BACKGROUND Detergent poisoning mostly occurs through oral ingestion(>85%),ocular exposure(<15%),or dermal exposure(<8%).Reports of detergent poisoning through an intravenous injection are extremely *** addition,there are very few cases of renal toxicity directly caused by ***,we report a unique case of acute kidney injury caused by detergent poisoning through an accidental intravenous *** SUMMARY A 61-year-old man was intravenously injected with 20 mL of detergent by another patient in the same room of a local *** surfactant and calcium carbonate accounted for the largest proportion of the *** patient complained of vascular pain,chest discomfort,and nausea,and was transferred to our *** hospitalization,the patient’s serum creatinine level increased to 5.42 mg/dL,and his daily urine output decreased to approximately 300 *** biopsy findings noted that the glomeruli were relatively intact;however,diffuse acute tubular injury was *** edema was also noted,and the patient underwent a total of four hemodiafiltration ***,the patient’s urine output gradually increased whereas the serum creatinine level *** patient was discharged in a stable status without any *** Detergents appear to directly cause renal tubular injury by systemic *** treating a patient with detergent poisoning,physicians should be aware that the renal function may also *** addition,timely renal replacement therapy may help improve the patient’s prognosis.

Objective To investigate CTLA4-Ig's potential role in therapy for allergic asthma by blocking B7/CD28 interactions with cytotoxic T lymphocyte antigen 4-Ig (CTLA4-Ig).Methods We divided BALB/C mice into the three groups: Sham/Sham (control), ovalbumin (OVA)/OVA and mCTLA4-Ig. Blood, bronchoalveolar lavage, histology and determination of cytokines were performed 24 hours after airway challenge. Results In the OVA/OVA group, the number of cells, the percentage of inflamed cells and the level of IL-4 in BALF were increased. Airways in our murine model for allergic asthma underwent pathological changes, which were significantly reduced after treatment with mCTLA4-Ig. Conclusion Blockage of co-stimulation with mCTLA4-Ig can inhibit allergy-specific response of T cells, and asthmatic response as well.

Asthma and chronic obstructive pulmonary disease t（COPD） are the two commonest causes of adultairflow obstruction. The fundamental dltterences anti similarities between the pathological mechanisms of asthma and COPD are well recognized.^1 Both asthma and COPD involve inflammation of airway, but the pattern and distribution of inflammation markedly differ between typical patients with asthma and COPD. Chronic lung disease is now the most important long-term pulmonary complication in daily practice, and is associated with prolonged hospitalization and long-term pulmonary and clinical problems.^2

AIM:To investigate if echocardiographic and hemodynamic determinations obtained at the time of transjugular intrahepatic portosystemic shunt(TIPS)can provide prognostic information that will enhance risk stratification of ***:We reviewed medical records of 467 patients who underwent TIPS between July 2003 and December 2011 at our *** recorded information regarding patient demographics,underlying liver disease,indication for TIPS,baseline laboratory values,hemodynamic determinations at the time of TIPS,and echocardiographic measurements both before and after *** recorded patient comorbidities that may affect hemodynamic and echocardiographic *** also calculated Model for Endstage Liver Disease(MELD)score and Child Turcotte Pugh(CTP)*** following pre-and post-TIPS echocardiographic determinations were recorded:Left ventricular ejection fraction,right ventricular(RV)systolic pressure,subjective RV dilation,and subjective RV *** recorded the following hemodynamic measurements:Right atrial(RA)pressure before and after TIPS,inferior vena cava pressure before and after TIPS,free hepatic vein pressure,portal vein pressure before and after TIPS,and hepatic venous pressure gradient(HVPG).RESULTS:We reviewed 418 patients with portal hypertension undergoing *** pressure increased by a mean ± SD of 4.8 ± 3.9 mmH g(P < 0.001),HVPG decreased by 6.8 ± 3.5 mmH g(P < 0.001).In multivariate linear regression analysis,a higher MELD score,lower platelet count,splenectomy and a higher portal vein pressure were independent predictors of higher RA pressure(R = 0.55).Three variables predicted 3-mo mortality after TIPS in a multivariate analysis:Age,MELD score,and CTP grade *** in the RA pressure after TIPS predicted long-term mortality(per 1 mm Hg change,HR = 1.03,95%CI:1.01-1.06,P < 0.012).CONCLUSION:RA pressure increased immediately after TIPS particularly in patients with worse liver function,portal hypertension,emergent TI

Interleukin-33(IL-33)is a crucial nuclear cytokine that induces the type 2 immune response and maintains immune *** fine-tuned regulation of IL-33 in tissue cells is critical to control of the type 2 immune response in airway inflammation,but the mechanism is still ***,we found that healthy individuals had higher phosphate-pyridoxal(PLP,an active form of vitamin B6)concentrations in the serum than asthma *** serum PLP concentrations in asthma patients were strongly associated with worse lung function and *** a mouse model of lung inflammation,we revealed that PLP alleviated the type 2 immune response and that this inhibitory effect relied on the activity of IL-33.A mechanistic study showed that in vivo,pyridoxal(PL)needed to be converted into PLP,which inhibited the type 2 response by regulating IL-33 *** mice heterozygous for pyridoxal kinase(PDXK),the conversion of PL to PLP was limited,and IL-33 levels were increased in the lungs,aggravating type 2 ***,we found that the mouse double minute 2 homolog(MDM2)protein,an E3 ubiquitin-protein ligase,could ubiquitinate the N-terminus of IL-33 and sustain IL-33 stability in epithelial *** reduced MDM2-mediated IL-33 polyubiquitination and decreased the level of IL-33 through the proteasome *** addition,inhalation of PLP alleviated asthma-related effects in mouse *** summary,our data indicate that vitamin B6 regulates MDM2-mediated IL-33 stability to constrain the type 2 response,which might help develop a potential preventive and therapeutic agent for allergy-related diseases.

Chronic obstructive pulmonary disease （COPD） is characterized by chronic irreversible inflammation and progressive decline in lung function. While smoking and other environmental agents （e.g., air pollutants） are common causes of COPD, the major challenges in managing this condition are the persistent inflammation and functional deterioration which are not reversed by removal of precipitating agents （e.g., smoking cessation） or treatment with anti-inflammatory agents.

Obtaining clean energy is of prime importance for planetary health and sustainable *** aimed to assess the association between residential energy transition and the risk of chronic respiratory *** data from the Global Health Observatory and Global Burden of Diseases,Injuries,and Risk Factors Study,we delineated the spatial distribution and temporal trends of the population using clean fuels for cooking at a global *** the China Health and Retirement Longitudinal Study,we performed rigorous and well-structured multistage analyses incorporating both cross-sectional and prospective data analyses to examine the associations between solid fuel use,residential energy transition,duration of solid fuel use,and the risk of chronic respiratory *** great progress,huge disparities in access to clean energy persist *** energy transition was associated with a lower risk of chronic respiratory *** the period of 2011–2013,compared with persistent solid fuel users,both participants who switched from solid to clean fuels(adjusted risk ratio[RR]0.78,95%confidence interval[CI]0.62–0.98)and persistent clean fuel users(adjusted RR 0.71,95%CI 0.57–0.89)had significantly lower risk of chronic respiratory diseases(prespiratory *** is a valuable lesson for policy-makers and the general public to accelerate energy switching to alleviate the burden of chronic respiratory diseases and achieve health benefits,particularly in low-and middle-income countries.
