The Prague C and M Criteria have been developed for the objective endoscopic diagnosis of Barrett's esophagus(BE).BE arises between the squamocolumnar junction and the gastroesophageal junction at the proximal margin of the gastric *** this study,we reported that 43.0% of the subjects examined were diagnosed with BE based on the Prague C and M *** criticism by John Dent proposed that our data should be considered invalid because the prevalence of BE reported in our study was extraordinarily high and discordant with previous *** predicted that the position of the gastroesophageal junction in our study was judged to be lower than the actual position due to the effacement of the proximal ends of the gastric folds because of the routine use of a high degree of air distension during typical Japanese endoscopic *** endoscopic evaluation of the superior gastric folds is certainly influenced by the degree of air distension of the ***,in our study,the proximal limit of the gastric mucosal folds was prospectively imaged while the oesophagus was minimally ***,under a high level of air distension,the distal ends of the palisade-shaped longitudinal vessels were imaged because they are more easily observed when *** the majority of patients,the distal ends of the palisade-shaped longitudinal vessels correspond to the proximal limit of the gastric mucosal *** endoscopic evaluation was appropriately performed according to the Prague C and M *** suspect that the high prevalence of BE in our study may be due to the inclusion of ultrashort-segment BE,which defines BE with an affected mucosal length under 5 mm,in our positive results.

Phase angle obtained by bioelectrical impedance analysis independently predicts mortality in patients with cirrhosis

AIM To evaluate the prognostic value of the phase angle(PA)obtained from bioelectrical impedance analysis(BIA) for mortality prediction in patients with cirrhosis. METHODS In total, 134 male cirrhotic patients prospectively completed clinical evaluations and nutritional assessment by BIA to obtain PAs during a 36-mo follow-up period. Mortality risk was analyzed by applying the PA cutoff point recently proposed as a malnutrition marker(PA ≤ 4.9°) in Kaplan-Meier curves and multivariate Cox regression models. RESULTS The patients were divided into two groups according to the PA cutoff value(PA > 4.9°, n = 73; PA ≤ 4.9°, n = 61). Weight, height, and body mass index were similar in both groups, but patients with PAs > 4.9° were younger and had higher mid-arm muscle circumference, albumin, and handgrip-strength values and lower severe ascites and encephalopathy incidences, interleukin(IL)-6/IL-10 ratios and C-reactive protein levels than did patients with PAs ≤ 4.9°(P ≤ 0.05). Forty-eight(35.80%) patients died due to cirrhosis, with a median of 18 mo(interquartile range, 3.3-25.6 mo) follow-up until death. Thirty-one(64.60%) of these patients were from the PA ≤ 4.9° group. PA ≤ 4.9° significantly and independently affected the mortality model adjusted for Model for End-Stage Liver Disease score and age(hazard ratio = 2.05, 95%CI: 1.11-3.77, P = 0.021). In addition, Kaplan-Meier curves showed that patients with PAs ≤ 4.9° were significantly more likely to die. CONCLUSION In male patients with cirrhosis, the PA ≤ 4.9° cutoff was associated independently with mortality and identified patients with worse metabolic, nutritional, and disease progression profiles. The PA may be a useful and reliable bedside tool to evaluate prognosis in cirrhosis.

Long-Term Postoperative Perfusion Indices in Surgically Resolved Myopic Traction Maculopathy

Background: Pathological myopia can be complicated by the presence of posterior staphyloma, macular atrophy, ruptures in Bruch’s membrane, pathologic choroidal neovascularization, and different degrees of myopic traction maculopathy. Purpose: To report the structural, functional and perfusional outcomes in patients underwent surgery for different stages of myopic traction maculopathy (MTM). Methods: A retrospective, consecutive, comparative, interventional, one-surgeon, case-control study was conducted in 46 eyes of 34 individuals between April 2015 and May 2021. Participants included normal emmetropic eyes (Control emmetropia, n = 25), healthy myopic eyes (Control high myopia, n = 20), and operated and structurally fully resolved myopic eyes with different stages of MTM (Surgically treated group, n = 46). Long-term postoperative functional and perfusional follow-up evaluations were performed with spectral domain-optical coherence tomography (SD-OCT) and OCT angiography. The primary outcome measure included long-term functional, structural and perfusion macular status across groups. Results: Forty-six eyes in 34 patients were included in the study group, with both eyes affected in 12 (26.3%) patients. The mean axial length was 29.89 ± 1.67 mm. The preoperative logMAR was 1.29 ± 0.54 and the postoperative logMAR was 0.60 ± 0.52 (P Conclusion: Compared to emmetropic and healthy myopic eyes, surgically-resolved MTM eyes generally have larger superficial foveal avascular zone area, lower vessel density, smaller choriocapillaris flow area, thinner central subfoveal thickness, and more macular defects. Eyes with stage III or IV MTM had larger deviation compared to eyes at earlier stages. Visual function change after surgery was associated with superficial foveal avascular zone area. Better functional, structural and perfusion index outcomes were observed when highly myopic eyes underwent early surgery.

Pulmonary embolism (PE) is a common cardiovascular and cardiopulmonary illness and the incidence increases exponentially with age. Because the clinical signs and symptoms are non-specific, its diagnosis is always difficult. It also may develop a complicating pericardial syndrome resembling that described by Dressler in myocardial infarction,which is far less common, simultaneous PE with pericardial syndromes and non-cardiogenic acute pulmonary edema (NCAPE) was an unusual event. Herein, we reported a rare case of elderly woman with functional limitations associated with knee osteoarthritis for several years,developed progressive exertional dyspnea and exercise intolerance two months,diagnosed NCAPE and Dressler syndrome associated PE.

Clinical pregnancy rate of women with unexplained infertility with or without cervical mucus aspiration before intrauterine insemination: A randomized controlled trial

Objective:To detect effect of removing cervical mucus before performing intrauterine insemination(IUI)on pregnancy rate in patients with unexplained ***:The randomized controlled trial was conducted in Infertility division of Department of Obstetrics and Gynaecology of a tertiary care *** 80 patients of unexplained infertility were *** computer generated block randomization in block size of 4 and 6,patients were randomly allocated at time of starting ovarian stimulation into the cervical mucus removal group or the non-removal of cervical mucus group before IUI,40 in each *** stimulation with clomiphene 100 mg from day 2-6 of menstrual cycle along with human menopausal gonadotropin 150 IU was given alternate day starting from day *** monitoring was done and further doses given as per response;trigger was planned when 1-3 follicle reach a diameter of>18 *** was planned after *** was done as per the group *** cleaning was done in the cervical mucus removal group by aspirating mucus with IUI syringe and sterile cotton swab before *** primary outcome was clinical pregnancy *** of difficult IUIs and cycle cancellation due to ovarian hyperstimulation were also ***:IUI was not done in 4 patients due to *** per IUI cycle occurred in 7.9%(3/38)in the cervical mucus removal group and 21.1%(8/38)in the non-removal of cervical mucus *** was no statistically significant difference in clinical pregnancy rate between two groups(P=0.19).Conclusions:There is a trend towards a lower clinical pregnancy rate with removal of cervical mucus before IUI in women of unexplained infertility though the difference is not statistically *** studies with large sample size need to be done on this intervention.

INTRODUCTION The benefit of mechanical thrombectomy for stroke associated with anterior circulation large vessel occlusion(LVO)is well established and among the most effective treatments in medicine.1 Stroke associated with basilar artery occlusion(BAO)was not included in the seminal thrombectomy randomised clin-ical trials(RCTs).BAO accounts for 1%of the ischaemic strokes and 5%-10%of the LVO,but 70%-80%of BAO victims evolve to death or full dependence.23 The terminal nature of the brainstem and thalamic circulations may predispose to such dismal *** two RCT designed to assess the efficacy of early(<6-8 hours since symptoms onset)thrombectomy for BAO treatment,the BEST and BASICS trials.24 Given the prior evidence for thrombectomy benefit for anterior circu-lation stroke is well settled;BAO stroke shares a similar physiopathology with very poor prog-nosis.

Traditional Chinese medicine wrist pulse-taking is associated with pulse waveform analysis and hemodynamics in hypertension

BACKGROUND： Pulse wave analysis（PWA） quantifies the phenomenon of pulse waveform propagation in patients with cardiovascular diseases, whereas pulse image analysis（PIA） is a subjective examination in traditional Chinese medicine. OBJECTIVE： This study evaluated the association of PIA with PWA and hemodynamics in patients with hypertension. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： This observational, cross-sectional study enrolled 45 patients（26 men,（55.2± 10.3） years, systolic blood pressure（155± 28） mm Hg, diastolic blood pressure（93± 17） mm Hg） for assessment of clinical and laboratorial data. MAIN OUTCOME MEASURES： Primary outcomes comprised： pattern differentiation based on an automated method; PIA at the radial artery using the ‘simultaneous pressing＇ method for identification of factors such as strength（strong/weak）, depth（superficial/deep）, and speed（fast/moderate/slow）; and PWA at the same artery using a noninvasive system. RESULTS： Significant multivariate main effects were observed for depth（l=0.648, F_（5,29）=3.149, P=0.022, h^2 =0.352）, strength（l=0.608, F_（5,29） =3.736, P=0.010, h^2 =0.392）, and speed（l=0.535, F_（5,29） =5.302, P=0.002, h^2 =0.465）. General effects comprised high values of PWA and blood pressure for superficial, strong, and fast pulse images. A strong pulse was found for pulse pressure ≥ 62.5 mm Hg and systolic blood pressure ≥ 149.5 mm Hg, whereas a superficial pulse was found for heart rate ≥ 58.25 beats/min; a fast pulse was found for heart rate ≥ 69.6 beats/min and pulse wave velocity ≥ 9.185 m/s. CONCLUSION： Associations were explained by La Place＇s law, arterial remodeling in hypertension, alongside the traditional criterion for classifying speed in pulse images. PIA is associated with PWA and hemodynamics in patients with hypertension. Systolic and pulse pressures, heart rate, and pulse wave velocity are quantitative variables that have information to describe the qualitative pulse images such as strength, depth and speed.

AIM:To determine the effect of oral sumatriptan on gastric emptying using a continuous 13 C breath test(BreathID system).METHODS:Ten healthy male volunteers participated in this randomized,2-way crossover *** subjects fasted overnight and were randomly assigned to receive a test meal(200 kcal/200 mL) 30 min after pre-medication with sumatriptan 50 mg(sumatriptan condition),or the test meal alone(control condition).Gastric emptying was monitored for 4 h after administration of the test meal by the 13 C-acetic acid breath test performed continually using the BreathID ***,using Oridion Research Software(β version),the time taken for emptying of 50% of the labeled meal(T 1/2) similar to the scintigraphy lag time for 10% emptying of the labeled meal(T lag),the gastric emptying coefficient(GEC),and the regression-estimated constants(β and κ) were *** statistical significance of any differences in the parameters were analyzed using Wilcoxon's signed-rank ***:In the sumatriptan condition,significant differences compared with the control condition were found in T 1/2 [median 131.84 min(range,103.13-168.70) vs 120.27 min(89.61-138.25);P = 0.0016],T lag [median 80.085 min(59.23-125.89) vs 61.11 min(39.86-87.05);P = 0.0125],and β [median 2.3374(1.6407-3.8209) vs 2.0847(1.4755-2.9269);P = 0.0284].There were no significant differences in the GEC or κ between the 2 ***:This study showed that oral sumatriptan significantly delayed gastric emptying of a liquid meal.
