背景右旋美托咪啶是一种仪，受体激动剂，目前正在研究其是否适合用于小儿麻醉。本研究旨在比较右旋美托咪啶一咪达唑仑与丙泊酚对于七氟烷麻醉儿童磁共振扫描（M对）中的药效学反应差异。方法40例1～10岁，ASA1或2级，拟行MRI检查的患儿，随机分为2组，两组均用七氟烷诱导，一组以右旋美托咪啶．咪达唑仑维持麻醉，另一组以丙泊酚维持麻醉。右旋美托咪啶组先静注负荷量（1μg／kg）后持续泵入（0．5μg·kg^-1·h^-1）。开始泵注后静注咪达唑仑（0．1mg／kg）。丙泊酚持续泵入（250—300μg·kg^-1·min^-1）。由一名不了解麻醉方案的护士来记录复苏时间及血流动力学变化。结果右旋美托咪啶组手术后完全恢复及出院时间明显长于（约15分钟）丙泊酚组。与丙泊酚组比较，右旋美托咪啶组心率减慢，收缩压增高。两组呼吸系统指标相近。恢复过程中，两组血流动力学反应相近。在麻醉及手术后恢复过程中，心肺指标均在同年龄段儿童的正常范围内。无麻醉意外发生。结论在MRＩ检查中，右旋美托咪啶．咪达唑仑提供足够的麻醉深度，但-I丙泊酚相比麻醉复苏时间稍长。与丙泊酚组比较，使用右旋美托咪啶的患儿心率慢、收缩压高。两种方式的呼吸系统指标相近。

Coronavirus disease 2019(COVID-19), caused by severe acute respiratory syndrome coronavirus 2(SARSCo V-2), has become a global pandemic. Clinical evidence suggests that the intestine is another high-risk organ for SARS-Co V-2 infection besides the lungs. However, a model that can accurately reflect the response of the human intestine to the virus is still lacking. Here, we created an intestinal infection model on a chip that allows the recapitulation of human relevant intestinal pathophysiology induced by SARSCo V-2 at organ level. This microengineered gut-on-chip reconstitutes the key features of the intestinal epithelium-vascular endothelium barrier through the three-dimensional(3 D) co-culture of human intestinal epithelial, mucin-secreting, and vascular endothelial cells under physiological fluid flow. The intestinal epithelium showed permissiveness for viral infection and obvious morphological changes with injury of intestinal villi, dispersed distribution of mucus-secreting cells, and reduced expression of tight junction(E-cadherin), indicating the destruction of the intestinal barrier integrity caused by ***, the vascular endothelium exhibited abnormal cell morphology, with disrupted adherent junctions. Transcriptional analysis revealed abnormal RNA and protein metabolism, as well as activated immune responses in both epithelial and endothelial cells after viral infection(e.g., upregulated cytokine genes), which may contribute to the injury of the intestinal barrier associated with gastrointestinal symptoms. This human organ system can partially mirror intestinal barrier injury and the human response to viral infection, which is not possible in existing in vitro culture models. It provides a unique and rapid platform to accelerate COVID-19 research and develop novel therapies.

Objective:Identifying novel strategies to prevent particulate matter(PM)-induced lung injury is crucial for the reduction of the morbidity of chronic respiratory *** combined intervention represented by herbal formulae for simultaneously targeting multiple pathological processes can provide a more beneficial effect than the single *** aim of this paper is therefore to design a safe and effective medicinal and edible Chinese herbs(MECHs)formula against PM-induced lung ***:PM-induced oxidative stress,inflammatory response and apoptosis A549 cell model were used to screen anti-oxidant,anti-inflammatory and anti-apoptotic MECHs,respectively.A network pharmacology method was utilized to rationally design a novel herbal *** performance liquid chromatography-mass spectrometer was utilized to assess the quality control of MECHs *** excretion of magnetic iron oxide nanospheres of the MECHs formula was estimated in *** MECH formula against PM-induced lung injury was investigated with mice ***:Five selected herbs were rationally designed to form a new MECH formula,including Citri Exocarpium Rubrum(Juhong),Lablab Semen Album(Baibiandou),Atractylodis Macrocephalae Rhizoma(Baizhu),Mori Folium(Sangye)and Polygonati Odorati Rhizoma(Yuzhu).The formula effectively promoted the magnetic iron oxide nanospheres excretion in *** mid/high dose formula significantly prevented PM-induced lung damage in mice by enhancing the activity of SOD and GSH-Px,reducing the MDA and ROS level and attenuating the upregulation of pro-inflammatory cytokine(IL-6,IL-8,IL-1βand TNFa),down regulating the protein expression of NF-κB,STAT3 and ***:Our findings suggest that the effective MECHs formula will become a novel strategy for preventing PM-induced lung injury and provide a paradigm for the development of functional foods using MECHs.

The Middle East respiratory Syndrome (MERS-CoV) is a potentially fatal respiratory viral infection that has had outbreaks in the Middle East and Asia region starting in 2012. As recent as 2015, MERS had outbreaks in South Korea, resulting in 36 deaths among 186 infected patients. The purpose of this thesis is to gain an in-depth understanding of how this viral infection behaves in outbreaks and how it is spread around the globe. To gain insight, real time analysis was performed under a case study methodology based on pooled data from the Ministry of Health & Welfare of South Korea. We calculated a striated mortality rate based on gender, age, and the presence of pre-existing health conditions ranging from heart, lung, and kidney diseases. We found that MERS outbreak follows the pattern of a point source outbreak, largely spurred by the patient-referral system under the universal healthcare system in South Korea. The purpose of this case is to discuss the benefits and challenges of such patient referral system, and to offer a possible solution in improving transparency between healthcare providers and the government under a universal healthcare system to prevent future outbreaks.

Dear Editor To date,tens of millions of people have been infected with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),causing the outbreak of the respiratory disease named the coronavirus disease 2019(COVID-19).As a newly emerged member of the coronavirus family,SARS-CoV-2 is an enveloped positive-strand RNA virus,which has probably the largest genome(approximately 30 kb)among all RNA *** nucleocapsid(N)protein of SARS-CoV-2 is mainly responsible for recognizing and wrapping viral RNA into helically symmetric structures(Malik,2020).

We evaluate recent claims opposing infant male circumcision,a procedure now supported by the evidencebased policy of the American Academy of *** find those criticisms depend on speculative claims about the foreskin and obfuscation of the strong scientific evidence supporting pediatric policy *** argument that circumcision should be delayed to allow a boy to make up his own mind as an adult fails to appreciate the psychological,scheduling and financial burdens later circumcision entails,so reducing the likelihood that it will *** contrast,early infant circumcision is convenient,safer,quicker,lower risk,healing is faster,cosmetic outcome is routinely good and the lifetime benefits accrue *** include reduction in urinary tract infections,inflammatory skin conditions,foreskin problems,and,when older,substantial protection against sexually transmitted infections and genital cancers in the male and his female sexual *** authorities regard the failure to offer parents early infant circumcision as unethical,just as it would be unethical to fail to encourage the vaccination of *** conclusion,the criticisms of evidence-based infant male circumcision policy are seriously flawed and should be dismissed as unhelpful to evidence-based development and implementation of pediatric policy intended to improve public health and individual wellbeing.
