Objective:This study aimed to determine the likelihood of gestational diabetes mellitus(GDM)in subsequent pregnancy among women without GDM history and to identify risk factors for GDM in subsequent ***:This retrospective cohort study involved participants who delivered twice in same hospital of 18 research centers when delivered the second baby from January 2018 to December *** 6204 women were enrolled and 5180 women without GDM history were analyzed *** were categorized as non-GDM or GDM based on the blood glucose values of the subsequent pregnancy,and the characteristics and GDM risk of these groups were compared.A univariate analysis of potential risk factors was performed using the Chi-squared test and/ort-test for qualitative or quantitative variables,*** withP values<0.1 were chosen to be included in the multivariate binary logistic regression ***:In primary analysis of 6204 women,the incidence of GDM in subsequent pregnancy is 48.9%(490/1002)in women with GDM history and 16.1%(835/5202)in women without GDM *** a further analysis for 5180 women without GDM at index pregnancy,compared with the non-GDM group,the GDM group had a significantly higher age,prepregnancy body mass index,and blood glucose value at each oral glucose tolerance test(OGTT)timepoint(fasting,1 h and 2 h)during the index and subsequent pregnancies,as well as higher weight retention during the interval between the two pregnancies(P<0.001).Age above 35 years in subsequent pregnancy(odds ratio(OR)=1.540,95%confidence interval(CI)=1.257-1.886,P<0.001),macrosomia in index pregnancy(OR=1.749,95%CI=1.277-2.395,P=0.001),OGTT blood glucose values in index pregnancy(fasting,OR=2.487,95%CI=1.883-3.285,P<0.001;1 h,OR=1.142,95%CI=1.051-1.241,P=0.002;2 h,OR=1.290,95%CI=1.162-1.432,P<0.001)and weight retention(OR=1.052,95%CI=1.035-1.068,P<0.001)were independent risk factors for GDM in subsequent ***:For women without G

The interaction of sperm with the egg＇s extracellular matrix, the zona pellucida （ZP） is the first step of the union between male and female gametes. The molecular mechanisms of this process have been studied for the past six decades with the results obtained being both interesting and confusing. In this article, we describe our recent work, which attempts to address two lines of questions from previous studies. First, because there are numerous ZP binding proteins reported byvarious researchers, how do these proteins act together in sperm-ZP interaction？ Second, why do a number of acrosomal proteins have ZP affinity？ Are they involved mainly in the initial sperm-ZP binding or rather in anchoring acrosome reacting/reacted spermatozoa to the ZP？ Our studies reveal that a number of ZP binding proteins and chaperones, extracted from the anterior sperm head plasma membrane, coexist as high molecular weight （HMW） complexes, and that these complexes in capacitated spermatozoa have preferential ability to bind to the ZP. Zonadhesin （ZAN）, known as an acrosomal protein with ZP affinity, is one of these proteins in the HMW complexes. Immunoprecipitation indicates that ZAN interacts with other acrosomal proteins, proacrosin/acrosin and sp32 （ACRBP）, also present in the HMW complexes. Immunodetection of ZAN and proacrosin/acrosin on spermatozoa further indicates that both proteins traffic to the sperm head surface during capacitation where the sperm acrosomal matrix is still intact, and therefore they are likely involved in the initial sperm-ZP binding step.

BACKGROUND: A twin gestation comprising a complete hydatidiform mole and a coexisting normal fetus is a rare and high-risk condition. Only a few such gestations have resulted in live infants. We report a case with a very large molar component presenting as a placenta previa. CASE: A live infant was delivered by cesarean at 31 weeks of gestation. The delivery incorporated prophylactic temporary balloon occlusion of the internal iliac arteries. The patient did not develop persistent gestational trophoblastic disease. CONCLUSION: We recommend that intra-arterial balloon catheters be considered before cesarean delivery in cases of complete hydatidiform mole with a coexisting normal fetus if the molar pregnancy presents as a previa. The size of the molar gestation is not an independent risk factor for persistent or metastatic disease.

目的比较组织学确认的宫颈上皮内瘤变的妇女在完成以连续细胞学涂片正常的治疗后随访并重返常规筛查与初始细胞学涂片结果正常的妇女发生宫颈癌风险。设计人群为基础的队列研究，数据来源于一个国家范围内的病理登记系统。研究背景荷兰，1994年至2006年。研究对象38956例经组织学确诊的宫颈上皮内瘤变1～3级的妇女，完成了治疗后随访。干预宫颈上皮内瘤变常规治疗后的随诊，推荐6、12、24个月涂片检测。主要结局测量宫颈上皮内瘤变后从完成阴性试验结果的随诊到下次基本检测期间宫颈癌的发生率。经调整随诊年，与初始涂片检测结果正常后时期比较的10年危害比。结果完成治疗后随诊的宫颈上皮内瘤变妇女在56956妇女年中共有20例宫颈癌被确诊。而既往细胞学检查阴性的妇女在25020697妇女年中发生宫颈癌1613例。两组人群宫颈癌的发生率分别为35．1（95％可信区间21．4～54．2）／每100000妇女年和6．4（6．1～6．8）／每100000妇女年，其校正危害比为4．2（95％可信区间2．7～6．5）。在所有年龄段中危害比都增加。宫颈上皮内瘤变不同级别间比较，宫颈癌的发生风险无明显差别。结论以往观察到的宫颈上皮内瘤变患者治疗后发生宫颈癌的额外风险也在完成治疗后随诊且连续3次细胞涂片都正常的妇女中观察到。经过校正后，这些妇女发生宫颈癌的风险（35例／每100000妇女年）是既往细胞学正常妇女（6例／每100 000妇女年）的4倍。
