The aim of this study was to evaluate the yield of clinical symptoms, signs, and radiological studies in the diagnosis of foreign body aspiration (FBA) in children. Methods: During the 2- year study, we performed on all children admitted to the pediatric department for suspected FBAa protocol that included the following: thorough medical history, physical examination, radiological studies (chest x-ray and fluoroscopy), and rigid bronchoscopy by a senior otolaryngologist. The yield of these measures for the diagnosis of FBA was evaluated. Results: Foreign bodies were found in 56 (57% ) of the 98 children with suspected FBA. Thirty-three (59% ) children were boys and 23 (41% ) girls (P < .05), with a mean age of 24 months (range, 8- 84 months). Main symptoms in the children with FBA were the following: choking (76.8% ), prolonged cough (14.3% ), dyspnea (3.6% ), and nonresolving pneumonia (1.8% )- . Physical examination, chest x-ray, and fluoroscopy findings were abnormal in 80.4% , 67.9% , and 46.9% of the children with FBA, respectively. The diagnostic yield of physical examination and radiological studies increased 24 hours after the event of FBA. In 45.2% of the children with positive history but with normal findings from physical examination and radiological studies, foreign bodies were found. Of the children with “ doubtful” history, physical examination, chest x-ray, and fluoroscopy findings were abnormal in 58% , 38% , and 12.5% , respectively. Foreign bodies were found in 9.5% of these children. Conclusion: Medical history is the key for the diagnosis of FBA. Choking followed by an acute episode of coughing is the most common presentation of FBA. The yield of physical examination and radiological studies in the diagnosis of FBA is relatively low but is increased when the presentation is delayed and when history is doubtful. If FBA is suspected, bronchos- copy should be performed.

Open-loop frequency response for a chaotic masking system

In this paper, a new numerical simulation approach is proposed for the study of open-loop frequency response of a chaotic masking system. Using Chua＇s circuit and the Lorenz system as illustrative examples, we have shown that one can employ chaos synchronization to separate the feedback network from a chaotic masking system, and then use numerical simulation to obtain the open-loop synchronization response, the phase response, and the amplitude response of a chaotic masking system. Based on the analysis of the frequency response, we have also proved that changing the amplitude of the exciting （input） signal within normal working domain does not influence the frequency response of the chaotic masking system. The new numerical simulation method developed in this paper can be extended to consider the open-loop frequency response of other systems described by differential or difference equations.

Gray mouse lemurs（Microcebus murinus） from Madagascar present an excellent model for studies of torpor regulation in a primate species. In the present study, we analyzed the response of the insulin signaling pathway as well as controls on carbohydrate sparing in six different tissues of torpid versus aroused gray mouse lemurs. We found that the relative level of phospho-insulin receptor substrate（IRS-1） was significantly increased in muscle, whereas the level of phospho-insulin receptor（IR） was decreased in white adipose tissue（WAT） of torpid animals, both suggesting an inhibition of insulin/insulin-like growth factor-1（IGF-1） signaling during torpor in these tissues. By contrast, the level of phospho-IR was increased in the liver. Interestingly, muscle,WAT, and liver occupy central roles in whole body homeostasis and each displays regulatory controls operating at the plasma membrane. Changes in other tissues included an increase in phosphoglycogen synthase kinase 3a（GSK3a） and decrease in phospho-ribosomal protein S6（RPS6） in the heart, and a decrease in phospho-mammalian target of rapamycin（m TOR） in the kidney. Pyruvate dehydrogenase（PDH） that gates carbohydrate entry into mitochondria is inhibited via phosphorylation by pyruvate dehydrogenase kinase（e.g., PDK4）. In the skeletal muscle, the protein expression of PDK4 and phosphorylated PDH at Ser 300 was increased, suggesting inhibition during torpor. In contrast, there were no changes in levels of PDH expression and phosphorylation in other tissues comparing torpid and aroused animals. information gained from these studies highlight the molecular controls that help to regulate metabolic rate depression and balance energetics during primate torpor.

BACKGROUND Venom-induced consumption coagulopathy(VICC)is characterized by coagulation dysfunction accompanied by decreased coagulation factor activity and fibrinogen(FBG)*** report a patient with VICC caused by snake bite who manifested persistent FBG deficiency without abnormal coagulation factor *** information may be helpful in diagnosing and treating *** SUMMARY A 49-year-old man who had been bitten by a snake 13 h previously was admitted to the Emergency department of our hospital with visible swelling of a finger and a bleeding puncture *** provisional diagnosis was VICC,this being made based on persistent bleeding from the puncture site and subcutaneous *** evidence of coagulation abnormalities,including fibrinolysis,and findings on thromboelastography confirmed *** had persistent afibrinogenemia requiring intravenous infusions of cryoprecipitate and fresh frozen plasma,together with continuous large doses of human *** this treatment,the patient’s right upper limb swelling improved significantly and his subcutaneous hemorrhage *** of his abnormal laboratory findings returned to normal by day *** 6 months’of follow-up,the patient had no further hemorrhagic *** Hemorrhagic snake venom can result in coagulation dysfunction characterized by persistent FBG deficiency without abnormal coagulation factor activity.

Assessment of health impacts of noise pollution in the Tarkwa Mining Community of Ghana using noise mapping techniques

Objective Communities in the developing countries such as Ghana have little knowledge of effects of noise pollution on human health,which is demonstrated by their attitude towards this *** study assessed the health impacts of noise pollution and its spatial distribution in the Tarkwa Mining Community(TMC)of *** To achieve the study objective,questionnaires were administered;as well as collation of health data from major health centers in the study *** levels were measured and noise map produced using geographic information system(GIS)*** maps of some diseases were done using overlay techniques in *** noise exposure and corresponding noise doses for churches,working sites and social centers were also calculated using the respective *** The noise levels were found to be high above the prescribed Ghana Environmental Protection Agency(EPA)standards,with traffic noise levels ranging from 65.00 dBA to 98 dBA,while that of churches ranged from 73.10 dBA to 107.00 dBA and that of working sites from 74.4 dBA to 115.2 *** calculated noise exposure and corresponding noise dose for churches ranged from 75.1 dBA to 104.6 dBA(i.e.,10%‒8000%),while that for workers’sites were from 75.8 dBA to 115 dBA(i.e.,12%‒90000%).Statistical regression and correlation analyses were done for diseases such as hypertension,ear problems and sleep *** The study has therefore revealed that the noise levels in the study area are very high and corresponding health impacts are *** and authorities should devise mitigating measures to combat this rising *** results revealed a strong positive correlation between noise and corresponding health *** the positive correlation there are other causes and effects to the mentioned diseases.

Large shift of the Pacific Walker Circulation across the Cenozoic

Fluctuations in the Pacific Walker Circulation(PWC), a zonally oriented overturning cell across the tropical Pacific, can cause widespread climatic and biogeochemical perturbations. It remains unknown how the PWC developed during the Cenozoic era, with its substantial changes in greenhouse gases and continental positions. Through a suite of coupled model simulations on tectonic timescales, we demonstrate that the PWC was ～38?broader and ～5% more intense during the Early Eocene relative to present. As the climate cooled from the Early Eocene to the Late Miocene, the width of the PWC shrank, accompanied by an increase in intensity that was tied to the enhanced Pacific zonal temperature gradient. However, the locations of the western and eastern branches behave differently from the Early Eocene to the Late Miocene,with the western edge remaining steady with time due to the relatively stable geography of the western tropical Pacific; the eastern edge migrates westward with time as the South American continent moves northwest. A transition occurs in the PWC between the Late Miocene and Late Pliocene, manifested by an eastward shift(both the western and eastern edges migrate eastward by >12?) and weakening(by ～22%),which we show here is linked with the closure of the tropical seaways. Moreover, our results suggest that rising CO2favors a weaker PWC under the same land-sea configurations, a robust feature across the large spread of Cenozoic climates considered here, supporting a weakening of the PWC in a warmer future.

Extrusion process of Acanthopanax senticosus leaves enhances the gastroprotective effect of compound 48/80 on acute gastric mucosal lesion in rats

OBJECTIVE:To investigate the gastroprotective effects of Acanthopanax senticosus leaves(ASLs)extrusion on acute gastric mucosal lesion in rats induced by compound 48/80(C48/80).METHODS:Rats were divided into six groups:normal;C48/80-induced gastric lesion control;gastric lesion positive control(famotidine 4 mg/kg);gastric lesion administered with two levels of extruded ASLs(ASLE,40 and 200 mg/kg);and gastric lesion treated with ASLs(ASL 200 mg/kg).Mucus secretion/damage was determined by immunohistological *** and western blotting were performed to determine gastric mucosal Bax and Bcl-2 *** mucosal oxidative-stress-related enzymes and malondialdehyde were ***:C48/80-induced mucus depletion and inflammation in the gastric mucosa were significantly attenuated by *** increased serum serotonin and histamine concentrations in C48/80-treated rats were also attenuated by *** mucosal Bax protein expression was increased and Bcl-2 expression was decreased after C48/80 treatment,and ASLs ameliorated Bax and Bcl-2 *** extrusion process significantly augmented the effects of ASLs in a dosedependent *** at 200 mg/kg normalized mucus damage/secretion,C48/80-induced increases of mucosal myeloperoxidase activity(index of inflammation),xanthine oxidase,and malondialdehyde content(index of lipid peroxidation).The effects of ASLs on Bax and Bcl-2expression were also enhanced by ***,these effects of ASLEs at 200 mg/kg were similar to those of famotidine,a histamine H2-receptor antagonist commonly used to treat gastric ***:ASLEs prevented acute gastric mucosal lesion progression induced by C48/80,possibly by inducing mucus production,and reduced inflammation and oxidative stress in gastric mucosa through an anti-apoptotic mechanism.

This study explores the influence of infill patterns on machine acceleration prediction in the realm of three-dimensional(3D)printing,particularly focusing on extrusion *** primary objective was to develop a long short-term memory(LSTM)network capable of assessing this *** conducted an extensive analysis involving 12 distinct infill patterns,collecting time-series data to examine their effects on the acceleration of the printer’s *** LSTM network was trained using acceleration data from the adaptive cubic infill pattern,while the Archimedean chords infill pattern provided data for evaluating the network’s prediction *** involved utilizing offline time-series acceleration data as the training and testing datasets for the LSTM ***,the LSTM model was devised to predict the acceleration of a fused deposition modeling(FDM)printer using data from the adaptive cubic infill *** testing yielded a root mean square error(RMSE)of 0.007144,reflecting the model’s *** refinement and testing of the LSTM model were conducted using acceleration data from the Archimedean chords infill pattern,resulting in an RMSE of ***,the developed LSTM model demonstrated superior performance compared to an optimized recurrent neural network(RNN)in predicting machine acceleration *** empirical findings highlight that the adaptive cubic infill pattern considerably influences the dimensional accuracy of parts printed using FDM technology.
