Immunoglobulin light chain amyloidosis(AL)is one of the most frequent causes of cardiac amyloidosis(CA),[1]with an estimated prevalence of 8 to 12 per million.[2−4]Multiple myeloma(MM)is hematological neoplasia originating from plasma cells,which is the most common disease that can lead to CA.The median age of patients with MM at diagnosis is about 65 years old.In this age group,cardiovascular diseases often co-exist,increasing the risk of adverse events related to MM treatment.By convention,the prognosis of AL-CA with MM is extremely poor,with a median survival time of five months.[5]The degree of cardiac involvement has a decisive impact on the prognosis of AL-CA pati-ents.

The role of electromechanical dyssynchrony in heart failure gained prominence in literature with the results of trials of cardiac resynchronization therapy(CRT).CRT has shown to significantly decrease heart failure hospitalization and mortality in heart failure patients with dyssynchrony.Current guidelines recommend the use of electrical dyssynchrony based on a QRS>150 ms and a left bundle branch block pattern on surface electrocardiogram to identify dyssynchrony in patients who will benefit from CRT implantation.However,predicting response to CRT remains a challenge with nearly one-third of patients gaining no benefit from the device.Multiple echocardiographic measures of mechanical dyssynchrony have been studied over the past two decade.However,trials where mechanical dyssynchrony used as an additional or lone criteria for CRT failed to show any benefit in the response to CRT.This shows that a deeper understanding of cardiac mechanics should be applied in the assessment of dyssynchrony.This review discusses the evolving role of imaging techniques in assessing cardiac dyssynchrony and their application in patients considered for device therapy.

In a recent study published in Nature,Takeuchi et al.used a multiomics strategy in humans to provide compelling evidence that increased carbohydrate metabolism by the gut microbiota contributes to insulin resistance(IR).The authors uncovered a previously unknown link between elevated gut monosaccharides and host inflammatory cytokines and suggest potential intervention opportunities to combat IR by targeting specific gut microbial species.

Coronary artery bypass grafting(CABG)tra-ditionally involves revascularization of the left anterior descending artery(LAD)as it is usually of greater prognostic value.[1]Isolated CABG for non-LAD lesions have also been descri-bed in literature.However,these were often in the setting of re-do CABG[2,3]or in patients with isol-ated right sided coronary lesion and concomitant valvular heart disease.

Dear editor,Post-cardiac injury syndrome(PCIS) refers to a group of syndromes characterized by inflammation of the pericardium, pleura, and lung parenchyma after various heart injuries, including cardiac surgery or trauma, acute myocardial infarction(AMI), radiofrequency catheter ablation, coronary intervention, pacemaker implantation and pericardiocentesis.[1-5]Although advanced PCIS is well understood after cardiac surgery and AMI, known as post-pericardiotomy syndrome and post-myocardial infarction syndrome,[6,7]early-onset cases of PCIS after coronary intervention are rarely reported in China.

BACKGROUND One of the efforts to reduce hypertension rates in the community is through an educational campaign that refers to the NIH's National Heart,Lung,and Blood Institute curricula or abbreviated as NHLBI.However,during the coronavirus disease 2019(COVID-19)pandemic,one of the hardest hit areas is health promotion,and there is a significant obstacle regarding the most effective way to transfer knowledge,attitude and practice towards society without transmitting the virus.AIM To evaluate the impact of the virtual anti-hypertensive educational campaign towards knowledge,attitude,and the practice of hypertension management in the primary care setting during the COVID-19 pandemic.METHODS An online action research with a randomized crossover-controlled trial using a pretest-posttest control group design.The study was conducted in October 2020-April 2021.The population in this study were patients with hypertension who were treated in the Mojo primary health care setting.A purposive sampling technique was done to receive 110 participants using an online questionnaire and invitation letter.RESULTS A total of 110 participants were included in the analysis,55 in the intervention group and 55 in the control group.Following the Virtual Anti-Hypertensive Educational Campaign implementation,the only parameter that showed significant improvement was knowledge and attitude(P<0.001).There is no significant change in the practice parameters(P=0.131).CONCLUSION The Virtual Anti-Hypertensive Educational Campaign implementation in our study population seems to be effective to improve knowledge and attitude of participants,nevertheless,this program seems to be ineffective to improve the practice of hypertension management aspect in participants.Future study with longer durations and more comprehensive programs need to be done to scrutinize the clinical impact of this program nationwide.

Ideal Cardiovascular Health Metrics and Coronary Artery Calcification in Northern Chinese Population:A Cross-sectional Study

Objective Coronary artery calcification（CAC） is a well-established risk predictor of coronary heart disease events and is recognized as an indicator of subclinical atherosclerosis.Methods A cross-sectional study consisting of 2999 participants aged ≥40 years from the Jidong community of Tangshan City,an industrial and modern city of China,was conducted between 2013 and 2014 to examine the association between the ideal cardiovascular health（CVH） metrics and CAC.The ideal CVH metrics were determined based on the definition of the American Heart Association（AHA）.The participants were then grouped into 4 categories according to the quartiles of their CVH metric scores as follows：first quartile（0-2）,second quartile（3）,third quartile（4）,and fourth quartile（5-7）.CAC was assessed by using high-pitch dual-source CT,and patients were identified based on thresholds of 0,10,100,or 400 Agatston units,as per common practice.Results The prevalence of subclinical atherosclerosis was 15.92%,13.85%,6.76%,and 1.93%,determined by using the CAC scores at thresholds of 0,10,100,and 400 Agatston units,respectively.Compared with the group in the first quartile,the other three CVH groups had a lower odds ratio of CAC 0 after adjusting for age,sex,income level,education level,and alcohol use in the logistic regression analysis.The odds ratios in these groups were 0.86 [95% confidence interval（CI）,0.63-1.17;P0.05],0.75（95% CI,0.55-1.02;P0.05）,and 0.49（95% CI,0.35-0.69;P0.05）,respectively.These associations of CAC with the CVH metrics were consistent when different CAC cutoff scores were used（0,10,100,or 400）.Conclusion The participants with more-ideal cardiovascular metrics had a lower prevalence of subclinical atherosclerosis determined according to CAC score.Maintaining an ideal cardiovascular health may be valuable in the prevention of atherosclerosis in the general population.

Background The benefits of exercise-based cardiac rehabilitation(EBCR)programs in post-acute myocardial infarction(AMI)patients have been demonstrated.Our aim was to assess the impact of EBCR in³70-years-old vs.younger post-AMI patients.Methods We retrospectively evaluated patients who underwent a supervised EBCR protocol,twice a week during 6-12 weeks.We evaluated changes in several outcomes based on pre-and post-CRP assessments.Results Of a total of 1607 patients,333(21%)were³70-years-old.After the EBCR,an overall improvement on functional capacity,daily physical activity,lipid profile,body mass index,glycated hemoglobin(HbA1c),N-terminal pro-brain natriuretic peptide(NT-pro-BNP)and C-reactive protein was observed in both younger and older patients(P<0.05).Older patients showed a smaller benefit on the increment of daily physical activity and lipid profile improvement,but a larger reduction in NT-pro-BNP.In the multivariate analysis,only improvements on daily physical activity and HbA1c were dependent on age.Conclusion As their younger counterparts,older patients,significantly improved functional capacity,metabolic parameters and level of daily physical activity after EBCR.

Patients with peripheral arterial disease(PAD)and critical limb ischemia are at risk for limb amputation and require urgent management to restore blood flow.Patients with PAD often have several comorbidities,including chronic kidney disease,diabetes mellitus,and hypertension.Diagnostic and interventional angiography using iodinated contrast agents provides excellent image resolution but can be associated with contrast-induced nephropathy(CIN).The use of carbon dioxide(CO2)as a contrast agent reduces the volume of iodine contrast required for angiography and reduces the incidence of CIN.However,CO2 angiography has been underutilized due to concerns regarding safety and image quality.Modern CO2 delivery systems with advanced digital subtraction angiography techniques and hybrid angiography have improved imaging accuracy and reduced the incidence of CIN.Awareness of the need for optimal imaging conditions,contraindications,and potential complications have improved the safety of CO2 angiography.This review aims to highlight current technological advances in the delivery of CO2 in vascular angiography for patients with PAD and critical limb ischemia,which result in limb preservation while preventing kidney damage.
