especially severe disease,has been increasing,DENV consist of 4 serotypes of single stranded RNA viruses(D1-D4) in the genus Flavivirus,family Flaviviridae,Majority of dengue infections are asymptomatic cases,which cause difficulty in disease control and are important in dengue surveillance,There is still no gold standard to diagnose asymptomatic dengue infection,Plaque reduction neutralization test(PRNT) has been developed for many purposes such as immunological study,clinical study,vaccine trial and is currently the most sensitive and specific method for serological surveillance,However,PRNT shows some degree of cross reaction among different dengue serotypes especially secondary dengue infection cases and to other flaviviruses,Moreover,various modification since the beginning make PRNT lack of inter-laboratory standardization which is an important issue,This paper discusses the important of asymptomatic dengue infection and its diagnostic method.

Patient-specific human-induced pluripotent stem cell-derived atrial cardiomyocytes(hiPSC-aCMs) may be produced,genome-edited,and differentiated into multiple cell types for regenerative medicine,disease modeling,drug testing,toxicity screening,and three-dimensional tissue *** is presently no complete model of atrial fibrillation(AF) available for studying human pharmacological responses and evaluating the toxicity of potential medication *** has been demonstrated that hiPSC-aCMs can replicate the electrophysiological disease phenotype and genotype of *** hiPSC-aCMs,however,are immature and do not reflect the maturity of a CMs in the native *** laboratories utilize a variety of methodologies and procedures to improve and promote a CM maturation,including electrical stimulation,culture duration,biophysical signals,and changes in metabolic *** review covers the current methods being explored for use in the maturation of patient-specific hiPSC-aCMs and their application towards a personalized approach to the pharmacologic therapy of AF.
