Furazolidone-based triple therapy for H pylori gastritis in children

AIM： To evaluate the furazolidone-based triple therapy in children with symptomatic H pylori gastdtis. METHoDS： A prospective and consecutive open trial was carried out. The study included 38 patients with upper digestive symptoms sufficiently severe to warrant endoscopic investigation. H pylori status was defined based both on histology and on positive ^13C-urea breath test. drug regimen was a seven-day course of omeprazole, clarithromycin and furazolidone （100 mg, 200 mg if over 30 kg） twice daily. Eradication of H pylori was assessed two months after treatment by histology and ^13C-urea breath test. Further clinical evaluation was performed 7 d, 2 and 6 mo after the treatment. RESULTS： Thirty-eight patients （24 females, 14 males） were included. Their age ranged from 4 to 17.8 （mean 10.9 ± 3.7） years. on intent-to-treat analysis （n = 38）, the eradication rate of H pylori was 73.7% （95% CI, 65.2%-82%） whereas in per-protocol analysis （n = 33） it was 84.8% （95% CI, 78.5%-91%）. All the patients with duodenal ulcer （n = 7） were successfully treated （100% vs 56.2% with antral nodularity）. Side effects were reported in 26 patients （68.4%）, mainly vomiting （14/26） and abdominal pain （n = 13）. Successfully treated dyspeptic patients showed improvement in 78.9% of H pylori-negative patients after six months and in 50% of Hpylori-positive patients after six months of treatment. CoNCLUSIoN： Triple therapy with furazolidone achieves moderate efficacy in H pylori treatment. The eradication rate seems to be higher in patients with duodenal ulcer.

Right liver lobe/albumin ratio:Contribution to non-invasive assessment of portal hypertension

AIM： To study the value of biochemical and ultrasonographic parameters in prediction of presence and size of esophageal varices.METHoDS： The study includes selected cirrhotic patients who underwent a complete biochemical workup, upper digestive endoscopic and ultrasonographic examinations. Albumin/right liver lobe diameter and platelet count/spleen diameter ratios were calculated. The correlation between calculated ratio and the presence and degree of esophageal varices was evaluated.RESULTS： Ninety-four subjects （62 males, 32 females）, with a mean age of 52.32 ± 13.60 years, were studied. Child-Pugh class A accounted for 42.6%, class 13 37.2%, whereas class C 20.2%. Esophageal varices （oE） were not demonstrated by upper digestive endoscopy in 24.5%, while oE grade Iwas found in 22.3% patients, grade Ⅱ in 33.0%, grade m in 16.0%, and grade iV in 4.3%. The mean value of right liver lobe diameter/ albumin ratio was 5.51± 1.82 （range from 2.76 to 11.44）, while the mean platelet count/spleen diameter ratio was 1017.75 ± 729.36 （range from 117.39 to 3362.50）, respectively. Statistically significant correlation was proved by Spearman＇s test between oE grade and calculated ratios. The P values were 0.481 and -0.686, respectively.CoNCLUSIoN： The right liver lobe diameter/albumin and platelet count/spleen diameter ratios are noninvasive parameters providing accurate information pertinent to determination of presence of esophageal varices, and their grading in patients with liver cirrhosis.

AIM:To investigate the association between prognosis of rectal cancer treated with chemoradiotherapy(CRT) and expression of sensitive-to-apoptosis(SAG),B-cell lymphoma-extra large(Bcl-X L) and Bcl-2 homologous antagonist/killer(Bak).METHoDS:Real-time quantitative polymerase chain reaction was used to determine the expression of proteins of interest,namely SAG,Bcl-X L,Bak and β-actin,in rectal carcinoma patients who had a follow-up period of 3 years after *** specimens were excised from the rectal tumor preceding ***:SAG,Bcl-X L and Bak proteins showed significant correlations with each *** multivariate analysis,patients with high vs low SAG expression showed a statistically significant difference in 2-year survival rates:56% vs 73%,respectively(P = 0.056).on the other hand,there were no significant correlations between the expression levels of all three genes and metastatic rates or tumor responses to *** overall survival in the patients with elevated SAG expression was 27.1 mo ± 3.9 mo [95% confidence interval(CI):19.3-34.9],and in patients with reduced expression,it was 32.1 mo ± 2.5 mo(95% CI:27.3-36.9).The corresponding values for Bcl-X L were 28.0 mo ± 4.1 mo(95% CI:19.9-36.1) and 31.7 mo ± 2.9 mo(95% CI:26.0-37.5),and those for Bak were 29.8 mo ± 3.7 mo(95% CI:22.5-37.2) and 30.6 mo ± 2.4 mo(95% CI:25.5-35.0),***:Two-year survival rates significantly correlated with low SAG expression,and SAG may be a candidate gene for good prognosis,independent of therapeutic response of different individuals.

Validation of the Glaucoma Quality of Life-15 Questionnaire in Serbian language

AIM：To translate the Glaucoma Quality of Life-15（GQL-15）to Serbian language and asses its validity and reliability in the population of Serbian ***：The study included 177 glaucoma *** parameters（visual acuity,mean defect and square root of loss variance of visual field）and socio-demographic data were *** were stratified according to the Nelson’s glaucoma staging system as mild,moderate and advanced *** patients filled out the GQL-15 and National Eye Institute Visual Function Questionnaire（NEI-VFQ 25）.The GQL-15 was translated following the internationally-accepted methodology,and its psychometric properties were assessed by using classical test theory and Rasch ***：The mean total score for the GQL-15 was 20.68±*** Cronbach’s alpha coefficient for the whole scale was 0.89（central and near vision,α=0.24;peripheral vision,α=0.85;glare and dark adaptation,α=0.83）.Factor analysis established 4 factors（70.3% of variance）：two corresponding to the original factors and two new factors specific for the Serbian *** GQL-15 score correlated positively with almost all clinical parameters and NEI-VFQ 25 proving good criterion *** of the GQL-15 total score on test-retest confirmed appropriate scale reproducibility（ρ=0.96,P〈0.001）.The GQL-15 discriminated well advanced from mild and moderate *** Rasch analysis we obtained adequate item（0.95）reliability *** all items had infit and outfit mean squares in the accepted ***：Serbian version of the GQL-15 demonstrates adequate reliability and *** version of the GQL-15is a valid instrument for evaluation of quality of life among Serbian speaking patients with glaucoma and can be applied in daily clinical work.

Gastroenterology training in Latin America

Latin America is characterized by ethnic, geographical, cultural, and economic diversity; therefore, training in gastroenterology in the region must be considered in this context. The continent’s medical education is characterized by a lack of standards and the volume of research continues to be relatively small. There is a multiplicity of events in general gastroenterology and in sub-disciplines, both at regional and local levels, which ensure that many colleagues have access to information. Medical education programs must be based on a clinical vision and be considered in close contact with the patients. The programs should be properly supervised, appropriately defined, and evaluated on a regular basis. The disparity between the patients’ needs, the scarce resources available, and the pressures exerted by the health systems on doctors are frequent cited by those complaining of poor professionalism. Teaching development can play a critical role in ensuring the quality of teaching and learning in universities. Continuing professional development programs activities must be planned on the basis of the doctors’ needs, with clearly defined objectives and using proper learning methodologies designed for adults. They must be evaluated and accredited by a competent body, so that they may become the basis of a professional regulatory system. The specialty has made progress in the last decades, offering doctors various possibilities for professional development. The world gastroenterology organization has contributed to the speciality through three distinctive, but closely inter-related, programs: Training Centers, Train-the-Trainers, and Global Guidelines, in which Latin America is deeply involved.

Human dental enamel： A masterpiece investigated natural nanotechnology by TEM and t-EBSD

The micro- and nanostructures of human dental enamel were investigated by transmission electron microscopy （TEM） and electron backscatter diffraction （EBSD）. For this purpose, TEM slices were prepared from teeth by focused ion-beam milling. These slices enabled TEM and transmission-EBSD （t-EBSD） investigations to be performed, while standard EBSD on bulk tooth samples resulted only in Kikuchi patterns. on the TEM slices, t-EBSD enabled automated mapping. The TEM images and the EBSD data dearly elucidated the arrangement of the hydroxyapatite crystals on the nanometer scale. Information regarding the crystallographic orientation of the apatite grains enabled the deduction of novel nanotechnological building principles of the enamel structure based on a chain-like arrangement of the crystallites.

The systemic nature of cirrhosis and portal hypertension has long been recognized,and the amount of data characterizing the interplay between each system is becoming ever so *** involvement was among the first described associated entities in cirrhosis,with reports dating back to the late nineteenth ***,it appears that throughout the years,interest in the pulmonary complications of portal hypertension has generally faded,especially in contrast to other decompensating events,as expertise in this field has primarily been concentrated in highly experienced tertiary care facilities and liver transplantation *** affecting up to 10%-15%of patients with advanced liver disease and having a proven prognostic impact,hepato-pulmonary syndrome,porto-pulmonary hypertension,and hepatic hydrothorax are frequently misdiagnosed,mistreated,or *** lack of precision might adversely impact patient care,referral to expert centers,and,ultimately,liver disease-related mortality and successful transplantation *** present minireview aims to increase awareness of the pulmonary complications of chronic liver disease by providing a brief overview of each of the three *** paper focuses on the essential theoretical aspects,addressing the most critical knowledge gaps on the one hand and,on the other hand,critically discussing one key issue for each complication.
