Recent trends in the epidemiology and clinical outcomes of inflammatory bowel disease in South Korea,2010-2018

BACKGROUND Inflammatory bowel disease(IBD)was previously regarded as a Western disease;however,its incidence is increasing in the East.The epidemiology of IBD in Asia differs significantly from the patterns in the West.AIM To comprehensively investigate the epidemiology of IBD in South Korea,inclu-ding its incidence,prevalence,medication trends,and outcomes.METHODS We analyzed claims data from the Health Insurance Review and Assessment Service and Rare and Intractable Diseases(RIDs),operated by the National Health Insurance Service of South Korea.Patients with IBD were identified based on the International Classification of Diseases,Tenth Revision,and RID diagnostic codes for Crohn’s disease(CD)and ulcerative colitis(UC)from 2010 to 2018.RESULTS In total,14498 and 31409 patients were newly diagnosed with CD and UC,respectively,between 2010 and 2018.The annual average incidence of CD was 3.11 cases per 105 person-years,and that of UC was 6.74 cases per 10^(5) person-years.Since 2014,the incidence rate of CD has been stable,while that of UC has steadily increased,shifting the peak age group from 50-year-olds in 2010 to 20-year-olds in 2018.The CD and UC prevalence increased consistently over the study period;the use of 5-aminosali-cylates and corticosteroids gradually decreased,while that of immunomodulators and biologics steadily increased in both CD and UC.The clinical outcomes of IBD,such as hospitalization and surgery,decreased during the study period.CONCLUSION The CD incidence has been stable since 2014,but that of UC has increased with a shift to a younger age at peak incidence between 2010 and 2018.IBD clinical outcomes improved over time,with increased use of immunomodu-lators and biologics.

Phytochemical analysis, antioxidant, acetylcholinesterase, and α-amylase inhibitors from Hippeastrum hybridum extract

OBJECTIVE:To investigate the effects of Hippeastrum hybridum（HH） as a free radical scavenger,and an inhibitor of the two enzymes i-e Alpha-amylase（α-amylase） and acetylcholinesterase（AChE）.METHODS:In this study,HH plant was preliminary analyzed for phytochemical screening and then tested for its antioxidant,anti-α-amylase,and anti-AChE efficiency via standard procedures.RESULTS:Phytochemical analysis shows the existence of different compounds;while Coumarins and quinones were absent.The total phenolic,flavonoid,and tannins content were found to be（78.52±0.69） mg GAE/g,（2.01±0.04） mg RUE/g,and（58.12±0.23） mg TAE/g of plant extract respectively.28.02%±0.02% alkaloid and 2.02%±0.05% saponins were present in the HH extract.The HH extract showed the anti-oxidant property with IC50（50%inhibition） of（151.01±0.13）(HH),（79.01±0.04）(Ascorbic acid) for ferric reducing,（91.48±0.13）(HH),（48.02±0.11）(Ascorbic acid) against Ammonium molybdenum,（156.02±0.31）(HH),（52.38±0.21）(Ascorbic acid) against DPPH,136.01±0.21（HH）,52.02±0.31（Ascorbic acid） against H2O2,and 154.12±0.03（HH）,（40.05±0.15）(Ascorbic acid)μg/mL against ABTS respectively.Statistical analysis indicated that HH caused a competitive type of inhibition of α-amylase(Vmaxremained constant and Km increases from 10.65 to84.37%) while Glucophage caused the un-competitive type of inhibition i-e both Km and Vmaxdecreased from40.49 to 69.15%and 38.86 to 69.61%respectively.The Ki,（inhibition constant）;KI,（dissociation constant）,Km,（Michaelis-Menten constant）,and IC50 were found to be62,364,68.1,and 38.08±0.22 for HH and 12,101.05,195,34.01±0.21 for Glucophage.Similarly,HH causes an anon-competitive type of inhibition of AChE i-e Km remains constant while Vmaxdecreases from 60.5%to74.1%.The calculated Ki,KI,Km,and IC50 were found to be 32,36.2,0.05,and 18.117±0.018.CONCLUSION:From the current results,it is concluded that HH extract contains bioactive compounds,and could be a good alternative to controlling oxidants,Alzheimer‘s and Type-Ⅱ diabetic diseases.

Application of ultrasonography-elastography score to suspect porto-sinusoidal vascular disease in patients with portal vein thrombosis

Background:Porto-sinusoidal vascular disease(PSVD)and portal vein thrombosis(PVT)are causes of portal hypertension characterized respectively by an intrahepatic and a pre-hepatic obstacle to the flow in the portal system.As PVT may be a consequence of PSVD,in PVT patients at presentation,a pre-existing PSVD should be suspected.In these patients the identification of an underlying PSVD would have relevant implication regarding follow-up and therapeutic management,but it could be challenging.In this setting ultrasonography may be valuable in differential diagnosis.The aim of the study was to use ultrasonography to identify parameters to discriminate between PSVD and“pure”PVT and then to suspect PVT secondary to a pre-existing PSVD.Methods:Fifty-three patients with histologically proven PSVD and forty-eight patients affected by chronic PVT were enrolled and submitted to abdominal ultrasonography with elastography by acoustic radiation force impulse(ARFI).Results:ARFI was higher and superior mesenteric vein(SMV)diameter was wider in PSVD patients than in PVT patients.Thus,a prognostic score was obtained as linear combinations of the two parameters with a good discrimination capacity between PSVD and PVT(the area under the curve=0.780;95%confidence interval:0.690-0.869).Conclusions:A score based on ARFI and SMV diameter may be useful to suspect an underlying PSVD in patients with PVT and to identify a subgroup of patients to be submitted to liver biopsy.

Complexed amino acid minerals vs. bis-glycinate chelated minerals:Impact on the performance of old laying hens

The present study was to evaluate the effect of trace minerals(Zn, Mn, and Cu) from complexed amino acid minerals(ZMCAA) and bis-glycinate chelated minerals(ZMCGly) in laying hen diets on performance,internal and external egg quality, yolk mineral deposition, intestinal morphometry, and bone characteristics. From 78 to 98 weeks of age, 400 White LSL-Lite strain laying hens were distributed in a randomized design with 4 treatments with 10 replicates per treatment. Treatments were distributed in a 2 ×2 factorial arrangement using either Zn, Mn, and Cu of ZMCAA or ZMCGly source at 2 levels: low(20, 20,and 3.5 mg/kg of Zn, Mn, and Cu, respectively) or high(40, 40, and 7 mg/kg of Zn, Mn, and Cu,respectively). The analysis of variance was performed, and in cases where differences were observed, the means were compared using Tukey?s test(P < 0.05). The source and level of trace mineral supplementation had a significant impact on the performance of laying hens. Hens fed ZMCAA had higher egg production(P = 0.01), egg weight(P = 0.02), egg mass(P = 0.01), and lower feed conversion ratio(P =0.05) compared to those fed ZMCGly. The ZMCAA supplementation showed higher albumen height(P =0.01), albumen weight(P = 0.01), and eggshell thickness(P < 0.01). The deposition of Zn(P < 0.01), Mn(P< 0.01), and Cu(P < 0.01) in the egg yolk was greater for hens received ZMCAA. Tibia weight(P = 0.04)and bone densitometry(P < 0.01) in the tibia were higher with ZMCAA supplementation. In the small intestine, ZMCAA resulted in longer villi(P = 0.02) and shorter crypt depth(P = 0.01) in the duodenum.Jejunum and ileum measurements were influenced by the level and source of trace minerals(P < 0.05).Laying hens fed ZMCAA exhibited superior performance, egg quality, deposition of trace minerals in the egg yolk, and bone density compared to hens fed ZMCGly. In this study, older laying hens supplemented with ZMCAA at lower levels demonstrated adequate levels of supplementation.

On the topological surface states of the intrinsic magnetic topological insulator Mn-Bi-Te family

We review recent progress in the electronic structure study of intrinsic magnetic topological insulators（MnBi2Te4）·（Bi2Te3）n（n = 0, 1, 2, 3） family. Specifically, we focus on the ubiquitously（nearly）gapless behavior of the topological Dirac surface state observed by photoemission spectroscopy, even though a large Dirac gap is expected because of surface ferromagnetic order. The dichotomy between experiment and theory concerning this gap behavior is perhaps the most critical and puzzling question in this frontier. We discuss various proposals accounting for the lack of magnetic effect on the topological Dirac surface state, which are mainly categorized into two pictures, magnetic reconfiguration and topological surface state redistribution. Band engineering towards opening a magnetic gap of topological surface states provides great opportunities to realize quantized topological transport and axion electrodynamics at higher temperatures.

Current status and future perspectives on stem cell transplantation for spinal cord injury

BACKGROUND Previous assessments of stem cell therapy for spinal cord injuries(SCI)have encountered challenges and constraints.Current research primarily emphasizes safety in early-phase clinical trials,while systematic reviews prioritize effectiveness,often overlooking safety and translational feasibility.This situation prompts inquiries regarding the readiness for clinical adoption.AIM To offer an up-to-date systematic literature review of clinical trial results concerning stem cell therapy for SCI.METHODS A systematic search was conducted across major medical databases[PubMed,Embase,Reference Citation Analysis(RCA),and Cochrane Library]up to October 14,2023.The search strategy utilized relevant Medical Subject Heading(MeSH)terms and keywords related to"spinal cord","injury","clinical trials","stem cells","functional outcomes",and"adverse events".Studies included in this review consisted of randomized controlled trials and non-randomized controlled trials reporting on the use of stem cell therapies for the treatment of SCI.RESULTS In a comprehensive review of 66 studies on stem cell therapies for SCI,496 papers were initially identified,with 237 chosen for full-text analysis.Among them,236 were deemed eligible after excluding 170 for various reasons.These studies encompassed 1086 patients with varying SCI levels,with cervical injuries being the most common(42.2%).Bone marrow stem cells were the predominant stem cell type used(71.1%),with various administration methods.Follow-up durations averaged around 84.4 months.The 32.7%of patients showed functional improvement from American spinal injury association Impairment Scale(AIS)A to B,40.8%from AIS A to C,5.3%from AIS A to D,and 2.1%from AIS B to C.Sensory improvements were observed in 30.9%of patients.A relatively small number of adverse events were recorded,including fever(15.1%),headaches(4.3%),muscle tension(3.1%),and dizziness(2.6%),highlighting the potential for SCI recovery with stem cell therapy.CONCLUSION In the realm of SCI treatment,stem cell-based thera

Cancer-related to the nervous system and brain tumors is a leading cause of mortality in various countries.Magnetic resonance imaging(MRI)and computed tomography(CT)are utilized to capture brain images.MRI plays a crucial role in the diagnosis of brain tumors and the examination of other brain disorders.Typically,manual assessment of MRI images by radiologists or experts is performed to identify brain tumors and abnormalities in the early stages for timely intervention.However,early diagnosis of brain tumors is intricate,necessitating the use of computerized methods.This research introduces an innovative approach for the automated segmentation of brain tumors and a framework for classifying different regions of brain tumors.The proposed methods consist of a pipeline with several stages:preprocessing of brain images with noise removal based on Wiener Filtering,enhancing the brain using Principal Component Analysis(PCA)to obtain well-enhanced images,and then segmenting the region of interest using the Fuzzy C-Means(FCM)clustering technique in the third step.The final step involves classification using the Support Vector Machine(SVM)classifier.The classifier is applied to various types of brain tumors,such as meningioma and pituitary tumors,utilizing the Contrast-Enhanced Magnetic Resonance Imaging(CE-MRI)database.The proposed method demonstrates significantly improved contrast and validates the effectiveness of the classification framework,achieving an average sensitivity of 0.974,specificity of 0.976,accuracy of 0.979,and a Dice Score(DSC)of 0.957.Additionally,this method exhibits a shorter processing time of 0.44 s compared to existing approaches.The performance of this method emphasizes its significance when compared to state-of-the-art methods in terms of sensitivity,specificity,accuracy,and DSC.To enhance the method further in the future,it is feasible to standardize the approach by incorporating a set of classifiers to increase the robustness of the brain classification method.

Flexible, efficient, and accurate tests for epidemics

Group testing involves discovering a small subset of distinguished subjects from a large population while efficiently reducing the total number of tests. It has been widely used for industrial testing, information technology, and biology, especially epidemic screening. Tests, in reality, are noisy for the presence of false outcomes. Some tests are accurate but time-consuming, while others are cheaper but less accurate. Exactly which test to use is constrained by various considerations, such as availability, cost, accuracy, and efficiency.In this paper, we propose flexible, efficient, and accurate tests(FEATs). FEATs are based on group testing with simple but careful designs by incorporating ideas such as close contact cliques and repeated tests. FEATs could dramatically improve the efficiency or accuracy of existing tests. For example, for accurate but slow tests, the FEAT can improve efficiency multiple times without compromising accuracy. On the other hand,for fast but inaccurate tests, the FEAT can sharply reduce the false-negative rate(FNR) and significantly increase efficiency. Theoretical justifications are provided. We point out some scenarios where the FEAT can be effectively employed.

Improving Video Watermarking through Galois Field GF(2^(4)) Multiplication Tables with Diverse Irreducible Polynomials and Adaptive Techniques

Video watermarking plays a crucial role in protecting intellectual property rights and ensuring content authenticity.This study delves into the integration of Galois Field(GF)multiplication tables,especially GF(2^(4)),and their interaction with distinct irreducible polynomials.The primary aim is to enhance watermarking techniques for achieving imperceptibility,robustness,and efficient execution time.The research employs scene selection and adaptive thresholding techniques to streamline the watermarking process.Scene selection is used strategically to embed watermarks in the most vital frames of the video,while adaptive thresholding methods ensure that the watermarking process adheres to imperceptibility criteria,maintaining the video's visual quality.Concurrently,careful consideration is given to execution time,crucial in real-world scenarios,to balance efficiency and efficacy.The Peak Signal-to-Noise Ratio(PSNR)serves as a pivotal metric to gauge the watermark's imperceptibility and video quality.The study explores various irreducible polynomials,navigating the trade-offs between computational efficiency and watermark imperceptibility.In parallel,the study pays careful attention to the execution time,a paramount consideration in real-world scenarios,to strike a balance between efficiency and efficacy.This comprehensive analysis provides valuable insights into the interplay of GF multiplication tables,diverse irreducible polynomials,scene selection,adaptive thresholding,imperceptibility,and execution time.The evaluation of the proposed algorithm's robustness was conducted using PSNR and NC metrics,and it was subjected to assessment under the impact of five distinct attack scenarios.These findings contribute to the development of watermarking strategies that balance imperceptibility,robustness,and processing efficiency,enhancing the field's practicality and effectiveness.

The widespread use of the Internet of Things(IoTs)and the rapid development of artificial intelligence technologies have enabled applications to cross commercial and industrial band settings.Within such systems,all participants related to commercial and industrial systems must communicate and generate data.However,due to the small storage capacities of IoT devices,they are required to store and transfer the generated data to third-party entity called“cloud”,which creates one single point to store their data.However,as the number of participants increases,the size of generated data also increases.Therefore,such a centralized mechanism for data collection and exchange between participants is likely to face numerous challenges in terms of security,privacy,and performance.To address these challenges,Federated Learning(FL)has been proposed as a reasonable decentralizing approach,in which clients no longer need to transfer and store real data in the central server.Instead,they only share updated training models that are trained over their private datasets.At the same time,FL enables clients in distributed systems to share their machine learning models collaboratively without their training data,thus reducing data privacy and security challeges.However,slow model training and the execution of additional unnecessary communication rounds may hinder FL applications from operating properly in a distributed system.Furthermore,these unnecessary communication rounds make the system vulnerable to security and privacy issues,because irrelevant model updates are sent between clients and servers.Thus,in this work,we propose an algorithm for fully homomorphic encryption called Cheon-Kim-Kim-Song(CKKS)to encrypt model parameters for their local information privacy-preserving function.The proposed solution uses the impetus term to speed up model convergence during the model training process.Furthermore,it establishes a secure communication channel between IoT devices and the server.We also use a lightweight secure transport protocol
