Proof-of-Concept and Feasibility Study to Evaluate the Effect of β-Glucan on Protective Qi Deficiency in Adults

Objective:To preliminarily explore the potential effect ofβ-glucan on Chinese medicine(CM)concept protective qi deficiency(PQD),and the methodology for future definitive ***:To have a standardized assessment of PQD,a list of 13 potentially PQD-relevant parameters were firstly created,each with defined quantitative or categorial *** the data from 37 participants with(21 cases)or without(16 cases)PQD,multivariate logistic modeling was conducted to create a preliminary diagnostic PQD risk ***,21 participants diagnosed with PQD were treated withβ-glucan in a dose of 200 mg/day for 8 *** were collected for trial acceptability measures(rate of recruitment,withdrawal,and compliance),and the participants were assessed for PQD status at baseline and every 2 weeks ***:The preliminary logistic model consisted of 3 parameters(low voice and apathy,aversion to wind and cold,and Cun pulse).The resulting risk score demonstrated a degree of PQD-predicting accuracy that,as evaluated by statistical(discrimination and classification)methods,was higher than those obtained from any of the individual candidate *** 21 PQD participants treated withβ-glucan demonstrated good receptibility and a time-dependent improvement in PQD status as evidenced by the decrease of PQD participant to 9.5%at the end of ***:This study demonstrated the effect of proof-of-concept ofβ-glucan on improving PQD and the proof-of-concept of a multivariate-model-derived diagnostic PQD risk *** also indicated feasibility for future definitive *** like this embody an innovative approach that uses therapies derived from the mainstream biomedicine to enrich therapeutics guided by CM principle.(Trial registration ***03829228).

β-Glucan Improves Protective Qi Status in Adults with Protective Qi Deficiency——A Randomized,Placebo-Controlled,and Double-Blinded Trial

Foreign body ingestion encompasses both foreign object ingestion(FOI) and esophageal food impaction(EFI) and represents a common and clinically significant scenario among patients of all ages. The immediate risk to the patient ranges from negligible to life-threatening, depending on the ingested substance,its location, patient fitness, and time to appropriate therapy. This article reviews the FOI and EFI literature and highlights important considerations and implications for pediatric and adult patients as well as their providers. Where published literature is insufficient to provide evidence-based guidance, expert opinion is included to supplement the content of this comprehensive review.

Narrow band imaging evaluation of duodenal villi in patients with and without celiac disease:A prospective study

BACKGROUND Duodenal biopsies are commonly obtained during esophagogastroduodenoscopy(EGD) but are very often histopathologically normal. Therefore, a more strategic method for evaluating the duodenal mucosa and avoiding unnecessary biopsies is *** To examine the clinical utility of narrow band imaging(NBI) for evaluating duodenal villous *** We performed a prospective cohort study of adult patients at Mayo clinic Rochester from 2013-2014 who were referred for EGD with duodenal biopsies. A staff endoscopist scored, in real-time, the NBI-based appearance of duodenal villi into one of three categories(normal, partial villous atrophy, or complete villous atrophy), captured ≥ 2 representative duodenal NBI images, and obtained mucosal biopsies therein. Images were then scored by an advanced endoscopist and gastroenterology fellow, and biopsies(gold standard) by a pathologist, in a masked fashion using the same three-category classification. Performing endoscopist, advanced endoscopist, and fellow NBI scores were compared to histopathology to calculate performance characteristics [sensitivity, specificity,positive and negative, negative predictive value(NPV), and accuracy]. Inter-rater agreement was assessed with Cohen's ***112 patients were included. The most common referring indications were dyspepsia(47%), nausea(23%), and suspected celiac disease(14%). Duodenal histopathology scores were: 84% normal, 11% partial atrophy, and 5% complete atrophy. Performing endoscopist NBI scores were 79% normal, 14% partial atrophy, and 6% complete atrophy compared to 91%, 5%, and 4% and 70%, 24%,and 6% for advanced endoscopist and fellow, respectively. NBI performed favorably for all raters, with a notably high(92%-100%) NPV. Immunomodulatory properties of dental tissue-derived mesenchymal stem cells: Implication in disease and tissue regeneration

Introduction: Leukocoria, a whitish pupillary reflection, is a common warning sign of various pediatric ocular pathologies, including Persistent hyperplastic primary vitreous (PHVP). This case report describes the observation of a 3-year-old child with melanoderma, presenting with a white spot in the left eye since birth. Observation: Uncorrected distance visual acuity was 2/10 in the right eye and hand movements were perceived at 2 meters in the left eye. Examination of the anterior segment in the left eye revealed leukocoria and microphthalmia. The fundus examination was normal in the right eye but revealed a white mass extending from the center of the optic disc towards the temporal region in the left eye. Ocular imaging, including ocular ultrasound and optical coherence tomography, confirmed the diagnosis of PHVP, illustrated by a residual white mass at the center of the optic disc in the left eye. Conclusion: PHVP should be considered in the presence of leukocoria in a child, with urgent exclusion of retinoblastoma. This observation underscores the importance of early diagnosis for appropriate management. .

Introduction: Morning Glory Syndrome (MGS) is a rare anomaly of the optic nerve characterized by a flower-shaped excavation in the optic disc. This anatomical peculiarity is of growing interest due to its impact on vision and the therapeutic challenges it presents. Despite research advances, gaps persist, requiring thorough exploration to better understand this rare condition. We present the case of a young girl. Observation: A 7-year-old girl presented with persistent deterioration of vision in the right eye since birth. Visual acuity was counting fingers at 2 meters in the right eye and 10/10 in the left eye. Biomicroscopy was normal in both eyes, and fundus examination revealed a funnel-shaped excavation at the optic disc of the right eye, associated with central glial proliferation, thin blood vessels in a radial distribution, and atrophy of the neuroretinal rim. Additional tests confirmed the diagnosis of isolated unilateral Morning Glory Syndrome in the right eye. Combined treatment included appropriate optical correction and functional rehabilitation. Regular ophthalmological follow-up over a one-year period was established to monitor visual acuity evolution, optic disc stability, and detect any potential complications. The reserved prognosis was marked by the absence of ocular complications and maintenance of initial visual acuity in the right eye. Conclusion: The positive response to management underscores the importance of an integrated approach, early diagnosis, therapeutic adherence, and personalized strategies.

Objective Previous studies have shown that meteorological factors may increase COVID-19 mortality,likely due to the increased transmission of the ***,this could also be related to an increased infection fatality rate(IFR).We investigated the association between meteorological factors(temperature,humidity,solar irradiance,pressure,wind,precipitation,cloud coverage)and IFR across Spanish provinces(n=52)during the first wave of the pandemic(weeks 10–16 of 2020).Methods We estimated IFR as excess deaths(the gap between observed and expected deaths,considering COVID-19-unrelated deaths prevented by lockdown measures)divided by the number of infections(SARS-CoV-2 seropositive individuals plus excess deaths)and conducted Spearman correlations between meteorological factors and IFR across the *** We estimated 2,418,250 infections and 43,237 *** IFR was 0.03%in<50-year-old,0.22%in 50–59-year-old,0.9%in 60–69-year-old,3.3%in 70–79-year-old,12.6%in 80–89-year-old,and26.5%in≥*** did not find statistically significant relationships between meteorological factors and adjusted ***,we found strong relationships between low temperature and unadjusted IFR,likely due to Spain’s colder provinces’aging *** The association between meteorological factors and adjusted COVID-19 IFR is *** age differences or ignoring COVID-19-unrelated deaths may severely bias COVID-19 epidemiological analyses.
