Previous reports have suggested that Ang-(1-7)may have a protective effect in endothelial cells against high glucose(HG)-induced cell injury thanks to a modulatory mechanism in the NF-κB signaling *** this study,we have examined whether NF-κB-IL-1βand Heme oxygenase-1(HO-1)pathways contribute to the protection of Ang-(1-7)against hyperglycemia-induced inflammation and oxidative stress in human umbilical vein endothelial cells(HUVECs).Our results indicate that time-varying exposures of HUVECs,from 1 h to 24 h,to high glucose concentrations result in an increased expression of phosphorylated(p)-p65 and HO-1 in a time-dependent *** an inhibitor of NF-κB,pyrrolidinedithiocarbamic acid(PDTC)suppressed IL-1βproduction induced by *** note,HUVECs previously treated with Ang-(1-7)(2μM)for 30 min before being exposed to HG concentrations significantly ameliorated the HG-increased in p-p65 and IL-1βexpression;whereas obviously up-regulated the level of HO-1,along with inhibition of oxidative stress,inflammation,and the HG-induced ***,when HUVECs were previously treated either with PDTC or IL-1Ra for 30 min before being exposed to HG,it significantly prevented damages caused by high glucose concentrations mentioned above,while the treatment of HO-1 inhibitor Sn-protoporphyrin(SnPP)before exposure to both HG and Ang-(1-7)significantly blocked the protective effect exerted by Ang-(1-7)on endothelial cells against injuries induced by HG mentioned *** conclude,the data of this study showed that activation and inhibition of the NF-κB-IL-1βpathway and HO-1 pathway may constitute an important defense mechanism against endothelial cell damage caused by HG *** additionally gave new evidence showing that exogenous Ang-(1-7)exerts a protective effect on HUVECs against the HG-induced cell injury via the inhibition and the activation of the NF-κB-IL-1βpathway and the HO-1 pathway,respectively.

AIM: Mitochondrial DNA（ mt DNA） copy number variation（ CNV）,which reflects the oxidant-induced cell damage,has been observed in a wide range of human diseases. However,whether it correlates with heart failure,which is closely related to oxidative stress,has never been elucidated before. We aimed to systematically investigate the association between leukocyte mt DNA CNV and heart failure risk and prognosis. METHODS: A total of 1 700 hospitalized patients with heart failure and 1 700 age- and gendermatched community population were consecutively enrolled in this observational study,as well as 1 638（ 96. 4%） patients were followed prospectively for a median of 17 months（12 ~ 24 months）. The relative mt DNA copy number in leukocyte of peripheral blood or cardiac tissue was measured in triplicate by quantitative real-time PCR method. RESULTS: Patients with heart failure possessed much lower relative mt DNA copy number compared with control subjects（ P cardiovascular deaths（P cardiovascular rehospitalization（P cardiovascular mortality（ P cardiovascular deaths in patients with heart failure.

Importance of Ankle/Brachial Pressure Index in Saudi Patients with Coronary Artery Disease

Background: The ankle/brachial pressure index (ABPI) has been shown to be a good marker of systemic atherosclerosis and a powerful predictor of cardiovascular morbidity and mortality. The objective of this study was to determine the importance of measuring ABPI in Saudi patients with coronary artery disease (CAD). METHODS: This is a hospital based cross-sectional study which was conducted on all Saudi patients who underwent coronary angiography without symptoms of peripheral arterial disease at King Fahd Hospital of the University, Al-Khobar Saudi Arabia. All patients underwent measurement of their ABPI. The study was carried out between December 2010 and November 2011. RESULTS: During the study period, two hundred and five patients were included. Fifty-nine (28.8%) patients were Group II (ABPI ≤ 0.90) and the rest was Group I (ABPI > 0.90). Significant correlation was also found between low ABPI and the extent of CAD (mean number of arteries involved in Group I was 1.78 ± 0.83 compared to 2.10 ± 0.736 in Group II p = 0.011). Nevertheless, the correlation between low ABPI, and the severity of presentation of CAD were also significant (Incidence of myocardial infarction with congestive heart failure was 0.5% in Group I compared to 12% in Group II p CONCLUSION: The ABPI is an important prognostic indicator for Saudi patients with CAD.

Stromelysin-1 Gene Promoter 5A/6A Polymorphism and Plasma C-Reactive Protein in Patients with Coronary Artery Disease and Myocardial Infarction

Objective: To study the associations of stromelysin-1 (MMP3) gene 5A/6A polymorphism with plasma high-sensitivity C-reactive protein (hs-CRP) level in coronary artery disease (CAD) and myocardial infarction (MI). Methods: The MMP3 5A/6A genotypes and plasma hs-CRP levels were determined in 405 non-CAD subjects and 395 angiography-documented CAD patients, 157 with MI and 238 with non-MI. Results: The percentage of the 5A/5A genotype was significantly (p < 0.001) greater in CAD than non-CAD subjects and in MI than non-MI patients. Plasma hs-CRP level of the 5A/5A genotype was significantly (p < 0.05) higher than that of the 6A/6A genotype in CAD and MI but not in non-MI patients. On logistic regression analysis, the odds ratio of the 5A/5A genotype for CAD was 2.11 (95% CI, 1.15 - 3.88, p < 0.05) and for MI was 3.05 (95% CI, 1.54 - 6.04, p < 0.005). Conclusions: This study showed a correlation of the 5A/5A genotype of MMP3 promoter with higher plasma hs-CRP level in CAD patients with MI.
