Associations between IL-1RN variable number of tandem repeat, IL-1β (-511) and IL-1β (+3954) gene polymorphisms and urolithiasis in Uighur children of China

Objective:Interleukin-1(IL-1)is a pro-inflammatory cytokine which may be related to urolithiasis.Genetic polymorphisms of the interleukin-1beta(IL-1β)have been proposed as markers for urolithiasis in some areas.Due to the high incidence of urolithiasis in Uighur children(Xinjiang,China)and existence of ethnic difference,our aim is to explore the potential of IL-1 gene polymorphisms and urolithiasis among these children.Methods:Genomic DNA extracted from peripheral blood of 115 patients and 98 controls were used for genotype polymorphisms analyses.IL-1 receptor antagonist(IL-1RN)gene variable number of tandem repeat(VNTR)gene polymorphisms were analyzed by PCR method.PCR-based restriction analysis was done for the IL-1β(-511)and IL-1β(+3954)gene polymorphisms by endonucleases Ava I and Taq I,respectively.The genotype distribution,allele frequencies,carriage rate,and haplotype frequencies were statistically analyzed.Results:No significant differences were observed in genotypic frequencies between pediatric urolithiasis patients and control group for IL-1RN gene(χ^(2)=1.906,p=0.605),IL-1β(-511)gene(χ^(2)=0.105,p=0.949),or IL-1β(+3954)gene(χ^(2)=3.635,p=0.169).There were yet no significant differences of the allele frequencies of IL-1RN VNTR gene(p=0.779),IL-1β(-511)gene(p=0.941),and IL-1β(+3954)gene(p=0.418)in the case and control groups,as well as the carriage rate and haplotype of them(all p>0.05).Conclusions:The associations between IL-1RN VNTR,IL-1β(-511)and IL-1β(+3954)genes polymorphisms and urolithiasis were not significant in Uighur children.The results need to be confirmed in studies with larger population sample size,as well as in other ethnic groups.

Efficacy of short duration versus conventional electroacupuncture in the treatment of obesity:a randomized crossover study

OBJECTIVE:To compare the efficacy of short duration electroacupuncture(EA)with conventional EA to reduce weight and other parameters in obese Thai women.METHODS:A randomized crossover study was conducted in 76 simple obese women.Participants were randomized to undergo either eight weeks of 30-minute EA(EA30)followed by an eight-week washout period,then eight weeks of five-minute EA(EA5),or EA5 followed by a washout period,then EA30.Electroacupuncture was performed at 14 acupoints for two sessions per week.Participants were randomized to undergo either eight weeks of EA30 followed by an eight-week washout period,then eight weeks of EA5,or EA5 followed by a washout period,then EA30.Electroacupuncture was performed using a stainless silver needle at 14 acupoints for two sessions per week.The needle was connected to an electric stimulator that delivered a constant current,40 Hz and 3 m A,for 30 or five minutes according to the assigned treatment period.The primary outcome was the difference in weight reduction between the short-duration and the conventional EA while differences in other anthropometric parameters and biochemical parameters were considered as secondary outcomes.RESULTS:After each treatment period,all anthro pometric characteristics of both groups decreased from the baseline regardless of the treatment sequence,including body weight(1.4-1.8 kg;P<0.01),body mass index(0.56-0.70 kg/m^(2);P<0.01),waist and hip circumference,skinfold thickness and body fat percentage as well as the improvement in fasting blood glucose and lipid profiles.There were no significant differences of the treatment effects on anthropometric parameters and biomedical chemistries between conventional and short-duration EA.No adverse effects were reported.CONCLUSION:Short duration EA at 14 acupuncture points had comparable efficacy to conventional EA in reduction of weight and other anthropometric parameters as well as to improve biochemistry parameters in obese women.

Global climate change and human health:Pathways and possible solutions

Global warming has been changing the planet’s climate pattern,leading to increasing frequency,intensity and duration of extreme weather events and natural disasters.These climate-changing events affect various health outcomes adversely through complicated pathways.This paper reviews the main signs of climate change so far,e.g.,suboptimal ambient temperature,sea-level rise and other conditions,and depicts the interactive pathways between different climate-changing events such as suboptimal temperature,wildfires,and floods with a broad range of health outcomes.Meanwhile,the modifying effect of socioeconomic,demographic and environmental factors on the pathways is summarised,such that the youth,elderly,females,poor and those living in coastal regions are particularly susceptible to climate change.Although Earth as a whole is expected to suffer from climate change,this review article discusses some potential benefits for certain regions,e.g.,a more liveable environment and sufficient food supply.Finally,we summarise certain mitigation and adaptation strategies against climate change and how these strategies may benefit human health in other ways.This review article provides a comprehensive and concise introduction of the pathways between climate change and human health and possible solutions,which may map directions for future research.

Helicobacter pylori Bab A in adaptation for gastric colonization

Helicobacter pylori(H. pylori) as a causative agent of gastric complications, is well adapted for the colonization of gastric mucosa. Although the infectious process depends on several factors, the adhesion to the gastric mucosa is the first and important step. Among several outer membrane proteins, Bab A is one of the significant protein involving in many inflammatory processes in addition to its role in the attachment for the persistent colonization. We performed a Pub Med search using the key words: "bab A ", "pylori", "gastric complications", "homologous recombination", "slipped strand mispairing"; a total of 249 articles were displayed. Of these we mainly focused on articles with the full text in English and published between 2005 and 2016. H. pylori Bab A is involved in binding with receptors; however, its synthesis is regulated by phase variation. In this review we confirm that H. pylori bab A can be modulated at the molecular and functional levels to adapt to the stress within the gastro-intestinal tract.

Performance-weighted-voting model:An ensemble machine learning method for cancer type classification using whole-exome sequencing mutation

Background:With improvements in next-generation DNA sequencing technology,lower cost is needed to collect genetic data.More machine learning techniques can be used to help with cancer analysis and diagnosis.Methods:We developed an ensemble machine learning system named performance-weighted-voting model for cancer type classification in 6,249 samples across 14 cancer types.Our ensemble system consists of five weak classifiers(logistic regression,SVM,random forest,XGBoost and neural networks).We first used cross-validation to get the predicted results for the five classifiers.The weights of the five weak classifiers can be obtained based on their predictive performance by solving linear regression functions.The final predicted probability of the performance-weighted-voting model for a cancer type can be determined by the summation of each classifier's weight multiplied by its predicted probability.Results:Using the somatic mutation count of each gene as the input feature,the overall accuracy of the performance-weighted-voting model reached 71.46%,which was significantly higher than the five weak classifiers and two other ensemble models:the hard-voting model and the soft-voting model.In addition,by analyzing the predictive pattern of the performance-weighted-voting model,we found that in most cancer types,higher tumor mutational burden can improve overall accuracy.Conclusion:This study has important clinical significance for identifying the origin of cancer,especially for those where the primary cannot be determined.In addition,our model presents a good strategy for using ensemble systems for cancer type classification.

Mucosal-associated invariant T cells from induced pluripotent stem cells:A novel approach for modeling human diseases

Mice have frequently been used to model human diseases involving immune dysregulation such as autoimmune and inflammatory diseases.These models help elucidatethe mechanisms underlying the disease and in the development of novel therapies.However,if mice are deficient in certain cells and/or effectors associated with human diseases,how can their functions be investigated in this species?Mucosal-associated invariant T(MAIT)cells,a novel innate-like T cell family member,are a good example.MAIT cells are abundant in humans but scarce in laboratory mice.MAIT cells harbor an invariant T cell receptor and recognize nonpeptidic antigens vitamin B2metabolites from bacteria and yeasts.Recent studies have shown that MAIT cells play a pivotal role in human diseases such as bacterial infections and autoimmune and inflammatory diseases.MAIT cells possess granulysin,a human-specific effector molecule,but granulysin and its homologue are absent in mice.Furthermore,MAIT cells show poor proliferation in vitro.To overcome these problems and further our knowledge of MAIT cells,we have established a method to expand MAIT cells via induced pluripotent stem cells(iP SCs).In this review,we describe recent advances in the field of MAIT cell research and our approach for human disease modeling with iP SCderived MAIT cells.

Ginger-indirect moxibustion plus acupuncture versus acupuncture alone for chronic fatigue syndrome:a randomized controlled trial

OBJECTIVE:To assess the efficacy and safety of gingerindirect moxibustion for chronic fatigue syndrome(CFS).METHODS:In this central randomized,controlled trial,290 CFS participants were recruited and randomly allocated to group A(ginger-indirect moxibustion plus acupuncture)or group B(acupuncture alone).The study consisted of a treatment period of 8 weeks with a total of 24 treatments(3 sessions per week,every other day),and a follow-up period of 12 weeks.The outcome was measured by Fatigue Severity Scale(FSS),Psychological Health Report(SPHERE),the Self-rating depression scale(SDS)and the Hamilton anxiety scale(HAMA)at baseline,2,4,6,8,12 and 20 weeks.RESULTS:With the treatment undergoing,the changes of FSS,SPHERE,SDS and HAMA scores in both groups increased gradually,and the effect maintained at the 12th week.Between groups,significantly higher score changes were seen in group A in FSS after 4 weeks treatment(11.94 vs 9.12,95%CI:0.94,4.7)and in SPHERE after 2 weeks treatment(3.7 vs 2.27,95%CI:0.56,2.31).But for SDS and HAMA,the improvement did not differ significantly between groups.No severe adverse events were reported.CONCLUSION:Ginger-indirect moxibustion is a safe and effective intervention to relieve fatigue and accompanying physical symptoms of CFS.

Influence of comorbidity on outcomes of older patients with acute pancreatitis based on a national administrative database

Little information is available on the influ- ence of comorbidities on outcomes of older patients with acute pancreatitis. This study aimed to investigate the influence of co- morbidities on outcomes of older patients with acute pancreati- tis using data from a national Iapanese administrative database. METHODS： A total of 14 322 older patients （〉70 years） with acute pancreatitis were referred to 1090 hospitals between 2010 and 2012 in Japan. We collected patients＇ data from the administrative database to compare the in-hospital mortal- ity and length of stay of older patients with acute pancreatitis. The patients were categorized into four groups according to comorbidity level using the Charlson Comorbidity Index （CCI）： none （CCI score=0; n=6890）; mild （1; n=3874）; moder- ate （2; n=2192） and severe （〉3; n=1366）. RESULTS： Multiple logistic and linear regression analyses revealed that severe comorbidity was significantly associated with higher in-hospital mortality and longer length of stay [odds ratio （OR）=2.26; 95% confidence interval （CI）： 1.75-2.92, P〈0.001 and coefficient 4.37 days; 95% CI： 2.89-5.85, P〈0.001, respectively]. In addition, cardiovascular and renal diseases were the most significant comorbidities affecting outcomes of the older patients. ORs of cardiovascular and renal diseases for mortality were 1.44 （95% CI： 1.13-1.85, P=0.003） and 2.69 （95% CI： 1.88-3.85, P〈0.001）, respectively, and coefficients forlength of stay were 3.01 days （95% CI： 1.34-4.67, P〈0.001） and 3.72 days （95% CI： 1.01-6.42, P=0.007）, respectively. CONCLUSION： This study demonstrated that comorbidities significantly influenced outcomes of older patients with acute pancreatitis and cardiovascular and renal comorbidities were significant factors affecting outcomes.

Isolation of aerobic bacteria from ticks infested sheep in Iraq

Objective:To highlight the presence of aerobic bacteria in live ticks infested sheep,in Diyala Governorate,Iraq.Methods:One hundred and thirty adult alive ticks were picked up from sheep which were reared in different farms in Diyala Governorate,Iraq,during the period from November 2012 to May 2013.Ticks were classified in the Natural History Museum in Baghdad.They were dissected aseptically for extraction of the salivary gland and midgut.The removed tissue from each organ was inoculated in buffer peptone water(1%)and incubated for 2 h at 37 °C,to maintain weak and/or injured bacterial cells,then transmitted to nutrient broth incubated at 37 °C for 18 h.Culturing was done on three solid bacteriological media(nutrient,blood and McConkey agars),and then incubated at37 ℃ for 24 h.Bacterial identification was performed by using multiple biochemical tests and API-20 strips.Data were analyzed by using Statistical Analysis System version 9.1,2010.Chi-square test was used for comparison at significant level of P ≤ 0.05.Results:Two species of ticks were identified[Rhipicephalus(Boophilus) annulatus and Hyalomma turanicum].High bacterial isolation rate was observed(483 isolates).A significant high isolation rate was recorded from Rhipicephalus annulatus(63.14%).Six bacterial species were identified[Escherichia coli(28.36%),Pseudomonas aeruginosa(18.01%),Bacillus cereus(14.69%),Staphylococcus aureus(13.66%),Citrobacter freundii(13.04%),and Enterobacter species(12.21%)].Also the high bacterial isolation rates were recorded in the temperate months(November,March and April);these coincided with high reproductive performance of ticks.Conclusions:The high isolation rate of aerobic pathogens from ticks might reflect the active contribution of this arthropod in environmental contamination and increase the probability of transmitting bacterial pathogens to their hosts.

Detrimental Effect of Electromagnetic Pulse Exposure on Permeability of In Vitro Blood-brain-barrier Model

Objective To study the effect of electromagnetic pulse （EMP） exposure on permeability of in vitro blood-brain-barrier （BBB） model. Methods An in vitro BBB model, established by co-culturing brain microvascular endothelial cells （BMVEC） and astroglial cells （AC） isolated from rat brain, was exposed to EMP at 100 kV/m and 400 kV/m, respectively. Permeability of the model was assayed by measuring the transendothelial electrical resistance （TEER） and the horseradish peroxidase （HRP） transmission at different time points. Levels of BBB tight junction-related proteins were measured at O, 1, 2, 4, 8, 12, 16, 20, 24 h after EMP exposure by Western blotting. Results The TEER level was lower in BBB model group than in control group at 12 h after EMP, exposure which returned to its normal level at 24 h. The 24 h recovery process was triphasic and biphasic respectively after EMP exposure at 100 kV/m and 400 kV/m. Following exposure to 400 kV/m EMP, the HRP permeability increased at 1-12 h and returned to its normal level at 24 h. Western blotting showed that the claudin-5 and ZO-1 protein levels were changed after EMP exposure. Conclusion EMP exposure at 100 kV/m and 400 kV/m can increase the permeability of in vitro BBB model and BBB tight junction-related proteins such as ZO-1 and claudin-5 may change EMP-induced BBB permeability.
