Activation of cerebral Ras-related C3 botulinum toxin substrate(Rac) 1 promotes post-ischemic stroke functional recovery in aged mice

Brain functional impairment after stroke is common;however,the molecular mechanisms of post-stroke recovery remain unclear.It is well-recognized that age is the most important independent predictor of poor outcomes after stroke as older patients show poorer functional outcomes following stroke.Mounting evidence suggests that axonal regeneration and angiogenesis,the major forms of brain plasticity responsible for post-stroke recovery,diminished with advanced age.Previous studies suggest that Ras-related C3 botulinum toxin substrate(Rac)1 enhances stroke recovery as activation of Rac1 improved behavior recovery in a young mice stroke model.Here,we investigated the role of Rac1 signaling in long-term functional recovery and brain plasticity in an aged(male,18 to 22 months old C57BL/6J)brain after ischemic stroke.We found that as mice aged,Rac1 expression declined in the brain.Delayed overexpression of Rac1,using lentivirus encoding Rac1 injected day 1 after ischemic stroke,promoted cognitive(assessed using novel object recognition test)and sensorimotor(assessed using adhesive removal tests)recovery on days 14–28.This was accompanied by the increase of neurite and proliferative endothelial cells in the periinfarct zone assessed by immunostaining.In a reverse approach,pharmacological inhibition of Rac1 by intraperitoneal injection of Rac1 inhibitor NSC23766 for 14 successive days after ischemic stroke worsened the outcome with the reduction of neurite and proliferative endothelial cells.Furthermore,Rac1 inhibition reduced the activation of p21-activated kinase 1,the protein level of brain-derived neurotrophic factor,and increased the protein level of glial fibrillary acidic protein in the ischemic brain on day 28 after stroke.Our work provided insight into the mechanisms behind the diminished plasticity after cerebral ischemia in aged brains and identified Rac1 as a potential therapeutic target for improving functional recovery in the older adults after stroke.

Ischemic stroke can cause blood-brain barrier(BBB)injury,which worsens brain damage induced by stroke.Abnormal expression of tight junction proteins in endothelial cells(ECs)can increase intracellular space and BBB leakage.Selective inhibition of mitogen-activated protein kinase,the negative regulatory substrate of mitogen-activated protein kinase phosphatase(MKP)-1,improves tight junction protein function in ECs,and genetic deletion of MKP-1 aggravates ischemic brain injury.However,whether the latter affects BBB integrity,and the cell type-specific mechanism underlying this process,remain unclear.In this study,we established an adult male mouse model of ischemic stroke by occluding the middle cerebral artery for 60 minutes and overexpressed MKP-1 in ECs on the injured side via lentiviral transfection before stroke.We found that overexpression of MKP-1 in ECs reduced infarct volume,reduced the level of inflammatory factors interleukin-1β,interleukin-6,and chemokine C-C motif ligand-2,inhibited vascular injury,and promoted the recovery of sensorimotor and memory/cognitive function.Overexpression of MKP-1 in ECs also inhibited the activation of cerebral ischemia-induced extracellular signal-regulated kinase(ERK)1/2 and the downregulation of occludin expression.Finally,to investigate the mechanism by which MKP-1 exerted these functions in ECs,we established an ischemic stroke model in vitro by depriving the primary endothelial cell of oxygen and glucose,and pharmacologically inhibited the activity of MKP-1 and ERK1/2.Our findings suggest that MKP-1 inhibition aggravates oxygen and glucose deprivation-induced cell death,cell monolayer leakage,and downregulation of occludin expression,and that inhibiting ERK1/2 can reverse these effects.In addition,co-inhibition of MKP-1 and ERK1/2 exhibited similar effects to inhibition of ERK1/2.These findings suggest that overexpression of MKP-1 in ECs can prevent ischemia-induced occludin downregulation and cell death via deactivating ERK1/2,thereby protecting the integrity of BBB

Neuroprotective effect of pretreatment with ganoderma lucidum in cerebral ischemia/reperfusion injury in rat hippocampus

Ganoderma lucidum is a traditional Chinese medicine, which has been shown to have both an-tioxidative and anti-inflammatory effects, and noticeably decreases both the infarct area and neuronal apoptosis of the ischemic cortex. This study aimed to investigate the protective effects and mechanisms of pretreatment with ganoderma lucidum （by intragastric administration） in cerebral ischemia/reperfusion injury in rats. Our results showed that pretreatment with ganoderma lucidum for 3 and 7 days reduced neuronal loss in the hippocampus, diminished the content of malondialdehyde in the hippocampus and serum, decreased the levels of tumor necrosis factor-a and interleukin-8 in the hippocampus, and increased the activity of superoxide dismutase in the hippocampus and serum. These results suggest that pretreatment with ganoderma lucidum was protective against cerebral ischemia/reperfusion injury through its anti-oxidative and an-tiinflammatory actions.

In recent years,the ancient yin-yang theory has been gradually adopted by modern researchers,especially European and American scholars,and it has also been applied to modern scientific research on sleep,viruses,metabolism,cancer,genes,autoimmune diseases,and so on.It is very promising and fruitful results have been reported.However,the understanding of the connotations of yin-yang theory is not sufficient and thorough enough in these studies.If we understand and apply yin-yang theory more comprehensively,it may provide us with additional potential mechanisms and research directions worthy of study.On the basis of promoting a comprehensive understanding of all three connotations of yin-yang theory,this review attempts to illustrate this theory,summarize its applications in modern scientific research,and reveal the potential research direction of modern medicine.

Prediction of large vessel occlusion for ischaemic stroke by using the machine learning model random forests

Backgrounds The timely identification of large vessel occlusion(LVO)in the prehospital stage is extremely important given the disease morbidity and narrow time window for intervention.The current evaluation strategies still remain challenging.The goal of this study was to develop a machine learning(ML)model to predict LVO using prehospital accessible data.Methods Consecutive acute ischaemic stroke patients who underwent CT or MR angiography and received reperfusion therapy within 8 hours from symptom onset in the Computer-based Online Database of Acute Stroke Patients for Stroke Management Quality Evaluation-II dataset from January 2016 to August 2021 were included.We developed eight ML models to integrate National Institutes of Health Stroke Scale(NIHSS)items with demographics,medical history and vascular risk factors to identify LVO and validate its efficiency.Results Finally,15365 patients were included in the training set and 4215 patients were included in the test set.On the test set,random forests(RF),gradient boosting machine and extreme gradient boosting presented area under the curve(AUC)of 0.831(95%CI 0.819 to 0.843),which were higher than other models,and RF presented the highest specificity(0.827).In addition,the AUC of RF was higher than other scales,and the accuracy of the model was improved by 6.4%compared with NIHSS.We also found the top five items of identifying LVO were total NIHSS score,gaze deviation,level of consciousness(LOC),LOC commands and motor left leg.Conclusions Our proposed model could be a useful screening tool to predict LVO based on the prehospital accessible medical data.

Background:Given that seizures may be triggered by vaccination,this study aimed to evaluate the risk and correlative factors of seizures in patients with epilepsy(PWE)after being vaccinated against coronavirus disease 2019(COVID-19).Methods:This study retrospectively enrolled PWE who were vaccinated against COVID-19 in the epilepsy centers of 11 hospitals in China.We divided the PWE into two groups as follows:(1)patients who developed seizures within 14 days of vaccination were assigned to the SAV(with seizures after vaccination)group;(2)patients who were seizure-free within 14 days of vaccination were assigned to the SFAV(seizure-free after vaccination)group.To identify potential risk factors for seizure reccurence,the binary logistic regression analysis was performed.Besides,67 PWE who had not been vaccinated were also included for elucidating the effects of vaccination on seizures recurrence,and binary logistic regression analysis was performed to determine whether vaccination would affect the recurrence rate of PWE who had drug reduction or withdrawal.Results:The study included a total of 407 patients;of which,48(11.8%)developed seizures within 14 days after vaccination(SAV group),whereas 359(88.2%)remained seizure-free(SFAV group).The binary logistic regression analysis revealed that duration of seizure freedom(P<0.001)and withdrawal from anti-seizure medications(ASMs)or reduction in their dosage during the peri-vaccination period were significantly associated with the recurrence of seizures(odds ratio=7.384,95%confidence interval=1.732–31.488,P=0.007).In addition,32 of 33 patients(97.0%)who were seizure-free for more than three months before vaccination and had a normal electroencephalogram before vaccination did not have any seizures within 14 days of vaccination.A total of 92(22.6%)patients experienced non-epileptic adverse reactions after vaccination.Binary logistic regression analysis results showed that vaccine did not significantly affect the recurrence rate of PWE who had the behavior of ASMs dose re

Background No study has thoroughly compared the effectiveness of combined antiplatelet treatments(other than clopidogrel–aspirin)versus clopidogrel–aspirin or aspirin alone for early secondary prevention in acute ischaemic stroke.Methods We identified patients with acute,minor,non-cardiogenic ischaemic stroke treated with aspirin alone,clopidogrel–aspirin or other combination treatment.Propensity scores considering the inverse probability of treatment weighting were used to adjust for baseline imbalances.The primary outcome was the composite of all strokes(ischaemic or haemorrhagic),myocardial infarction and all-cause mortality at 3 months.Results Among 12234 patients(male:61.9%;age:65.5±13 years)who met the eligibility criteria,aspirin,clopidogrel–aspirin and other combination treatments were administered in 52.2%,42.9%and 4.9%of patients,respectively.In the crude analysis,the primary outcome event at 3 months occurred in 14.5%of the other combination group,14.4%of the aspirin group and 13.0%of the clopidogrel–aspirin group.In the weighted Cox proportional hazards analysis,the 3-month primary outcome event occurred less frequently in the clopidogrel–aspirin group than in the other combination group(weighted HR:0.82(0.59–1.13)),while no association was found between the aspirin group(weighted HR:1.04(0.76–1.44))or other combination group and the 3-month primary outcome.Conclusion Other combined antiplatelet treatment,compared with aspirin alone or clopidogrel–aspirin,was not associated with reduced risks of primary composite vascular events or recurrent stroke during the first 3 months after stroke.Therefore,the results suggest that other combination treatments,particularly the cilostazol-based combination,may not be effective alternatives for clopidogrel–aspirin to prevent early vascular events in patients with acute minor stroke.Further exploration in clinical trials will be needed.
