Increased hepatic peroxisome proliferator-activated receptor coactivator-la expression precedes the development of insulin resistance in offspring of rats from severe hyperglycemic mothers

Background Prenatal hyperglycaemia may increase metabolic syndrome susceptibility of the offspring. An underlying component of the development of these morbidities is hepatic gluconeogenic molecular dysfunction. We hypothesized that maternal hyperglycaemia will influence her offsprings hepatic peroxisome proliferator-activated receptor coactivator-la （PGC-la） expression, a key regulator of glucose production in hepatocytes. Method We established maternal hyperglycaemia by streptozotocin injection to induce the maternal hyperglycaemic Wistar rat model. Offspring from the severe hyperglycemia group （SDO） and control group （CO） were monitored until 180 days after birth. Blood pressure, lipid metabolism indicators and insulin resistance （IR） were measured. Hepatic PGC-la expression was analyzed by reverse transcription polymerase chain reaction and Western blotting, mRNA expression of two key enzymes in gluconeogenesis, glucose-6-phospha-tase （G-6-Pase） and phosphoenolpyruvate carboxykinase （PEPCK）, were analyzed and compared. Results In the SDO group, PGC-la expression at protein and mRNA levels were increased, so were expression of G-6-Pase and PEPCK （P〈0.05）. The above effects were seen prior to the onset of IR. Conclusion The hepatic gluconeogenic molecular dysfunction may contribute to the metabolic morbidities experienced by this population.

Objective:To evaluate the effectiveness and safety of Zhenqi Buxue Oral Liquid(ZQ),progesterone capsules,and their combination in treating oligomenorrhea and hypomenorrhea with qi-blood and Kidney(Shen)essence ***:This was a prospective,randomized,multi-center controlled trial between June 2022 to December ***-six oligomenorrhea and hypomenorrhea patients with qi-blood and Shen essence deficiency were randomly assigned to receive ZQ(ZQ group,29 cases),progesterone capsules(PG group,32 cases),or the combined Chinese and Western medicine(COM group,31 cases)at a ratio of 1:1:*** in the ZQ or PG group took daily 10 m L twice a day of ZQ or 200 mg once a day of progesterone capsules for 10 consecutive days on day 15 of the menstrual cycle respectively,and patients in the COM group received the same ZQ combined with progesterone *** treatment course lasted for 3 months and follow-up was performed at 1 and 3 months after the end of *** endpoint was the menstrual Traditional Chinese Medicine Syndrome Scale(TCMSS)*** endpoints included pictorial blood loss assessment chart(PBAC)scores,clinical efficacy rate,36-item Short Form health Survey(SF-36)scores,sex hormones and thickness of *** events(AEs)were ***:TCMSS scores after 1-and 3-month treatment in all groups were significantly lower than those at baseline(P<0.05).Only TCMSS scores after 3-month treatment in the ZQ and COM groups continuously decreased compared with those after 1-month treatment in the same group(P<0.01).TCMSS scores after 3-month treatment in the ZQ and COM groups were significantly lower than those in the PG group(P<0.05,P<0.01).Compared with baseline,PBAC scores in the ZQ and COM groups after 3 months of treatment were also significantly higher(both P<0.01).The total effective rates of TCM syndrome of 3-month treatment were significantly improved in all groups compared with that after 1 month of treatment(P<0.

Objective:To investigate the risk factors of severe postpartum hemorrhage(PPH)in cesarean section of twin pregnancy,and to provide clinical basis for pregnancy management and perioperative obstetric management of twin ***:The clinical data of 631 twin pregnancies with gestational age28 weeks delivered by cesarean section at Peking University People's hospital(PKUPH)from January 2004 to January 2017 were retrospectively *** of conception,the combined weight of twins,serum albumin level before cesarean section,operation time and other factors on the amount of blood loss during cesarean section were ***:The proportion of severe PPH was significantly higher in in vitro fertilization-embryo transfer(IVF-ET)group,the combined weight of twins>6000g group,serum albumin before cesarean section0.05).Moreover,according to the surgical indications,the emergency surgery group was divided into premature rupture of membranes(PROM),labor,fetal distress and others groups,no significant difference were detected among these groups(P>0.05).Conclusion:IVF-ET,the combined weight of twins,serum albumin before operation were significantly correlated with severe PPH of twin pregnancies delivered by cesarean section,revealing that it is necessary to strengthen pregnancy management of twin pregnancy.

INTRODUCTION Gestational diabetes mellitus （GDM） was earlier defined as ＂hyperglycemia first recognized during pregnancy＂ and has more recently been described by American Diabetes Association （ADA）（2012） as diabetes diagnosed during pregnancy that is not clearly overt diabetes. The hyperglycemia and adverse pregnancy outcomes study （HAPO） demonstrated that the risk of adverse maternal,fetal,and neonatal outcomes continuously increase as a function of maternal glycemia at 24-28 weeks,even within ranges previously considered normal for pregnancy.

Prodrug self-delivery carriers with targeting that specifically responded to tumor microenvironments have good potential to improve the application dilemma of approved clinical therapeutic drugs(systemic distribution and side effects).It's noted the conversion of gemcitabine(GEM)to inactive ingredients under the action of cytidine deaminase(CDA)during metabolism in vivo limits its clinical effect.A high level of reactive oxygen species(ROS)results in a high level of oxidative stress in tumor cells,which changes the expression of CDA and optimizes the metabolism of GEM in vivo and overcome drug *** this study,the ROS responsive and ROS self-supplied prodrug of artemisia(ART)-thioacetal bond(TK)-GEM was synthesized and self-vectors based on ART-TK-GEM(TK@FA NPs)was prepared by using nano *** responsive characteristics ensure specific release of prodrugs in tumor cells with high level of ROS thereby reducing side effects on normal cells and *** endogenous ROS and newly generated ROS by ART can reduce the expression of CDA and optimizes the metabolism of GEM,and the accumulated ROS can also induce apoptosis of tumor cells,realizing synergistic anti-tumor effect of chemical drugs and traditional Chinese *** paper proposes a simple method by using clinically approved drugs to improve the insufficient effect of existing chemotherapy and overcome resistance,which has potential to appropriately shorten the drug development cycle and accelerate the clinical investigation of drugs.

China's universal two child policy was released in October of *** would this new policy influence the rate of overall cesarcan delivery(CD)in China?The objective of this paper is to investigate the trend of overall CD rate with the increase of number of multiparous women based on a big childbirth center of China(a tertiary hospital)in *** this study,22530 cases from the medical record department of a big childbirth center of China from January 1 to December 31 in 2016 were entolled as research *** health records of these selected objects were *** t0 the history of childbirth,the selected cases were divided into primiparous group containing 16340 cases and multiparous group containing 6190 ***-square test was carried out to compare the rate of CD,neuraxial labor analgesia,maternity insurance between the two groups;1-test was performed to compare the in-hospital days and gestational age at birth between the two *** corrclation coefficient was used to evaluate the rclationship among observed monthly rate of multiparas,overall CD rate,and Elective Repeat Cesarean Delivery(ERCD)*** results showed that the CD rate in multiparous group was 55.46%,which was higher than that in primiparous group(34.66%,Phospital cost and in-hospital days in multiparas group were higher,and the gcstational age at birth in multiparas group was lower than in primiparas group(P<0.05).The overall CD rate slightly dropped in the first 4 months of the year(P<0.05),then increased from 36.27%(April)to 43.21%(Dcember)(P<0.05).The rate of multiparas women and ERCD had the same trend(P<0.05).There were linear correlations among the rate of overall CD,the rate of multiparas women and t
