BACKGROUND： It is crucial to understand cerebral perfusion and vascular reserve in stegnotic arterial blood-supply regions to treat ischemic cerebrovascular diseases. However, effects on symptomatic intracranial arterial stenosis （SICAS） need to be further studied in additional applications. OBJECTIVE： To evaluate and summarize the effects of cerebral perfusion and vascular reserve on the treatment of SICAS. RETRIEVAL STRATEGY： A computer-based online search of English language publications from January 2000 to July 2007 was conducted in PubMed to identify publications that addressed cerebral perfusion and vascular reserve of SICAS. Search key words were ＂intracranial stenosis, perfusion, brain reserve＂. Relevant data were also searched with the China Journal Net, using the same key words in Chinese from January 2000 to January 2007. In total, 101 articles were retrieved. Inclusion criteria：（1） Articles describing the current status for the diagnosis and treatment of SICAS; （2） Articles concerning research developments of cerebral perfusion and vascular reserve of SICAS. Exclusion criteria： duplicated articles. LITERATURE EVALUATION： This study included 21 articles of experimental studies and conference reports. DATA SYNTHESIS： When performing interventional surgery in SICAS patients, it is important to understand cerebral perfusion and vascular reserve in addition to knowing the clinical symptoms and degrees of arterial stenosis. In recent years, there are a growing number of reports on measurements of vascular reserve through the use of magnetic resonance perfusion imaging （MR-PWI）. Investigations demonstrate cerebral perfusion and vascular reserve decrease in many SICAS patients. Many studies show that both improve after surgical intervention. CONCLUSION： Cerebral perfusion could provide direct evidence of whether ischemia has occurred in the brain. It remains unclear whether limitations in activities of daily living(ADL) increase the risk of stroke in older Chinese *** longitudinal study used data from the Chinese Longitudinal Healthy Longevity Survey to investigate the effects of limitations in ADL on the incidence of stroke in older *** 2002 and 2011,46,728 participants from 22 provinces in China were included in this *** participants,11,241 developed limitations in ADL at baseline.A 3-year follow-up was performed to determine the incidence of *** the 3-year follow-up,929 participants(8.26%) and 2434 participants(6.86%) experienced stroke in the ADL limitations group and non-ADL limitations group,*** regression was used to analyze the effect of ADL limitations on the risk of *** results showed that after adjusting for the confounding factors gender,age,weight,hypertension,diabetes,heart disease,natural teeth,hearing impairment,visual impairment,smoking,alcohol abuse,exercise,ethnicity,literacy,residential area,and poverty,the ADL limitations group had a 77% higher risk of developing stroke than the non-ADL limitations *** propensity score matching,the ADL limitations group still had a 33% higher risk of developing stroke than the non-ADL limitations group(OR = 1.326,95% CI:1.174–1.497).These findings suggest that limitations in ADL are a stroke risk factor.

Objective:To study the effect of aspirin on healing process of osteoporotic fracture(OPF)in ***:A total of 50 female Wistar rata aged 3 months were randomly divided into observation group and control group,castration method was adopted to establish the osteoporosis(OP)*** artificial preparing fractures on the midpoint of left femur,fixing gram needle *** modeling was *** lavage of 33 mg once a day was adoptde in observation group after modeling,same amount of normal saline was used in the control as *** eash group,selected 5 rats at the 2nd.4th,8th and 12th week after modeling to measure the bone mineral density(BMD)and histogical examination of the fracture callus,radiology observation was conducted at the 8th and 12th *** femur biomechanical measurement was taken at the 12th ***:BMD values of observation group at each time point were significantly higher than that of the control group after modeling(P<0.05);Histological observation showed that at the 8th week,the endochondral ossification process of observation group was faster than that of observation group,with fuzzy fracture line in observation group and clear fracture line in observtion group;at the 12th week,fracture line disappeared in observation group,fracture line of the control group was fuzzy at the same time;three-point bending load of the left femur in observation group rats was significantly higher than that of control group after12 weeka(P<0.05).Conclusions:Asporin can accelerate the healing of new callus in OPF rats,increase bone density and biomechanics strength,and promote fracture of osteporotic rats.

Our previous studies showed that miR-23b was downregulated in patients with intracerebral hemorrhage(ICH). This indicates that miR-23b may be closely related to the patho-physiological mechanism of ICH, but this hypothesis lacks direct evidence. In this study, we established rat models of ICH by injecting collagenase Ⅶ into the right basal ganglia and treating them with an injection of bone marrow mesenchymal stem cell(BMSC)-derived exosomal miR-23b via the tail vein. We found that edema in the rat brain was markedly reduced and rat behaviors were improved after BMSC exosomal miR-23b injection compared with those in the ICH groups. Additionally, exosomal miR-23b was transported to the microglia/macrophages, thereby reducing oxidative stress and pyroptosis after ICH. We also used hemin to mimic ICH conditions in vitro. We found that phosphatase and tensin homolog deleted on chromosome 10(PTEN) was the downstream target gene of miR-23b, and exosomal miR-23b exhibited antioxidant effects by regulating the PTEN/Nrf2 pathway. Moreover, miR-23b reduced PTEN binding to NOD-like receptor family pyrin domain containing 3(NLRP3) and NLRP3 inflammasome activation, thereby decreasing the NLRP3-dependent pyroptosis level. These findings suggest that BMSC-derived exosomal miR-23b exhibits antioxidant effects through inhibiting PTEN and alleviating NLRP3 inflammasome-mediated pyroptosis, thereby promoting neurologic function recovery in rats with ICH.

BACKGROUND Delayed intracranial hemorrhage(DICH),a potential complication of ventriculoperitoneal(VP)shunts,has been associated with high mortality,but its risk factors are still *** To investigate the risk factors of DICH after VP *** We compared the demographic and clinical characteristics of DICH and non-DICH adult patients with VP shunts between January 2016 and December *** The 159 adult VP shunt patients were divided into 2 groups according to the development of DICH:the DICH group(n=26)and the non-DICH group(n=133).No statistically significant difference was found in age,sex,laboratory examination characteristics or preoperative modified Rankin Scale(mRS)score between the DICH and non-DICH groups(P>0.05);however,a history of an external ventricular drain(EVD)[P=0.045;odds ratio(OR):2.814;95%CI:1.024-7.730]and postoperative brain edema around the catheter(P<0.01;OR:8.397;95%CI:3.043-23.171)were associated with a high risk of DICH.A comparison of preoperative mRS scores between the DICH group and the non-DICH group showed no significant difference(P=0.553),while a significant difference was found in the postoperative mRS scores at the 3-mo follow-up visit(P=0.024).CONCLUSION A history of EVD and postoperative brain edema around the catheter are independent risk factors for DICH in VP shunt *** patients with a high mRS score are vulnerable to poor clinical outcomes.
