Craniotomy with endoscopic assistance in the treatment of nasopharygeal fibroangioma

Background Nasopharygeal fibroangioma （NPF） can be approached through lateral rhinotomy, the middle skull fossa approach and the transcranial-facial combined approach. It is complicated and thus results in more insults, and when adopted, the total resection rate of tumor is still low. The nasal endoscope is minimally invasive, the dead angles of a craniotomy, such as sphenoid sinus, maxillary sinus, and nasopharynx are easily approached by an endoscope. Lateral rhinotomy have to make facial incision and affects maxillary bone development. We combined the craniotomy and endoscopic approach intending to take advantages of the two approaches. Methods Twelve NPF patients who underwent craniotomy with endoscopic assistance from March 2002 to July 2008 at the Beijing Tongren Hospital were selected. All patients were male. Their ages ranged from 11 to 33 years. The main symptoms were visual deterioration, exophthalmos, nasal obstruction, epistaxis and pharynx nasalis neoplasm. The diagnosis was based on CT, MRI and digital subtraction angiography （DSA）. All patients had intracranial encroachment and all underwent DSA and embolism treatment were taken before surgery. Seven patients had a pterional craniotomy, five had a frontal-temporal-orbital-zygomatic craniotomy. Most of the tumor was resected piecemeal, then removed through the sphenoidal sinus. Finally, using an endoscope in the nasal cavity, tumor in nasal cavity was resected and removed through the sphenoidal sinus, observing the dead angle of the craniotomy and confirming that sinus drainage was unobstructed. Results The tumor was removed completely in 11 patients and partially resected in one patient because of hemorrhage One patient had an infection after the operation and one patient had cerebrospinal rhinorrhea 3 years after surgery that was remediated by endoscopic repair. Because of the unique features of spherical symmetry,angle-independency,good monodispersity,controllable components and morphologies,structural color particles(SCPs)have found great significances in various fields such as sensing,monitoring,biological assays,***,inspired by the melanosome-derived bright structural colors and the self-adhesivity of mussels,we present a kind of bioinspired SCPs assembled from polydopamine(PDA)-adhered multi-layer graphene oxide(GO)encapsulated silica nanoparticles(SNs).It is demonstrated that compared with traditional SCPs,the designed particles possess brighter and more vibrant structural colors,and no complicated modification is required during the functionalization process due to the abundant inherent functional groups of *** resultant SCPs are verified to be capable for direct hybridization chain reaction and multiplexed nucleic acid *** properties indicate the promising prospects of our designed SCPs.

Effect of 17β-estradiol on the Brain Damage and Metabolic Changes in Rats

An in vivo model of glutamate excitotoxicity in which glutamate is applied to the cortex of rats through a microdialysis probe has been used to investigate the neuroprotective processes initiated by 17β estradiol. Rats were pre treated with 17β estradiol i.v. before local application of glutamate. The experimental results showed that pre treatment with 17β estradiol significantly reduced the size of the glutamate induced lesion. In the microdialysates, the peak of lactate observed immediately after glutamate application was significantly higher and longer lasting after 17β estradiol pre treatment. The level of extracellular glucose was markedly decreased concomitantly to the increase in lactate, but no difference could be observed with and without 17β estradiol pre treatment. These suggest a new neuroprotective mechanism of 17β estradiol by activating glutamate induced lactate production. This effect on lactate production and lesion reduction is estrogen receptor dependent and is abolished totally by estrogen antagonist tamoxifen. It was also demonstrated here that high lactate subserves estrogen neuroprotection during glutamate toxicity.

Background:Conventional treatment has limited efficacy in relapsed/refractory B-cell *** chimeric antigen receptor T-cell(CAR-T)technology has shown high safety and results in high remission rates,we investigated its efficacy and safety in B-cell lymphoma treatment and analyzed potential affecting factors to provide evidence for therapeutic strategies and ***:We searched databases including PubMed,Embase,and Cochrane up to July ***-analysis 1 was conducted to study the efficacy of CAR-T cell for treating B-cell lymphoma,measuring the response rate and complete remission rate as ***-group analysis was performed for age,pathological type,target antigen,co-stimulatory molecule,and conditioning ***-analysis 2 was undertaken on the safety of the treatment with the incidence rate of toxicity(cytokine-releasing syndrome[CRS],neurotoxicity)as an ***:Seventeen studies were included in the systematic review and *** was found that CAR-T cells had good therapeutic effects in the following cases:B-cell lymphoma(patients≥65 years old);diffuse large B-cell lymphoma pathological type;patients with treatment target antigen other than CD19;patients treated with co-stimulatory molecules other than CD28,including 4-1BB+CD28 or 4-1BB;and patients treated with cyclophosphamide/fludarabine pre-treatment protocol conditioning *** the CRS and neurotoxicity incidences were high,most were reversible with minimal risk of ***:CAR-T cell treatment is safe for clinical application;however,toxicity effects should be monitored.

Background: Endoscopic transnasal skull base surgery had started long time ago in different centers around the world for excision of skull base lesions with good results and more cost effectiveness. The aim of this study is to discuss our early results in endoscopic skull base surgery and the development of the learning curve. Patients and Methods: We analyzed our experience regarding 25 patients presented to us in neurosurgery department, Assiut university Hospital, Assiut university, Assiut, Egypt in a period of 3 years (2015, 2016, 2017) and operated by endoscopic transnasal approach. All patients signed an informed consent. Results: With the highest percentage was pituitary adenoma 56%, pituitary apoplexy 12%, craniopharyngioma 12%, CSF rhinorrhea 12%, Planum sphenoidal meningioma 4% and suprasellar granuloma 4%. 88% of patients were operated without complications, 8% mortality rate postoperative, 12% complication rate and 76% complete improvement postoperative. Conclusion: Endoscopic skull base surgery is a safe approach to the skull base that needs a good experience, practice and good anatomical knowledge. Teamwork between a Neurosurgeon and ent surgeon is a must for patient safety.

“Ski”-Shaped Microneedle Facilitates Suturing in a Deep Narrow Surgical Field

Background: Microsurgical suturing requires high level of dexterity and practice. Suturing in a deep and narrow operative field poses additional difficulties mainly due to the long microsurgical instruments and the difficulty of manipulating a curved needle. Objective: In this technical note, we describe a simple modification of a classical curved suture needle into a “ski”-shaped needle. Methods: A classical curved needle is stretched using a needle holder. Only the tip of the needle is left slightly bent, imitating a ski. Once the needle is bent into the ski shape, it can be grasped virtually in any position with the needle holder. Results: We have used the ski needle technique suturing in several patients in which the operative field was deep and limited such as dura repair in spinal surgery in obese patients, facial nerve grafting in the internal acoustic meatus and posterior inferior cerebellar artery suturing between the caudal cranial nerves. Conclusion: This modification facilitates free positioning of the needle in the needle holder and as a consequence results in easy and faster microsuturing in a deep and narrow operative field. The ski needle technique was felt to be superior to the classical curved needle technique.

Today, the ultrasound guidance (USG) in regional anesthesia is gold standard more and more often used for medial or paramedian approaches around to the spine, such as the paravertebral (PV) block. Local anatomical changes may greatly handicap the performance of this type of block. We present clinical, sonographic, and radiological data on successful PV block and catheter placement in four patients with vertebral diseases, targeting thoracotomy or lumbotomy postoperative pain after stabilization of the involved vertebral body and preliminary arthrodesis with laminectomy by the posterior approach. We emphasize the importance of USG in this special context involving local anatomical disturbance.
