Radiologic factors related to double-bar insertion in minimal invasive repair of pectus excavatum

Background:Pectus excavatum is the most common congenital chest wall deformity,with a high incidence in live *** study aimed to evaluate the measured factors on CT images related to the number of pectus bars for surgical ***:A total of 497 patients who had undergone minimally invasive repair between April 2007 and July 2011 were classified into single-bar(n=358)and doublebar(n=139)insertion *** measured eight distinct distances and one angle on CT scans to reflect quantitative *** analysis and multivariate logistic regression analysis were performed to detect statistically significant association between radiologic measurements and the pectus bars ***:After adjusting for age and gender,the transverse distance(T),the transverse distance of the depression area(A),the inclined distance of the depression area(B),the AP distance of the depression area(C),the depression angle(G),and the eccentric distance of deformity(E)were significantly correlated with double-bar *** regression model showed that age(,P<0.0001),gender(P<0.0001),depression angle(G)(P<0.0001),direction of the depression(DD)(P<***1)and depression depth(D)(P<0.0001)were significantly associated with doublebar ***:CT scan provides useful factors which can be of assistance in predicting the number of pectus bars for the surgical correction of pectus excavatum.

AIM To investigated the association between adenoma detection rate(ADR) and sessile serrated ADR(SSADR) and significant predictors for sessile serrated adenomas(SSA) *** This study is a retrospective, single-center analysis. Total colonoscopies performed by the gastroenterologists at the University of Tokyo Hospital between January and December 2014 were retrospectively identified. Polyps were classified as low-grade or high-grade adenoma, cancer, SSA, or SSA with cytological dysplasia, and the prevalence of each type of polyp was investigated. Predictors of adenoma and SSA detection were examined using logistic generalized estimating equation models. The association between ADR and SSADR for each gastroenterologist was investigated by calculating a correlation coefficient weighted by the number of each gastroenterologist's *** A total of 3691 colonoscopies performed by 35 gastroenterologists were assessed. Overall, 978 (26.5%) low-and 84 (2.2%) high-grade adenomas, 81 (2.2%) cancers, 66 (1.8%) SSAs, and 2 (0.1%) SSAs with cytological dysplasia were detected. Overall ADR was 29.5%(men 33.2%, women 23.8%) and overall SSADR was 1.8%(men 1.7%, women 2.1%). In addition, 672 low-grade adenomas (68.8% of all the detected lowgrade adenomas), 58 (69.9%) high-grade adenomas, 29 (34.5%) cancers, 52 (78.8%) SSAs, and 2 (100%) SSAs with cytological dysplasia were found in the proximal colon. Adenoma detection was the only significant predictor of SSA detection (adjusted OR: 2.53, 95%CI: 1.53-4.20; P < 0.001). The correlation coefficient between ADR and SSADR weighted by the number of each gastroenterologist's examinations was 0.606(P < 0.001).CONCLUSION Our results demonstrated that ADR is correlated to SSADR. In addition, patients with adenomas had a higher prevalence of SSAs than those without adenomas.

Gallstones are a common public health problem,especially in developed *** are an increasing number of patients who are diagnosed with gallstones due to increasing awareness and liberal use of imaging,with 22.6%-80% of gallstone patients being asymptomatic at the time of *** being asymptomatic,this group of patients are still at life-long risk of developing symptoms and complications such as acute cholangitis and acute biliary ***,while early prophylactic cholecystectomy may have some benefits in selected groups of patients,the current standard practice is to recommend cholecystectomy only after symptoms or complications *** reviewing the current evidence about the natural course of asymptomatic gallstones,complications of cholecystectomy,quality of life outcomes,and economic outcomes,we recommend that the option of cholecystectomy should be discussed with all asymptomatic gallstone *** of material information is essential for patients to make an informed choice for prophylactic *** is for the patient to decide on watchful waiting or prophylactic cholecystectomy,and not for the medical community to make a blanket policy of watchful waiting for asymptomatic gallstone *** patients with high-risk profiles,it is clinically justifiable to advocate cholecystectomy to minimize the likelihood of morbidity due to complications.

Zinc deficiency in patients with chronic pancreatitis

BACKGROUND Zinc is a key element in numerous proteins and plays an important role in essential cell functions such as defense against free radicals and DNA damage repair. Chronic pancreatitis(CP) is a chronic inflammation with progressive fibrosis of pancreas ultimately resulting in pancreatic exocrine insufficiency(PEI),which is associated with malnutrition. Studies analyzing zinc levels in patients with CP are sparse and lead to conflicting *** To investigate serum zinc levels in patients with CP of various *** Between October 2015 and March 2018, patients with a diagnosis of CP were identified and recruited from the Pancreatic Outpatient Clinic at the Karolinska University Hospital in Stockholm, Sweden. Demographic, clinical and laboratory data were analyzed. Etiology of CP was determined according to the MANNHEIM classification system into the following etiological subcategories:alcohol consumption, nicotine consumption, hereditary factors, efferent pancreatic duct factors and immunological factors. Effects of resveratrol and methylprednisolone on biochemical,neurobehavioral and histopathological recovery after experimental spinal cord injury

Aim:To investigate the neuroprotective effect of resveratrol in an experimentalspinal cord injury(SCI)model in ***:Male Wistar albino rats weighing200-250 g were randomized into six ***-drop trauma was performedfor *** 1 underwent laminectomy *** 2 underwent laminectomyfollowed by *** 3,4,5,and 6 underwent laminectomy followed by SCIand received resveratrol(100 mg/kg),methylprednisolone(MP)(30 mg/kg),resveratrol(100 mg/kg)plus MP(30 mg/kg),and ethanol(2%),*** were divided into two subgroups for biochemical analysis(killed at 24 h aftersurgery)and for neurobehavioral and histopathological evaluation(killed at 6weeks after surgery).Posttraumatic neurological recovery after surgery was re-corded ***:Groups 3 and 5 revealed significantly lower malon-dialdehyde,nitric oxide,xanthine oxidase,and higher glutathione levels than group4(P<0.05).Neurological recovery rates were significantly better in groups 3 and5 than group 4(P<0.05).When spinal trauma size ratios were compared,there wasno significant difference between treatment ***:Resveratroltreatment revealed better biochemical recovery in the acute stage of trauma thanMP *** resveratrol and combined treatment revealed betterneurobehavioral recovery than MP treatment;resveratrol,MP,and combined treat-ment modalities improved histopathological recovery at the same level in the finalstage of the *** studies involving different doses of resveratroland different doses combinations with MP could promise better results as eachdrug has a different anti-oxidative mechanism of action.

Prognostic analysis of patients with gastrointestinal stromal tumors： a single unit experience with surgical treatment of primary disease

Background Gastrointestinal stromal tumor （GIST）, the most common type of mesenchymal tumors of the gastrointestinal tract, is a recently recognized tumor. The biological behavior of GIST is highly variable. Surgical resection remains the major treatment for GIST. In this study we retrospectively analyzed our surgical experience with 181 GIST patients to determine the effects of the treatment and the pathological features and prognosis factors of these GIST patients. Methods The clinicopathological features and follow-up data of the 181 patients with GIST who had received surgical resection between January 1999 and December 2007 at Ren Ji Hospital were retrospectively reviewed. Immunohistochemical stains including CDl17 （KIT）, CD34, and other markers were used. Tumor size, mitotic index and other pathological parameters were recorded. According to the consensus of NIH risk-group stratification system based on maximum tumor size and mitotic index （per 50 high power field）, tumors were classified into very-low-risk group （15 tumors, 8.3%）, low-risk group （48, 26.5%）, intermediate-risk group （52, 28.7%） and high-risk group （66, 36.5%）. Prognostic factors were analyzed by Cox analysis including age, sex, tumor size, tumor site, mitotic index, NIH categories and surgical procedures. Results One hundred and seven （59.1%） of the 181 tumors were located in the stomach, 51 （28.2%） in the small intestine, 9 （5.0%） in the colon and rectum, and 14 （7.7%） in other sites including the omentum and mesentery. The median age of the patients was 58 （range, 24-84） years, and 102 patients （56.4%） were male. Tumor size ranged from 0.5 to 30 cm, while the mean size was 7.02 cm. Metastasis was found in 7 patients. One hundred and seventy-six （97.2%） of the 181 patients underwent radical resection, and among them 26 patients received extensive resection with the adjacent organ adherent to the tumors. Trends with neoadjuvant radiotherapy and clinical staging for those with rectal malignancies

AIM To see how patterns of care changed over time,and how institution type effected these *** A retrospective analysis was performed using the National Cancer Database,looking at all patients that were diagnosed with rectal cancer from 1998 to 2011. We tested differences in rates of treatment and stage migration using χ~2 tests and logistic regression models. RESULTS A review of ninety thousand five hundred and ninety four subjects underwent multimodality therapy for cancer of the rectum. Staging and response to treatment varied greatly between centers. Forty-six percent of the time staging was missing in academic practices,vs fiftyfour percent of the time in community centers(P surgery in *** two years this increased to fifty-three percent. This increased to eighty-six percent by 2011(P surgery in academic centers in 1998. Thirty-nine percent followed this pattern in the same year in the community(P < 0.001). By 2011,ninety-one percent received radiation before their procedure in academic centers,vs eighty-four percent in the community(P < 0.001). Rates of adoption were better in academia,although an increase was seen in both center types. CONCLUSION From the study dates of 1998 to 2011,preoperative treatment with radiation has been on the rise. There is certainly an increased rate of use of radiation in academia,however,this trend is also seen in the community. Practice patterns have evolved over time,although rates of assigning clinical stage are grossly underreported prior to initiation of preoperative therapy.
