Context: Diuretic-based therapy is at least as effective as newer classes of agents for hypertension. However, many patients with hypertension require treatment with more than 1 drug class to achieve blood pressure control. The relative benefits or risks of 2-drug-class combinations are not well known. Objective: To prospectively evaluate if there are differences in cardiovascular mortality among postmenopausal women with hypertension but no history of cardiovascular disease(CVD)treated with different classes of antihypertensive agents, singly or in combination. Design, Setting, and Participants: Women with hypertension enrolled in the Womens health Initiative Observational Study, a longitudinal multicenter cohort study of 93 676 women aged 50 to 79 years at baseline(1994-1998), assessed for a mean of 5.9 years. Main Outcome Measures: Relationship between baseline use of ACE inhibitors, β-blockers, calcium channel blockers, or diuretics, or a combination of these, and incidence of coronary heart disease, stroke, and CVD mortality. Results: Among 30219 women with hypertension but no history of CVD, 11 294(57%)were receiving monotherapy with an ACE inhibitor, β-blocker, calcium channel blocker, or diuretic, and 4493(23%)were treated at baseline with a combination of diuretic plus either ACE inhibitor, β-blocker, or calcium channel blocker or ACE inhibitor plus calcium channel blocker. Monotherapy with calcium channel blockers vs diuretics was associated with greater risk of CVD death(hazard ratio, 1.55; 95%confidence interval, 1.02-2.35), controlling for multiple covariates. Women treated with a diuretic plus a calcium channel blocker had an 85%greater risk of CVD death vs those treated with a diuretic plus a β-blocker, after adjustment for age, race, smoking, high cholesterol levels requiring medication, body mass index, physical activity, use of hormone therapy, and diabetes. Context: Recent cervical cancer screening guidelines state that the interval between screenings can be safely extended to 3 years in healthy women 30 years or older who have normal cytology results and have negative test results for oncogenic human papillomavirus (HPV)-DNA. Objective: To determine the incidence of squamous intraepithelial lesions (SILs) in HIV-seropositive women with nor mal cytology results, by baseline HPV DNA results. Design, Setting, and Patients: Participants were HIV-seropositive (n = 855; mean age, 36 years) and HIV-seronegative (n = 343; mean age, 34 years) US women with normal baseline cervical cytology who were enrolled in the Womens Interagency HIV Study (WIHS), a large, multi-institutional prospective cohort study. Since their recruitment during 1994-1995, WIHS participants have been followed up semi-annually with repeated Pap smears for a median of 7 years. Main Outcome Measure: The cumulative incidence of any SIL and high-grade SIL or cancer (HSIL+) was estimated according to baseline HPV DNA results, stratified by HIV serostatus and CD4 T-cell count. Results: Development of any SIL in women with negative HPV results (both oncogenic and nononcogenic) at 2 years was as follows: in HIV-seropositive women with CD4 counts less than 200/μL, 9%(95%CI, 1%-18%); with CD4 counts between 200/μL and 500/μL, 9%(95%CI, 4%-13%); and with CD4 counts greater than 500/μL, 4%(95%CI, 1%-7%). The CIs for these estimates overlapped with those for HIV-seronegative women with normal baseline cytology who were HPV-negative (3%; 95%CI, 1%-5%), indicating that at 2 years, there were no large absolute differences in the cumulative incidence of any SIL between groups. Furthermore, no HPV-negative participants in any group developed HSIL+lesions within 3 years. Multivariate Cox models showed that on a relative scale, the incidence of any SIL among HIV-seropositive women with CD4 counts greater than 500/μL (hazard ratio [HR], 1.2; 95%CI, 0.5-3.0), but not those w

Objective: To examine the association between chronic infection and cumulative burden of infection and acute coronary syndrome. Design: The 5C(Cork coronary care case-control)-study was a community based case-control study. Patients and controls underwent a standard physical examination and had blood samples taken for serological analysis for Helicobacter pylori(IgG), Chlamydia pneumoniae(IgA, IgM, and IgG), cytomegalovirus(IgG), and herpes simplex virus types 1 and 2(IgG). Setting: Patients were recruited from four hospitals in Cork City and Mal lowTown, Controls, individually matched on age and sex, were selected by incident density sampling from the same general practices as the referent case. Main outcome measures: Age and sex adjusted and fully adjusted odds ratios for acute coronary syndrome by seropositivity and by increasing number of infections. Results: Cases and controls did not differ significantly in seropositivity to C pneumoniae, cytomegalovirus, herpes simplex viruses, and H pylori. In unconditional logistic regression analysis adjusted for age, sex, waist to hip ratio, smoking, physical activity, alcohol consumption, and social class there was no evidence of an increasing risk for acute coronary syndrome with increasing burden of infection. Conclusions: The findings do not support an association between specific infectious agents and acute coronary syndrome and do not provide evidence of a burden of infection effect.

Objective: To investigate the association between the S447X polymorphis m of th e lipop.otein lipase (LPL) gene and lipop.otein lipid and blood pressure (BP) le vels in 904 Chinese subjects with essential hypertension. Method: Five hundred a nd sixty-three male and 341 female patients (aged 35-69 years) were randomly s elected from hypertensive patients diagnosed in the Community-based Comprehensi ve Studies on Prevention and Control of Hypertension Project in China (CCPACH) a nd not treated with antihypertensive medications for at least 2 weeks immediatel y before blood collection. Results: After multivariate adjustment for age, body mass index (BMI), smoking status, alcohol intake and serum glucose, the X447 all ele was significantly associated with low triglyceride, log triglyceride: high- density lipop.otein (HDL)-cholesterol ratio and high HDL-cholesterol levels co mpared with the S447S genotype, but not with BP levels in the whole study pop.la tion. However, upon stratification for dyslipidemic status, the X447 allele was associated with higher systolic blood pressure (SB P) (P< 0.05) and pulse pressure (PP) (P< 0.05) compared with the S447S genotype after multivariate adjustment in dyslipidemic subjects, but not in those without dyslipidemia. A statistically significant interaction between the LPL S447X pol ymorphism and dyslipidemic status was observed for SBP and PP levels, suggesting that dyslipidemic status might modify the effect of the LPL S447X polymorphismo n BP levels. Conclusions: Our findings suggest that a high concentration of trig lyceride and/or low concentration of HDL-cholesterol are associated with high S BP and PP in hypertensive patients with the X447 allele of the LPL gene.

Background and Purpose:Declining trends in the incidence and mortality of str oke have been observed in Finland since the beginning of the 1980s until 1997. I n this study we analyzed the trends in fatal and nonfatal strokes in Finland dur ing 1991-2002. Methods The Finnish Hospital Discharge Register was linked to th e National Causes of Death Register to produce a Cardiovascular Disease Register , which includes data on 410760 cerebrovascular events (International Statistica l Classification of Diseases, 10th Revision [ICD-10] codes 160-169) in patient s aged ≥35 years in 1991-2002. Results:Age-standardized incidence of first- ever stroke (ICD-10 codes 160-164,excluding 163.6) per 100 000 persons decline d during 1991-2002 annually by 2.2%(95%CI, -2.4%to -1.9%) among men and 2 .5%(-2.8%to -2.2%) among women aged 35 to 74 *** patients aged 75 to 8 4 years, the change in the incidence of first-ever stroke was -2.6%per year ( -3.0%to -2.2%) among men and -3.2%per year (-3.5%to -2.9%) among women . A similar trend was observed also in the oldest age group, in patients aged ≥ 85 years. Among patients aged 35 to 74 years,the 28-day case fatality of first -ever stroke declined annually by 3.2%(-3.9%to -2.5%) among men and by 3.0 %(-3.8%to -2.2%) among women. A significant decrease was found in the 28-d ay case fatalities of all subtypes of stroke in this age group. Conclusions:The favorable development in stroke incidence, mortality, and case-fatality has co ntinued in Finland during 1991-2002.

Objectives: To investigate the occurrence of rape/attempted rape and verbal sexual coercion among an ethnically diverse adolescent pop.lation and to evaluate whether unique risk factors existed for victims of either type of forced sexual experience. Design: Cross-sectional survey assessing sexual violence within the past 12 months, behaviors that occurred on each subject’s most recent date, and the occurrence of unwanted sexual experiences on any dates in the past year and details about that date. Subjects identified as experiencing either rape/attempted rape or verbal sexual coercion were compared with nonvictims using bivariate and multivariate analyses. Setting: Urban adolescent health care facility. Participant: Female adolescents and young adults (n = 689) between the ages of 14 and 23 years who presented for care and met the eligibility criteria. Results: Approximately 30%of youngwomen reported having an unwanted sexual experience in the past year. The risk of rape/attempted rape was increased by past mild to moderate (adjusted odds ratio [AOR] = 4.27) or severe (AOR = 15.24) physical aggression by the dating partner; date-specific behaviors, including decreased level of romantic involvement [AOR] = 0.63 per point decline)-and going to the perpetrator’s house to be alone (AOR = 3.01); past sexual victimization as an adolescent (AOR = 4.70); and lower levels of self-reported ethnic identity (AOR = 1.07 per point decline). More important, alcohol use by the victim or perpetrator was unrelated to an increased risk of rape/attempted rape. The risk of verbal sexual coercion was increased by past mild to moderate (AOR = 4.38) or severe (AOR = 13.79) verbal aggression from the dating partner; date-specific behaviors, including decreased level of romantic involvement (AOR = 0.70 per point decline) and greater number of past dates (AOR = 7.53); going to the perpetrator’s house to be alone (AOR = 3.52); past sexual victimization as an adolescent (AOR = 9.83); pressures
