BACKGROUND Pancreatic panniculitis is an extremely rare condition associated with different underlying pancreatic disorders and characterized by subcutaneous fat necrosis induced by elevated serum lipase *** lesions usually affect the lower extremities and may precede abdominal symptoms of pancreatic *** cell carcinoma(ACC)of the pancreas is a rare pancreatic neoplasm,accounting for only 1%-2%of pancreatic tumors in *** present the case of a 72-year-old man with ACC of the pancreatic head and synchronous liver *** the primary tumor and liver metastases were *** lipase was elevated before surgery and decreased to normal *** serum lipase levels at follow-up led to the diagnosis of hepatic *** disease progression was then accompanied by pancreatic panniculitis,with subcutaneous fat necrosis and acute *** the best of our knowledge,only 4 cases have been reported in the literature and each showed a similar association of serum lipase levels with pancreatic panniculitis and progression of *** Clinical symptoms and progression of ACC may correlate with serum lipase levels,suggesting potential usefulness as a follow-up biomarker.

Ameliorated stress related proteins are associated with improved cardiac function by sarcoplasmic reticulum calcium ATPase gene transfer in heart failure

Background Previous studies showed that overexpression of sarco-endoplasmic retieulum calcium ATPase （SERCA2a） in a variety of heart failure （HF） models was associated with greatly enhanced cardiac performance. However, it still undefined the effect of SERCA2a overexpression on the systemic inflammatory response and neuro-hormonal factors. Methods A rapid right ventricular pacing model of experimental HF was used in beagles. Then the animals underwent recombinant adeno-associated vires 1 （rAAV1） mediated gene trans- fection by direct intra-myocardium injection. HF animals were randomized to receive the SERCA2a gene, enhanced green fluorescent protein （control） gene, or equivalent phosphate buffered saline. Thirty days after gene delivery, the cardiac function was evaluated by echocardiographic testing. The protein level of SERCA2a was measured by western blotting. The proteomic analysis of left ventricular （LV） sample was determined using two-dimensional （2-D） gel el^ctrophoresis and MALDI-TOF-MS. The serum levels of the systemic inflammatory and neuro-hormonal factors were assayed using radioimmunoassay kits. Results The cardiac function improved after SERCA- 2a gene transfer due to the significantly increased SERCA2a protein level. Beagles treated with SERCA2a had significantly decreased serum levels of the inflammatory markers （interleukin-6 and tumor necrosis factor-a） and neuro-hormonal factors （brain natriuretic peptide, endothelin-1 and angiotensin II） compared with HF animals. The myocardial proteomic analysis showed that haptoglobin heavy chain, heat shock protein （alpha-crystallin-related, B6） were down-regulated, and galectin-1 was up-regulated in SERCA2a group compared with HF group, companied by up-regulated contractile proteins and NADH dehydrogenase. Conclusions These findings demonstrate that regional intramyocardial injections of rAAV 1-SERCA2a vectors may improve global LV function, correlating with reverse activation of the systemic inflammatory,

The relationship between activities of hepatic and gastric alcohol dehydrogenase and occurrence of chronic alcoholic liver disease

Objective: To investigate the role of hepatic and gas-tric alcohol dehydrogenase (ADH) in different pa-thologic stages of alcoholic liver disease (ALD).Methods: Thirty-nine Wistar rats were divided ran-domly into two groups: model group (24) and con-trol group (15). The ALD model was established byinfusing alcohol into the stomach. After hepatic andgastric tissues had been stained by enzyme histo-cyto-chemistry assay, the activity varieties of hepatic andgastric ADH were observed by an optical micro-scope, and the activity alterations were also deter-mined by LUZEX-F image analysis as a semi-quanti-tative ***: The activity of hepatic ADH gradually in-creased, but that of gastric ADH gradually decreasedin the different pathologic stages of alcoholic liverdisease. There was a significant difference betweenthe model group and control group (P<0.05).Conclusions: Along with occurrence of ALD, the ac-tivity of hepatic ADH gradually increased, but thatof gastric ADH gradually decreased, showing thatthe activity alterations of hepatic and gastric ADHmay play an important role in the onset and develop-ment of ALD.

Chemoradiotherapy for a patient with a giant esophageal fistula

We describe our experience of treatment for a giant esophageal malignant fistula, which has not been reported previously. A 36-year-old woman who was diagnosed as having massive esophageal small cell carcinoma with metastases was treated with chemoradiotherapy. However, a giant esophagomediastinal fistula appeared due to shrinkage of the massive tumor, and all anti-cancer treatment was suspended. However, chemoradiotherapy was restarted at the request of the patient despite the presence of the fistula. After restarting treatment, the giant esophageal fistula was naturally closed despite intensive chemoradiotherapy, and the patient became able to eat and drink. Although the patient finally died, her QOL and prognosis seemed to be improved by the chemoradiotherapy. Anti-cancer treatment could be safely performed despite the presence of a giant fistula. The giant fistula closed while intensive chemotherapy was administered to the patient. Therefore, the presence of a fistula may not be a contraindication for curative chemoradiotherapy. Completion of treatment with proper management and maintenance of patients would be of benefit to patients with fistula.

Changes in the nutritional status of nine vitamins in patients with esophageal cancer during chemotherapy
