Laparoscopic versus open resection of gastric gastrointestinal stromal tumors

The aims of this study were to explore whether laparoscopic surgical resections of gastric gastrointestinal stromal tumors （GISTs） would produce better perioperative and similar oncologic outcomes compared with open surgical resection in Chinese patients. Thirty-six gastric GISTs cases were divided into a minimally invasive laparoscopic group and open resection group, depending on the surgical approach that was used. The general preoperative information, operative time, incision length, intraoperative blood loss, postoperative time to first flatulence, postoperative complications, postoperative hospital stay, total hospitalization costs, and such follow-up data as recurrence, metastasis, and mortality rates were compared between two groups. Among the 36 gastric GISTs, 15 received laparoscopic surgical treatment （laparoscopy group, n=15）, and 21 received routine open resection treatment （open resection group, n=21）. The laparoscopy group and the open resection group showed statistically significant differences （P〈0.05） in incision length （7.8±2.3 vs. 16.9±3.8 cm）, postoperative time to first flatulence （3.8±1.3 vs. 5.1±2.1 d）, postoperative hospitalization time （7.6±2.5 vs. 11.3±3.7 d）, and total cost of hospitalization （RMB 28,239±5,521 vs. RMB 23,761±5,362）. There were no statistically significant differences （P〉0.05） between the laparoscopy group and the open resection group in operative time （147.8±59.3 vs. 139.2±62.1 min） and intraoperative blood loss （149.8±98.9 VS. 154.2±99.3 mL）. Both groups had no postoperative complications, no recurrence and metastasis, and no postoperative mortality. There were no statistically significant differences between the two groups in postoperative complications, postoperative recurrence and metastasis, and postoperative mortality. In conclusion, compared with open resection, the laparoscopic resection of gastric GISTs offers the advantages of less trauma, faster recovery, and shorter hospital stay.

Effects of probiotic therapy on hepatic encephalopathy in patients with liver cirrhosis:an updated meta-analysis of six randomized controlled trials

BACKGROUND： Liver cirrhotic patients with hepatic encephalopathy have poor prognosis. Probiotics alter the intestinal microbiota and reduce the production of ammonia. We conducted a meta-analysis about the role of probiotics on liver cirrhotic patients with hepatic *** SOURCES： We collected the relevant literatures up to February 21, 2014 from databases of PubMed, EMBASE and the Cochrane Central Register of Controlled Trials. A statistical analysis was conducted by RevMan 5.2 and STATA 12.0 ***： Six randomized controlled trials involving 496 liver cirrhotic patients were included. The results showed that probiotic therapy significantly reduced the development of overt hepatic encephalopathy（OR [95% CI]： 0.42 [0.26, 0.70],P=0.0007）. However, probiotics did not affect mortality, levels of serum ammonia and constipation（mortality： OR [95% CI]：0.73 [0.38, 1.41], P=0.35; serum ammonia： WMD [95% CI]：-3.67 [-15.71, 8.37], P=0.55; constipation： OR [95% CI]： 0.67 [0.29,1.56], P=0.35）.CONCLUSION： Probiotics decrease overt hepatic encephalopathy in patients with liver cirrhosis.

Folic acid contributes to peripheral nerve injury repair by promoting Schwann cell proliferation, migration, and secretion of nerve growth factor

After peripheral nerve injury, intraperitoneal injection of folic acid improves axon quantity, increases axon density and improves electromyography results. However, the mechanisms for this remain unclear. This study explored whether folic acid promotes peripheral nerve injury repair by affecting Schwann cell function. Primary Schwann cells were obtained from rats by in vitro separation and culture. Cell proliferation, assayed using the Cell Counting Kit-8 assay, was higher in cells cultured for 72 hours with 100 mg/L folic acid compared with the control group. Cell proliferation was also higher in the 50, 100, 150, and 200 mg/L folic acid groups compared with the control group after culture for 96 hours. Proliferation was markedly higher in the 100 mg/L folic acid group compared with the 50 mg/L folic acid group and the 40 ng/L nerve growth factor group. In Transwell assays, the number of migrated Schwann cells dramatically increased after culture with 100 and 150 mg/L folic acid compared with the control group. In nerve growth factor enzyme-linked immunosorbent assays, treatment of Schwa nn cell cultures with 50, 100, and 150 mg/L folic acid increased levels of nerve growth factor in the culture medium compared with the control group at 3 days. The nerve growth factor concentration of Schwann cell cultures treated with 100 mg/L folic acid group was remarkably higher than that in the 50 and 150 mg/L folic acid groups at 3 days. Nerve growth factor concentration in the 10, 50, and 100 mg/L folic acid groups was higher than that in the control group at 7 days. The nerve growth factor concentration in the 50 mg/L folic acid group was remarkably higher than that in the 10 and 100 mg/L folic acid groups at 7 days. In vivo, 80 μg/kg folic acid was intraperitoneally administrated for 7 consecutive days after sciatic nerve injury. Osteopontin is a promoter for hepatocellular carcinoma metastasis： a summary of 10 years of studies

In this review, we summarize the novel findings from our series of studies on the leading metastasis- related gene, osteopontin （OPN）. In our previous gene expression profiling study, OPN was identified as one of the leading genes associated with the metastasis of hepatocellular carcinoma （HCC）. We focused on OPN to evaluate its prognostic values and important roles in HCC metastasis. A retrospective study of large cohorts of HCC patients demonstrated that plasma OPN level was one of the leading independent prognostic factors for HCC patients, even in the early stage of HCC, and could serve as a surrogate serologic biomarker for monitoring the treatment response and tumor recurrence after HCC resection. Using both in vitro and in vivo investigations, we found that OPN has an important role in metastasis and tumor growth of HCC and is an attractive potential therapeutic target for combating HCC metastasis. We also found that OPNx HCC cells have much more amplifications at chromosomal regions, and promoter polymorphisms are important in the regulation of OPN expression and tumor growth and lung metastasis of HCC.

LIVER ALVEOLAR HYDATIDOSIS IN XINJIANG

In 45 cases of alveolar hydatidosis caused by Echinococcus multilocularis, the chief clinical manifestation was firm, round and rough masses located on the upper right abdomen for 2 months to 7 years. Jaundice developed in 10. Casoni’s intradermal test was positive in all; but indirect hemagglutination test and immunoelectrophoresis were positive in 8. Eosinophilia was always noted. Other examinations included ultrasonic exploration, roentgenography and radioisotope liver scanning. Hemihepatectomy was performed in cases that met the indications for operation. Most of the patients had palliative resection. The general condition of all these patients improved after operation.

A RETROSPECTIVE STUDY ON DIAGNOSIS AND TREATMENT OF SEVERE ACUTE PANCREATITIS

Severe acute pancreatitis is highly controversial on its diagnositic criteria, the optimum time for surgery, the selection of surgical procedures, and the prevention and treatment of complications. We treated 40 patients with severe acute pancreatitis from July 1983 to July 1988. The comparison of clinical and laboratory data of severe acute pancreatitis and mild acute pancreatitis showed that in some patients neither Ranson’s nor Bank’s criteria are reliable in classifying or predicting the severity of the disease. The coexistence of acute peritonitis and bloody ascites with elevated amylase level is very helpful to identify the local conditions of pancreatic necrosis and *** suggest early operation (within 48 hours) be applied in severe acute pancreatitis. In our series, five types of surgical procedures were used. We consider that proper treatment of acute respiratory distress syndrome (ARDS) is most important in the management of severe pancreatitis.

Historical perspective of living donor liver transplantation

Living donor liver transplantation (LDLT) has gone through its formative years and established as a legitimate treatment when a deceased donor liver graft is not timely or simply not available at all. Nevertheless, LDLT is characterized by its technical complexity and ethical controversy. These are the consequences of a single organ having to serve two subjects, the donor and the recipient, instantaneously. The transplant community has a common ground on assuring donor safety while achieving predictable recipient success. With this background, a reflection of the development of LDLT may be appropriate to direct future research and patient-care efforts on this life-saving treatment alternative.

The relationship between multiple clinicopathological features and nerve invasion in pancreatic cancer

BACKGROUND:Nerve invasion is a specific type of tumor expansion and characteristic manifestation of pancreatic cancer(PC),with an incidence rate ranging from 50% to 100%.It is an important prognostic factor for pancreatic cancer,and its early detection is helpful in the management of the *** study was undertaken to analyze retrospectively the relationship between neural invasion and multiple clinicopathological features and to provide evidences for clinicians in the management of neural invasion in patients with ***:Formalin-fixed paraffin-embeded specimens of PC taken from 215 patients were examined for the presence of neural invasion under a light *** was the relationship between neural invasion and multiple clinicopathological feature including preoperative fasting blood glucose level,amylase level,serum CA19-9 level,abdominal pain,lumbar and back pain,and the expressions of p53 and Ki67 in tumor ***:Preoperative fasting blood glucose level,serum CA19-9 level and p53 positive cells in cancer tissue were increased with the rise of pathological grade(P<0.05).These indices were significantly higher in patients with neural invasion than in those without(P<0.05).Further analysis revealed a positive correlation between p53 and Ki67 overexpression and lymphatic metastasis(P<0.05).Referred pain was positively correlated with neural invasion(P<0.05).Patients with PC perineural invasion were more likely to have a higher pathological grade(P<0.05).CONCLUSIONS:Our data indicated that the preoperative fasting blood glucose level,serum CA19-9 level,and referred pain are novel predictive markers for neural invasion in patients with PC.p53 and Ki67 play important roles in neural invasion of *** of hyperglycemia may serve as an auxiliary treatment to curb neural invasion in PC.
