Instant evaluation of contrast enhanced endoscopic ultrasound helps to differentiate various solid pancreatic lesions in daily routine

BACKGROUND Contrast enhanced harmonic endoscopic ultrasound(CEH-EUS) is a spreading technique; some studies have shown its value in the diagnosis of pancreatic adenocarcinoma using quantitative *** To examine the value of CEH-EUS for differentiating various pancreatic lesions in everyday routine with qualitative and quantitative *** Data of 55 patients with pancreatic lesions who underwent CEH-EUS were analysed retrospectively. Perfusion characteristics were classified by the investigator qualitatively immediately upon investigation, quantitative analysis was performed later on. Samples from fine needle aspiration(EUS-FNA) or surgical specimen served as gold *** CEH-EUS showed 39 hypoenhanced lesions, 3 non-enhanced and 13 hyperenhanced lesions. Concordance of the investigators qualitative classification of peak contrast enhancement with quantitative analysis later on was 100%, while other parameters such as arrival time, time to peak or area under the curve did not show additional value. 34 of 39 hypoenhanced lesions were pancreatic adenocarcinoma; of the hyperenhanced lesions 4 were inflammatory, 3 neuroendocrine carcinomas, 1 lymphoma, 1 insulinoma and 4 metastases(2 of renal cell carcinoma, 2 of lung cancer). Non-enhanced lesions showed up as necroses. Sensitivity for the detection of pancreatic adenocarcinoma was 100%,specificity 87.2% for hypoenhancement alone; in otherwise healthy pancreatic tissue all hypoenhanced lesions were pancreatic adenocarcinoma(sensitivity and specificity 100%, PPV and NPV for adenocarcinoma 100%).CONCLUSION This study again shows the excellent value of CEH-EUS in everyday routine for diagnostics of various focal pancreatic lesions suggesting that qualitatively assessed hypoenhancement is highly predictive for adenocarcinoma. Objective:To determine the causes of death among human immunodeficiency virus/acquired immunodeficiency syndrome(HIV/AIDS) patients as a step to planning strategies to improve mortality from this ***:This study retrospectively analyzed the mortality pattern of adult HIV/AIDS patients in the university of Calabar teaching Hospital from January 2005 to December *** data were obtained from sexually transmitted infection/acquired immunodeficiency syndrome(STI/AIDS) clinic register,admissions and discharge/death registers as well as the patients’ case records and the hospitals monthly mortality *** obtained included age,sex,diagnosis and cause(s) of *** causes of death considered were the direct causes of death,since the originating antecedent cause of death is the same in all the patients,in this case,HIV/*** was analysed using Epi Info ***:The total number of mortalities during the study period was 350,100 were HIV positive representing 28.6%of all *** advanced HIV/AIDS disease was the leading cause of death in our study representing 27.0%,tuberculosis was the single leading cause of deaths in HIV/AIDS patients constituting about 24.0%of *** was followed by sepsis and septicaemia(13.0%), meningitis and encephalitis,and anaemia accounting for 11.0%,while respiratory diseases constituted 5.0%of the mortality *** highest number of deaths occurred in those aged between 21-50 years(82.0%).Conclusions:The study has shown that HIV/AIDS is a major cause of morbidity and mortality in our *** causes of death reflect the varied spectrum of infection and other forms of organ involvement that affect HIV/AIDS *** present dismal situation of adult patients living with HIV/AIDS calls for enhanced strategies to decrease the mortality trend observed in Nigeria and sub-Saharan Africa.

Epidemiology and Histology Aspects of Breast Cancers of Women in Ivory Coast

Breast cancer is a major public health problem both in developed countries, where it represents the first female cancer, and in developing countries, where its incidence is increasing. Purpose: The authors report the results of a study whose objective was to describe the epidemiological and pathological features of women’s breast cancers in Ivory Coast. Material and Methods: This is a retrospective descriptive study on breast cancer histologically confirmed and made from the records of pathology laboratories of the university Hospital in Abidjan. The study period was 24 years (1984-2007). The parameters studied were: frequency, age, risk factors, the macroscopic and histological aspects. Results: Women’s breast cancer was the second cancer among women (13.69%) after cervical cancer and represented 6.51% of cancers. The average age was 45.21 years ranging from 10 years to 85 years. Subjects from 35 to 44 years were the most affected (30.7%) and 8% of patients were under 30 years (n=61). Multiparity (53.55%) and lower socioeconomic level (79.63%) were associated. At the macroscopic level nodular aspects represented 47.62% of cases (n=301). Carcinomas were 92% (n=710) of histological types with 94.36% (n=692) of invasive carcinomas. Sarcomas (5.7%), lymphoma (2.1%) and Paget’s disease (0.3%) were also observed. With regard to prognostic, the grading of Scarff-Bloom and Richardson showed 26.75% (n=156) grade I, 60.64% (n=354) grade II and 12.52% (n=73%) grade III. Conclusion: In consideration of prognosis of advanced forms, an effort should be made for diagnosis and early treatment of women’s breast cancer in Ivory Coast.

Childhood malaria in the Niger delta area of Nigeria:mothers/care givers'perception,definition and treatment practices

Objective:The objective of the study was to evaluate mothers/care givers perception of malaria,their treatment practices and the effects on the outcome of ***:Four hundred and sixty children were enrolled and their mothers/care givers *** children were screened for malaria parasitaemia and there after,blood specimens were obtained for biochemical and haematological evaluation from those children who met the criteria and tested positive to *** *** cell volume,electrolytes,urea, creatinine,plasma glucose,and serum bilirubin were ***:A total of 460 children were studied, 233(50.7%) males and 227(49.3%) *** malaria cases were 112(24.3%) and severe malaria 348(75.7%).Those who presented early 106(23.0%) and those who presented late 354(77.0%).Perception and definition of malaria as well as the treatment seeking behaviors vary significantly with the level of education of the mothers and care *** without formal education 68(51.9%) wrongly perceived that the etiology of malaria can only be diagnosed by native doctors compared to those with primary six education 61 (26.5%) and junior secondary education 10(10.1%).Only 43(9.3%) gave the correct dose of chloroquine syrup to their sick children,while 32(7.0%) gave at sub optimal ***:Wrong perception of malaria especially the complicated malaria and wrong treatment practices are major contributory factors to the high mortality and morbidity of malaria in *** is therefore a need for health education to correct the wrong ideas about the cause and treatment practices of malaria as part of malaria control programme.

Comparative Analysis of Indoor Levels of Suspended Particulates and Nitrogen Dioxide a Few Hours later after an Asthmatic Attack

Suspended particulates （TSP） and nitrogen dioxide （NO2） are known respiratory irritants linked to asthma aggravation. This pilot study was designed to investigate the role of these pollutants on the frequency of asthmatic attack on two of the inhabitants of a household. The surveillance of TSP and NO2 in this household commenced a few hours later, after one of the occupants suffered an attack. The TSP load determination was done using a High Volume Gravimetric sampler and a light scattering method via a Haz-Dust 10 μm particulate monitor. Palmes Diffusion tubes for NO2 and a portable Crowcon Gasman toxic gas detector were utilized for NO2 screening. In the first day of monitoring in the living room, the in situ particulate sampler （Haz-Dust） recorded a mean TSP level of 26,000 μg·m^-3. A confirmatory test with the eight hour average Gravimetric sampler gave 25,833 μg·m^-3. With the use of the Gasman toxic gas detector for NO2, the NO2 concentration for the first few hours of sampling was lower than 188 μg·m^-3, the detection limit of this instrument. However, the exact NO2 concentrations for the 7 day monitoring after the attack were 27.50 μg·m^-3 （kitchen） and 12.03 μg·m^-3 （living room） as recorded by the Palmes diffusion tubes.

Seroprevalence of HDV among non-hospitalized HBs Ag positive patients from KPK-region of Pakistan

Objective: To study the seroprevalence of hepatitis B virus(HBV) and hepatitis delta virus(HDV) infections in patients visiting outpatient department of a major tertiary care hospital in Khyber Pakhtunkhwa region of ***: Blood samples were collected from non-hospitalized patients. Serological analysis was done by ELISA and viral DNA was amplified by PCR. The amplified DNA was analyzed by agarose gel ***: Altogether, 946 blood samples were screened, overall percentage of HBs Agpositive patients remained 22.41%(prevalence: 224.10/1 000; CI: 0.197 5 ± 0.250 7) with the highest incidence rates among relatively younger age groups(20–29 years). The prevalence of HBV–HDV co-infection was found to be 46.75/1 000; CI: 0.031 8 ± 0.061 *** HBs Ag-positive patients, anti-HBc-total was detected in 86.79% while 25.00% were positive for anti-HBc-immunoglobulin M. Similarly, among these patients, HBV DNA was detected in 64.13% and 10.85% were co-infected with HDV. Different symptoms were associated with the prevailing infection, including malaise(62%), anorexia(66%) and fatigue(73%). The most commonly associated symptom was abdominal discomfort. Among these patients, certain risk factors, including surgery, visit to dentist and intravenus infusions were frequently associated with the infection(x^2= 95.23; df = 11; P < 0.000 1).Conclusions: Overall, this study confirmed higher prevalence of active HBV/HDV infection, among young patients from Khyber Pakhtunkhwa region having no prior history of viral hepatitis.

Malaria parasitaemia and disorders of plasma electrolytes

Objective:To assess the plasma electrolyte status of patients with diagnosis of malaria but without the symptoms of diarrhea,vomiting or altered sensorium and correlation of the plasma electrolyte changes and the degree of parasitaemia. Methods:All the participants were adults,who met the clinical case definition of malaria but without the symptoms of diarrhea,vomiting or other medical *** slides were screened microscopically for malaria parasite and the parasite positive patients were grouped into A(mild),B(moderate) and C(heavy) malaria parasiteamia, depending on the malaria parasite count per high power microscopic *** sodium,potassium and chloride were estimated using the ion selective electrode method,while bicarbonate ions were determined by simple titration ***:A total of 200 subjects were studied which comprised of 150 patients and 50 *** mean plasma sodium was significantly lower in patients with heavy parasitaemia[group C,(128.8±1.2) mmol/L] compared to those with mild and moderate parasitemia[(133.5±2.8) mmol/L and(133.5±3.5) mmol/L,P<0.0001].The mean plasma chloride was lowest in those patients with heavy parasitaemia(group C) than those patients of group A and B(P<0.0001).Patients in group C also had significant hypokalaemia[(3.2±0.5) mmol/ L]when compared to those in groups A and B[(3.6±0.3) mmol/L and(4.1±0.6) mmol/L respectively,P<0. 0001].Conclusion:A disorder of plasma electrolytes in malaria patients that had no symptoms of diarrhea and vomiting was *** the severity of hyponatraemia and hypokalaemia correlate with the severity of the patients’ malaria *** data should alert clinicians on the need to assess electrolytes status of patients with malaria even without the symptoms of fluid loss,especially when malaria parasitaemia is heavy.

FLOW CYTOM ETRIC ANALYSIS OF CELLULAR DNA CONTENT IN EPITHELIAL OVARIAN TUMOR

The DNA content of tumor all was analyzed by flow cytometry on parafflnembedded specimens in 73 patients with epithelial ovarian tumor, and its clinical significance was evaluated. One of the 5 benign (20%), 2 of the 11 borderline (18.18%), and 30 of the 57 malignant (52. 63%) tumors were aneuplold. The occurrence rate of aneuploidy In malignant tumors was higher than In benign and borderline tumors ( P < 0. 05 ). Furthermore, aneuploidy was more frequently In the advanced stages (Ⅲ -Ⅳ ) (77. 7%) than in the early stages (Ⅰ - Ⅱ ) (9. 5%) (P<0. 005). The occurrence rate of DNA aneuploidy was higher in patients associated with ascites and the residual tumor≥.2 cm. Patients with aneuploid tumors had more of ten ascites (P<0. 005) and residual tumor size≥2cm (P< 0.005). There was no apparent correlation between the DNA ptoidy and the histologic grade, histologic type of the tumors. G0/G1 cell proportion of DNA diplold tumors in advanced carcinoma (64. 6%) was less than those of early stage carcinoma (75. 9% ) (P<0. 05). The survival rate of diplold tumor patients was higher than that of aneuploid tumor patients in the different time after operation, and the median survival time was 30. 2 months and 10. 3 months, respectively. Multivariate analysis revealed that cellular DNA ploidy was the most Important predictive factor (P = 0. 007) of prognosis, followed by residual tumor size (P= 0. 05). Different tumor specimen of the same patient can exhibit variation sometime (38. 9%).The results revealed that the DNA ploidy may reflect tumor biological characteristics, I. e. , Its proliferative ability. Analysis of cellular DNA content of epithelial ovarian tumors would help us to predict the prognosis of the patients better.

Objective: To study the effects of regenerated tissue extracts after liver injury on the proliferation, differentiation, migration and invasion of SK-HEP1 cells. Methods: Regenerated tissue extracts after liver injury were used to induce SK-HEP1 cells after enrichment, their effects on the proliferation, differentiation, migration and invasion of SK-HEPI cells were observed through in vitro cell culture, MTT, flow cytometry and transwell assays. Results:In response to the action of regenerated tissue extracts after liver injury, SK-HEP1 cells were blocked in G_0/G_1 phase, their growth rate was distinctly reduced. The number of SK-HEP1^(-fj)colonies decreased. The migration ability of SK-HEPI cells showed a decreased trend on day7 and day 11 after induction. SK-HEPl's invasion ability clearly decreased on days 7 and11 after induction, especially on day 7. Conclusions: To a certain extent, regenerated tissue extracts after liver injury can inhibit the proliferation, differentiation, migration and invasion of hepatoma cells, showing an important potential of being a differentiating agent for the treatment of liver cancer.
