Dear Editor,Tumor-suppressor protein p53 is important for cell function and genome integrity.1 The decrease of p53 protein is a common feature of human malignant tumors,which leads to the deficiency of cell cycle detection point control and apoptosis *** enormous amount of research effort goes into small molecules that regulate p53,including wild-type repair of mutant p53 gene and interruption of the binding between p53 and an E3-ubiquitin ligase Mdm2 to prevent ubiquitination degradation of p53 and rescue the protein level of p53.2 However,it is not clear whether it is necessary to continue to inhibit the ubiquitin activity of proteasome in order to restore the p53 protein level.

Background Myxoma is the most frequent cardiac benign tumor which requires surgical removal even though in right atrium. Minimally invasive surgery has become the standard approach for removal of myxoma in our center. Herein, we report our initial experience of cardiac myxoma removal through mini-port totally thoracoscopic particular with focus on its feasibility, efficacy and short-term outcomes, as compared with median sternotomy. Methods From April 2015 to March 2019, 31 consecutive cases were ultimately identified and divided into two groups(Mini-port thoracoscopic group, n=14;versus conventional median sternotomic group, n=17). An echocardiography was routinely performed prior to discharge, at 1 st month, 6 th month, 12 th month and every year postoperatively. Retrospective analyses of perioperative data, postoperative outcomes and complications,and follow up data were performed. Results All the 31 patients successfully underwent right atrial myxoma removal surgery with no perioperative death, re-exploration for bleeding, residual mass or central nervous event in both groups. Intraoperative data from two group showed absence of significant difference except the intraoperative blood loss(150±50.01 m L in mini-port thoracoscopic group vs. 255.88±93.35 m L in conventional median sternomtomic group, P=0.01). There was no significant difference in perioperative blood transfusion and postoperative outcomes between the two groups. But the mini-port thoracoscopic group had a much less 24-hour postoperative drainage(136.71±148.20 m L vs. 341.17±211.29 m L, P=0.03). No significance difference could be identified between two groups about the incidence of postoperative morbidities and follow-up adverse events. Conclusion Totally mini-port video-assisted thoracoscopic approach for right atrial myxoma resection on peripheral cardiopulmonary bypass is feasible, effective with favorable safety in experienced cardiac center.[S Chin J Cardiol 2019;20(4):236-244]

Background: Severe burns injury is a serious pathology, leading to teratogenicity and significant mortality, and it also has a long-term social impact. The aim of this article is to describe the hospitalized population with severe burns injuries in eight burn centers in China between 2011 and 2015 and to suggest future preventive strategies. Methods: This 5-year retrospective review included all patients with severe burns in a database at eight institutions. The data collected included gender, age, month distribution, etiology, location, presence of inhalation injury, total burn surface area, depth of the burn, the length of hospitalization, and mortality. SPSS 19.0 software was used to analyze the data. Results: A total of 1126 patients were included: 803 (71.3%) male patients and 323 (28.7%) female patients. Scalds were the most common cause of burns (476, 42.27%), followed by fire (457, 40.59%). The extremities were the most frequently affected areas, followed by the trunk. The median length of hospitalization was 30 (15, 52) days. The overall mortality rate was 14.21%. Conclusions: Although medical centers have devoted intensive resources to improving the survival rates of burn patients, expenditures for prevention and education programs are minimal. Our findings suggest that more attention should be paid to the importance of prevention and the reduction of injury severity. This study may contribute to the establishment of a nationwide burn database and the elaboration of strategies to prevent severe burns injury.

Crosstalk between nerves and bone is essential for bone repair,for which Schwann cells(SCs)are crucial in the regulation of the *** that exosomes are critical paracrine mediators for intercellular communication that exert important effects in tissue repair,the aim of this study is to confirm the function and molecular mechanisms of Schwann cell-derived exosomes(SC-exos)on bone regeneration and to propose engineered constructs that simulate SC-mediated nerve-bone *** promoted the proliferation and differentiation of bone marrow mesenchymal stem cells(BMSCs)through *** molecular mechanism studies demonstrated that SC-exos promoted BMSC osteogenesis by regulating the TGF-βsignaling pathway via ***,SC-exos promoted the migration and tube formation performance of endothelial progenitor ***,the SC-exos@G/S constructs were developed by bioprinting technology that simulated SC-mediated nerve-bone crosstalk and improved the bone regeneration microenvironment by releasing SC-exos,exerting the regulatory effect of SCs in the microenvironment to promote innervation,vascularization,and osteogenesis and thus effectively improving bone repair in a cranial defect *** study demonstrates the important role and underlying mechanism of SCs in regulating bone regeneration through SC-exos and provides a new engineered strategy for bone repair.

PURPOSE: Clostridium difficile diarrhea is common in elderly patients and leads to prolonged hospitalization. Patients with severe or recurrent Clostridium difficile diarrhea have poor antitoxin antibody responses. Intravenous immunoglobulin has been advocated in these patients. This study was designed to assess the response of patients with refractory, recurrent, or severe Clostridium difficile diarrhea to intravenous immunoglobulin. METHODS: Retrospective review (November 2003-January 2005) of 14 patients with severe, refractory, recurrent Clostridium difficile diarrhea treated with intravenous immunoglobulin (Flebogamma, 150-400 mg/kg) from 264 Clostridium difficile toxin-positive patients. RESULTS: Median age was 79 (range, 54-91) years. Median length of symptoms before intravenous immunoglobulin was 29 (range, 3-90) days. Patients received a median of three (range, 1-5) courses of vancomycin or metronidazole before intravenous immunoglobulin. All had hypoalbuminemia (median, 22 g/l; range, 18-33) and raised C-reactive protein (median, 47 mg/l; range, 25-255) at time of infusion. The median white cell count was 15.3×109/liters (range, 4-24). Eight patients had evidence of pancolitis on abdominal imaging, suggesting severe Clostridium difficile diarrhea. All patients tolerated intravenous immunoglobulin without side effects. Nine (64 percent) responded with bowels normalizing in a median of ten (range, 2-26) days; one patient received two doses. One patient had a partial response from two doses but died two months later after a recurrence. The other four patients died of other causes within three weeks of infusion. CONCLUSIONS: Intravenous immunoglobulin may be effective for severe, refractory, or recurrent Clostridium difficile diarrhea after failed conventional treatment.

BACKGROUND Laparoscopic colorectal surgery is still developing in the Anglophone Caribbean,having been first performed in the region in the year *** report the initial outcomes using a robot camera holder to assist in laparoscopic colorectal *** To report our initial experience using the FreeHand®robotic camera holder(Freehand 2010 Ltd.,Guildford,Surrey,United Kingdom)for laparoscopic colorectal surgery in Trinidad&*** We retrospectively collected data from all patients who underwent laparoscopic colorectal resections using the Freehand®(Freehand 2010 Ltd.,Guildford,Surrey,United Kingdom)robotic camera holder between September 30,2021 and April 30,*** following data were recorded:patient demographics,robotic arm setup time,operating time,conversions to open surgery,conversions to a human camera operator,number and duration of intra-operative lens *** the termination of the operation,before operating notes were completed,the surgeons were administered a questionnaire recording information on ergonomics,user difficulty,requirement to convert to a human camera operator and their ability to carry out effective movements to control the robot while *** Nine patients at a mean age of 58.9±7.1 years underwent colorectal operations using the FreeHand robot:Right hemicolectomies(5),left hemicolectomy(1),sigmoid colectomies(2)and anterior resection(1).The mean robot docking time was 6.33 minutes(Median 6;Range 4-10;SD±1.8).The mean duration of operation was 122.33±78.5 min and estimated blood loss was 113.33±151.08 *** were no conversions to a human camera *** laparoscope was detached from the robot for lens cleaning/defogging an average of 2.6±0.88 times per case,with cumulative mean interruption time of 4.2±2.15 minutes per *** mean duration of hospitalization was 3.2±1.30 days and there were no complications *** the surgeons were interviewed after operation,the surgeons reported that there were

Background and purpose The prenatal diagnosis of cleft palate is an important component of sequential therapy,but the relevant diagnostic methods are still *** aimed here,to explore the possibility of an early prenatal diagnosis of cleft palate by assessing metabolites in pregnant *** Twenty-four inseminated females were randomly divided into retinoic acid(RA)-treated(treated with retinoic acid at 10.5 gestation days)and control *** metabolites of the embryonic palatal tissue,maternal amniotic fluid,and serum were characterized using 9.4T magnetic resonance spectroscopy in ***,a predictive model was established through the principal component analysis(PCA),and the correlations between the metabolites of amniotic fluid and palatal tissue were explored using orthogonal-2 partial least squares(O2-PLS).Results The incidences of cleft palate were 100%and 0%in the RA-treated and control groups,respectively.A predictive PCA model with a high specificity and sensitivity was established for the early prenatal diagnosis of isolated cleft palate using amniotic fluid metabolic *** RA-treated and control mice,we found that two metabolites in the amniotic fluid and palatal tissue were *** showed the same trend in the palatal tissue and amniotic fluid,while choline showed opposite trends in the two ***,the data for serum metabolites could not be used to establish a prediction *** This study indicates that assessing the metabolites of amniotic fluid is a potential approach for the prenatal diagnosis of isolated cleft palate.

The pernicious bacterial proliferation and emergence of super-resistant bacteria have already posed a great threat to public health,which drives researchers to develop antibiotic-free strategies to eradicate these fierce *** enormous achievements have already been achieved,it remains an arduous challenge to realize efficient sterilization to cut off the drug resistance ***,photothermal therapy(PTT)has emerged as a promising solution to efficiently damage the integrity of pathogenic bacteria based on hyperthermia beyond their *** now,numerous photothermal agents have been studied for antimicrobial *** them,MXenes(a type of two-dimensional transition metal carbides or nitrides)are extensively investigated as one of the most promising candidates due to their high aspect ratio,atomic-thin thickness,excellent photothermal performance,low cytotoxicity,and ultrahigh dispersibility in aqueous ***,the enormous application scenarios using their antibacterial properties can be tailored via elaborated designs of MXenes-based *** this review,the synthetic approaches and textural properties of MXenes have been systematically presented first,and then the photothermal properties and sterilization mechanisms using MXenes-based materials are ***,recent progress in diverse fields making use of the photothermal and antibacterial performances of MXenes-based materials are well summarized to reveal the potential applications of these materials for various purposes,including in vitro and in vivo sterilization,solar water evaporation and purification,and flexible antibacterial *** but not least,the current challenges and future perspectives are discussed to provide theoretical guidance for the fabrication of efficient antimicrobial systems using MXenes.
