Monochloramine is a well-known disinfectant for drinking water with several advantages over chlorine and chlorine dioxide. With these two biocides, monochloramine is included in the US-EPA list of disinfectants for drinking water. At the beginning of XXI century, epidemiological studies demonstrated its superior ability to control Legionella in hot water health-care premises. In 2005 a research program started to set up a reliable and effective method to produce monochloramine directly in domestic hot water systems, at the safest concentration and without accumulation of by-products. Results of these researches, which has been carried out with the collaboration of Italian and American Institutions showed that monochloramine can be safely and reliably prepared and that it is the best approach to Legionella remediation in health-care facilities.

Objectives:This study aimed to test the association between of type and number of D'Amico high-risk criteria(DHRCs)with cancer-specific mortality(CSM)in high-risk prostate cancer patients treated with radical *** and methods:In the Surveillance,Epidemiology,and End Results database(2004–2016),we identified 31,281 radical prostatectomy patients with at least 1 DHRC,namely,prostate-specific antigen(PSA)>20 ng/mL(hrPSA),biopsy gleason grade group(hrggg)score of 4 and 5,or clinical tumor stage≥T3(hrcT).Multivariable Cox regression models and competing risks regression models(adjusting for other cause mortality)tested the association between DHRCs and 5-year ***:Of 31,281 patients,14,394(67%)exclusively harbored hrggg,3189(15%)harbored hrPSA,and 1781(8.2%)harbored *** 2132 patients(6.8%)harbored a combination of the 2 DHRCs,and 138(0.6%)had all 3 ***-year CSMrates ranged from 0.9%to 3.0%when any individual DHRC was present(hrcT,hrPSA,and hrggg,in that order),1.6%to 5.9%when 2 DHRCs were present(hrPSA-hrcT,hrcT-hrggg,and hrPSA-hrggg,in that order),and 8.1%when all 3 DHRCs were *** regression models and competing risks regression confirmed the independent predictor status of DHRCs for 5-year CSM that was observed in univariable analyses,with hazard ratios from 1.00 to 2.83 for 1 DHRC,2.35 to 5.88 for combinations of 2 DHRCs,and 7.13 for all 3 ***:Within individual DHRCs,hrcT and hrPSA exhibited weaker effects than hrggg ***,a dose-response effect was identified according to the number of ***,the type and number of DHRCs allow further risk stratification within the high-risk subgroup.

BACKgROUND Radicular cyst is a lesion of odontogenic origin that arises from epithelial remains due to periapical periodontitis caused by inflammatory reactions generated at the apex of affected teeth with infected or necrotic *** therapeutic mana gement of radicular cysts is *** is only one case report of enucleation of a radicular cyst managed with microsurgery and apicoectomy,but without the use of the guided tissue regeneration(gTR)technique in the same surgical *** present clinical case describes the management of a radicular cyst with microsurgical approach,performance of an apicoectomy of the tooth associated with the entity,application of gTR technique,use of a resorbable membrane of type I bovine collagen,and bovine *** SUMMARY A 68-year-old patient presented with a radicular cyst from an upper lateral *** microsurgical management used was aimed at enucleating the chemical membrane,performing apicoectomy of the tooth along with careful and precise retrograde filling,and implementing gTR technique using a resorbable collagen membrane and bovine *** diagnosis of radicular cyst was confirmed using histopathological *** patient underwent follow-up evaluations at 10 and 30 d *** 4 months postoperative evaluation,she remained asymptomatic,and radiographs showed significant periapical healing with adequate bone *** These results suggest that microsurgical management using the gTR technique with collagen membrane and xenograft,contributes to bone regeneration.

Obesity is increasing worldwide,and this has major implications in the setting of kidney *** with obesity may have limited access to transplantation and increased posttransplant morbidity and *** transplant centers incorporate interventions aiming to target obesity in kidney transplant candidates,including dietary education and lifestyle *** those failing nutritional restriction and medical therapy,the use of bariatric surgery may increase the transplant candidacy of patients with obesity and endstage renal disease(ESRD)and may potentially improve the immediate and late *** surgery in ESRD patients is associated with weight loss ranging from 29.8% to 72.8% excess weight loss,with reported mortality and morbidity rates of 2% and 7%,*** most commonly performed bariatric surgical procedures in patients with ESRD and in transplant patients are laparoscopic sleeve gastrectomy(LSg)and laparoscopic Roux-en-Y gastric ***,the correct timing of bariatric surgery and the ideal type of surgery have yet to be determined,although pretransplant LSg seems to be associated with an acceptable risk-benefit *** review the impact of obesity on kidney transplant candidates and recipients and in potential living kidney donors,exploring the potential impact of bariatric surgery in addressing obesity in these populations,thereby potentially improving posttransplant outcomes.

Acute Coronary Syndrome of Young Subjects at the Luxembourg Mother-Child University Hospital in Bamako

Introduction: Acute coronary syndrome is often the first event of coronary disease of young subjects. Objective: To study sociodemographic, clinical, paraclinical, therapeutic and evolutionary aspects of acute coronary syndrome of young subjects. Patients and Methods: Descriptive cross-sectional study with prospective recruitment from October 01, 2020 to March 31, 2022. Were included all patients admitted for acute coronary syndrome whose age was less or equal to 45 years and who had undergone coronary angiography at the Mother-Child University Hospital on Luxembourg from Bamako. Results: During the study period, we collected 60 patient files out of 198. These 60 patient files met our inclusion criteria. Hospital frequency was 30.30%. Average age of patients was 40.43 ± 3.9 years. Sex ratio M/f was 5.3. Main cardiovascular risk factor was smoking tobacco (23.42%), followed by dyslipidemia (13.92%). functional signs were dominated by angina 62% followed by dyspnea 25.3%. Persistent ST-segment elevation on electrocardiogram was present in 76.4%. Left ventricular systolic dysfunction was present in 26.5%. Time to first medical contact was more than 12 hours in 62.7% of cases. Radial approach was adopted in 94% of cases. Coronary angiography was pathological in 85% (n = 51) of cases. Lesions were single-vessel disease in 47.1% and culprit artery was anterior interventricular in 51% of cases. Dual anti-platelet aggregation was aspirin and ticagrelor in 91.8% of cases. Angioplasty was performed in all patients who had significant abnormalities at coronary angiography. In-hospital mortality was 3.9%. Conclusion: Acute coronary syndromes exist in young Africans with a male predominance. Main cardiovascular risk factor is smoking tobacco. Coronary lesions are single vessel disease in the majority of case. Most of patients meet medical team after 12 hours. Time to first medical contact is a main challenge in our country.

Psychiatric Variables in Cytomegalovirusor Toxoplasma gondii—Infected Pregnants: Prospects for Cure with a Program of “Counseling about Behavioral Change”

TORCH complex, one of the potential infections that may occur during pregnancy, may contribute also to prenatal pregnant women’s anxiety and depression. The aim of this study was to explore the presence of psychiatric symptoms, in relation with infection of TORCH agents, specifically the Toxoplasma gondii (T. gondii) and Cytomegalovirus (CMV). The study was conducted on 58 pregnant women recruited from the Clinical Virology Unit, A.O.U. Policlinico-Vittorio Emanuele, P.O. “gaspare Rodolico”, from September 2012 to March 2014. Psychiatric symptoms were evaluated through the Symptom Checklist-90 Revised (SCL-90 R) in pregnant women with CMV or T. gondii infection. Moreover, pregnant women were invited to participate in a program of “Counseling about behavioral change”, an effective psychotherapeutic training for the development of new motivational strategies to the infection acceptance, the greater self-confidence and greater adherence to treatment. The age of the subjects was positively correlated with Depression (r = 0.119, p ? 0.178, p ? 0.231, r = ? 0.320, p ? 0.208, p ? 0.105, p ? 0.236, p finally, we can notice an improvement of the variables Anxiety and Somatization in the whole sample after the attendance at the counseling psychotherapic program. Our study suggests the presence of anxiety and somatization in pregnant women with infection, expression of a phase of emotional fragility with the concern of transmitting a serious disease in the unborn child. The program “Counseling about behavioral change” allowed us to create an atmosphere of trust and to understand the usefulness of “empathic listening”. giving correct and accurate information about the disease reassures the patient and prevents the realization of negative thoughts and further concern and discomfort.
