Efficacy of biofeedback therapy via a mini wireless device on sleep bruxism contrasted with occlusal splint:a pilot study

The putative causes of bruxism are multifactorial and there are no definite measures for bruxism *** aim of this study was to evaluate the efficacy of biofeedback therapy on sleep bruxism,compared with occlusal ***-four volunteers with sleep bruxism were divided into two groups：the GTB group that were treated with biofeedback therapy（n 512） and the GTO group that were treated with occlusal splint（n 512）.A mini pressure sensor integrated with a monitoring circuit by use of a maxillary biofeedback splint was *** foster the relaxation of the masticatory muscles and the nervous system,the wireless device received signals from bruxism events and vibrations alerted the bruxer when the threshold was *** episodes and average duration of bruxism events during8 hours of sleep were analyzed with the monitoring program（TRMY1.0）.After 6 and 12 weeks,the episodes（P 50.001） and duration（P,0.05） in the GTB group declined *** contrast,there were no significant differences in the GTO group after the treatment（P.0.05）.Furthermore,the episodes had significant differences between the GTB group and the GTO group after the same period of treatment（P 5 0.000）.The results suggest that biofeedback therapy may be an effective and convenient measure for mild bruxers,when compared with occlusal splint *** mini wireless biofeedback method may be of value for the diagnosis and management of bruxism in the future.

The progressive ageing of the population and the high incidence of acute coronary syndromes(ACS)in the elderly is leading to an important increase in the number of elderly patients admitted for ACS.[1]Nevertheless,the information about the optimal clinical management in this age group is scarce due to the exclusion of older patients from clinical trials.[2]Higher comorbidity and frailty are common in this clinical setting and they are associated with higher rates of complications and consumption of healthcare resources.[3]These are the main reasons for launching this special issue about management of ACS in the elderly,in which several experts in the field address some of the most important questions about how to deal with ACS in patients at older ages.

Background:Chronic obstructive pulmonary disease(COPD)is a common public health problem *** studies have reported that socioeconomic status(SES)is related to the incidence of *** study aimed to investigate the association between SES and COPD among adults in Jiangsu province,China,and to determine the possible direct and indirect effects of SES on the morbidity of ***:A cross-sectional study was conducted among adults aged 40 years and above between May and December of 2015 in Jiangsu province,*** were selected using a multistage sampling ***,the outcome variable,was diagnosed by physicians based on spirometry,respiratory symptoms,and risk ***,occupation,and monthly family average income(FAI)were used to separately indicate SES as the explanatory ***-effects logistic regression models were introduced to calculate odds ratios(ORs)and 95%confidence intervals(CIs)for examining the SES-COPD relationship.A pathway analysis was conducted to further explore the pulmonary function impairment of patients with different ***:The mean age of the 2421 participants was 56.63±9.62 *** prevalence of COPD was 11.8%(95%CI:10.5%–13.1%)among the overall sample *** adjustment for age,gender,residence,outdoor and indoor air pollution,body weight status,cigarette smoking,and potential study area-level clustering effects,educational attainment was negatively associated with COPD prevalence in men;white collars were at lower risk(OR:0.60,95%CI:0.43–0.83)of experiencing COPD than blue collars;compared with those within the lower FAI subgroup,participants in the upper(OR:0.68,95%CI:0.49–0.97)tertiles were less likely to experience *** negative associations between all these three SES indicators and COPD were significant among men ***,FAI,and occupation had direct or indirect effects on pulmonary function including post-bronchodilator forced expiratory volume in 1 s/forced

BackgroundSeveral prognostic biomarkers have been validated for acute myeloid leukemia(AML),a heterogeneous hematopoietic ***,the factors associated with the cumulative incidence of relapse(CIR)and leukemia-free survival(LFS)in real-world patients with AML have not been well *** study examined clinical and mutational data of 246 patients with newly diagnosed AML who received the traditional“3+7”regimen in PLA General Hospital from January 2008 to August *** associated with CIR and LFS in patients newly diagnosed with AML were analyzed using next-generation *** sex combs-like 1(ASXL1)and Serine/arginine-rich splicing factor 2(SRSF2)mutations were found to be associated with an increased risk of CIR and a reduced LFS in univariate analysis,while only SRSF2 mutations were associated with these factors in the multivariate *** maintained an independent effect on LFS in the multivariate *** stem cell transplantation conferred a significant prognostic benefit on both CIR and LFS in our ***,we validated the risk classification of patients with AML receiving traditional induction regimens across a broad age *** on next-generation sequencing results,we concluded that SRSF2 mutations were predictive of an increased risk of relapse,inferior LFS rates,and non-relapse mortality in patients with newly diagnosed *** findings indicate that patients with SRSF2 mutations might not benefit from the conventional“3+7”*** results may help in developing molecular stratification strategies and could guide treatment decisions for patients with newly diagnosed AML.

Background：Since 2010,two versions of National Guidelines aimed at promoting the management of ST-segment elevation myocardial infarction （STEMI） have been formulated by the Chinese Society of ***,little is known about the changes in clinical characteristics,management,and in-hospital outcomes in rural ***：In the present multicenter,cross-sectional study,participants were enrolled from rural hospitals located in Liaoning province in Northeast China,during two different periods （from June 2009 to June 2010 and from January 2015 to December 2015）.Data collection was conducted using a standardized *** total,607 and 637 STEMI patients were recruited in the 2010 and 2015 cohorts,***：STEMI patients in rural hospitals were older in the second group （63 years vs.65 years,P =0.039）.We found increases in the prevalence of hypertension,prior percutaneous coronary intervention （PCI）,and prior *** the past 5 years,the cost during hospitalization almost *** proportion of STEMI patients who underwent emergency reperfusion had significantly increased from 42.34% to 54.47% （P 〈 0.0001）.Concurrently,the proportion of primary PCI increased from 3.62% to 10.52% （P 〈 0.0001）.The past 5 years have also seen marked increases in the use of guideline-recommended drugs and clinical ***,in-hospital mortality and major adverse cardiac events did not significantly change over time （13.01% vs.10.20%,P =0.121;13.34% vs.13.66%,P =0.872）.Conclusions：Despite the great progress that has been made in guideline-recommended therapies,in-hospital outcomes among rural STEMI patients have not significantly ***,there is still substantial room for improvement in the quality of care.

Eucommia lignans alleviate the progression of diabetic nephropathy through mediating the AR/Nrf2/HO-1/AMPK axis in vivo and in vitro

Lignans derived from Eucommia ulmoides Oliver(Eucommia lignans)inhibit the progression of inflammatory diseases,while their effect on the progression of diabetic nephropathy(DN)remained *** work was designed to assess the function of Eucommia lignans in *** major constituents of Eucommia lignans were analyzed by UPLC-Q-TOF-MS/*** binding between Eucommia lignans and aldose reductase(AR)was predicted by molecular *** lignans(200,100,and 50 mg·kg^(-1))were used in model animals to evaluate their renal function *** glomerular mesangial cells(HBZY-1)were transfected with sh-AR,sh-AMPK,and oe-AR in the presence of high glucose(HG)or HG combined with Eucommia lignans to evaluate whether Eucommia lignans affected HG-induced cell injury and mitochondrial dysfunction through the AR/Nrf2/HO-1/AMPK *** lignans significantly attenuated the progression of DN in *** lignans notably reversed HG-induced upregulation of inflammatory cytokines and mitochondrial injury,while downregulating the levels of Cyto c,caspase 9,AR,and NOX4 in HBZY-1 *** contrast,HG-induced downregulation of Nrf2,HO-1 and p-AMPKαlevels were abolished by Eucommia ***,knockdown of AR exerted similar therapeutic effect of Eucommia lignans on DN progression,and AR overexpression reversed the effect of Eucommia *** lignans alleviated renal injury through the AR/Nrf2/HO-1/AMPK ***,these findings might provide evidence for the use of Eucommia lignans in treating DN.

Anesthetic Management of a Patient with Sick Sinus Syndrome during General Anesthesia for Maxillofacial Surgery

Sick sinus syndrome (SSS) is a generalized abnormality of cardiac impulse formation. Patients with SSS occasionally need temporary pacing during general anesthesia. The most common issue arising in the perioperative period is electromagnetic interference with device function. We report a case of a 66-year-old man who required temporary cardiac pacing during maxillary cyst extirpation using electrocautery.
