Carcinosarcoma of the breast:Facing the challenge of a rare nosologic entity

Carcinosarcoma(CS),also known as metaplastic breast carcinoma with mesenchymal differentiation,is one of the five distinct subtypes of metaplastic breast *** is considered as a mixed,biphasic neoplasm consisting of a carcinomatous component combined with a malignant nonepithelial element of mesenchymal origin without an intermediate transition *** cellular origin of this neoplasm remains controversial,most researchers declare that neoplastic cells derive from a cellular structure with potential biphasic *** recent research on the therapeutic strategies against CS neoplastic disorders,surgical resection appears the only potentially curative *** CS metastasize by the lymphatic route,axillary assessment with sentinel lymph node biopsy and/or axillary lymph node dissection is always ***,the tumor also presents a hematogenous metastatic pattern including pleural,pulmonary,liver,brain and less commonly bone ***,surgical removal of breast CS does not necessarily ensure patient’s long-term ***,alternative therapies,such as radio-and chemotherapy proved insufficient and 5-year survival rate is ***,there is evidence that following surgery,the combination of radio and chemotherapy is associated with a better prognosis than either treatment *** aim of this review is to evaluate the results of surgical treatment for breast CS with special reference to the extent of its histological *** features,histogenesis,morphological and immunochemical findings are discussed,while the role of current diagnostic and therapeutic management of this aggressive neoplasm is emphasized.

The pathophysiology of ischemic stroke is complex and multifactorial,involving various forms of cell death such as apoptosis,autophagy,and necrosis.A recent study suggests that oxidative and inflammatory stress can induce ferroptosis,a specialized form of cell death characterized by the accumulation of lipid peroxides dependent on intracellular iron overload(Li and Jia,2023).

Hepatocellular carcinoma in chronic hepatitis B patients under antiviral therapy

Patients with chronic hepatitis B are at increased risk of hepatocellular carcinoma(HCC),while the inhibition of viral replication can represent a reasonable target for HCC ***-αtherapy results in decreased HCC risk,which is more evident in patients with high baseline HCC *** majority of chronic hepatitis B patients are treated with a nucleos(t)ide analogue(NA)for several reasons including the nonsustained response after interferon-α.The effect of the first licensed and low genetic barrier NA,lamivudine,on HCC incidence,has been repeatedly ***,compared to no treatment,reduces the HCC incidence,which may increase again in cases with lamivudine *** data with the currently first-line NAs,entecavir and tenofovir,suggest that they also reduce the HCC *** treatment benefit in reduction of the HCC incidence is always greater in patients with high baseline HCC risk,particularly cirrhotics,and without virological remission under entecavir/***,the HCC risk is not eliminated even in the vast majority of patients who remain in virological remission under entecavir/***,patients at increased baseline HCC risk should continue to undergo HCC surveillance even if they have achieved complete long-term inhibition of viral replication and improvements in liver histology.

Surgical treatment of acute cholecystitis in patients with confirmed COVID-19:Ten case reports and review of literature

BACKGROUND Research concerning postoperative outcomes of confirmed coronavirus disease 2019(COVID-19)patients revealed unfavorable postoperative results with increased morbidity,pulmonary complications and *** reports have suggested that COVID-19 is associated with more aggressive presentation of acute *** aim of the present study is to describe the perioperative assessment and postoperative outcomes of ten patients with confirmed severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection with concomitant acute cholecystitis who underwent *** SUMMARY We report a total of 10 SARS-CoV-2 positive patients with concomitant acute cholecystitis that underwent *** patients were males,the mean age was 47.1 *** patients had moderate acute cholecystitis,and one patient had severe acute *** patients were treated with urgent/early laparoscopic *** the Parkland grading scale,two patients received a Parkland grade of 3,two patients received a Parkland grade of 4,and six patients received a Parkland grade of *** patients required a bail-out *** patients developed biliary leakage and required endoscopic retrograde cholangiopancreatography with biliary *** surgery,five patients developed acute respiratory distress syndrome(ARDS)and required intensive care unit(ICU)*** patient died after cholecystectomy due to ARDS *** mean total length of stay(LOS)was 18.2 *** histopathology demonstrated transmural necrosis(n=5),vessel obliteration with ischemia(n=3),perforation(n=3),and acute peritonitis(n=10).CONCLUSION COVID-19 patients with acute cholecystitis had difficult cholecystectomies,high rates of ICU admission,and a prolonged LOS.

drug-induced autoimmune hepatitis(DIAIH)is a specific phenotype of druginduced liver injury that may lead to the devastating outcome of acute liver failure requiring liver *** implicated in DIAIH include antimicrobials such as nitrofurantoin and minocycline,non-steroidal anti-inflammatory drugs,statins as well as anti-tumor necrosis *** clinical features of druginduced liver injury are indistinguishable from idiopathic autoimmune hepatitis(AIH)as both may have positive AIH-related autoantibodies,elevated immunoglobulin G,as well as similar histopathological *** patients who show no clinical improvement,or there is progressive liver injury despite cessation of the suspected drug,a liver biopsy should be considered,whereby the presence of advance fibrosis on histology favors the diagnosis of idiopathic *** treatment with corticosteroids may be required in patients with non-resolving liver injury.A typical clinical scenario supportive of DIAIH includes a history of drug exposure with spontaneous resolution of liver injury after drug withdrawal and the absence of relapse after rapid steroid *** this article we report two cases of DIAIH secondary to Sorafenib and Atorvastatin along with a review of currently available *** identification and treatment often lead to a favorable outcome in DIAIH.

Circulating biomarkers of hepatocellular carcinoma response after locoregional treatments: New insights

Hepatocellular cancer is the 5th most common cancer in the world and the third cause of death by malignant disease. Locoregional therapies are the most usual treatment of choice for patients with early or intermediate stage of disease. The main diagnostictools for the detection of recurrence are the radiological techniques such as 4-phase computed tomography or dynamic contrast enhanced magnetic resonance imaging. However, in order to achieve best evaluation of treatment outcome and recurrence rates, there is a great need for the identification of specific and easily measured circulating biomarkers. The aim of this review is to analyze the existing data considering the prognostic significance of changes of serum diagnostic markers such as alpha-fetoprotein, des-gamma-carboxy prothrombin, alpha-fetoprotein-L3, angiogenetic factors(vascular endothelial growth factor, hypoxia inducible factor-1a) and immune parameters before and after radiofrequency ablation or transarterial chemoembolization.

Peripheral blood RNA biomarkers can predict lesion severity in degenerative cervical myelopathy

Degenerative cervical myelopathy is a common cause of spinal cord injury,with longer symptom duration and higher myelopathy severity indicating a worse *** numerous studies have investigated serological biomarkers for acute spinal cord injury,few studies have explored such biomarkers for diagnosing degenerative cervical *** study involved 30 patients with degenerative cervical myelopathy(51.3±7.3 years old,12 women and 18 men),seven healthy controls(25.7±1.7 years old,one woman and six men),and nine patients with cervical spondylotic radiculopathy(51.9±8.6 years old,three women and six men).Analysis of blood samples from the three groups showed clear differences in transcriptomic *** analysis identified 128 differentially expressed genes that were enriched in patients with neurological *** least absolute shrinkage and selection operator analysis,we constructed a five-gene model(TBCD,TPM2,PNKD,EIF4G2,and AP5Z1)to diagnose degenerative cervical myelopathy with an accuracy of 93.5%.One-gene models(TCAP and SDHA)identified mild and severe degenerative cervical myelopathy with accuracies of 83.3%and 76.7%,*** of two immune cell types(memory B cells and memory-activated CD4^(+)T cells)predicted levels of lesions in degenerative cervical myelopathy with 80%*** results suggest that peripheral blood RNA biomarkers could be used to predict lesion severity in degenerative cervical myelopathy.

动脉粥样硬化仍然是全球发病率和死亡率的主要原因,斑块内新生血管形成显著促进了动脉粥样硬化的进展和不稳定性.血管内皮生长因子A (VEGFA)通过诱导内皮细胞增殖和迁移,刺激斑块中异常微血管的生长.色素上皮衍生因子(PEDF)能够高效抑制VEGFA依赖的新生血管形成.本研究介绍了一种由泊洛沙姆407(F127)和海藻酸钠(SA)制备的温敏水凝胶(PFSgel),将PEDF局部递送至动脉粥样硬化斑块周围. PFSgel在体温(37°C)下表现出优异的液固转变,随后在SA与生理环境中的Ca2+凝胶化后形成稳定的三维网络结构,实现控制释放的效果.流变学分析证实其相变温度为28.7°C,并具有显著的自愈特性.体外实验显示, PFSgel可通过调节CD31和MMP-2/MMP-9信号通路,抑制VEGFA诱导的内皮细胞增殖和迁移.体内降解测试验证了PFSgel的控制释放性能.在Apoe缺陷的动脉粥样硬化小鼠中,超声引导的PFSgel注射到腹主动脉周围,可逐步在原位释放包封的PEDF,有效减少了斑块负担、新生血管形成和腔狭窄.总之,这一原位注射的PFSgel凝胶通过CD31和MMP-2/MMP-9通路,有效抑制病理性新生血管形成,通过干预VEGFA依赖的新生血管形成来稳定动脉粥样硬化中的高风险斑块.

Prevalence and Factors Associated with Concomitant Chinese Medicine Use by Rheumatoid Arthritis Patients in A Multi-Ethnic Asian Population

Objective:To determine the prevalence,factors associated with and patterns of concomitant Chinese medicine(CM)with Western treatment use among patients with rheumatoid arthritis(RA)in a tertiary referral centre(Singapore general hospital)in ***:We conducted a cross-sectional intervieweradministered survey of a consecutive sample of patients with RA in Singapore general hospital centre regarding their CM use including data on patient demographics,disease characteristics,concomitant use of CM and reasons,concerns and disclosure patterns from March to August *** and multivariate logistic regression analyses were performed to determine the associations of CM ***:Prevalence of CM use among the 258 patients surveyed(male:female 42:216;Chinese:Malay:Indian 191:29:34;mean age:61 years;mean duration of RA:10 years)was 46.1%(119/258).On muutivariate analysis,Chinese ethnicity(OR,95%CI:4.11,1.49-11.36),Chinese speakers(OR,95%CI:2.35,1.03-5.54),middle-income group(OR,95%CI:2.53,1.01-6.31)and greater learned helplessness(OR,95%CI:1.13,1.04-1.22)were significantly associated with CM *** CM users disclosed their CM use to CM physicians(87.3%,96/110),sought advice from them on treatment interactions(59.4%,57/96)and how best to combine treatments(49.0%,47/96)than did so with rheumatologists(42.0%,50/119;40.0%,20/50;and 42.0%,21/50,respectively).Forty-two percentage(29/69)of patients who concealed CM use from rheumatologists because their rheumatologists did not specifically enquire about CM ***:Concomitant CM use among patients with RA treated in a tertiary referral centre in Singapore is high but voluntary disclosure is *** associations identified can help doctors identify and enquire about CM use,minimizing potential adverse interactions.
