External quality assessment of the detection of rickettsioses in China

Objective:To report a training course on the laboratory diagnoses of rickettsioses that 10 provincial/city CDCs participated in laboratory external quality assurance(EQA) based on the serological specific antibodies detection and rapid PCR amplifying targeted genes of rickettsiae. Methods:An EQA program to evaluate the following laboratory procedures was developed to detect rickettsiae:(1) immunofluorescent assay(IFA) to detect specific antibodies of A. phagocytophilum,*** and *** respectively.(2) Two sets of nested PCR were used amplifying groEL genes for most members of the family Rickettsiaceae and amplifying 16SrRNA genes for the most members of family anaplasmae,respectively.A scoring scheme based on the distribution of the median antibody titer values of the serologic assays was designed and a ranking list of the scores of the PCR results based on the detected minimal copy numbers of reference DNA was ***:Among nine laboratories who reported the results on time,eight laboratories gave acceptable serologic results,the other one provided an unacceptable antibody titer(1:2 vs median 1:64) results for *** limits of detection(LOD) for the PCR amplifying for five references DNA ranged from 1copy/μL to 10~6 copy/μ***:We successfully trained and popularized modern diagnostic methods of rickettsiae in 10 provincial CDCs in China and first conducted the EQA projects and evaluated the results.

Objective:To observe the effect of common salt(NaCl) on immature stages of laboratory reared Aedes aegypti(L).Methods:A laboratory colony of Aedes aegypti mosquitoes of Rajahmundry strain was established in the laboratory of National Institute for communicable Disease(NICD), Rajahmundry unit at(26±2)℃with relative humidity of(70±10)%.1.00%,1.25%and 1.50% solutions of common salt(NaCl) were selected to observe the susceptibility status of immature stages of Aedes aegypti in ***:Fifty percent larvae of Aedes aegypti died within 19,31 and 48 hours when exposed to 1.50%,1.25%and 1.00%common salt solution, *** percent of the larvae died within 29,57 and 108 hours when exposed to the same salt solutions,*** high pupal mortality was observed varying from 81.8% to 40.0%.Formation of pupae was found inversely proportional in the presence of concentration of common salt in breeding ***:With easy availability,less toxicity and long lasting nature,common salt may be applied in unused containers,especially in junkyards where surveillance mechanism is poor along with other conventional vector control methods in order to control breeding of Aedes aegypti,the vector of dengue/ dengue hemorrhagic fever and chikungunya.

Due to exponential increases in incidences,low risk papillary thyroid microcarcinoma(PTMC)has become a clinical and social issue in recent *** active surveillance(AS)management approach is an alternative to immediate surgery for patients with low risk *** decreased doubts about the safety and validity due to evidence from a large number of studies,the AS approach has become increasingly popular ***,Chinese thyroid surgeons still lag behind other countries in their knowledge of clinical practices and research related to *** promote the implementation of AS in China,thyroid surgeons should understand the implications,advantages,and disadvantages of management approaches for AS,and should also consider the willingness of Chinese patients,the impact on the medical billing system,and the enthusiasm of ***,a management approach for AS based on the Chinese population should be developed to reduce the risk of disease progression and enhance patient ***,we summarize the recent research achievements and deficiencies in AS approaches,and describe the initial experiences regarding AS in the Chinese population,in order to assist Chinese thyroid surgeons in preparing for AS management in the era of PTMC precision medicine.

The Boston Model describes a successful rodent management plan that succeeded in a first-world city in the *** third-world cities, which often contain informal shack settlements, it is debatable whether the Boston Modelwould apply. In Durban, a major harbor city of three million people on the east coast of South Africa, we investigatedthe sanitary risks due to rodents in both formal (residential and commercial) and informal (shacks) sectors,and we evaluated the relative merits of different management interventions suggested by the Boston Model. Bloodand tissue samples of six species (Rattus norvegicus, R. tanezumi, R. rattus, Mus musculus, Mastomys natalensis,Tatera brantsi) from 262 live-trapped rodents from 54 localities were tested for antibodies or DNA for plague (n =193: antibody test), leptospirosis (n = 221 for antibody test;n = 69 for polymerase chain reaction test for DNA) andtoxoplasmosis (n = 217: antibody test). We conducted a socioeconomic survey of 90 household to determine environmentaland socioeconomic disease risk factors in the shack settlement of Cato Crest . No rodents wereseropositive for plague, but nine Norway rats, R. norvegicus (4.1% of the sample tested) were seropositive fortoxoplasmosis, and 22 R. norvegicus (10.0% of sample tested) were seropositive for leptospirosis. Disease endemicareas were concentrated in Cato Crest and the commercial district of Durban. Serology tests of humans living inCato Crest (n = 219) showed 0% exposure to plague, 23% to leptospirosis and 35% to toxoplasmosis. Comparedwith shack-dwellers, the residents of brick houses had slightly lower levels of exposure to leptospirosis andtoxoplasmosis. Based on our results, environmental hygiene and rodent-trapping campaigns were launched in CatoCrest. The initiative owes much of its current success to implementation of the principles inherent in the BostonModel, even though certain elements were lacking.

Objectives: To investigate the mutation of the basic core promoter (BCP) of hepatitis B virus (HBV) and clarify the significance of HBV quasispecies groups in patients with chronic HBV infection. Methods: A set of specific primers was synthesized according to the HBV DNA sequence of a Chinese strain. The BCP was amplified by PCR method from the serum of 40 patients with chronic HBV infection, and the PCR products of 2 patients were subcloned into pGEM Teasy vectors. Polyacrylamide gel elec- trophoresis (PAGE) was employed to display the de- letion mutations, and clones with differential length were selected to be sequenced. Sequence comparison was made to find the difference. Results: Two or three bands were displayed by PAGE in 60% patients. The results of sequence anal- ysis showed that there are some kinds of mutations in the BCP region. The substitution always occurs in TATA-like boxes, especially from T to C on 140 site. The deletion mutations were detected in TA1, TA2 and TA3. The 8bp, 20bp deletion mutations fre- quently happened. Conclusions: There is a hot deletion region in the BCP. The deletion and the substitution in the TATA- like box may influence the expression of preC/C pro- tein. The sequencing results indicate that there are HBV quasispecies groups in patients with chronic HBV infection.

Zambia is among the sub-Saharan countries highly burdened with tuberculosis(tb) and has an estimated prevalence rate of 638 per 100 000 population in those aged 15 years and *** mining industry is the main contributor to the country's gross national product,although it is associated with public health challenges,with tb in the mines being among the occupational health diseases having a negative economic impact and threatening to delay the control of tb in the *** reviewed available evidence on the extent of the burden of tb in the mines so as to inform the development of targeted interventions for the post-2015 End tb *** was a review of published data from Medline/Pubmed,Cochrane Library and Embase,including unpublished "grey"literature on the burden of tb and the risk factors of tb in the mines of *** is limited research in Zambia to fully understand the burden of tb and risk factors associated with tb in the ***,the few studies and data available have shown that tb is a significant health problem requiring interventions to improve the quality of life of miners,ex-miners and surrounding *** is a potential problem in the mines of Zambia and the actual burden needs to be *** to silica as a risk factor needs further investigation.

Preliminary Study on Drug Susceptibility Profile and Resistance Mechanisms to Macrolides of Clinical Isolates of Non-tuberculous Mycobacteria from China

Objective Macrolide susceptibility and drug resistance mechanisms of clinical non-tuberculous mycobacteria（NTM） isolates were preliminarily investigated for more accurate diagnosis and treatment of the infection in China. Methods Four macrolides, including clarithromycin（CLAR）, azithromycin（AZM）, roxithromycin（ROX）, and erythromycin（ERY）, were used to test the drug susceptibility of 310 clinical NTM isolates from six provinces of China with the broth microdilution method. Two resistance mechanisms, 23 S r RNA and erm, were analyzed with nucleotide sequence analysis. Results Varied effectiveness of macrolides and species-specific resistance patterns were observed. Most Mycobacterium abscessus subsp. massiliense were susceptible and all M. fortuitum were highly resistant to macrolides. All the drugs, except for erythromycin, exhibited excellent activities against slow-growing mycobacteria, and drug resistance rates were below 22.2%. Only four highly resistant strains harbored 2,058/2,059 substitutions on rrl and none of other mutations were related to macrolide resistance. G2191 A and T2221 C on rrl were specific for the M. abscessus complex（MABC）. Seven sites, G2140 A, G2210 C, C2217 G, T2238 C, T2322 C, T2404 C, and A2406 G, were specifically carried by M. avium and M. intracellulare. Three sites, A2192 G, T2358 G, and A2636 G, were observed only in M. fortuitum and one site G2152 A was specific for M. gordonae. The genes erm（39） and erm（41） were detected in M. fortuitum and M. abscessus and inducible resistance was observed in relevant sequevar. Conclusion The susceptibility profile of macrolides against NTM was demonstrated. The well-known macrolide resistance mechanisms, 23 S r RNA and erm, failed to account for all resistant NTM isolates, and further studies are warranted to investigate macrolide resistance mechanisms in various NTM species.
