This study aims to investigate the toxicity and possible effects and mechanism of action of the essential oil obtained from the Piper tuberculatum Jacq.‘s fruit(EOPT)on the central nervous system(CNs)of *** the Hippocratic test,it was observed ambulation reduction,sedation,piloerection and mortality of 25%at the dose of 1000 mg/kg and 50%at the dose of 5000 mg/*** results found demonstrate that the EOPT is endowed with a possible depressant effect,and is characterized by a likely sedative(open field test),anxiolytic-like(elevated plus maze test),anticonvulsant(pentylenetetrazole-induced seizure test)and hypnotic action(pentobarbital-induced sleeping time).These effects may possibly occur due to the interaction between the monoterpenes,present in its essential oil,and the GABAergic pathway confirmed by reversion of the effects by ***,these results demonstrate the therapeutic potential and the validation of the ethnopharmacological use of this species in the treatment of disorders affecting the central nervous system.

Importance of computed tomography in posterior malleolar fractures:Added information to preoperative X-ray studies

BACKGROUND Ankle fractures are common lesions of the lower *** 40%of ankle fractures affect the posterior malleolus(Pm).Historically,Pm osteosynthesis was recommended when Pm size in X-ray images was greater than 25%of the ***,computed tomography(CT)has been gaining traction in the preoperative evaluation of ankle *** To elucidate the similarity in dimensions and to correlate Pm size in X-ray images with the articular surface of the affected tibial plafond in the axial view on CT(AXCT)of a Pm *** Eighty-one patients(mean age:39.4±13.5 years)were evaluated(54.3%were male).Two independent examiners measured Pm size in profile X-ray images(PmXR)and sagittal CT(sAGCT)*** correlation of the measurements between the examiners and the difference in the Pm fragment sizes between the two images were ***,the Pm size in PmXR was compared with the surface of the tibial plafond involved in the fracture in AXCT according to the Haraguchi *** The correlation rates between the examiners were 0.93 and 0.94 for PmXR and sAGCT,respectively(Pments were 2.12%larger in sAGCT than in PmXR(P=0.018).In PmXR,there were 56 casesses≥25%.When PmXR wassponded to 10.13%of the tibial *** PmXR was≥25%,AXCT was 24.52%(Pssification,fracture types I and II had similar PmXR measurements that were greater than those of type *** analyzing AXCT,a significant difference was found between the three types,with II>I>III(PsION Pm fractures show different sizes using X-ray or CT *** showed a larger Pm in the sagittal plane and allowed the visualization of the real dimensions of the tibial plafond surface.

dermatomyositis Post COVID-19 Vaccine: A Case Report

dermatomyositis (Dm) is a rare systemic autoimmune disease characterized by symmetrical and predominantly proximal muscle weakness in the limbs, associated with typical skin lesions. Autoimmune diseases have already been described as an adverse effect of vaccines. Case-report: A 65-year-old Caucasian female patient evolved, 5 days after the first dose of the AZD1222 vaccine against the COVID-19 virus, with skin lesions suggesting heliotrope and Gottron’s sign, with partial improvement of the condition with use of topical glucocorticoids in the lesions. After two months, the patient received the second dose of AZD1222 vaccine, and evolved with proximal muscle weakness of upper and lower limbs, dysphagia, increase of muscle enzymes, in addition to skin lesions compatible with heliotrope and Gottron’s sign again. After treatment with pulse therapy with methylprednisolone 1000 mg per day for three days and treatment with immunosuppressants, she evolved with significant clinical improvement. Conclusions: This case-report demonstrating an important chronological relationship between dermatomyositis and the vaccine AZD1222 alerts to the possibility of the disease as an important adverse reaction of vaccine against COVID-19 virus, as has been recently described by other authors.

Acute pancreatitis complicated with splenic rupture: A case report

Atraumatic splenic rupture is an uncommon complication of acute pancreatitis. This report describes the case of a 30-year-old man with acute pancreatitis and splenic vein thrombosis complicated by splenic rupture. The patient was admitted to the emergency department with pain in the upper abdomen that had been present for six hours and was associated with vomiting and sweating. He was diagnosed with acute pancreatitis of alcoholic etiology. Upon computed tomography(CT) of the abdomen, the pancreatitis was scored as Balthazar C grade, and a suspicious area of necrosis affecting 30% of the pancreas with splenic vein thrombosis was revealed. seventytwo hours after admission, the patient had significant improvement in symptoms. However, he showed clinical worsening on the sixth day of hospitalization, with increasing abdominal distension and reduced hemoglobin levels. A CT angiography showed a large amount of free fluid in the abdominal cavity, along with a large splenic hematoma and contrast extravasation along the spleen artery. The patient subsequently underwent laparotomy, which showed hemoperitoneum due to rupture of the splenic parenchyma. A splenectomy was then performed, followed by ultrasound-guided percutaneous drainage.

Branch retinal vein thrombosis and visual loss probably associated with pegylated interferon therapy of chronic hepatitis C

Ophthalmological complications with interferon therapy are usually mild and reversible, not requiring the withdrawal of the treatment. We report a case of a patient who had visual loss probably associated with interferon therapy. Chronic hepatitis C virus infection （genotype la） was diagnosed in a 33-year old asymptomatic man. His past medical history was unremarkable and previous routine ophthalmologic check-up was normal. Pegylated interferon alpha and ribavirin were started. Three weeks later he reported painless reduction of vision. Ophthalmologic examination showed extensive intraretinal hemorrhages and cottonwool spots, associated with inferior branch retinal vein thrombosis. Antiviral therapy was immediately discontinued, but one year later he persists with severely decreased visual acuity. This case illustrates the possibility of unpredictable and severe complications during pegylated interferon therapy.

Epidemiological characteristics of live births with esophageal atresia in sao paulo state,Brazil,from 2005 to 2018

Background The estimated prevalence of esophageal atresia(EA)is 1 in 2500–4500 live births(LBs).Researchers have already identified risk factors,but the mechanisms are still *** aim of this study is to identify EA prevalence trends and its risk factors in the são paulo state(sPs)population *** We conducted a population-based study using all EA cases identified by the Live Births Information system across 14 years(2005–2018)to estimate EA prevalence trends in recent years,stratified by maternal age and sPs geographical *** calculated the prevalence trends,regression coefficient(β),annual percent change(APC),and 95%confidence interval(ci).Results We found 820 EA cases among 8,536,101 LBs with a prevalence of 1.0/10,000 LBs in sPs,*** was no significant difference in distribution by *** all the cases,the majority(65%)were Caucasian;51.8%were born at term;43%had weight of≥2500 g;95.4%were singleton;and 73.4%of births were by cesarean *** 2005 to 2018,there was an increasing trend of EA prevalence(APC=6.5%)with the highest APC of 12.2%.The highest EA prevalence rate(1.7/10,000 LB)was found in the group with maternal age of≥35 *** significant seasonal variation was found based on the conception month(p=0.061).Conclusions EA had an increasing prevalence trend in sPs,Brazil,in recent years,with the highest prevalence rate in the group with maternal age of≥35 *** seasonality was *** population-based study is the first to summarize the current epidemiology of EA in sPs LB.

There is still no definitive treatment for knee osteoarthritis(OA). We are certainly far from a consensus on the best form of treatment or on an effective treatment recommendation. There are reasons for the current equivocal treatment recommendations in the face of this very serious health problem. The greatest of these reasons, undoubtedly, is the great complexity of the factors involved in the development and progression of knee OA and the complex pathophysiology including mechanical, inflammatory, metabolic, post-traumatic, molecular,genetic, and psychological changes. For several years, an attempt has been made to correlate different patient phenotypes to different patterns of response to treatment, thus creating the possibility of developing specific treatments for certain groups of patients and theoretically allowing better treatment ***, in practice we still find totally different responses and evolutions even in individuals belonging to the same phenotype. Thus, classification by phenotypes, despite being an advance, is not sufficient. The present article proposes a fragmented look at each of the many factors or targets involved in the genesis and evolution of OA. Therefore, we propose not the treatment of OA per se but the management of an individual set of targets to achieve personalized OA management. We believe that, paradoxically, by fragmenting the view of the disease we will be able to treat our patients more holistically in an individualized way.

AIm:To analyze the role of rs12979860 and rs8099917polymorphisms in hepatitis C virus(HCV)genotype 1infection of ***:A total of 145 adult patients diagnosed with genotype 1 chronic hepatitis C(CHC)who had completed a 48-wk regimen of pegylated-interferonα-2a or-2b plus ribavirin combination therapy were recruited from six large urban healthcare centers and199 healthy blood donors(controls)from a single site between January 2010 and January *** on the patients’response to treatment was *** chain reaction-restriction fragment length polymorphism genotyping of the interleukin(IL)28B gene fragment encompassing the single nucleotide polymorphisms(sNPs)rs12979860(C/T)and rs8099917(T/G)was carried out for 79 of the CHC patients and 199 of the ***-directional amplicon sequencing of the two sNPs was carried out for the remaining 66 CHC ***:sNP rs12979860 genotyping was successful in 99.5%of the controls and 97.2%of the CHC patients,whereas the sNP rs8099917 genotyping was successful in 95.5%of the controls and 100%of the CHC *** genotype and allele distributions for both rs12979860 and rs8099917 were significantly different between the control and CHC patient groups,with significantly higher genotype frequencies of CC and TT in the controls(P=0.037 and 0.046,respectively)and of TT and GG in the CHC patients(P=0.0009and 0.0001,respectively).Analysis of the CHC patients who achieved sustained virological response(sVR)to treatment(n=55)indicated that the rs12979860 C allele and CC genotype were predictors of sVR(P=0.02).No significant correlation was found between rs8099917 genotypes and treatment response,but carriers of the T allele showed significantly higher rates of sVR(P=0.02).Linkage disequilibrium analysis of the group that achieved sVR showed a significant association between rs12979860 and rs8099917(P=0.07).CONCLUsION:The higher allele frequency of rs12979860 C and rs8099917 T observed in non-HCVinfec
