Esomeprazole regimens for reflux symptoms in Chinesepatients with chronic gastrit

AIM To compare symptom control with esomeprazoleregimens for non-erosive reflux disease and chronicgastritis in patients with a negative ***： This randomized, open-label study wasdesigned in line with clinical practice in China. Patientswith typical reflux symptoms for ≥ 3 mo and a negativeendoscopy who had a Gastroesophageal Reflux DiseaseQuestionnaire score ≥ 8 were randomized to initialtreatment with esomeprazole 20 mg once daily eitherfor 8 wk or for 2 wk. Patients with symptom reliefcould enter another 24 wk of maintenance/on-demandtreatment, where further courses of esomeprazole 20mg once daily were given if symptoms recurred. Theprimary endpoint was the symptom control rate at week24 of the maintenance/on-demand treatment *** endpoints were symptom relief rate, successrate （defined as patients who had symptom reliefafter initial treatment and after 24 wk of maintenancetreatment）, time-to-first-relapse and satisfaction ***： Based on the data collected in the modifiedintention-to-treat population （MITT; patients in the ITTpopulation with symptom relief after initial esomeprazoletreatment, n = 262）, the symptom control rate showeda small but statistically significant difference in favorof the 8-wk regimen （94.9% vs 87.3%, P = 0.0473）.Among the secondary endpoints, based on the datacollected in the ITT population （n = 305）, the 8-wkgroup presented marginally better results in symptomrelief after initial esomeprazole treatment （88.3% vs83.4%, P = 0.2513） and success rate over the wholestudy （83.8% vs 72.8%, P = 0.0258）. The 8-wkregimen was found to provide a 46% reduction in riskof relapse vs the 2-wk regimen （HR = 0.543; 95%CI：0.388-0.761）. Objective Poly（rC）-binding protein 1 （PCBP1） belongs to the heterogeneous nuclear ribonucleoprotein family and participates in transcriptional and translational regulation. Previous work has identified transcripts targeted by both knockdown and overexpression of PCBP1 in SH-SY5Y neuroblastoma cells using a microarray or ProteomeLabTM protein fractionation 2-dimensions （PF-2D） and quadrupole time-of-flight mass spectrometer. The present study aimed to further determine whether these altered transcripts from major pathways （such as Wnt signaling, TGF-β signaling, cell cycling, and apoptosis） and two other genes, H2AFX and H2BFS （screened by PF-2D）, have spatial relationships. Methods The genes were studied by qRT-PCR, and dynamic Bayesian network analysis was used to rebuild the coordination network of these transcripts. Results PCBP1 controlled the expression or activity of the seven transcripts. Moreover, PCBP1 indirectly regulated MAP2K2, FOS, FST, TP53 and WNT7B through H2AFX or regulated these genes through SAT. In contrast, TP53 and WNT7B are regulated by other genes. Conclusion The seven transcripts and PCBP1 are closely associated in a spatial interaction network.

AIM： To analyze the local and systemic complications of high intensity focused ultrasound （HIFU） for patients with recurrent and metastatic abdominal tumors. METHODS： From Aug 2001 to Aug 2004, 17 patients with recurrent and metastatic abdominal tumors were enrolled in this study. Real-time sonography was taken, and vital signs, liver and kidney function, skin burns, local reactions, and systemic effects were observed and recored before, during, and after HIFU. CT and MR/were also taken before and after HIFU. RESULTS： All 17 patients had skin burns and pain in the treatment region; the next common complication was neurapraxia of the stomach and intestines to variable degrees. The other local and systemic complications were relatively rare. Severe complications were present in two patients; one developed a superior mesenteric artery infarction resulting in necrosis of the entire small intestines, and the other one suffered from a perforation in terminal ileum due to HIFU treatment. CONCLUSION： Although HIFU is a one of noninvasive treatments for the recurrent and metastatic abdominal tumors, there are still some common and severe complications which need serious consideration.

Background:Financial issues are major barriers for rural-to-urban migrants accessing tuberculosis(TB)care in *** paper discusses the effectiveness of providing financial incentives to migrant TB patients(with a focus on poor migrants in one district of Shanghai using treatment completion and default rates),the effect of financial incentives in terms of reducing the TB patient cost,and the incremental cost-effectiveness ratio of the ***:Ninety and ninety-three migrant TB patients were registered in the intervention and control districts *** treatment completion rates significantly improved by 11%(from 78%to 89%)in the intervention district,compared with only a 3%increase(from 73%to 76%)in the control district(P=0.03).Default rates significantly decreased by 11%(from 22%to 11%)in the intervention district,compared with 1%(from 24%to 23%)in the control district(P=0.03).In the intervention district,the financial subsidy(RMB 1,080/US$170)accounted for 13%of the average patient direct cost(RMB 8,416/US$1,332).Each percent increase in treatment completion costs required an additional RMB 6,550(US$1,301)and each percent reduction in defaults costs required an additional RMB 5,240(US$825)in the intervention ***:Overall,financial incentives proved to be effective in improving treatment completion and reducing default rates among migrant TB patients in *** results suggest that financial incentives can be effectively utilized as a strategy to enhance case management among migrant TB patients in large cities in China,and this strategy may be applicable to similar international settings.

To investigate the nature of the Ψ（3770） resonance and to measure the cross section for e^＋e^-→DD, a cross-section scan data sample, distributed among 41 center-of-mass energy points from 3.73 to 3.89 GeV, was taken with the BESIII detector operated at the BEPCII collider in the year 2010. By analyzing the large angle Bhabha scattering events, we measure the integrated luminosity of the data sample at each center-of-mass energy point. The total integrated luminosity of the data sample is 76.16±0.04±0.61 pb^-1, where the first uncertainty is statistical and the second systematic.
