Improved guided bone regeneration by combined application of unmodified, fresh autologous adipose derived regenerative cells and plasma rich in growth factors:A first-in-human case report and literature review

BACKGROUND Novel strategies are needed for improving guided bone regeneration(GBR) in oral surgery prior to implant placement, particularly in maxillary sinus augmentation(GBR-MSA) and in lateral alveolar ridge augmentation(LRA). This study tested the hypothesis that the combination of freshly isolated, unmodified autologous adipose-derived regenerative cells(UA-ADRCs), fraction 2 of plasma rich in growth factors(PRGF-2) and an osteoinductive scaffold(OIS)(UAADRC/PRGF-2/OIS) is superior to the combination of PRGF-2 and the same OIS alone(PRGF-2/OIS) in GBR-MSA/*** SUMMARY A 79-year-old patient was treated with a bilateral external sinus lift procedure as well as a bilateral lateral alveolar ridge augmentation. GBR-MSA/LRA was performed with UA-ADRC/PRGF-2/OIS on the right side, and with PRGF-2/OIS on the left side. Biopsies were collected at 6 wk and 34 wk after GBRMSA/LRA. At the latter time point implants were placed. Radiographs(32 mo follow-up time) demonstrated excellent bone healing. No radiological or histological signs of inflammation were observed. Detailed histologic,histomorphometric, and immunohistochemical analysis of the biopsies evidenced that UA-ADRC/PRGF-2/OIS resulted in better and faster bone regeneration than PRGF-2/*** GBR-MSA with UA-ADRCs, PRGF-2, and an OIS shows effectiveness without adverse effects.

Bone remodeling is balanced by bone formation and bone resorption as well as by alterations in the quantities and functions of seed cells, leading to either the maintenance or deterioration of bone status. The existing evidence indicates that micro RNAs（mi RNAs）, known as a family of short non-coding RNAs, are the key post-transcriptional repressors of gene expression,and growing numbers of novel mi RNAs have been verified to play vital roles in the regulation of osteogenesis, osteoclastogenesis,and adipogenesis, revealing how they interact with signaling molecules to control these processes. This review summarizes the current knowledge of the roles of mi RNAs in regulating bone remodeling as well as novel applications for mi RNAs in biomaterials for therapeutic purposes.

Non-Diabetic Renal Disease in Patients with Type 2 Diabetes Mellitus with Proteinuria

Background: Diabetes mellitus (DM) is the leading cause of end stage renal disease (ESRD) worldwide. Although DM with proteinuria is the ultimate result of diabetic nephropathy (DN), a wide spectrum of non-diabetic renal diseases (NDRD) can occur in such patients. Objective: To observe the frequency and histological pattern of NDRD in diabetic patients with proteinuria and to explore their association with clinical and laboratory parameters. Methods: This cross-sectional study was conducted in the department of Nephrology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from April 2016 to September 2017. In this study a total of 38 cases of DM with proteinuria (>1 gm/24-hour) were selected purposively. Renal biopsy was done in all patients. Based on histological findings they were categorized into two groups;Group 1 with NDRD and Group II with DN. Their clinical and laboratory parameters were analyzed and compared. Results: Among the total study subjects, 21 (55.3%) were male and 17 (44.7%) were female, mean (±SD) age 43.45 ± 9.99 years in the NDRD group and 41.57 ± 9.50 years in the DN group. Thirty one cases (81.6%) out of thirty eight had NDRD and seven (18.4%) cases had isolated DN;therefore more than two third cases had NDRD. Duration of DM was found to be significantly shorter (p = 0.004) in the NDRD group. Diabetic retinopathy was present in 12.9% cases in NDRD group vs. 57.1% cases in DN group (p = 0.025). Frequency of microscopic hematuria was significantly higher (90.3%) in NDRD patients (p = 0.002). Conclusion: The frequency of NDRD in type 2 diabetic patients other than diabetic nephropathy is relatively high. Membrano proliferative glomeru-lonephritis and membranous nephropathy are more common in NDRD. Absence of diabetic retinopathy, presence of hematuria and shorter duration of DM are markers associated with NDRD in type 2 DM, which are important indicators for renal biopsy in diabetic patients with proteinuria.

Comparison of skeletal, dental and soft tissue changes in young adults with Class II malocclusion, treated either by camouflage, fixed functional appliance or orthognathic surgery—A prospective study on Indian subjects

Introduction: Borderline Class II malocclusion due to deficient mandible can be treated either by orthodontic camouflage, fixed functional appliances or by orthodontics followed by surgical mandibular advancement. Methodology: A prospective study was designed on young adults with Class II malocclusion on account of a deficient mandible. A total of 45 subjects were divided into three groups of 15 individuals each. The patients were treated either by camouflage, fixed functional appliances or by orthognathic surgery. Pre and post treatment cephalograms were used to assess the skeletal, dental and soft tissue changes. Pre and post treatment profile photographs were assessed on a Visual Analogue Scale (VAS) by orthodontists, oral surgeons and laypersons. Results: Each group achieved a reduction in facial convexity, but the results obtained from the surgical group were more pronounced than the camouflage and the fixed functional group. Conclusion: The reduction in convexity in the camouflage group was by retracting the upper anteriors, which increases the nasolabial angle. In the fixed functional appliance a combination of skeletal and dentoalveolar changes can be observed. However the most appropriate reduction in profile convexity can be obtained by combined orthodontic and surgical treatment of malocclusion.

Presurgical Nasoalveolar Moulding—An Adjunct to Facilitate Surgical Repair in Infants with Cleft Lip and Palate

Presurgical orthopedics forms an integral part of the treatment of patients with facial clefting. The primary aim of presurgical nasoalveolar moulding (PNAM) is reduction in the soft tissue and cartilaginous deformity to facilitate surgical soft tissue repair in optimal conditions under minimum tension to minimize scar formation. It also helps in reducing the nasal deformity with a nasal stent which brings the deformity near to the normal and forms an important part of the primary nasal repair at the time of primary lip repair. A soft acrylic relining material is added or removed to line the appliance to a thickness of approximately 1 - 1.5 mmin the region from where the bone is to be resorbed or apposed. This article describes the fabrication procedure of the modified nasal alveolar molding appliance in eight infants with unilateral cleft lip and palate in whom a significant reduction in cleft width and nasoalveolar deformity was achieved. The results showed that the greater segment shows more favourable changes and can be molded more positively and efficiently than the lesser segment.

Evaluation of Orthodontic Treatment Needs among Widou Schoolchildren in Ferlo (SENEGAL): A Transversal Study

The aim of this study was to evaluate orthodontic treatment needs among Ferlo scholars. The design of the study was descriptive and transversal. For Inclusion criteria, children must live in Widou or live around Widou’s school at least 6 km, be students at primary school of Widou Thiengoly, have a general health state which permits a clinical oral examination and teacher’s and/or parent’s agreement. The investigation was exhaustive and concerned the only school of the locality. More of 4/5 schoolchildren was 10.25 years ± 2.15 old. The majority (83.74%) of children presented an overjet between 0 and 3.5 mm, 1.63% presented a crossbite and 15.45% had OIM and RC different over to 2 mm. More of 30% of schoolchildren presented an overbite between 1 and 2 mm. More of 1/10 of them had an aesthetic teeth perception classified levels 8.9 and 10 corresponding to an orthodontic treatment needed according to IOTN while 20.33% had a score of 4 and 3.25% recorded score of 5 corresponding to a high treatment needed according to IOTN. The results of this study reveal that people need to be informed better about the therapeutic possibilities that orthodontics offer.

Polyetherketoneketone(PEKK)exhibits admirable biocompatibility and mechanical performances but bioinert while tantalum(Ta)possesses excellent osteogenesis and osseointegration but high elastic modulus and density,and processing is too difficult and *** the present study,combining of the advantages of both PEKK and Ta,implantable composites of PEKK/Ta were fabricated by blending PEKK with Ta microparticles of 20 v%(PT20)and 40 v%(PT40)*** comparison with PT20 and PEKK,the surface hydrophilicity,surface energy,roughness and proteins adsorption as well as mechanical performances of PT40 significantly increased because of the higher Ta particles content in ***,PT40 exhibited the mechanical performances(e.g.,compressive strength and modulus of elasticity)close to the cortical bone of *** with PT20 and PEKK,PT40 with higher Ta content remarkably enhanced the responses(including adhesion,proliferation and osteogenic differentiation)of MC3T3-E1 cells in ***,PT40 markedly improved bone formation as well as osseointegration in *** short,incorporation of Ta microparticles into PEKK created implantable composites with improved surface performances,which played key roles in stimulating cell responses/bone formation as well as promoting ***40 might have great potential for bear-loading bone substitute.

As an implantable biomaterial,polyetherketoneketone(PEKK)exhibits good mechanical strength but it is biologically inert while tantalum(Ta)possesses outstanding osteogenic bioactivity but has a high density and elastic ***,silicon nitride(SN)has osteogenic and antibacterial *** this study,a microporous surface containing both SN and Ta microparticles on PEKK(STP)exhibiting excellent osteogenic and antibacterial activity was created by *** with sulfonated PEKK(SPK)without microparticles,the surface properties(roughness,surface energy,hydrophilicity and protein adsorption)of STP significantly increased due to the SN and Ta particles presence on the microporous *** addition,STP also exhibited outstanding antibacterial activity,which inhibited bacterial growth in vitro and prevented bacterial infection in vivo because of the presence of SN ***,the microporous surface of STP containing both SN and Ta particles remarkably induced response(e.g.,proliferation and differentiation)of rat bone mesenchymal stem(rBMS)cells in ***,STP significantly improved new bone regeneration and osseointegration in *** the induction of cellular response in vitro and improvement of osseointegration in vivo,the microporous surface containing Ta was better than the surface with SN *** conclusion,STP with optimized surface properties activated cellular responses in vitro,enhanced osseointegration and prevented infection in ***,STP possessed the dual biofunctions of excellent osteogenic and antibacterial activity,showing great potential as a bone substitute.

Skeletal health relies on architectural integrity and sufficient bone mass, which are maintained through a tightly regulated equilibrium of bone resorption by osteoclasts and bone formation by osteoblasts. Genetic studies have linked the gene coding for low-density lipoprotein receptor-related protein1(Lrp1) to bone traits but whether these associations are based on a causal molecular relationship is unknown. Here, we show that Lrp1 in osteoblasts is a novel regulator of osteoclast activity and bone *** lacking Lrp1 specifically in the osteoblast lineage displayed normal osteoblast function but severe osteoporosis due to highly increased osteoclast numbers and bone resorption. Osteoblast Lrp1 limited receptor activator of NF-κB ligand(RANKL) expression in vivo and in vitro through attenuation of platelet-derived growth factor(PDGF-BB) signaling. In co-culture, Lrp1-deficient osteoblasts stimulated osteoclastogenesis in a PDGFRβ-dependent manner and in vivo treatment with the PDGFR tyrosine kinase inhibitor imatinib mesylate limited RANKL production and led to complete remission of the osteoporotic phenotype. These results identify osteoblast Lrp1 as a key regulator of osteoblast-to-osteoclast communication and bone mass through a PDGF–RANKL signaling axis in osteoblasts and open perspectives to further explore the potential of PDGF signaling inhibitors in counteracting bone loss as well as to evaluate the importance of functional LRP1 gene variants in the control of bone mass in humans.
