For the past few decades,the internet of underwater things(IoUT)otained a lot of attention in mobile aquatic applications such as oceanography,diver network monitoring,unmanned underwater exploration,underwater surveillance,location tracking system,*** of the IoUT applications rely on acoustic *** current IoUT applications face difficulty in delivering a reliable communication system due to the various technical limitations of IoUT environment such as low data rate,attenuation,limited bandwidth,limited battery,limited memory,connectivity problem,*** of the significant applications of IoUT include monitoring underwater diver *** order to perform a reliable and energy-efficient communication system in the underwater diver networks,a smart underwater hybrid softwaredefined modem(UHSDM)for the mobile ad-hoc network was developed that is used for selecting the best channel/medium among acoustic,visible light communication(VLC),and infrared(IR)based on the criteria established within the ***,due to the mobility of underwater divers,the developed UHSDMmeets the challenges such as connectivity errors,frequent link failure,transmission delay caused by re-routing,*** emergency,the divers are most at the risk of *** deal with diver mobility,connectivity,energy efficiency,and reducing the latency in ADN,a handover mechanism based on pre-built UHSDM is proposed in this *** paper focuses on(1)design of UHSDM for ADN(2)propose the channel selection mechanism in UHSDM for selecting the best medium for handover and(3)propose handover protocol *** implementation result shows that the proposed mechanism can be used to find the new route for divers in advance and the latency can be reduced ***,this paper shows the real field experiment of air tests and underwater tests with various *** research will contribute much to the profit of researchers in underwater diver networks and underwater net

An Adaptive User Service Deployment Strategy for Mobile Edge Computing

Mobile edge computing(MEC) is a cloud server running at the edge of a mobile network, which can effectively reduce network communication delay. However, due to the numerous edge servers and devices in the MEC, there may be multiple servers and devices that can provide services to the same user simultaneously. This paper proposes a userside adaptive user service deployment algorithm ASD(Adaptive Service Deployment) based on reinforcement learning algorithms. Without relying on complex system information, it can master only a few tasks and users. In the case of attributes, perform effective service deployment decisions, analyze and redefine the key parameters of existing algorithms, and dynamically adjust strategies according to task types and available node types to optimize user experience delay. Experiments show that the ASD algorithm can implement user-side decision-making for service deployment. While effectively improving parameter settings in the traditional Multi-Armed Bandit algorithm,it can reduce user-perceived delay and enhance service quality compared with other strategies.

BACKGROUND: The epidemiological data on elderly patients attending Accident and emergency departments(AEDs) in Hong Kong is lacking. The study aimed to examine the epidemiology of geriatric patient visits to AEDs in Hong Kong, including demographic data and predictors of life-saving interventions(LSI) and ***: A retrospective cross-sectional study of geriatric patients older than 64 years old attending three AEDs during the year 2012, with a sample of 1 200 patient visits recruited. The data were retrieved from the medical records of the respective hospitals. Descriptive characteristics of the visits were provided. Multivariate logistic regression was performed to evaluate the predictors of LSI and hospital ***: The mean age of the patients was 79.1 years. Totally 49.7% of the patients were male. "Diseases of the respiratory system" was the commonest diagnosis in AEDs as well as that required admission. The admission rate was 56.8%. Logistic regression demonstrated that dependent activity of daily living(ADL), arrival by ambulance, and the higher number of co-morbidities were predictors of LSI, while advanced age, dependent ADL, institutionalized patients, arrival by ambulance, and higher number of co-morbidities were predictors of hospital ***: Ageing population is creating an imminent burden on the emergency service in Hong Kong. Previously unavailable epidemiological information about geriatric attendance to AEDs was described. This forms the basis for development of future studies concerning the medical services on this specific group of patients.

BACKGROUND:To assess the efficacy of the epidemic prevention measures of the“closed-loop”system adopted by the Beijing 2022 Olympic Winter Games(BOWG).METHODS:We retrospectively collected and analyzed information,including age,sex,nationality,vaccination status,date of diagnosis,and date of entry,from 280 SARS-CoV-2-positive individuals identified during the BOWG.A susceptibility-exposed-infectious-remove model was employed to evaluate the effectiveness of epidemic prevention strategies on controlling the spread of SARS-CoV-2 under different scenarios during the ***:Regarding SARS-CoV-2-positive cases,97.9%were imported,and 96.4%were *** median age was 37 years(range:29–47 years),and 73.9%were male,with the majority of cases being broadcasters and European *** vaccination status,93.5%were fully vaccinated,and six cases were considered to have been infected in the closed-loop system during the *** that the BOWG adopted a semi-closed-loop management system,the cumulative number of confirmed cases would be 1,137 for quick quarantine measures(3 d later)implemented and 5,530 for delayed quarantine measures(9 d later)*** modeling revealed that stringent pandemic prevention measures and closed-loop management effectively controlled the spread of SARS-CoV-2 during the ***:Imported cases are considered the main risk factor for SARS-CoV-2 transmission during mass gatherings,but a comprehensive closed-loop system could minimize transmission among attendees and general personnel.

BACKGROUND: There is little consensus as to the effects of insurance expansion on emergency department(ED) utilization for mental health purposes. We aimed to study the race specifi c association between the dependent coverage provision of the Affordable Care Act(ACA) and changes in young adults' usage of emergency department services for psychiatric ***: We utilized a Quasi-Experimental analysis of ED use in California from 2009–2011 for behavioral health diagnoses of individuals aged 19 to 31 years. Analysis used a difference-indifferences approach comparing those targeted by the ACA dependent provision(19–25 years) and those who were not(27 to 31 years), evaluating changes in ED visit rates per 1 000 in California. Primary outcomes measured included the quarterly ED visit rates with any psychiatric diagnosis. Subgroups were analyzed for differences based on race and ***: The ACA dependent provision was associated with 0.05 per 1 000 people fewer psychiatric ED visits among the treatment group(19–25 years) compared to the control group(27–31 years). Hispanics and Asian/Pacific Islanders were the only racial subgroups who did not see this significant reduction and were the only racial subgroups that did not see significant gains in the proportion of psychiatric ED visits covered by private ***: The ACA dependent provision was associated with a modest reduction in the growth rate of ED use for psychiatric reasons, however, racial disparities in the effect of this provision exist for patients of Hispanic and Asian/Pacifi c Islander racial groups.

BACKGROUND: Timely reperfusion in ST-segment elevation myocardial infarction(STEMI)improves outcomes. System delay is that between first medical contact and reperfusion therapy,comprising prehospital and hospital components. This study aimed to characterize prehospital system delay in ***: A retrospective chart review was performed for 462 consecutive STEMI patients presenting to a tertiary hospital from December 2006 to April 2008. Patients with cardiac arrest secondarily presented were excluded. For those who received emergency medical services(EMS),ambulance records were reviewed. Time intervals in the hospital were collected prospectively. The patients were divided into two equal groups of high/low prehospital system delay using visual binning ***: Of 462 patients, 76 received EMS and 52 of the 76 patients were analyzed. The median system delay was 125.5 minutes and the median prehospital system delay was 33.5minutes(interquartile range [IQR]=27.0, 42.0). Delay between call-received-by-ambulance and ambulance-dispatched was 2.48 minutes(IQR=1.47, 16.55); between ambulance-dispatch and arrival-at-patient-location was 8.07 minutes(IQR=1.30, 22.13); between arrival-at- and departurefrom-patient-location was 13.12 minutes(IQR=3.12, 32.2); and between leaving-patient-location to ED-registration was 9.90 minutes(IQR=1.62, 32.92). Comparing patients with prehospital system delay of less than 35.5 minutes versus more showed that the median delay between ambulancedispatch and arrival-at-patient-location was shorter(5.75 vs. 9.37 minutes, P<0.01). The median delay between arrival-at-patient-location and leaving-patient-location was also shorter(10.78 vs.14.37 minutes, P<0.01).CONCLUSION: Prehospital system delay in our patients was suboptimal. This is the first attempt at characterizing prehospital system delay in Singapore and forms the basis for improving efficiency of STEMI care.

As the first international sporting event scheduled since the outbreak of the novel coronavirus disease 2019(COVID-19)pandemic,the Beijing XXIV Olympic Winter Games were officially held in China between the 4th and 20th of February 2022.1 According to the International Olympic Committee(IOC),there were approximately 2900 elite athletes from 91 National Olympic Committees competing in 109 events during the 16 days of the Games.2 In fact,under the conditions of a global pandemic,mass gatherings during large-scale sporting events represent a conducive environment for viral transmission given the close,prolonged,and frequent interactions between participants;this includes an increased risk of transmission of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).
