Can patients with mild non-neoplastic lesions diagnosed at baseline screening be safely exempt from surveillance: evidence from multicenter community-based cohorts

Surveillance recommendations for gastric cancer（GC） in current guidelines focused on advanced precancerous lesions and were based on precise diagnosis of severity/extent of baseline lesions. We aimed to develop a less endoscopy-related equipment-dependent risk-stratification tool, and assessed whether mild-precursor-lesion patients can be safely exempt from surveillance. In the multicenter communitybased cohort, 75,051 participants receiving baseline endoscopy were enrolled during 2015–2017 and followed-up until 2021. Cumulative incidence rates（CIRs） of GC for precancerous-conditions were calculated by Kaplan-Meier method and compared by Log-rank tests. Mixedeffects Cox regression models were used to detect potential factors for progression towards GC. A risk score was calculated as counts of selected factors. An independent cohort, including 26,586 participants was used for external validation. During a median follow-up of 6.25 years, CIRs of GC were 0.302%, 0.436%, and 4.756% for normal group, non-neoplastic（atrophic gastritis/intestinal metaplasia） and neoplastic lesions（low-grade/high-grade dysplasia）, respectively(Ptrend<0.001). Four predictors, including male, ≥60 years, smoking, and limited vegetable consumption, were selected for risk-stratification. High-risk patients（≥3 risk factors） with non-neoplastic lesions showed higher GC risks（adjusted HR=7.73, 95%CI: 4.29–13.92）, and their four-year CIR reached the one-year CIR of neoplastic lesions. Further categorizing non-neoplastic lesions by histological grade, both patients with moderate-to-severe lesions（aHR=3.07, 95%CI: 1.67–5.64） and high-risk patients with mild lesions（aHR=7.29, 95%CI: 3.58–14.86） showed higher risks. Consistent trends were observed in validation cohort. High-risk mild-precursor-lesion patients should receive surveillance within 3–5 years after baseline screening. Our study provides evidence on supplementing current guideline recommendations.

AIM:To investigate the efficacy and safety of capecitabine plus irinotecan±bevacizumab in advanced or metastatic colorectal cancer patients. METHODS:Forty six patients with previously untreated,locally-advanced or metastatic colorectal cancer(mCRC) were recruited between 2001-2006 in a prospective open-label phaseⅡtrial,in German community-based outpatient *** received a standard capecitabine plus irinotecan(CAPIRI) or CAPIRI plus bevacizumab(CAPIRI-BEV) regimen every 3 wk. Dose reductions were mandatory from the first cycle in cases of>grade 2 *** treatment choice of bevacizumab was at the discretion of the *** endpoints were response and toxicity and secondary endpoints included progression-free survival and overall survival. Several factors have led to the decline of electricity generation from coal over the past decade, and projections forecast high rates of growth for wind and solar technologies in coming years. This analysis uses hourly generation data from large coal-fired power stations to determine how operations have been modified in recent years and describes the implications of these changes for plant equipment and unit reliability. The data shows increasing variability in intraday generation output that affects nearly all of the units in the sample, but the magnitude of increase varies widely among plants. Outage patterns were examined as was the relationship between renewable energy growth in a region and the changes in coal plant operations. Aggregate direct and indirect costs associated with running coal plants as load-following units have not yet been quantified in large-scale studies on a sector-wide basis, largely due to differences in how specific equipment responds to output fluctuations. Due to findings from the hourly generation data analysis and the high degree of potential impact on coal plant equipment, the study suggests the development of a new modeling tool that will represent the costs of running coal-fired power plants at lower capacity factors.

The title compound C18H18N4OS has been synthesized by the reaction of 3-（2-hydroxy- benzyl）-4-amino-（1H）-1,2,4-triazole-5-thione with 4-isopropylbenzaldehyde in ethanol and characterized by IR, ^1H NMR spectra and elemental analysis. Its structure was determined by X-ray diffraction analysis. The crystal belongs to monoclinic, space group P21/c with a = 11.605（2）, b = 7.401（1）, c = 20.339（2） A, β= 103.05（2）°, V= 1701.8（4） A^3, Z = 4, Mr = 338.42,μ = 0.202 mm^-1, Dc = 1.321 g/cm^3 and F（000） = 712. The structure was solved by direct methods and refined to R = 0.0428 and wR = 0.1069. Due to the intramolecular O-H…N hydrogen bond and π-π stacking interactions between the benzene （C（1）～C（6）） and triazole rings, the two planes are essentially coplanar. Their biological activities have been measured, showing this type of compound has certain antibacterial activity for Staphylococous aureus and Bacillus subtilis. Based on the quantum chemistry calculation at the RHF/6-31G level, the frontier orbitals and electrostatic potential of the title compound were also discussed.

We have carried out a bibliometric analysis on the development of ametropia literature to determine its growth rule and tendency, and to provide the basis for the problems related to ametropia research. Literatures that contained the descriptors of ametropia in title or paper published before Nov. 10, 2010 in PubMed databases (***/Pubmed) were selected. As bibliometric indicators of ametropia, biomedical journals referring to ophthalmology by ISSN were calculated. The principal bibliometric indicators: Price's and Bradford's laws were applied on the increase or dispersion of scientific literature, the participation index of languages and the journals. By means of manual coding, literatures were classified according to documents study and statistical analysis. The literatures cited in ametropia, astigmatism, myopia and hypermetropia had accumulated to 26475, which consists of Review (n =1560), Randomized Controlled Trial (n =776), Practice Guideline (n =10), Meta-Analysis (n=23), Letter (n=1222), Editorial (n =328), Clinical Trial ( n =1726) and Others (n=20830); and Humans (n=23073), Animals (n=1434) and Others ( n=1968). 1136 literatures were included in PubMed Central, 22384 in MEDLINE and 2955 in others. The ametropia literatures rose every 5 years which of the ametropia-year cumulated amount of the literatures had three periods: before 1900, slowly increasing from 1901 to 1950, rapidly rising from 1951 to 2010 (increased approximate exponentiation exponent). Sixty kinds of languages were listed in PubMed databases, of which English was dominant for aborting to ametropia research documents before 2010 (77.32%, 20471/26475). The document language of top eight accounted for 95.58% (English, German, French, Japanese, Russian, Italian, Spanish, Chinese), and others for 4.42% (1171/26475). AIM：To quantify the association between diabetes and glaucoma using ***：Pub Med and Embase were searched using medical subject headings and key words related to diabetes and *** inclusion criteria were：1）the study design was a prospective cohort study;2）the exposure of interest was diabetes;3）the outcome of interest was primary open angle glaucoma（POAG）;4）risk ratios（RR）and the corresponding 95%confidence interval（CI）.Data were pooled using fixed effects models to take into account heterogeneity between *** prospective studies were *** increased the incidence of glaucoma by 36%（OR=1.36,95%CI=1.25-1.50）.There was no evidence of statistical heterogeneity（I2=0,P=0.53）or publication bias（the funnel plot did not identify obvious asymmetry）.RESULTS：Seven prospective cohort studies were incorporated in this *** pooled RR of the association between POAG and diabetes based on the risk estimates of the seven cohort studies was 1.36（95%CI=1.24-1.50）,with no significant heterogeneity across studies（I2=0;P=0.526）.The sensitivity analysis yielded a range of RRs from 1.34（95%CI=1.22-1.48）to1.40（95%CI=1.18-1.67）.CONCLUSION：Diabetes is associated with a significantly increased risk of glaucoma.

AIM: To investigate the effects of Japanese apricot(JA) consumption on gastroesophageal reflux disease(GERD)-related ***: Participants included individuals living in Minabe-cho,a well-known JA-growing region,who received specific medical check-ups by the local community health service in ***-related symptoms were examined in 1303 Japanese individuals using a validated questionnaire,the Frequency Scale for Symptoms of GERD(FSSG),which consists of 7 questions associated with acid reflux symptoms and 5 questions asking about gastrointestinal dysmotility *** question was answered using a 4-point scale,with higher scores indicating more severe GERDrelated *** were divided into two groups according to their intake of dried and pickled JA: daily intake(≥ 1 JA daily)(392 subjects) and none oroccasional intake(< 1 JA daily)(911 subjects).FSSG scores were compared between subjects who consumed JA daily and those who did ***,subjects were stratified by age,gender and Helicobacter pylori(***) status for ***: Those who ate JA daily were significantly older than those who did not(60.6 ± 10.5 years vs 56.0 ± 11.0 years,P < 0.001).Total FSSG scores were significantly lower in subjects with daily JA intake than in those with none or only occasional intake(2.13 ± 3.14 vs 2.70 ± 3.82,P = 0.005).In particular,subjects who consumed JA daily showed significantly improved FSSG dysmotility scores compared with subjects who did not(1.05 ± 1.58 vs 1.46 ± 2.11,P < 0.001).In contrast,the FSSG reflux score did not differ between subjects with and without daily intake of JA(1.08 ± 1.90 vs 1.24 ± 2.11,P = 0.177).Subanalysis indicated that improvement in dysmotility by JA intake was specifically observed in non-elderly(1.24 ± 1.68 vs 1.62 ± 2.22,P = 0.005) and ***-negative subjects(0.99 ± 1.58 vs 1.57 ± 2.06,P < 0.001).GERD patients(total FSSG score ≥ 8) were less frequently observed among subjects with daily intake of JA as
