Purpose.Tumour diameter,tumour height,and tumour volume are considered important pro gnostic indicators of survival in choroidal melanomas.In this study,we inves-tigated the prognostic impact on sur vival of the easily cal-culated volume estimate based on the assumption of a half-rotation ellipsoid.Methods.The largest tumour diam-eter and tumour height were measured by ultrasound A-and B-scan in 93patients with choroidal melanoma.Tumour volume was calculated by the half volume of a rotation el-lipsoid formula,rotated around the y-axis,and compared to tumour diameter and tumour height.A ll parameters were correlated to the clinical outcome o f the patients.Results.At the time of diagnosis,the mean dia meter was 10.4mm(range 4.1-18.9mm),and the mean height was 5.7mm(range 1.74-14.9mm).The range of the calculated tu-mour volume was between 11and 628mm 3 (mean volume 190mm 3 ).Among all patients,distant metastases occurred in 10patients(10.8%).In a univariate approach using Mantel-Haenszel log-rank test,the calculated tumour vol-ume at the time of diagnosis was the best prognostic indicator of survival followed by tu mour diameter and tu-mour height (P=0.028).When tumour volume,hori-zontal and vertical tumour diameter,age,sex,and primary tumour therapy were considered in a multivariate approach using Cox proportional Hazard model,only tumour volume turned out as a significant prognostic parameter(P=0.001).Conclusions.Calculated tumour volume is a bet-ter prognostic indicator of survival of patients with choroidal melanomas than the largest tumour di ameter and tumour height,and might be established in d aily routine.

Peripheral arterial disease, manifested as intermittent claudication or critical ischaemia, or identified by an ankle/brachial index < 0.9, is present in at least one in every four patients with type 2 diabetes mellitus.Several reasons exist for peripheral arterial disease indiabetes. In addition to hyperglycaemia, smoking and hypertension, the dyslipidaemia that accompanies type2 diabetes and is characterised by increased triglyceride levels and reduced high-density lipoprotein cholesterol concentrations also seems to contribute to this association. Recent years have witnessed an increased interest in postprandial lipidaemia, as a result of various prospective studies showing that non-fasting triglycerides predict the onset of arteriosclerotic cardiovascular disease better than fasting measurements do. Additionally,the use of certain specific postprandial particle markers,such as apolipoprotein B-48, makes it easier and more simple to approach the postprandial phenomenon. Despite this, only a few studies have evaluated the role of postprandial triglycerides in the development of peripheral arterial disease and type 2 diabetes. The purpose of this review is to examine the epidemiology and risk factors of peripheral arterial disease in type 2 diabetes, focusing on the role of postprandial triglycerides and particles.

We report the case of an 11-year-old boy suffering from a severe progressive chronic skin disease with clinical features of progressive systemic scleroderma , systemic lupus erythematosus and dermatomyositis. Skin biopsies revealed fibro sis and lichenoid changes and muscle biopsy a myositis. Immunohistology of the s kin showed a lichen-ruber-like pattern. Despite repeated extensive investigati ons, no autoantibodies were detectable. Some of these findings looked like those described in juvenile dermatomyositis. Finally, it could be demonstrated that the boy showed microchimerism with approximately 1%maternalCD4+lymphocytesinhisperipheralbloodleukocytes. Furthermore maternal cells could be demonstrated in inflamed muscle tissue. So a graft-versus-host -disease-like pathomechanism appears to be likely. Several systemic therapies have been used with limited success to improve the condition including corticost eroids, azathioprine, cyclosporine A and mycophenolate mofetil. A distinct impro vement of erythemas and sclerosis could be achieved by means of low-dose UVA1 p hototherapy which was applied with escalating single doses of 3-12 J/cm2 for 35 consecutive days.

Background:Loiasis is an uncommon and poorly understood parasitic disease outside endemic areas of Africa.The aim of this study was to describe the clinical and biological patterns and treatment of imported loiasis by sub-Saharan migrants diagnosed in Madrid,Spain.Methods:A retrospective study was conducted with sub-Saharan immigrants seen at the Tropical Medicine Unit of the Carlos III Hospital in Madrid,Spain,a reference center,over 19 years.Categorical variables were expressed as frequency counts and percentages.Continuous variables were expressed as the mean and standard deviation(SD)or median and interquartile range(IQR:Q3–Q1).Chi-square tests were used to assess the association between categorical variables.The measured outcomes were expressed as the odds ratio(OR)with a 95%confidential interval.Continuous variables were compared by Student’s t-tests or Mann-Whitney U tests.Binary logistic regression models were used.P<0.05 was considered a statistically significant difference.Results:One hundred thirty-one migrants from tropical and subtropical areas with loiasis were identified.Forty-nine patients were male(37.4%).The migrants’mean age(±SD)was 42.3±17.3 years,and 124(94.7%)were from Equatorial Guinea.The median time(IQR)between arrival in Spain and the first consultation was 2(1–7)months.One hundred fifteen migrants had eosinophilia,and one hundred thirteen had hyper-IgE syndrome.Fifty-seven patients had pruritus(43.5%),and thirty patients had Calabar swelling(22.9%).Seventy-three patients had coinfections with other filarial nematodes(54.2%),and 58 migrants had only Loa loa infections(45.8%).One hundred two patients(77.9%)were treated;45.1%(46/102)patients were treated with one drug,and 54.9%(56/102)patients were treated with combined therapy.Adverse reactions were described in 14(10.7%)migrants.Conclusions:Our patients presented early clinical manifestations and few atypical features.Thus,physicians should systematically consider loiasis in migrants with a typical presentation.However,considering that 7

Oralinvolvement in cutaneous CD30+T-cell lymphoid proliferations is rare and has received littleattenti on in the dermatologic literature. The authors repor t 2 patients with self-healing, recurrent papulonodular eruptions with the clas sic clinical, histopathological and immunophenotypic features of lymphomatoid pa pulosis, which developed two ulcerated papules and an ulcerative nodule on the d orsum of the tongue, respectively. The lesions appeared coincident with a new cu taneous relapse of the disease. Histopathological and immunophenotypic features were similar to those of the cutaneous lesions. All lesions regressed spontaneou sly after several weeks. Since then, and after follow-up periods of 3 and 7 yea rs, respectively, no evidence of extracutaneous involvement has been detected. O ral involvement in lymphomatoid papulosis is an uncommon event, probably without prognostic significance. Previously reported cases are reviewed. The differenti al diagnosis of atypical T-cell lymphoid infiltrates observed in the oral mucos a is discussed.

Background: Tinea capitis is the most common type of dermatophytosis in child ren, but is uncommon in the first year of life. Objectives: To review clinical, mycological and epidemiological data in a series of 10 infants aged under 1 year diagnosed as having tinea capitis in three Spanish hospitals between 1998 and 2 002. Methods: A retrospective case note study. Results There were six boys and f our girlswith a mean of age 7 months (range 1· 5- 12). All the children were b orn in Spain, but in five cases the parents were immigrants from Africa. In thes e cases the isolated dermatophytes were two Trichophyton tonsurans, one T. verru cosum and two Microsporum audouinii. Four autochthonous cases were caused byM. c anis and one by M. audouinii (but this one was in contact with African immigrant s). In two of the five cases produced by anthropophilic dermatophytes other fami ly members were infected by the same fungus. Most cases were treated successfull y with griseofulvin. Conclusions: Although tinea capitis is rare in infants in t heir first year of life, the condition should be investigated if scaling and/or alopecia are present. A thorough epidemiological study of other family members i s mandatory.

We describe a 65-year-old Caucasian man with Waldenstrm’s macroglobulinae mia who developed paraneoplastic pemphigus (PNP) 3 years after his haematologica l diagnosis. This is a very rare malignancy that is associated with PNP. The evo lution of PNP in this patient appears to exhibit the postulated immunological ph enomenon of epitope spreading.

Multiple cutaneous and uterine leiomyomas is an autosomal dominant condition t hat results in benign smooth muscle tumours of the skin and, in females, uterine fibroids. This syndrome overlaps with hereditary leiomyomatosis and renal cell cancer syndrome in which affected individuals may develop the rare type II papil lary renal cell cancer, in addition to skin leiomyomas. Recently, heterozygous m utations in the gene encoding fumarate hydratase have been found to underlie bot h conditions. Fumarate hydratase is an enzyme that catalyses the conversion of f umarate to malate in the Kreb’s cycle and may also function as a tumour suppres sor gene. We report a family with multiple leiomyomas, uterine fibroids and papi llary renal cell cancer. The proband is a 77-year-old Polish woman who develop ed multiple cutaneous leiomyomas on her right upper arm in her thirties and subs equently underwent a hysterectomy for uterine fibroids in her forties. She has f our offspring: her eldest daughter also has skin and uterine leiomyomas with a s imilar onset; her son has multiple skin leiomyomas and in addition was diagnosed with metastatic papillary renal cell cancer at the age of 50 years; the two you ngest daughters are unaffected. DNA sequencing in all the affected individuals d isclosed a heterozygous G ?úC substitution at nucleotide 173 ofthefumaratehydra tasegene,thatconvertsanarginineresidue (CGA) to proline (CCA). This missense mut ation has not been reported previously and is designated R58P. Interestingly, th e clinically asymptomatic 20-year-old son of the individual with renal cancer was also found to be heterozygous for R58P. It is likely that he will develop sk in leiomyomas in the future but the risk of renal cancer is difficult to predict . Nevertheless, detection of this mutation has important implications for screen ing and genetic counselling in this and other family members.
