Pregnant Women’s Knowledge Gaps about Breastfeeding in Northern Portugal

Introduction: To explore Portuguese pregnant women’ knowledge on breastfeeding and to correlate knowledge scores with the socio-demographic variables. Methods: A cross-sectional design was used to assess 621 pregnant women’ knowledge on breastfeeding at three health centers in the North of Portugal. The face-to-face interviewing was used to collected data using a clinical instrument composed by 18 knowledge breastfeeding descriptors. The reliability of the instrument was very good (KR-20 = 0.88). descriptive and inferential statistics were used to examine pregnant women’ knowledge and the relation with socio-demographics variables. Results: Pregnant women were found to be more knowledgeable in benefits of breastfeeding. In 14 knowledge descriptors more than 60% of pregnant women revealed lack of knowledge. The most usual lack of knowledge concerned how to increase lactation and how to deal with breastfeeding complications. The pregnant women more knowledgeable were the oldest, with higher education, and with previous experience on breastfeeding. Discussion: Pregnant women revealed a significant lack of knowledge to support basic breastfeeding decisions. The focus of care should emphasize on the younger, the first-time mothers and less well-educated women as particular vulnerable and poorer prepared groups concerning breastfeeding knowledge.

The increasing number of elderly people with behavioral changes, agitation, aggressiveness, and refusal to receive care imposes new challenges to caregivers, who need to take ownership of innovative care methodologies to better take care of this group of people. Objective: To evaluate the benefits of the Humanitude Care Methodology (HCM) in order to improve the health condition of people admitted in a Portuguese Integrated Continuing Care Unit (UCCI). Methods: An action research study was conducted between September and december 2016. The sample consisted of 33 people who were admitted in this UCCI for at least 90 days. Monthly data collection, during four periods, using the Braden Scale, Morse Scale, Barthel Index and Mini Mental State Examination. The data was processed using the Statistical Package for Social Science, version 17.0. Results: The following decreases were observed: in individuals with cognitive deficit (93.94 percent to 81.82 percent);in individuals considered totally dependent (60.61 percent to 30.30 percent);at risk developing pressure ulcers (PU) (84.85 percent to 51.52 percent);in individuals who had PU (24.24 percent to 15.15 percent);in people with high risk of fall (45.45 percent to 39.39 percent);in the number of individuals with two antipsychotic prescriptions (27.27 percent to 9.09 percent). Conclusions: These results allowed to understand the positive benefits, of implementing the HCM, on the people receiving care, specifically at cognitive level, dependency level in activities of daily living, and the avoidance of physical and mental deterioration.

Epidemiological Characteristics, Resistance Patterns and Spread of Gram-Negative Bacteria Related to Colonization of Patients in Intensive Care Units

Our aim was to determine the epidemiological characteristics, the resistance patterns and the spread of Gram negative bacteria related to colonization of patients in adult Intensive Care Units. Methods: A prospective cohort of patients colonized and/or infected with Gram negative bacteria was conducted at two adult ICUs from hospitals in Brazil (April 2012 to February 2013). Nasal, groin and perineum swabs were performed. Samples were incubated on MacConkey and cetrimide agar (48 h at 37℃) and identification tests (Vitek-BioMérieux), antibiogram (Bauer-Kirby method), Carba NP test, Polymerase Chain Reaction (PCR) and sequencing were performed. The patterns of resistant microorganisms were compared by rep-PCR (Diversilab). Results: There were 53 cases of colonization. In these cases, we identified imipenem-resistant Acinetobacter baumannii (51%), Pseudomonas aeruginosa (32%), Klebsiella pneumoniae ESBL (38%) or imipenem resistant (5.6%). The use of antimicrobials and medical devices were related to colonization (p The resistance patterns expressed by Klebsiella pneumoniae were ESBL (CTX-M, SHV e TEM) and KPC2. A verified profile of Acinetobacter baumannii was related to OXA-23 and OXA-253 (OXA-143 variant). The profiles ESBL and KPC2 expressed by Klebsiella pneumoniae were distributed between the both ICUs. The distribution of OXA-23 and OXA-253 was verified only in one ICU. The similarity of strains ranged from 80% to 95%, highlighting the horizontal transference of these microorganisms.

Behavioral Risk Factors for Chronic Non-Communicable Diseases in Adults and Elderly People in the City of Ribeirão Preto—SP

Aims: to analyze the presence and simultaneity of behavioral risk factors for chronic non-communicable disease (NCD) in adults and the elderly. Methods: quantitative, cross-sectional and observational study. Healthy and unhealthy eating are classified according to the frequency of food consumption. Smoking is expressed by the percentage of smokers. Harmful alcohol consumption by the percentage of individuals who consumed alcoholic beverages at least once in the last 30 days. Classification of the level of physical inactivity required to practice at least 150 minutes of physical activity. Results: the sample constituted 719 people, 535 adults and 184 elderly. The behavioral risk factor reported by most adults 499 (93.3%) and elderly 156 (84.8%) was unhealthy eating. Frequency 72% higher of smoking, four times higher of harmful alcohol consumption and 10% higher of unhealthy food among adults when compared to the elderly. The simultaneity between risk factors was 39% greater in adults than in the elderly. Discussion: risk factors evaluated in this study are key aspects of the development of NCDs. Adults have 2.43 times the chance of presenting two risk factors and 7.73 times the chance of presenting three in relation to the elderly. Conclusion: To achieve more effective and differentiated results, knowing and directing measures to control behavioral risk factors, whether isolated or concurrent, requires specific knowledge.

Objective: To compare the learning of visually impaired individuals after the use of the educational game “Drugs: playing it clean”. Method: Quasi-experimental, comparative, before-after study. Results: The participants’ mean age in Brazil was lower than in Portugal;a significant difference in information acquisition was found between the pre and post-test for the low-complexity (Brazil p = 0.018 and Portugal p = 0.002), without a difference in the number of correct answers for the medium/high-complexity questions between the two countries (p = 0.655 and p = 0.0792);when comparing the number of correct answers before and after the game intervention, an increase was found in Brazil and Portugal, respectively (21.8% - 61.1%;11.2% - 38.9%);a significant difference was found in the number of correct answers between the low and medium/high-complexity questions (p = 0.030). Conclusion: The educational game permits information access and can be used as a teaching-learning strategy.

Chemical and microbiological characterization of tinctures and microcapsules loaded with Brazilian red propolis extract

The aim of this study was to characterize tinctures and microcapsules loaded with an ethanol extract of red propolis through chemical, physicochemical and microbiological assays in order to establish quality control tools for nutraceutical preparations of red propolis. The markers(isoflavonoids, chalcones, pterocarpans,flavones, phenolic acids, terpenes and guttiferones) present in the tinctures A and B were identified and confirmed using LC/ESI/FTMS/Orbitrap. Four compositions(A, B, C and D) were prepared to contain B tincture of the red propolis with some pharmaceutical excipients and submitted to two drying processes, i. ***-drying and freeze-drying to obtain microcapsules loaded with the red propolis extract. The tinctures and microcapsules of the red propolis were submitted to the total flavonoid content and antioxidant activity *** antibacterial activity and minimum inhibitory concentration(MIC) were tested using Staphylococcus aureus ATCC 25293 and Pseudomonas aeruginosa ATCC 27853 strains. The tinctures and microcapsules presented high flavonoid quantities from 20.50 to 40.79 mg/100 mg of the microcapsules. The antioxidant activity and IC50 were determined for the tinctures A and B(IC50: 6.95 μg/m L and 7.48 μg/m L), the spray-dried microcapsules(IC50: 8.89–15.63 μg/m L) and the freeze-dried microcapsules(IC50: 11.83–23.36 μg/m L). The tinctures and microcapsules were proved to be bioactive against gram-positive and gram-negative bacteria with inhibition halos superior to 10 mm at concentration of 200 μg/m L and MIC values of 135.87–271.74 μg/m L using gram-positive strain and 271.74–543.48 μg/m L using gram-negative strain. The tinctures and microcapsules of the red propolis have a potential application for nutraceutical products.

The influences of life habits on the cardiovascularsystem may have important implications for publichealth, as cardiovascular diseases are among the lead-ing causes of shorter life expectancy worldwide. A linkbetween excessive ethyl alcohol(ethanol) consumptionand arterial hypertension was first suggested early lastcentury. Since then, this proposition has received con-siderable attention. Support for the concept of ethanolas a cause of hypertension derives from several epi-demiologic studies demonstrating that in the generalpopulation, increased blood pressure is significantlycorrelated with ethanol consumption. Although thelink between ethanol consumption and hypertension iswell established, the mechanism through which etha-nol increases blood pressure remains elusive. Possiblemechanisms underlying ethanol-induced hypertensionwere proposed based on clinical and experimental ob-servations. These mechanisms include an increase insympathetic nervous system activity, stimulation of therenin-angiotensin-aldosterone system, an increase of intracellular Ca2+ in vascular smooth muscle, increased oxidative stress and endothelial dysfunction. The pres-ent report reviews the relationship between ethanol intake and hypertension and highlights some mecha-nisms underlying this response. These issues are of interest for the public health, as ethanol consumption contributes to blood pressure elevation in the population.

Mixtures of cereals and legumes result in products with higher content of high biological value proteins. The thermoplastic extrusion promotes the transformation of chemical, physical and nutritional characteristics of the food such as starch gelatinization and promotes the inactivation of anti-nutritional compounds and enzymes. This study aimed to develop products extruded using broken rice and lupine and to evaluate the physical and chemical characteristics of those products. The expansion index (EI), water solubility (WSI), water absorption (WAI), texture and color were determined for the extruded rice, and Scanning Electron Microscopy (SEM) of the extruded rice was also performed. As the rice concentration in the mixture increased, the EI, WSI and WAI values as well as the a* color parameter were increased too, and the brightness was reduced. An increase in the levels of essential amino acids with the addition of lupine grain to the extrudates was observed. SEM analysis identified intact loose starch granules or pellets in the raw materials, and the extruded samples showed a compact and amorphous shape without apparent starch granules. Due to the structural changes and reduced expansion rate of the extruded mixture prepared with lupine and rice grits, use of the extruded mixtures as snacks would not be suitable. However, these mixtures could be used in pre-gelatinized flour to formulate various products to increase the protein level.
