Level of Stress at Nurses Working in emergency clinic and Central Intensive Care: University clinical Centre of Kosovo

Background: emergency clinic (EC) and Central Intensive Care (CIC) are high-stress environments that directly affect the health status of nurses. Methods: The aim of this study was to assess the level of stress at nurses working in the emergency clinic and nurses working in the Central Intensive Care. The study included 90 nurses, 45 nurses working in emergency clinic and 45 nurses working in Central Intensive Care. The study applied the emergency Nurse Stress Questionnaire as an instrument to gather the data. This questionnaire was adopted from the Operational Police Stress Questionnaire in order to serve for the function and aim of the present study. Results: Nurses’ cohort-age ranged from 20 to 62 years. The largest proportion of respondents 40 (44.4%) was in the 20 - 30 age group, 58 (64.4%) were married, 60 (66.6%) hold bachelor degree and (33.3%) were with secondary school educational level. In terms of work-related fatigue, significant difference was found in working hours, participants reported that there was a significant difference in fatigue between 12 hours shift nurses (61 ± 10.5) compared to 8 hours nurses (41 ± 23.6) with P Conclusion: These findings emphasize the role of using and assuring adequate strategies for ensuring quality management and finding ways of facilitating the increase in the number of nursing staff in these two departments because workplace overload and fatigue are potential factors that increase nurses’ stress levels.

Helicobacter pylori:a foodborne pathogen?

Helicobacter pylori(H. pylori) is an organism that is widespread in the human population and is sometimes responsible for some of the most common chronic clinical disorders of the upper gastrointestinal tract in humans, such as chronic-active gastritis, duodenal and gastric ulcer disease, low-grade B-cell mucosa associated lymphoid tissue lymphoma of the stomach, and gastric adenocarcinoma, which is the third leading cause of cancer death worldwide. The routes of infection have not yet been firmly established, and different routes of transmission have been suggested, although the most commonly accepted hypothesis is that infection takes place through the faecal-oral route and that contaminated water and foods might play an important role in transmission of the microorganism to humans. Furthermore, several authors have considered H. pylori to be a foodborne pathogen because of some of its microbiological and epidemiological characteristics. H. pylori has been detected in drinking water, seawater, vegetables and foods of animal origin. H. pylori survives in complex foodstuffs such as milk, vegetables and ready-to-eat foods. This review article presents an overview of the present knowledge on the microbiological aspects in terms of phenotypic characteristics and growth requirements of H. pylori, focusing on the potential role that foodstuffs and water may play in the transmission of the pathogen to humans and the methods successfully used for the detection of this microorganism in foodstuffs and water.

Esophageal inserted subclavian catheter:a rare complication

Central venous catheterization(CVC) is an invasive procedure for administering fluids,nutrients,and drugs;monitoring central venous pressure;performing pulmonary artery catheterization;and placing transvenous pacemakers in intensive care units and all specialties,from anesthesia to emergency medicine,for the treatment of trauma and hemodynamically unstable pediatric and adult patients.[1,2]Complications have been observed in more than 15% of patients who underwent ***,infectious,and thrombotic complications have been reported in 5%–19%,5%–26%,and 2%–26% of patients,respectively.[3] Malposition,on the other hand,is common in subclavian catheter insertion and is usually associated with an initially misplaced guidewire.[4]

Dear editor,Anaphylaxis and acute coronary syndromes(ACS)are discrete clinical presentations that usually present independently and are commonly treated in the emergency departments(EDs).[1]Kounis syndrome(KS)is a rare condition characterized by the coexistence of allergic reactions and *** was firstly described in detail by Kounis and Zavras in 1991 as an allergic angina syndrome.[2]The primary pathophysiological mechanism of KS is believed to be coronary artery vasospasm secondary to increased levels of infl ammatory mediators such as histamine,tryptase,platelet-activating factor,and various cytokines.[3]These inflammatory mediators are released through mast cell activation in response to a range of stimuli during episodes of allergic or hypersensitivity reactions.

Use of short stems in revision of standard femoral stem:A case report

BACKGROUND Short stems are usually uncemented prosthetics and are recommended in the treatment of traumatic or degenerative diseases of *** revision procedures for elderly patients with serious comorbidity,applying a short stem could reduce peri-and post-operative secondary surgical risks to femoral osteotomy,which are necessary for the removal of parts of the implant or acrylic cement from the medullary *** are no cases in the literature that apply a short stem for prosthetic revision by acrylic cement *** SUMMARY A male patient had a left hip replacement in 1995 due to *** the age of eighty the patient reported an accidental trauma and walked with pain in the left *** X-ray highlighted the stem breakage in the distal section without fracturing the femoral *** patient had various comorbidities(diabetes,anaemia,heart deficiency,and arrhythmia)presenting a high operation risk(ASA 4).During the revision procedure,the distal apex of the stem could not be removed from the femoral *** of the poor general health of the patient,the surgeon decided not to perform a Wagner femoral osteotomy to remove the distal section of the stem and decided to implant a short stem to avoid removing the stem section of the previous *** patient had his left femur X-rayed 15 d *** A field of application of short stem may be the development of a cemented short stem to reduce the complexity of the revision procedure.

BACKGROUND Secondary hypertension is a relatively rare condition most commonly caused by renovascular disease due to atherosclerotic vascular disease or fibromuscular *** accessory renal arteries are frequent,to date,only six cases of secondary hypertension determined by their existence have been *** SUMMARY We describe a case of a 39-year-old female who came to the emergency department with an urgent hypertensive crisis and hypertensive *** normal renal arteries,the computed tomography angiography revealed an inferior polar artery with 50%stenosis of its *** treatment with amlodipine,indapamide and perindopril was adopted,leading to blood pressure control within one *** To the best of our knowledge,there are controversies regarding accessory renal arteries as a potential etiology for secondary hypertension,but the seven similar cases already described,along with the current case,could reinforce the necessity of more studies concerning this subject.

BACKGROUND Adrenocortical carcinoma(ACC),the second most aggressive malignant tumor,lacks epidemiological data worldwide;therefore,every new case can improve the understanding of the pathology and treatment of this *** SUMMARY We present the case of a 66-year-old Caucasian woman with a giant androgenproducing ACC(21 cm×17 cm×12 cm;2100 g),without metastases,which unusually presented with an acute onset of atrial flutter and congestive heart *** cardiac complications observed in our case support the hypothesis that androgen excess in women is a cardiovascular risk *** excess in women can be a rare cause of reversible dilated cardiomyopathy,therefore a comprehensive approach to the patient is essential to improve the recognition of androgen-secreting *** atrial flutter was remitted after initiation of drug treatment during *** severe heart failure was totally remitted at 6 mo after radical open surgery to remove the giant *** Radical open surgery to remove a giant androgen-producing ACC was the firstline treatment to cure the excess of androgen,which determined the total remission of cardiac complications at 6 mo after surgery in the women of this case report.
