Testicular fine needle aspiration as a diagnostic tool in nonobstructive azoospermia

Aim： To report the fine needle aspiration cytology （fNAC） of the testes used as a diagnostic tool in non-obstructive azoospermic patients. Methods： One hundred and twenty-five non-obstructive azoospermic male candidates to intracytoplasmic sperm injetion （ICSI） were analysed for follicle stimulating hormone （fSH）, luteinizing hormone （LH）, testosterone and inhibin B plasma levels. They were classified into three groups on the basis of fNAC： 1） Sertoli cell-only syndrome （SCOS）（70）; 2） severe hypospermatogenesis （42）; and 3） maturation arrest （13）, Then, all men underwent testicular sperm extraction （TESE） for sperm recovery for ICSI. Results： Mature spermatozoa were detected by fNAC in 24 of 42 men with severe hypospermatogenesis and nine of 13 men with maturation arrest; while they were retrieved by TESE in 29 of 70 men with SCOS, 35 of 42 men with severe hypospermatogenesis （including the 24 by fNAC） and 10 of 13 men with maturation arrest （including the nine by fNAC）. The sensitivity and specificity of fNAC were 44.6 % and 100 %, respectively. There was no difference on testicular volume and hormonal parameters in men with and without sperm retrieved. Conclusion： These findings suggest that fNAC may be a simple and valid diagnostic parameter in non-obstructive azoospermic men and it may represent a valid positive prognostic parameter for sperm recovery at TESE, （Asian J Androl 2005 Sep; 7： 289-294）

Abdominal wall defects and incisional hernias represent a challenging problem. In particular, when a synthetic mesh is applied to contaminated wounds, its removal is required in 50%-90% of cases. Biosynthetic meshes are the newest tool available to surgeons and they could have a role in ventral hernia repair in a potential-ly contaminated field. We describe the use of a sheet of bovine pericardium graft in the reconstruction of abdominal wall defect in two patients. Bovine pericardium graft was placed in the retrorectus space and secured to the anterior abdominal wall using polypropylene sutures in a tension-free manner. We experienced no evidence of recurrence at 4 and 5 years follow-up.

BACKgROUND Pancreatic cancer(PC)is one of the deadliest malignancies with an alarming mortality *** significant advancement in diagnostics and therapeutics,early diagnosis remains elusive causing poor prognosis,marred by mutations and epigenetic modifications in key genes which contribute to disease *** To evaluate the various biological tumor markers collectively for early diagnosis which could act as prognostic biomarkers and helps in future therapeutics of PC in Kashmir *** A total of 50 confirmed PC cases were included in the study to evaluate the levels of carbohydrate antigen 19-9(CA 19-9),tissue polypeptide specific antigen(TPS),carcinoembryonic antigen(CEA),vascular endothelial growth factor-A(VEgf-A),and epidermal growth factor receptor(EgfR).Mutational analysis was performed to evaluate the mutations in Kirsten rat sarcoma(KRAS),Breast cancer type 2(BRCA-2),and deleted in pancreatic cancer-4(DPC-4)***,epigenetic modifications(methylation of Cpg islands)were performed in the promoter regions of cyclin-dependent kinase inhibitor 2A(p16;CDKN2A),MutL homolog 1(hMLH1),and Ras association domain-containing protein 1(RASSf1A)*** We found significantly elevated levels of biological markers CA 19-9(P≤0.05),TPS(P≤0.05),CEA(P≤0.001),and VEgf(P≤0.001).Molecular genetic analysis revealed that KRAS gene mutation is predominant in codon 12(16 subjects,P≤0.05),and 13(12 subjects,P≤0.05).However,we did not find a mutation in DPC-4(1203g>T)and BRCA-2(617delT)***,epigenetic modification revealed that Cpg methylation in 21(P≤0.05)and 4 subjects in the promoter regions of the p16 and hMLH1 gene,*** In conclusion,CA 19-9,TPS,CEA,and VEgf levels were significantly elevated and collectively have potential as diagnostic and prognostic markers in *** data of mutation in the KRAS gene commonly in codon 12 and rare in codon 13 could augment the predisposition towards ***,methylatio

Objective： To investigate the effectiveness of curcumin, a natural polyphenolic compound with antioxidant and anti-inflammatory activities, on the frequency of symptoms of anxiety and depression in obese individuals. Methods： In this double blind, cross-over trial, 30 obese subjects were randomized to receive either curcumin （1 g/day） or placebo for a period of 30 days. following a wash-out interval of 2 weeks, each subject was crossed over to the alternative regimen for a further 30 days. Severity of anxiety and depression was assessed at baseline and at weeks 4, 6 and 10 of the trial using the Beck Anxiety Inventory （BAI） and Beck Depression Inventory （BDI） scales, respectively. Results： Mean BAI score was found to be significantly reduced following curcumin therapy （P=0.03）. However, curcumin supplementation did not exert any significant impact on BDI scores （P=0.7）. Conclusion： Curcumin has a potential anti-anxiety effect in individuals with obesity.

Conventional sutures used in surgical procedures often lack the capability to effectively monitor physical and chemical activities or the microbial environment of surgical wounds due to their inadequate mechanical properties, insufficient electrical accuracy and unstability. Here, we present a straightforward layer-by-layer coating technique that utilizes 3-glycidoxypropyltrimethoxysilane(CA), graphene oxide(gO), and ascorbic acid(AA) to develop conductive silk-based surgical sutures(CA-rgSfS). The CA-rgSfS feature a continuous reduced graphene oxide(rgO) film on their surface, forming robust hydrogen bonds with silk fibroin. The reduction process of rgO is confirmed through Raman analysis, demonstrating an enhanced D peak to g peak ***, the CA-rgSfS exhibit exceptional mechanical properties and efficient electron transmission, with a knot-pull tensile strength of 2089.72 ± 1.20 cN and an electrical conductivity of 130.30 ± 11.34 S/m, respectively, meeting the requirements specified by the United States Pharmacopeia(USP) for 2-0 sutures. These novel CA-rgSfS demonstrate the ability to accurately track resistance changes in various fluid environments with rapid response, including saline, intestinal, and gastric fluids. The suture also retains remarkable stretchablility and stability even after enduring 3000 tensile cycles, highlighting their potential for precise surgical site monitoring during the wound healing process.

Colonic perforation during endoscopic diagnostic or therapeutic procedures,represents an uncommon occurrence even if,together with haemorrhage,it is still the most common complication of colonoscopy,with an incidence ranging between 0.1% and 2% of all colonoscopic procedures. The ideal treatment in these cases remains elusive as the endoscopist and the surgeon have to make a choice case by case,depending on many factors such as how promptly the rupture is identified,the condition of the patient,the degree of contamination and the evidence of peritoneal irritation. surgical interventions both laparotomic and laparoscopic,and other medical non-operative solutions are described in the literature. Only three cases have been reported in the literature in which the endoscopic apposition of endoclips was used to repair a colonic perforation during colonoscopy. Ours is the first case that the perforation itself was caused by the improper functioning of a therapeutic device.

Rare cause of odynophagia: giant esophageal ulcer

gastrointestinal complications are a frequent cause of morbidity after transplantation and may affect up to 40% of kidney transplant recipients. Here we report a rare case of idiopathic giant esophageal ulcer in a kidney transplant recipient. A 37-year-old female presented with a one-week history of odynophagia and weight loss. Upon admission, the patient presented cold sores, and a quantitative cytomegalovirus polymerase chain reaction was positive(105 copies/ml). An upper endoscopy demonstrated the presence of a giant ulcer. Serological test and tissue biopsies were unable to demonstrate an infectious origin of the ulcer. Immunosuppression was reduced and everolimus was introduced. An empirical i.v. therapy with acyclovir was started, resulting in a dramatic improvement in symptoms and complete healing of the ulcer. Only two cases of idiopathic giant esophageal ulcer in kidney transplant recipients have been reported in the literature; in both cases, steroid therapy was successful without recurrence of symptoms or endoscopic findings. However, this report suggests that correction of immune imbalance is mandatory to treat such a rare complication.
